Objectives: To investigate the presence of mutations in the pncA gene in 31 pyrazinamide-resistant Mycobacterium tuberculosis and 5 susceptible strains. MICs and pyrazinamidase (PZase) activity were also determined.Methods: All 36 M. tuberculosis clinical isolates were genotyped by mycobacterial interspersed repetitive units (MIRUs) and most were also typed by spoligotyping. The MIC value necessary to inhibit 99% of the resistant mycobacterial isolates was determined by microplate Alamar Blue assay (MABA) and by Lowenstein-Jensen assay (LJA). The PZase activity was measured by pyrazinamide deamination to pyrazinoic acid and ammonia, and the entire pncA sequence including the 410 by upstream from the start codon was determined by DNA sequencing of purified PCR products.Results: of the 31 isolates resistant to pyrazinamide, 26 (83.9%) showed at least one mutation in the pncA gene or in its putative regulatory region: Among the 22 different mutations detected in the pncA gene and in its regulatory region, 9 (40.9%) mutations (consisting of six substitutions, two insertions and one deletion) have not been described in previous studies. Three pyrazinamide-resistant isolates, confirmed by MIC varying from 800 to 1600 mg/L, carried the wild-type pncA sequence and retained PZase activity.Conclusions: These results contribute to the knowledge of the molecular mechanism of pyrazinamide resistance in Brazil and also expand the profile of pncA mutations worldwide. The MABA was successfully used to determine the MICs of pyrazinamide.

INTRODUCTION: The aim of this work was to evaluate the prevalence of Mycobacterium tuberculosis (MT) strains with mutations that could result in resistance to the main drugs used in treatment in a region with one of the highest numbers of tuberculosis (TB) cases in southern Brazil. METHODS: Deoxyribonucleic acid (DNA) from 120 sputum samples from different patients suspicious of pulmonary tuberculosis who attended the Municipal Public Laboratory for Mycobacterium sp. diagnosis was directly amplified and analyzed by PCR-SSCP. The DNA was amplified in known hotspot mutation regions of the genes rpoB, ahpC, embB, katG, inhA, and pncA. RESULTS: The percentage of samples positive by culture was 9.2% (11/120); 5% (6/120) were positive by bacilloscopy and MT-PCR, and DNA fragments of the aforementioned resistance genes could be amplified from seven (7) of the eleven (11) samples with positive results, either by culture or PCR/bacilloscopy. All presented a SSCP pattern similar to a native, nonresistant genotype, with the ATCC strain 25177 as control, except for one sample (0.01%), which presented a SSCP profile demonstrating mutation at the embB gene. CONCLUSIONS: These results are consistent with the empirical observations by physicians treating TB patients in our region of a low occurrence of cases that are refractory to conventional treatment schemes, in contrast to other parts of the country. Continued surveillance, especially molecular, is essential to detect and monitor the outbreak of MT-resistant strains.

In this cross-sectional study, mycobacteria specimens from 189 tuberculosis (TB) patients living in an urban area in Brazil were characterised from 2008-2010 using phenotypic and molecular speciation methods (pncA gene and oxyR pseudogene analysis). Of these samples, 174 isolates simultaneously grew on Löwenstein-Jensen (LJ) and Stonebrink (SB)-containing media and presented phenotypic and molecular profiles of Mycobacterium tuberculosis, whereas 12 had molecular profiles of M. tuberculosis based on the DNA analysis of formalin-fixed paraffin wax-embedded tissue samples (paraffin blocks). One patient produced two sputum isolates, the first of which simultaneously grew on LJ and SB media and presented phenotypic and molecular profiles of M. tuberculosis, and the second of which only grew on SB media and presented phenotypic profiles of Mycobacterium bovis. One patient provided a bronchial lavage isolate, which simultaneously grew on LJ and SB media and presented phenotypic and molecular profiles of M. tuberculosis, but had molecular profiles of M. bovis from paraffin block DNA analysis, and one sample had molecular profiles of M. tuberculosis and M. bovis identified from two distinct paraffin blocks. Moreover, we found a low prevalence (1.6%) of M. bovis among these isolates, which suggests that local health service procedures likely underestimate its real frequency and that it deserves more attention from public health officials.

O presente trabalho consiste no estudo do Plano Nacional de Colheita de Amostras (PNCA), realizado pela ASAE e, a apresentação de contributos/sugestões para a sua melhoria. Os principais objectivos do plano consistem em assegurar e verificar se os géneros alimentícios cumprem a legislação comunitária e nacional, e não colocam em risco a segurança alimentar e a saúde humana e, assegurar a autenticidade das informações constantes dos rótulos dos géneros alimentícios existentes no mercado, de forma a garantir que os consumidores não sejam induzidos em erro. Assim sendo, analisaram-se os resultados de amostras de géneros alimentícios colhidas no âmbito do PNCA 2009. Os resultados evidenciaram que a maioria das não conformidades corresponderam a irregularidades de rotulagem (45%), não representando um perigo para a saúde dos consumidores. Os incumprimentos detectados passíveis de constituir risco imediato para a saúde do consumidor, correspondem a situações em que se detectaram microrganismos potencialmente patogénicos (20%), em alimentos que estão prontos a ser consumidos, tais como produtos à base de carne e queijo. O estudo realizado indicia que, de uma forma geral, os géneros alimentícios colocados à venda no mercado nacional, recolhidos no âmbito do Plano Nacional de Colheita de Amostras, constituem alimentos seguros.

Large-scale vaccination with BCG, the live attenuated strain of Mycobacterium bovis, is being adopted around the world, although sporadic complications have occurred after the procedure. Lymphadenopathy is not uncommon especially in babies under one year (0.73% of vaccinated infants), but the swelling subsides within 2 months in most cases, with no medical or surgical treatment. Brazil adopted BCG vaccination program earlier in the seventies and by 1995 more than 96% of the infant population received this immunization. We report here the occurrence of lymphadenopathy in a two-year-old child vaccinated with the Brazilian BCG strain. The diagnosis was made using a lymph node biopsy and intestinal aspirates that yielded a positive mycobacterial culture. The isolate was resistant to isoniazid, rifampicin, pyrazinamide and thiophen-2-carbonic acid hydrazide, sensitive to streptomycin, ethambutol, and p-nitrobenzoic acid, and reacted positively to cyclo-serine and negatively to niacin. The pncA gene involved in bacterial activation of pyrazinamide contains in M. bovis a point mutation that renders pyrazinamidase unable to catalyze drug activation. Therefore, this polymorphism is a good option for developing methods to differentiate M. bovis and M. tuberculosis. Taking advantage of this difference we further analyzed the isolates by single-stranded conformation polymorphism electrophoresis of DNA following PCR of the pncA gene. The isolate identity was confirmed by RFLP electrophoretic analysis of the amplified fragment following Eco065I digestion, which selectively cleaves M. tuberculosis DNA. From this result it is proposed that RFLP of pncA gene represents an alternative for differential diagnosis of M. bovis.

Ziel der Studie war die Untersuchung inwieweit PCNA als immunhistochemisches Hilfsmittel eine Verbesserung der Abgrenzung von uterinen LM gegenüber uterinen LMS und eine Hilfe bei der Prognoseaussage bei uterinen und gastrointestinalen LMS darstellt. Dabei wurden auch weitere, herkömmliche, in einer Reihe von Studien untersuchte Prognoseparameter mit in Betracht gezogen.rnDie Auswertung der Färbungen erfolgte quantitativ mittels einer PC-gesteuerten Bildverarbeitung, die neben der positiven Zellfläche und Zellzahl auch die Kernfläche berechnet. Vorteil der quantitativen Messung ist die Möglichkeit der Reproduzierbarkeit unabhängig vom Benutzer, im Gegensatz zu semiquantitativen Analysen, die häufiger angewendet werden.rnEs fällt dabei auf, dass sowohl die Zellkerngröße generell als auch die Zellkerngröße der positiv gefärbten Zellen signifikant größer bei LMS als bei LM ist. (p< 0,0001). Zwischen den Zellkerngrößen der uterinen und der gastrointestinalen LMS fanden sich keine signifikanten Unterschiede.rnBezüglich der Abgrenzung zwischen uterinen LM und LMS zeigten sich signifikant höhere PCNA-Meßwerte für LMS (mittlere positive Zellkernzahl 15,08%, mittlere positive Zellkernzahl 16,67%) gegenüber LM (mittlere positive Zellkernzahl 1,95%, mittlere positive Zellkernzahl 2,02%). Zu den gastrointestinalen LMS-PCNA Messwerten gab es keinen signifikanten Unterschied (mittlere positive Zellkernfläche 19,07%, mittlere positive Zellkernzahl 16,43).rnKein signifikanter Zusammenhang zeigte sich bei der Höhe der PCNA-Meßwerte und der Prognose sowohl der uterinen als auch der gastrointestinalen LMS.rnIn dieser Studie stellten sich als signifikant prognoserelevant lediglich das Alter sowohl bei den uterinen und gastrointestinalen LMS heraus. Bei den uterinen LMS war auch noch ein schlechteres Grading und das Vorhandensein von Nekrosen signifikant mit der Prognose korreliert.rnNicht signifikant prognoserelevant waren sowohl bei den uterinen wie den gastrointestinalen LMS die Tumorgröße und die PCNA-Meßwerthöhe. Bei den gastrointestinalen Leiomyosarkomen zusätzlich noch das Vorhandensein von Nekrosen.rnEin signifikanter Zusammenhang wurde zwischen PCNA-Meßwerten und der Uterusgröße bzw. auch der Myomgröße bei den uterinen Leiomyomen sowie analog der Tumorgröße der uterinen und der gastrointestinalen LMS festgestellt. Innerhalb der Gruppe der uterinen Myome zeigten sich für zellreiche Myome zwar höhere Messwerte, die jedoch nicht signifikant waren.rnKein Zusammenhang zeigte sich dagegen für die Höhe der PNCA-Meßwerte und dem Grading der uterinen sowohl als auch der gastrointestinalen Leiomyosarkome, dem Alter der Patientinnen bei den uterinen LMS, dem Vorhandensein von Nekrosen bei den uterinen sowohl als auch bei gastrointestinalen LMS und dem Geschlecht bei den gastrointestinalen LMS.rnDie Ergebnisse stehen bezüglich der Aussage der Prognose teilweise in Widerspruch, teilweise in Einklang mit den bisherigen Veröffentlichungen. Ein großes Hemmnis ist die geringe Fallzahl, die vor allem durch die Seltenheit der Tumore bedingt ist, desweiteren die ungenügende Nachverfolgung der Patienten, die auch daraus resultiert, dass es sich um eine retrospektive Studie handelt. Hier bedarf es größerer Studien, möglicherweise als multizentrische Studien mit genauer Verlaufsbeobachtung, um auch im deutschsprachigen Raum eine weitere Erforschung dieser seltenen, umsomehr sehr interessanten und an den untersuchenden Pathologen besondere Ansprüche stellende Tumore zu geben.rn

The spirochete Borrelia burgdorferi (Bb) is the causative agent of Lyme disease. During infection, a strong immune response is elicited towards Bb by its host; however, the organism is able to persist and to disseminate to many different tissues. The vls locus is located on the linear plasmid lp28-1, a plasmid shown to be important for virulence in the mouse model. During infection, vlsE undergoes antigenic variation through a series of gene conversions, which results in the insertion of sequences from the silent, unexpressed cassettes into the vlsE cassette. We hypothesize that this antigenic variation is important in the spirochete's ability to persist within mammals by allowing it to evade the immune system. To define the role of vls in immune evasion, the immune response against VlsE was determined by using a recombinant form of VlsE (VlsE1-His) as an antigen to screen patient sera. Lyme patients produce antibodies that recognize VlsE, and these antibodies are present throughout the course of disease. Immunization with the VlsE1-His protein provided protection against infection with Bb expressing the same variant of VlsE (VlsE1), but was only partially protective when mice were infected with organisms expressing VlsE variants; however, subsequent VlsE immunization studies yielded inconsistent protection. Successful immunizations produced different antibody reactivities to VlsE epitopes than non-protective immunizations, but the reason for this variable response is unclear. In the process of developing genetic approaches to transform infectious Bb, it was determined that the transformation barrier posed by plasmids lp25 and lp56 could be circumvented by replacing the required lp25 gene pncA. To characterize the role of vlsE in infectivity, Bb lacking lp28-1 were complemented with a shuttle plasmid containing the lp25 encoded virulence determinant pncA and vlsE. Complemented spirochetes express VlsE, but the gene does not undergo antigenic variation and infectivity in the mouse model was not restored, indicating that either antigenic variation of vlsE is necessary for survival in the mouse model or that other genes on lp28-1 are important for virulence. ^

Borrelia burgdorferi, a spirochete and the causative agent of Lyme disease, infects both mammals and ticks. Its genome, sequenced in 1997, consists of one linear chromosome and over 20 linear and circular plasmids. Continuous passage of organisms in culture causes them to lose certain plasmids and also results in loss of infectivity in mammals. In this work, 19 B. burgdorferi clonal isolates were examined for infectivity in mice and for plasmid content utilizing polymerase chain reaction (PCR). Two plasmids, a 28 kilobase (kb) linear plasmid (Ip28-1) and a 25 kb linear plasmid (Ip25) were found to be required for full infectivity. Previous studies had demonstrated that Ip28-1 contains the vls locus, which is involved in antigenic variation and immune evasion. Gene BBE22 on Ip25 is predicted to encode the nicotinamidase PncA, an enzyme that converts nicotinamide to nicotinic acid as part of a pathway for NAD synthesis. To examine the potential role of BBE22 in infectivity, a shuttle vector containing BBE22 (pBBE22) was constructed and used to transform B. burgdorferi clone 5A13, which contains all plasmids except lp25. Transformation with pBBE22 restored infectivity of clone 5A13 in mice, whereas 5A13 transformed with the shuttle vector alone was not infectious. To determine whether BBE22 acts as a nicotinamidase in vivo, a Salmonella typhimurium pncA− nadB− transposon mutant was transformed with pBBE22 or with pQE30:BBE22, which contained BBE22 in an E. coli expression vector. Both constructs complemented the Salmonella mutant, permitting growth in minimal media plus nicotinamide. Salmonella cells over-expressing BBE22 also exhibited nicotinamidase activity, as determined by ammonia production in the presence of nicotinamide. Site-directed mutagenesis of BBE22 at the predicted active site (resulting in a Cys120Ala substitution) abrogated the ability to restore infectivity to B. burgdorferi 5A13 and to complement the pncA mutation in S. typhimurium. These studies indicate that BBE22 is a nicotinamidase required for NAD synthesis and survival of B. burgdorferi in mammals. This is also the first demonstration of ‘molecular Koch's postulates’ in B. burgdorferi, i.e. that a specific gene is essential for infectivity of the Lyme disease spirochete. ^
