The Work Disability Diagnosis Interview (WoDDI) is a structured interview guide developed by the University of Sherbrooke, Canada to help clinicians detect the most important work-related disability predictors and to identify one or more causes of prolonged absenteeism. This methodological study aims for the cross-cultural adaptation of the WoDDI for the Brazilian context. The method followed international guidelines for studies of this kind, including the following steps: initial translation, synthesis of translations, back translation, evaluation by an expert committee and testing of the penultimate version. These steps allowed obtaining conceptual, semantic, idiomatic, experiential and operational equivalences, in addition to content validity. The results showed that the translated WoDDI is adapted to the Brazilian context and can be used after training.

OBJECTIVE: Visual hallucinations are under-reported by patients and are often undiscovered by health professionals. There is no gold standard available to assess hallucinations. Our objective was to develop a reliable, valid, semi-structured interview for identifying and assessing visual hallucinations in older people with eye disease and cognitive impairment. METHODS: We piloted the North-East Visual Hallucinations Interview (NEVHI) in 80 older people with visual and/or cognitive impairment (patient group) and 34 older people without known risks of hallucinations (control group). The informants of 11 patients were interviewed separately. We established face validity, content validity, criterion validity, inter-rater agreement and the internal consistency of the NEVHI, and assessed the factor structure for questions evaluating emotions, cognitions, and behaviours associated with hallucinations. RESULTS: Recurrent visual hallucinations were common in the patient group (68.8%) and absent in controls (0%). The criterion, face and content validities were good and the internal consistency of screening questions for hallucinations was high (Cronbach alpha: 0.71). The inter-rater agreements for simple and complex hallucinations were good (Kappa 0.72 and 0.83, respectively). Four factors associated with experiencing hallucinations (perceived control, pleasantness, distress and awareness) were identified and explained a total variance of 73%. Informants gave more 'don't know answers' than patients throughout the interview (p = 0.008), especially to questions evaluating cognitions and emotions associated with hallucinations (p = 0.02). CONCLUSIONS: NEVHI is a comprehensive assessment tool, helpful to identify the presence of visual hallucinations and to quantify cognitions, emotions and behaviours associated with hallucinations.

Purpose: To perform a psychometric analysis of the Brazilian version of the Brief Social Phobia Scale (BSPS). Materials and methods: Hundred and seventy-eight university students of both genders aged on average 21.2 years and identified as Social Anxiety Disorder (SAD) cases and non-cases was studied, with the structured clinical interview for DSM-IV being used as a parameter. The different instruments were applied in an individual manner in the presence of a rater and of an observer. Results: The BSPS showed adequate internal consistency (0.48-0.88) and concurrent and divergent validity with the Beck Anxiety Inventory (BAI) (0.21-0.62), Social Phobia Inventory (0.24-0.82) and Self Statements During Public Speaking Scale (SSPS) (0.23-0.31). Discriminative validity revealed a sensitivity of 0.88-0.90 and a specificity of 0.81(0.83 for cut-off notes of 18/19. Factorial analysis demonstrated the presence of six factors that jointly explained 71.79% of data variance. Construct validity indicated some limits of the scale regarding the diagnosis of SAD. Inter-rater reliability was strong (0.86-1.00, p < 0.001). Conclusions: The BSPS is adequate for use with university students, although further studies in different cultures, samples and contexts are still necessary. (C) 2009 Elsevier Masson SAS. De l’experiència professional de l’equip del SATAF al llarg dels anys, s’ha constatat que un dels conflictes més recurrents, existents en els procediments contenciosos, és el rebuig filial envers un dels progenitors, habitualment, el no-custodi. Un tipus de rebuig filial és la Síndrome d’Alienació Parental (SAP), descrita per Gardner, inicialment, l’any 1985. A partir dels estudis realitzats d’ençà l’any 2004, aquest grup d’investigació pretén centrar la present recerca en el perfil de competències parentals del progenitor alienat. Paral·lelament, s’ha elaborat una entrevista semiestructurada d’exploració de la SAP. Altrament, també s’ha creat una guia d’observadors per tal d’aconseguir certa operativitat diagnòstica. Els resultats obtinguts han partit d’una anàlisis estadística no-paramètrica, considerant el petit tamany de la mostra. Amb l’anàlisi, es constata que la gravetat de la SAP no manté relació quant a presència d’habilitats parentals en el progenitor alienat. D'altra banda, l’actitud d’aquests progenitors incideix en el manteniment de la Síndrome. Disposar d’eines específiques per a l’avaluació d’aquests casos optimitza i operatitvitza les intervencions amb aquestes famílies.

RÉSUMÉ L‘hébergement en centre de soins de longue durée d'un parent âgé atteint de démence marque une transition pour les personnes qui occupent un rôle d’aidant familial principal. Ces personnes, principalement des femmes, poursuivent leur engagement après l’hébergement et souhaitent être impliquées dans les décisions concernant les soins offerts à leur parent. Souvent l'hébergement survient au moment où la personne âgée n'est plus en mesure, compte tenu de ses déficiences cognitives, d'exprimer clairement ses besoins; les aidantes accordent alors une place centrale au rôle de représentante de leur proche hébergé. Cette étude avait pour but d’expliquer la transformation du rôle de représentante chez des aidantes familiales dont le parent atteint de démence vit dans un centre d’hébergement et de soins de longue durée (CHSLD). La méthode qualitative de la théorisation ancrée a été retenue pour expliquer ce processus social. Les résultats obtenus reposent sur 14 entrevues en profondeur réalisées auprès de filles dont le parent atteint de démence était hébergé depuis plus de six mois dans un CHSLD. Ces aidantes ont été sélectionnées selon une procédure d’échantillonnage théorique et l’analyse des données a été réalisée à partir de la transcription intégrale des entrevues en suivant trois niveaux d'analyse, soit la codification ouverte, axiale et sélective. Une proposition théorique, générée par voie inductive, met en lumière trois processus intermédiaires interreliés expliquant la transformation du rôle de représentante pendant l’hébergement du proche : 1) l’intégration dans le milieu ; 2) l’évaluation de la qualité des soins et 3) le développement de la confiance envers le milieu d’hébergement. Plus précisément, les aidantes déploient différentes stratégies d’intégration dans le milieu d’hébergement, soit l’établissement de relations de réciprocité et d’une collaboration avec le personnel soignant, ainsi que l’utilisation d’un style de communication diplomatique. Parallèlement, elles évaluent la qualité des soins en trois étapes : jugement, pondération et action. Finalement, une relation de confiance avec les membres du personnel de l’établissement se développe en lien avec cinq facteurs spécifiques, soit les premières impressions, la comparaison avec d’autres CHSLD, l’intérêt démontré par le personnel envers le proche, le fait d’être entendue et prise au sérieux et la transparence du milieu d’hébergement. Ces trois processus contribuent au bien-être du parent hébergé et à celui de l’aidante. Le développement de la confiance étant associé aux deux autres processus intermédiaires ainsi qu’au bien-être de l’aidante, il est le processus central de la théorie contextuelle qui émerge de cette recherche. Cette étude contribue au développement des connaissances, notamment en fournissant plusieurs éléments inédits de compréhension du processus de transformation du rôle de représentante des aidantes familiales, de même que des pistes pour soutenir ces aidantes dont le parent, souffrant de démence, n’est plus en mesure de prendre des décisions. La théorie contextuelle proposée dans le cadre de cette étude constitue les prémices d’une théorie de niveau intermédiaire portant sur le rôle de représentant des aidants familiaux dans le contexte plus général du système de santé. Des études réalisées dans d’autres contextes de soins et auprès d’aidants de proches vulnérables ayant d’autres types d’affections sont ainsi recommandées.

Le but de cette étude était d’explorer et de comprendre l’expérience que peuvent vivre les proches aidants au regard de leurs interactions avec leur parent âgé qui erre, lors de leurs visites au centre d’hébergement et de soins de longue durée. Une étude qualitative exploratoire a été réalisée en s’inspirant de la conception des interactions sociales de Khosravi (2003, 2007) et de la théorie de l’être humain unitaire de Rogers (1970, 1990). À l’aide d’un guide d’entretien semi-dirigé, des entrevues individuelles ont été effectuées auprès de cinq filles aidantes ayant un parent errant hébergé. Les résultats de l’analyse thématique suggèrent que ces aidantes sont en mesure d’expliquer l’errance en identifiant plusieurs causes à ce comportement, entre autres la recherche de repères connus, les habitudes de vie antérieures, le besoin de liberté et le désir de retourner chez soi. Être en mesure d’expliquer le comportement semble en favoriser l’acceptation. Cette compréhension et cette acceptation du besoin constant de se mouvoir permettent aux aidantes d’entretenir des interactions avec leur parent et de passer de « bons moments » lors des visites. Pour maintenir ces interactions encore importantes, les aidantes ont développé des stratégies de communication non verbales comme le toucher et la marche rapide. Elles ont également su se familiariser avec ce comportement en évitant de confronter leur parent et en faisant preuve de créativité. Ces résultats mettent en lumière des pistes d’interventions infirmières afin de favoriser des interactions harmonieuses entre les résidents errants et leur proche aidant.

Cancer of the cervix (cervical cancer) is the second most prevalent cancer among Brazilian women. The high rates of cervical cancer in Brazil justify the implementation of effective strategies to control this, which include actions to promote health, primary prevention, early detection, screening, treatment and palliative care. Despite the existence of the National Programme for Control of the CCU there was no reduction in the incidence and mortality of this disease in Brazil. The Family Health Strategy (FHS) has the potential to facilitate such control and, in this context, one should consider that nurses play a central role. The study aimed to know the general intervention strategies used by nurses FHS of Natal / RN in CCU control, and how specific: analyzing the knowledge of these nurses on the CCU, the actions developed in the ESF for the control of CCU and identify the difficulties faced by them to perform it. This is a descriptive exploratory quantitative developed through a structured interview guide with 106 nurses who have experience in controlling the CCU in FHS teams of Natal / RN. Data analysis was performed using descriptive statistic s. The results pointed to actions taken in the FHS to control the CCU, collection of cervical cancer screening, health education activities, nursing consultation, referral of suspected cases for medical monitoring and active women with abnormal test result . The actions that were not mentioned by the nurses included: forming groups of prevention and health promotion; expand coverage of exams and office hours of consultations, establishment of alternatives to end the pent-up demand in the health units, participation in treatment or rehabilitation process users with the CCU; interventions for pain management, alliances and partnerships with schools, in dustry and the use of protocols. This study can be seen that the practice nurses partially shares to the CCU in Natal / RN. The participants of this study, when asked about the CCU, specifically for signs and symptoms of disease and risk factors in general showed important gaps. Difficulties such as lack of materials for collection of Pap smear; inadequate physical space in the Health Units; pent-up demand in the service, delay in arrival of the test results; obstacles in the actions of referral and counter-referral and cultural factors make the CCU control is compromised. It is believed in this research contributed to a reflection on the importance of the role of nurses in the development of the ESF control actions CCU, pointing out the factors that affect these. It is important to involve all nurses who comprise the ESF as knowledgeable of the risk factors, signs and symptoms, and existing tools for the early detection of cervical cancer in the pursuit of quality improvement actions to promote women`s health, contributing in planning future interventions that may reduce mortality from this disease in Natal / RN.

The purpose of this qualitative study was to gain an understanding of the experiences of Mexican American women living with intimate partner abuse relevant to the process of disclosure of abuse. Limited research exists on the experiences of women who are of Mexican descent living with intimate partner abuse and their disclosure of abuse. Factors that influence disclosure for other populations are well articulated in the literature however, these factors have not been adequately verified in persons of Mexican descent. Data are reported from in-depth interviews with 26 clients at a shelter and an outreach agency in a south Texas-Mexico border community. Semi-structured interview guide was used to elicit information over an 11 month period. A grounded theory ethnography approach was used to analyze data. Verification strategies and constant comparison techniques (e.g. investigator responsiveness, methodological coherence, sampling adequacy, an active analytic stance, and saturation) enhanced rigor of analysis. Nineteen Mexican immigrant women and seven Mexican American women participated in the study. Several themes were discerned related to women's experiences in abuse: painful living, questioning endurance, and confronting reality. In almost every participant's account there was a description of repeated victimization by her intimate partner or partners, and again, by others within and outside her network. The participants discussed several cultural factors (e.g. embarrassment, concerns for family, avoidance of causing pain to family, protection of partner, avoidance of being judged) that hindered their decisions whether or not to disclose. Participants noted that healthcare workers rarely asked probing questions regarding abuse. The timing and process of disclosure took many turns for women in this study. Some of the factors hindering women from disclosing were found to be influenced by cultural practices. The consequences of disclosure for many of the women led them to re-victimization. Implications for practice to avoid missed opportunities with women living in abuse are to: ask questions routinely to encourage disclosure of abuse and offer community resource information for women living in abuse or both.^

A presente dissertação tem por objetivo analisar os aspectos religiosos islâmicos e as implicações das relações de gênero no islam sobre a assistência de saúde às mulheres muçulmanas, e através disto, discutir a importância do conhecimento prévio do islamismo pelos profissionais de saúde para propor uma assistência de saúde congruente a estas mulheres, tendo por referência a Política Nacional de Atenção Integral à Saúde da Mulher. Esta pesquisa tem abordagem qualitativa, com o desenvolvimento de pesquisa de campo e aplicação de um roteiro de perguntas semi-estruturado, com questões relacionadas ao islamismo e à saúde das mulheres. Ao todo, foram entrevistadas dez pessoas, sendo estas: quatro mulheres revertidas ao islam, três mulheres de família muçulmana, dois sheiks e uma assistente social. As entrevistas foram realizadas no Centro de Divulgação do Islam para a América Latina e o Caribe (CDIAL) e na Assembléia Mundial da Juventude Islâmica na América Latina (WAMY).

The postpartum period can be challenging for many women as they adjust to physical and social changes. Breastfeeding may be more difficult than expected. Additionally, many women may feel that their postpartum body fails to meet an idealized image, leading to body dissatisfaction. Mindfulness-based interventions have been developed for stress reduction in a variety of health contexts, including pregnancy. The purpose of this study is to explore whether participants in a mindfulness based childbirth and parenting class (MBCP) during pregnancy found mindfulness skills beneficial to their breastfeeding experiences and postpartum body image. Women who participated in a ten week MBCP course during pregnancy were interviewed within the first year postpartum to discuss their experiences. The semi-structured interview guide included questions on how participants may have used mindfulness to approach a variety of positive and negative experiences. Findings have implications for future research on the postpartum experience and intervention design.
