OBJECTIVE: To identify the instruments used to assess polymyalgia rheumatica (PMR) in published studies. METHODS: A systematic literature review of clinical trials and longitudinal observational studies related to PMR, published from 1970 to 2014, was carried out. All outcome and assessment instruments were extracted and categorized according to core areas and domains, as defined by the OMERACT (Outcome Measures in Rheumatology) Filter 2.0. RESULTS: Thirty-five articles (3221 patients) were included: 12 randomized controlled trials (RCT); 3 nonrandomized trials; and 20 observational studies. More than 20 domains were identified, measured by 29 different instruments. The most frequently used measures were pain, morning stiffness, patient global assessment and physician global assessment, erythrocyte sedimentation rate, and C-reactive protein. The definition of outcomes varied considerably between studies. CONCLUSION: The outcome measures and instruments used in PMR are numerous and diversely defined. The establishment of a core set of validated and standardized outcome measurements is needed.

We worked toward developing a core outcome set for clinical research studies in polymyalgia rheumatica (PMR) by conducting (1) patient consultations using modified nominal group technique; (2) a systematic literature review of outcome measures in PMR; (3) a pilot observational study of patients presenting with untreated PMR, and further discussion with patient research partners; and (4) a qualitative focus group study of patients with PMR on the meaning of stiffness, using thematic analysis. (1) Consultations included 104 patients at 4 centers. Symptoms of PMR included pain, stiffness, fatigue, and sleep disturbance. Function, anxiety, and depression were also often mentioned. Participants expressed concerns about diagnostic delay, adverse effects of glucocorticoids, and fear of relapse. (2) In the systematic review, outcome measures previously used for PMR include pain visual analog scores (VAS), morning stiffness, blood markers, function, and quality of life; standardized effect sizes posttreatment were large. (3) Findings from the observational study indicated that asking about symptom severity at 7 AM, or "on waking," appeared more relevant to disease activity than asking about symptom severity "now" (which depended on the time of assessment). (4) Preliminary results were presented from the focus group qualitative study, encompassing broad themes of stiffness, pain, and the effect of PMR on patients' lives. It was concluded that further validation work is required before a core outcome set in PMR can be recommended. Nevertheless, the large standardized effect sizes suggest that pain VAS is likely to be satisfactory as a primary outcome measure for assessing response to initial therapy of PMR. Dissection of between-patient heterogeneity in the subsequent treatment course may require attention to comorbidity as a potential confounding factor.

A Febre Reumática (FR) é uma doença com significativa prevalência na população de pacientes em idade escolar. Constitui-se na principal causa de cardiopatia crônica em adultos jovens em países em desenvolvimento. A prevenção é de fácil manejo terapêutico, embora com reduzido índice de adesão. Até o momento, a melhor forma de evitarmos as seqüelas cardíacas é através da identificação e tratamento precoces e a manutenção de uma adequada profilaxia secundária. São raras as publicações referentes ao acompanhamento da profilaxia secundária após a alta hospitalar. O objetivo deste trabalho é avaliar a adesão ao acompanhamento dos pacientes com FR internados no Hospital de Clínicas de Porto Alegre, no período de janeiro de 1977 a junho de 1999. A coorte foi constituída de 112 indivíduos com FR que tinham indicação de retorno ao ambulatório após a alta. Quarenta pacientes (35,7%) foram atendidos no primeiro episódio de FR. A maioria apresentou seqüelas cardíacas (52,7%), ocorrendo registro de casos a partir de 8 anos de idade. As manifestações clínicas nos 40 pacientes atendidos no primeiro episódio de FR foram: 21 (52,5%) com cardite, 30 (75%) com artrite e 14 (35%) com coréia. Apenas 77 (68,7%) retornaram ao ambulatório após a alta e somente 13 (21% pelo método de Kaplan-Meier) mantiveram acompanhamento por no mínimo 5 anos. A idade menor ou igual a 16 anos foi fator preditivo de maior tempo de acompanhamento. Local de procedência, presença de seqüelas e renda familiar não mostraram associação significante. A interrupção do acompanhamento pela maioria dos pacientes e a verificação do pior prognóstico cardíaco nos pacientes com recidivas sugere a necessidade da adoção de um programa de orientação e busca para garantir a efetividade da profilaxia secundária.

Investigations in the field of pharmaceutical analysis and quality control of medicines require analytical procedures with good perfomance characteristics. Calibration is one of the most important steps in chemical analysis, presenting direct relation to parameters such as linearity. This work consisted in the development of a new methodology to obtain calibration curves for drug analysis: the stationary cuvette one. It was compared to the currently used methodology, and possible sources of variation between them were evaluated. The results demonstrated that the proposed technique presented similar reproducibility compared to the traditional methodology. In addition to that, some advantages were observed, such as user-friendliness, cost-effectiveness, accuracy, precision and robustness. Therefore, the stationary cuvette methodology may be considered the best choice to obtain calibration curves for drug analyis by spectrophotometry

A fibromialgia é uma síndrome reumática que atinge cerca de 2% da população brasileira, sendo 90% dos pacientes do gênero feminino. Os principais sintomas são dor crônica generalizada, depressão, desânimo e fadiga acentuada, provocando dificuldades sociais e afetivas cotidianas. Objetivo: O objetivo principal deste estudo foi apreender e interpretar sentidos e significados que mulheres com fibromialgia atribuem às práticas terapêuticas corporais realizadas no Projeto de Extensão: Tratamento Multidisciplinar para Pacientes Portadores de Fibromialgia realizado na Universidade do Estado do Rio de Janeiro (UERJ). Todas as mulheres foram diagnosticadas em ambulatórios públicos e privados e depois se inscreveram voluntariamente no tratamento gratuito disponibilizado. Nossa principal hipótese de estudo procura relacionar o adoecimento dessas mulheres com o regime social de trabalho, que aumenta o sofrimento e, por conseguinte, provoca somatização do mal-estar gerado. Métodos: Trata-se de um estudo socioantropológico com campo etnográfico, no qual foram observadas as práticas corporais semanais nos anos 2009-2010. Também foram realizadas fotografias e entrevistas em profundidade com todas as mulheres que participavam regularmente das práticas corporais. De igual modo, realizamos entrevistas por computador e por telefone com mulheres diagnosticadas por fibromialgia em diversas cidades brasileiras. Resultados: Foi possível compreender que estar em um local onde podem se relacionar com outras pessoas com os mesmos sinais e sintomas e que compartilhar situações de sofrimento semelhantes contribui para construção de uma identidade de grupo baseada no cuidado e no acolhimento. Assim, elas constroem coletivamente valores de cuidado com o corpo e com a saúde. Conclusões: A participação assídua no tratamento oferecido aumenta a qualidade de vida e a vitalidade de mulheres com fibromialgia, contribuindo para a promoção da saúde não apenas na sua dimensão físico-orgânica, mas em sua totalidade sócio-afetiva.
