Purpose Cadaveric study at our institution has demonstrated that optimal basaplate fixation of a reversed shoulder arthroplasty (RSA) could be achieved with screws in three major columns. Our aim was to review our early rate of aseptic glenoid loosening in a series of baseplate fixed according to this principle. Material and Methods Between 2005 and 2008, 48 RSA (Aequalis Reversed) were implanted in 48 patients with an average age of 74.4 years (range, 56 to 86 years). There were 37 women and 11 men. Twenty-seven primary RSAs were performed for cuff tear arthropathy, 3 after failed rotator cuff surgery, 6 for failed arthroplasties, 7 for acute fractures and 5 after failed ORIF. All baseplate fixation were done using a nonlocking posterior screw in the spine, a nonlocking anterior screw in the glenoid body, a locking superior screw in the coracoid and a locking inferior screw in the pillar. All patients were reviewed with standardized radiographs. The number of screws were reported. We measured the position of the screws in relation to the scapular spine and the coracoid process in two different views. We defined screw positions as totally, partially or out of the target. Finally we reported glenoid aseptic loosening which was defined as implant subsidence. Results Four patients were lost to follow-up. Thus, 44 shoulders could be reviewed after a mean follow-up of 13 months (range, 6 to 32 months). All baseplates were fixed with 4 screws. Thirty-seven (84%) screws were either partially or totally in the spine. Thus, 7 (16%) scapular spine screws were out of the target. No coracoid screw was out the target. Two (4.5%) patients had glenoid loosening. Both had a scapular spine and a coracoid screw partially in the bone. Conclusion Early aseptic glenoid loosening occurred before the two years follow-up and is most of time related to technical problems and/or insufficient bone stock and bone quality. Our study demonstrate that baseplate fixation according to the three columns principle is a reproducible technique and a valuable way to prevent early glenoid loosening.

Ce travail consiste en l'analyse et la revue des différentes prises en charge de la pseudarthrose de la diaphyse fémorale dans un groupe nommé « Echantillon CHUV » formé par 16 patients. Les patients ont tous été opérés au Centre Hospitalier Universitaire Vaudois, CHUV, entre 2008 et 2011 pour cure de pseudarthrose. Une présentation succincte des fractures de la diaphyse fémorale ainsi que de la pseudarthrose de la diaphyse fémorale consiste en la première partie du rapport. La deuxième partie est une revue des cas des patients de l'échantillon CHUV. Des paramètres tels que le mode initial de fracture, la durée effective de la pseudarthrose, le nombre de révisions avant la consolidation ou la répartition des pseudarthroses selon leur type font partie des différents éléments caractéristiques répertoriés puis analysés dans cette revue. 25 cures de pseudarthroses sont effectuées sur les 16 patients. Les techniques de fixations les plus utilisées sont la fixation par plaque après réduction ouverte (ORIF, 64%, 16 cures sur 25) et l'enclouage centromédullaire (24%, 6 cures sur 25). L'utilisation de substituts osseux est très souvent complémentaire à la refixation mécanique lors des cures de pseudarthrose (recours à la greffe osseuse dans 72% des cures). Le taux d'union après la première cure de pseudarthrose s'établit à 63% et il monte à 100% après l'ensemble des cures. Le retour à une fonction adéquate du membre inférieur est obtenu chez 14 des 16 patients (1 patient est traité par arthrodèse du genou sur pseudarthrose du tiers distal de la diaphyse fémorale et 1 patient tétraplégique est traité par résection de la tête et du col fémoral sur pseudarthrose du tiers proximal de la diaphyse fémorale). La revue quantifie la durée de l'invalidité causée par la pseudarthrose de la diaphyse fémorale : la longueur totale du traitement avant union est d'au minimum 12 mois pour 75% des patients. 8 patients sur 16 ont un temps avant union supérieur à 20 mois. La pseudarthrose de la diaphyse fémorale doit être considérée comme une complication grave. La localisation au niveau du fémur réduit voire abolit l'autonomie de marche du patient et limite ses activités de la vie quotidienne. Elle entrave le retour au travail et réduit terriblement la qualité de vie pendant souvent plus d'une année. L'ensemble des symptômes ont un effet dévastateur sur la rééducation et peuvent parfois laisser d'importantes séquelles physiques ou psychologiques sur le long terme. Pour ces raisons, le traitement doit être adapté du mieux possible à la pseudarthrose du patient afin d'augmenter les chances de réussite de la cure.

OBJECTIVES: The elbow joint is vulnerable to stiffness, especially after trauma. The aim of this study was to evaluate the results of open arthrolysis for posttraumatic elbow stiffness. DESIGN: Cohort retrospective study. PATIENTS: Eighteen consecutive patients were evaluated by an independent observer at an average of 16 months (6 to 43) after open elbow arthrolysis was performed for posttraumatic stiffness. Initial traumas were: isolated fractures (11) or dislocation (1) and complex fracture-dislocations (6). Initial treatments were: nonoperative (3), radial head resection (1), and ORIF (14). Patients presented predominantly with mixed contractures (combined extrinsic and intrinsic contractures). INTERVENTION: Open elbow arthrolysis. MAIN OUTCOME MEASUREMENTS: Elbow function and patient satisfaction were the principal outcome measures. At follow-up European Society for Shoulder and Elbow Surgery (SECEC) elbow scores were calculated. RESULTS AND CONCLUSIONS: Three patients had minor postoperative complications: 1 partial wound dehiscence, 1 subcutaneous infection, and one seroma. None of these complications influenced the final result clinically. The mean total increase in range of motion was 40 degrees (13 to 112 degrees), with a mean gain in flexion of 14 degrees (0 to 45 degrees) and 26 degrees in extension (5 to 67 degrees). No patient showed signs of elbow instability. There was no radiographic evidence of osteoarthritis progression at follow-up. We did not find any correlations between the type of stiffness, the approaches used, and the results. However, patients with the greatest preoperative stiffness had significantly better improvement of mobility (P<0.001). The best results were obtained in patients who had arthrolysis done within 1 year after the initial trauma (P=0.008). The mean SECEC scores were 88 (52 to 100) for the injured elbows, and 96 (88 to 100) for the contralateral elbows. CONCLUSION: Open elbow arthrolysis for patients with posttraumatic stiffness improves joint function and provides patient satisfaction. The best results, in terms of gain of motion and patient satisfaction, were obtained in patients with severe stiffness who had operations within the first year after initial trauma.

O presente trabalho apresenta uma modelagem detalhada de processos de combustao turbulentos para um jato concentrico de combustıvel e ar. A modelagem é fundamentada nas equacões de conservacão de massa, de quantidade de movimento, de energia e de espécies quımicas. A turbulencia é resolvida pela utilizacão do modelo k- padrão. Dois modelos de reacões quımicas são apresentados. O modelo SCRS – Simple Chemically-Reacting Systems, que assume taxas instantâneas de reacões quımicas. Também é abordado o modelo E-A – Eddy Breakup - Arrhenius, que assume taxas finitas de reacões quımicas. A radiacão térmica, fenômeno de grande importância devido as altas temperaturas alcancadas em processos de combustão,é modelada através do Método das Zonas. O modelo da soma ponderada de gases cinzas – WSGGM,é usado para determinar o espectro de emissão e absorcão dos gases no processo. Para a solucão destas equacões diferenciais, juntamente com os modelos de turbulência, de reaçõoes químicas e radiação térmica, faz-se o uso do Método dos Volumes Finitos. Para validar a modelagem apresentada resolve-se o processo de combustão em uma câmara cilíndrica. A câmara de combustão usada áa mesma abordada no First Workshop on Aerodynamics of Steady State Combustion Chambers and Furnaces, organizado pela ERCORTAC - European Research Community On Flow Turbulence And Combustion, em outubro de 1994, que apresenta dados experimentais de temperatura e concentração das espécies químicas para várias posições de interesse no interior da câmara. Utiliza-se o gás natural como combustível e o ar atmosférico como oxidante. O processo de combustão sem pré-mistura é resolvido para a condição de excesso de combustível de 5 % para ambos os modelos, onde o gás natural é injetado por um duto circular central, e o ar atmosférico por um orifício anular externo a esse duto, no mesmo plano Uma reação química não estagiada é assumida para o modelo SCRS. Para o modelo E-A duas situações são resolvidas: combustão não estagiada, com uma etapa global de reação química; e reação quımica estagiada, com duas etapas globais. Os resultados obtidos com o modelo SCRS para a distribuição de temperaturas, em termos de tendências gerais, são razoáveis. Já as concentrações de espécies químicas não apresentam dados satisfatórios para este modelo. Para o modelo E-A os resultados apresentam boa concordância com os dados experimentais, principalmente para a situação em que o processo de combustão é assumido em duas etapas globais. ´E analisado em detalhe o papel desempenhado pela transferencia de calor por radiacao, com meio participante. Para melhor verificar as trocas de calor, assume-se uma camara de combustao cilındrica com paredes d’agua. A injecao do combustıvel e do oxidante e feita atraves de um queimador central, semelhante ao usado para validar a modelagem, porem com dois orifıcios concentricos para injecao de combustıvel. Nesta situação o efeito do turbilhonamento (swril), assumido como 20 % da velocidade axial de entrada, sobre a injecao de ar e computado atraves da condicao contorno da equacao de conservacao da quantidade de movimento angular. Nesta fase apenas o modelo E-A, com duas etapas globais de reacoes quımicas, e considerado, ja que o mesmo apresenta os melhores resultados. O processo de combustao e simulado com e sem a presenca da radiacao termica. Verifica-se que a presenca da radiacao termica homogeneiza a temperatura dos gases no interior da camara. Com isso verifica-se tambem alterações nas taxas de reacoes quımicas, modificando a magnitude das fracoes das especies quımicas Quando a radiacao termica e considerada efeitos de extinção local da chama sao verificados nas regioes de temperaturas mais altas, diminuindo o consumo de oxigenio e aumentando a producao de monoxido de carbono, caracterizando assim uma combustao incompleta. Em algumas situacoes tem-se uma variacao de temperatura de ate 500 K, a montante da chama. A radiacao termica tambem aumenta a taxa de transferencia de calor dos gases quentes para as paredes da camara, e desta para o seu exterior. Com os resultados obtidos a partir desta modelagem e possıvel determinar o perfil da zona de combustao, a distribuicao de concentracoes de especies quımicas, o campo de velocidades e as taxas de transferencia de calor para as paredes da camara de combustao, total, por conveccao superficial e por radiacao. Estes resultados sao de extrema importancia para prever a performance de camaras de combustao, assim como auxiliar na sua otimizacao.

Background: This is a position paper from the 2nd International Bone Research Association (IBRA) Symposium for Condylar Fracture Osteosynthesis 2012 was held at Marseille, succeeding the first congress in Strasbourg, France, in 2007. The goal of this IBRA symposium and this paper was to evaluate current trends and potential changes of treatment strategies for mandibular condylar fractures, which remain controversial over the past decades.Methods: Using a cross-sectional study design, we enrolled the consensus based on the panel of experts and participants in the IBRA Symposium 2012. The outcomes of interest were the panel and electronic votes on management of condylar base, neck and head fractures, and panel votes on endoscopic and paediatric condylar fractures. Appropriate descriptive and univariate statistics were used.Results: The consensus derived from 14 experts and 41 participant surgeons, using 12 case scenarios and 27 statements. The experts and participants had similar decision on the treatment of condylar base, neck and head fractures, as well as similar opinion on complications of condylar fracture osteosynthesis. They had a parallel agreement on using open reduction with internal fixation (ORIF) as treatment of choice for condylar base and neck fractures in adults. Endoscopic approaches should be considered for selected cases, such as condylar base fractures with lateral displacement. There was also a growing tendency to perform ORIF in condylar head fractures. The experts also agreed to treat children (> 12 years old) in the same way as adults and to consider open reduction in severely displaced and dislocated fractures even in younger children. Nevertheless, non-surgical treatment should be the first choice for children <6 years of age. The decision to perform surgery in children was based on factors influencing facial growth, appropriate age for ORIF, and disagreement to use resorbable materials in children.Conclusions: The experts and participating surgeons had comparable opinion on management of condylar fractures and complications of ORIF. Compared to the first Condylar Fracture Symposium 2007 in Strasbourg, ORIF may now be considered as the gold standard for both condylar base and neck fractures with displacement and dislocation. Although ORIF in condylar head fractures in adults and condylar fractures in children with mixed dentition is highly recommended, but this recommendation requires further investigations. HYPOTHESIS: This study addresses the outcome after osteosynthesis or hemiarthroplasty, using a cohort of patients that was enrolled in a previous prospective study on humeral head perfusion and was consequently treated using a common conceptual approach. MATERIALS AND METHODS: Between 1998 and 2001, 98 patients with 100 fractures of the proximal humerus were treated surgically by a single surgeon with open reduction and internal fixation (ORIF) (51/100, group A, median age 54 years; range, 21-88) or with hemiarthroplasty (49/100, group B, median age 66 years; range, 38-87). Seventy-six of 98 patients were available for re-evaluation at a mean follow-up of five years (3.3-7.3) using the Constant-Murley score (CMS), the Subjective Shoulder Value (SSV), and conventional radiographs. RESULTS: The median total CMS was 77 (range, 37-98) for group A and 70 (range, 39-84) for group B. The median SSV was 92 (range, 40-100) for group A and 90 (range, 40-100) for group B. Avascular necrosis occured in 6/40 fractures treated with ORIF. CONCLUSION: Osteosynthesis and hemiarthroplasty yield similar functional results and comparable patient satisfaction following the applied decision making process in this selected patient cohort. Osteosynthesis with preservation of the humeral head is worth considering when adequate reduction and stable conditions for revascularization can be obtained. In patients with osteopenic bone and/or comminuted fractures, hemiarthroplasty is a viable alternative. LEVEL OF EVIDENCE: Level 2; Prospective non-randomized comparison study.

As an application of this result, a partial answer is given to a question posed by Sahoo and Riedel.

Gridded multibeam bathymetry from Poseidon cruise 408 and Pelagia cruises 64PE350 and 64PE351 within the Jeddah Transect Project. The raw-data were post-processed and gridded at a resolution of 30 m with QPS Fledermaus Pro. For smaller file size and better handling 11 tiles were created with GlobalMapper (5 columns, 5 lines).
