Con il seguente lavoro di tesi si propone un excursus dell’evoluzione della tecnica che ha permesso l’amplificazione del DNA in laboratorio, spiegandone i principi elementari di base. La scoperta che il DNA è il depositario dell’informazione genica ha aperto la strada a una nuova disciplina: la biologia molecolare, dove molte delle tecniche utilizzate limitano le funzioni naturali degli acidi nucleici. Dalla sua introduzione, la tecnologia PCR ha modificato il modo nel quale l’analisi del DNA viene condotta nei laboratori di ricerca e di diagnostica. Con lo scopo di rilevare direttamente sequenze genomiche specifiche in un campione biologico nasce l’esigenza di avere a disposizione metodologie con un’elevata soglia di sensibilità e specificità. Il primo capitolo di questo elaborato introduce la PCR nella sua prima formulazione. A partire da quantità estremamente ridotte di DNA, questa tecnica di amplificazione in vitro, consente di ottenere rapidamente milioni di molecole identiche della sequenza bersaglio di acido nucleico. A seguire, nel secondo capitolo, verrà trattata un’implementazione della PCR: la real-time PCR. La RT-PCR, introduce nuove opportunità poiché rende possibile la misurazione diretta e la quantificazione della reazione mentre è in atto. Con l’utilizzo di molecole fluorescenti che vengono incorporate nel prodotto in formazione o che si intercalano alla doppia elica, si può monitorare la formazione del prodotto di amplificazione in tempo reale seguendone l’assorbimento con un sistema spettrofotometrico, in un sistema automatizzato che provvede anche alle routine della PCR. Nel terzo e ultimo capitolo si analizza una nuova tecnologia recentemente commercializzata: la PCR in formato digitale. Verranno prese in esame essenzialmente due metodologie, la dPCR su chip e la ddPCR (Droplet Digital Polymerase Chain Reaction).

Campylobacter est l’agent pathogène zoonotique responsable de la majorité des gastro-entérites d’origine bactérienne chez l’homme. Les produits de volaille représentent la principale source d’infection; toutefois, l’exposition peut également découler de contacts directs avec les animaux ou avec l’eau. Une forte variation saisonnière est présente dans les cas rapportés, qui n’est toujours pas élucidée : les eaux environnementales, sources d’infection connues, sont soupçonnées. Cette étude transversale a été réalisée dans la région Sud-Est du Québec (Canada) où Campylobacter fut quantifié et génotypé à partir de différentes sources d’eau (eaux de captage, récréatives et usées) et de cas cliniques afin d’évaluer les risques potentiels posé par l’eau environnementale. Différents essais PCR en temps réel furent appliqués à l’eau environnementale et comparés: 2 ont été sélectionnés pour leur spécificité et sensibilité de quantification. Les courbes standards ont été calibrées en utilisant la PCR digitale pour déterminer précisément les concentrations. Les isolats environnementaux et cliniques furent comparés génétiquement en utilisant le CGF (« comparative genomic fingerprinting »). Les eaux usées étaient plus contaminées que les eaux de captage et récréatives (3.9Log, 1.7Log et 1.0Log cellules/L en moyenne, respectivement). Six pour cent des isolats d’eaux environnementales étaient génétiquement similaires (100 % homologie) aux isolats cliniques. Les cas cliniques de campylobactériose d’été montraient des isolats avec davantage de similarités génétiques avec les isolats retrouvés dans l’eau environnementale comparativement aux autres saisons (p<0.01). Les faibles concentrations et similarités génétiques entre les isolats d’eau et cliniques suggèrent un risque de transmission possible, mais faible.

Hand, foot and mouth disease (HFMD) is a contagious viral disease that frequently affects infants and children and present with blisters and flu-like symptoms. This disease is caused by a group of enteroviruses such as enterovirus 71 (EV71) and coxsackievirus A16 (CA16). However, unlike other HFMD causing enteroviruses, EV71 have also been shown to be associated with more severe clinical manifestation such as aseptic meningitis, brainstem and cerebellar encephalitis which may lead to cardiopulmonary failure and death. Clinically, HFMD caused by EV71 is indistinguishable from other HFMD causing enteroviruses such as CA16. Molecular diagnosis methods such as the use of real-time PCR has been used commonly for the identification of EV71. In this study, two platforms namely the real-time PCR and the droplet digital PCR were compared for the detection quantitation of known EV71 viral copy number. The results reveal accurate and consistent results between the two platforms. In summary, the droplet digital PCR was demonstrated to be a promising technology for the identification and quantitation of EV71 viral copy number.

Schistosomiasis is a chronically debilitating helminth infection with a significant socio-economic and public health impact. Accurate diagnostics play a pivotal role in achieving current schistosomiasis control and elimination goals. However, many of the current diagnostic procedures, which rely on detection of schistosome eggs, have major limitations including lack of accuracy and the inability to detect pre-patent infections. DNA-based detection methods provide a viable alternative to the current tests commonly used for schistosomiasis diagnosis. Here we describe the optimisation of a novel droplet digital PCR (ddPCR) duplex assay for the diagnosis of Schistosoma japonicum infection which provides improved detection sensitivity and specificity. The assay involves the amplification of two specific and abundant target gene sequences in S. japonicum; a retrotransposon (SjR2) and a portion of a mitochondrial gene (nad1). The assay detected target sequences in different sources of schistosome DNA isolated from adult worms, schistosomules and eggs, and exhibits a high level of specificity, thereby representing an ideal tool for the detection of low levels of parasite DNA in different clinical samples including parasite cell free DNA in the host circulation and other bodily fluids. Moreover, being quantitative, the assay can be used to determine parasite infection intensity and, could provide an important tool for the detection of low intensity infections in low prevalence schistosomiasis-endemic areas.

Droplet digital PCR (ddPCR) can be used to detect low frequency mutations in oncogene-driven lung cancer. The range of KRAS point mutations observed in NSCLC necessitates a multiplex approach to efficient mutation detection in circulating DNA. Here we report the design and optimisation of three discriminatory ddPCR multiplex assays investigating nine different KRAS mutations using PrimePCR™ ddPCR™ Mutation Assays and the Bio-Rad QX100 system. Together these mutations account for 95% of the nucleotide changes found in KRAS in human cancer. Multiplex reactions were optimised on genomic DNA extracted from KRAS mutant cell lines and tested on DNA extracted from fixed tumour tissue from a cohort of lung cancer patients without prior knowledge of the specific KRAS genotype. The multiplex ddPCR assays had a limit of detection of better than 1 mutant KRAS molecule in 2,000 wild-type KRAS molecules, which compared favourably with a limit of detection of 1 in 50 for next generation sequencing and 1 in 10 for Sanger sequencing. Multiplex ddPCR assays thus provide a highly efficient methodology to identify KRAS mutations in lung adenocarcinoma.

Segundo O`Kane et al, em 2008, foi demonstrado que 88,9% dos erros laboratoriais são realizados na fase pré-analítica. No laboratório de análises clínicas, o sistema de controle da qualidade pode ser definido como toda a ação sistemática necessária para dar confiança e segurança em todos os exames e prevenir a ocorrência de erros. O tempo de armazenamento pode variar de dias a meses ou mesmo anos, influenciando na definição da temperatura de estocagem. O armazenamento de longo prazo pode resultar na criopreservação inadequada para determinados analitos e pode desnaturar as lipoproteínas. Objetivos: Este trabalho teve como objetivo avaliar a fase pré-analítica, controle da qualidade interno e também avaliar o efeito do congelamento (- 80C) quanto ao tempo de armazenamento do soro e plasma de sangue colhido com ácido etilenodiamino tetra-acético (EDTA). As dosagens foram realizadas no Laboratório de Lípides LabLip, e estocadas em - 80 C por três anos, as mesmas foram redosadas no Serviço de Patologia Clínica da Policlínica Piquet Carneiro da UERJ com metodologias iguais e realizada a comparabilidade dos resultados. Foram analisados o perfil lipídico (CT, HDLc e TG) e PCR-US de 103 amostras, 73 no soro e 30 no plasma em dois laboratórios altamente qualificados. Discussão: Após redosagem foram encontrados nas dosagens de HDLc e CT resultados diminuídos respectivamente (correlação coeficiente no soro 0,48 e 0,62) teste t pareado no soro (CT p 0,0012 e HDLc p 0,0001). Conclusões: Os dados obtidos nas avaliações dos resultados de diferentes laboratórios e tempo de estocagem revelaram que as amostras quando armazenadas por um longo período após redosagem no soro, apresentaram diferenças em certos analitos, tais como CT e HDLc, no qual obteve-se resultados significativamente diminuídos, diferentemente no plasma, que após três anos de estocagem a -80C foram redosados e aplicados no teste t pareado, os analitos CT e PCR-US mantiveram a estabilidade.

Specimens taken postmortem from typical lesions of digital dermatitis in two dairy cows were tested by the polymerase chain reaction (PCR) for the presence of a spirochaetal 16S rRNA gene. Seven different assays detected the gene in the samples from both cows. Two of the PCR products were sequenced and a comparison of the nucleotide sequences revealed that the spirochaete belonged to the genus Treponema and was closely related to Treponema denticola. A PCR specific for the detection of the digital dermatitis-associated treponeme was developed.

A study was carried out to determine whether spirochaetes are frequently associated with digital dermatitis in United Kingdom (UK) dairy cattle. Histopathological examination of lesions using a silver stain showed a large number of unidentified spirochaete-like organisms present in digital dermatitis hoof skin tissue in all examined biopsies. Immunocytochemical staining demonstrated that spirochaetes in skin lesions were identified by polyclonal antisera to Borrelia burgdorferi, Treponema denticola and Treponema vincentii (again all biopsies were positively stained), whereas monoclonal antibodies to B. burgdorferi and any Treponema pallidum did not stain any organisms in all biopsies. A PCR of 16S rRNA, previously shown to be specific for a new treponeme, was employed and produced positive results from 82.4% of digital dermatitis tissues. It is concluded that this spirochaete (or related spirochaetes), which is similar to human oral treponemes, is frequently associated with, and may be responsible for, pathological changes in digital dermatitis. The isolation of spirochetes from severe ovine foot disease has been reported recently by our research group. In this study we describe the preliminary classification of this spirochete based on nucleotide sequence analysis of the PCR-amplified 16S rRNA gene. Phylogenetic analysis of this sequence in comparison with other previously reported 16S rRNA gene sequences showed that the spirochete belonged to the treponemal phylotype Treponema vincentii which has been associated with bovine digital dermatitis and human periodontal disease. Further work is required to define the common virulence determinants of these closely related treponemes in the aetiology of these tissue destructive diseases. (C) 2000 Elsevier Science B.V. Contagious ovine digital dermatitis (CODD) is a recently recorded, apparently new infection of the ovine hoof, which differs clinically from footrot caused by Dichelobacter nodosus and which fails to respond well to accepted treatment practices for footrot. Despite the welfare implications of such an infection, very little research has been performed on CODD to date and the aetiology remains confused. Suggestions have been made that there is a potential role for treponemes in the pathogenesis of CODD but that D. nodosus is apparently not involved. Six farms were therefore targeted in this study to provide a more in-depth investigation into the bacterial flora of CODD lesions. Dark ground microscopy, culture and PCR techniques were used, concentrating on the presence of D. nodosus and spirochaetes, particularly those of the genus Treponema. The results demonstrated that isolates of D. nodosus were indeed present in a high percentage (74%) of CODD lesions compared with 31% of apparently healthy feet. The isolates were shown to be of similar virulence type to those reported previously in cases of footrot, and the range of serogroups was also found to be similar to footrot, with serogroup H being prevalent. Treponemes were present in 70% of CODD lesions and 38% of apparently healthy feet, supporting a possible association between CODD and treponemes. However, any further progress on the aetiology of CODD and the potential for novel, effective treatment will depend on an improved ability to culture these organisms routinely in the laboratory thereby enabling their complete characterisation.
