Ubiquitous computing systems operate in environments where the available resources significantly change during the system operation, thus requiring adaptive and context aware mechanisms to sense changes in the environment and adapt to new execution contexts. Motivated by this requirement, a framework for developing and executing adaptive context aware applications is proposed. The PACCA framework employs aspect-oriented techniques to modularize the adaptive behavior and to keep apart the application logic from this behavior. PACCA uses abstract aspect concept to provide flexibility by addition of new adaptive concerns that extend the abstract aspect. Furthermore, PACCA has a default aspect model that considers habitual adaptive concerns in ubiquitous applications. It exploits the synergy between aspect-orientation and dynamic composition to achieve context-aware adaptation, guided by predefined policies and aim to allow software modules on demand load making possible better use of mobile devices and yours limited resources. A Development Process for the ubiquitous applications conception is also proposed and presents a set of activities that guide adaptive context-aware developer. Finally, a quantitative study evaluates the approach based on aspects and dynamic composition for the construction of ubiquitous applications based in metrics

This work proposes a model based approach for pointcut management in the presence of evolution in aspect oriented systems. The proposed approach, called conceptual visions based pointcuts, is motivated by the observation of the shortcomings in traditional approaches pointcuts definition, which generally refer directly to software structure and/or behavior, thereby creating a strong coupling between pointcut definition and the base code. This coupling causes the problem known as pointcut fragility problem and hinders the evolution of aspect-oriented systems. This problem occurs when all the pointcuts of each aspect should be reviewed due to any software changes/evolution, to ensure that they remain valid even after the changes made in the software. Our approach is focused on the pointcuts definition based on a conceptual model, which has definitions of the system's structure in a more abstract level. The conceptual model consists of classifications (called conceptual views) on entities of the business model elements based on common characteristics, and relationships between these views. Thus the pointcuts definitions are created based on the conceptual model rather than directly referencing the base model. Moreover, the conceptual model contains a set of relationships that allows it to be automatically verified if the classifications in the conceptual model remain valid even after a software change. To this end, all the development using the conceptual views based pointcuts approach is supported by a conceptual framework called CrossMDA2 and a development process based on MDA, both also proposed in this work. As proof of concept, we present two versions of a case study, setting up a scenario of evolution that shows how the use of conceptual visions based pointcuts helps detecting and minimizing the pointcuts fragility. For the proposal evaluation the Goal/Question/Metric (GQM) technique is used together with metrics for efficiency analysis in the pointcuts definition

Nowadays, there are many aspect-oriented middleware implementations that take advantage of the modularity provided by the aspect oriented paradigm. Although the works always present an assessment of the middleware according to some quality attribute, there is not a specific set of metrics to assess them in a comprehensive way, following various quality attributes. This work aims to propose a suite of metrics for the assessment of aspect-oriented middleware systems at different development stages: design, refactoring, implementation and runtime. The work presents the metrics and how they are applied at each development stage. The suite is composed of metrics associated to static properties (modularity, maintainability, reusability, exibility, complexity, stability, and size) and dynamic properties (performance and memory consumption). Such metrics are based on existing assessment approaches of object-oriented and aspect-oriented systems. The proposed metrics are used in the context of OiL (Orb in Lua), a middleware based on CORBA and implemented in Lua, and AO-OiL, the refactoring of OIL that follows a reference architecture for aspect-oriented middleware systems. The case study performed in OiL and AO-OiL is a system for monitoring of oil wells. This work also presents the CoMeTA-Lua tool to automate the collection of coupling and size metrics in Lua source code

Many challenges have been imposed on the middleware to support applications for digital TV because of the heterogeneity and resource constraints of execution platforms. In this scenario, the middleware must be highly configurable so that it can be customized to meet the requirements of applications and underlying platforms. This work aims to present the GingaForAll, a software product line developed for the Ginga - the middleware of the Brazilian Digital TV (SBTVD). GingaForAll adds the concepts of software product line, aspect orientation and model-driven development to allow: (i) the specification of the common characteristics and variables of the middleware, (ii) the modularization of crosscutting concerns - both mandatory and concepts variables - through aspects, (iii) the expression of concepts as a set of models that increase the level of abstraction and enables management of various software artifacts in terms of configurable models. This work presents the architecture of the software product line that implements such a tool and architecture that supports automatic customization of middleware. Two hundred and six patients with severe head injury (Glasgow Coma Scale of 8 points or less after nonsurgical resuscitation on admission), managed at Intensive Care Unit-Hospital das Clínicas - Universidade Estadual de Campinas were prospectively analysed. All patients were assessed by CT scan and 72 required neurosurgical intervention. All patients were continuously monitored to evaluate intracranial pressure (ICP) levels by a subarachnoid device (11 with subarachnoid metallic bolts and 195 with subarachnoid polyvinyl catheters). The ICP levels were continuously observed in the bedside pressure monitor display and their end-hour values were recorded in a standard chart. The patients were managed according to a standard protocol guided by the ICP levels. There were no intracranial haemorrhagic complications or hematomas due the monitoring method. Sixty six patients were punctured by lateral C1-C2 technique to assess infectious complications and 2 had positive cerebrospinal fluid samples for Acinetobacter sp. The final results measured at hospital discharge showed 75 deaths (36,40%) and 131 (63,60%) survivors. ICP levels had significantly influenced the final results (p<0,001). The subarachnoid method to continuously assess the ICP levels was considered aplicable, safe, simple, low cost and useful to advise the management of the patients. The ICP record methodology was practical and useful. Despite the current technical advances the subarachnoid method was considered viable to assess the ICP levels in severe head injury.

Purpose: A survey was carried out on one hundred patients of the Emergency Service of the Ophthalmology Department of the Hospital das Clinicas of the University of Campinas (UNICAMP), in order to analyze the personal characteristics and the barriers against getting resolving ophthalmologic assistance. Variables, were the following: sex, age, home town, average distance between the place of initial symptoms and first visit to the hospital, time spent between the first examination (if performed in any other service) and the examination performed at the Hospital das Clinicas of University of Campinas, diagnosis, veracity of emergency, need to refer patients previously seen in other services to our Service and possibility of assistance and treatment at a secondary level. Methods: The sample showed the following characteristics: distances between 20 and 100 kilometers covered by 50.0% of the patients to be seen at University of Campinas. 75.0% of those patients needed someone to stay with them and 67.0% came from other municipalities. The long distances covered meant additional expenses for the treatment of diseases which should be treated locally. Results: Among the patients referred to University of Campinas by ophthalmologists of other services, 87.5% could have their diseases treated at a secondary level of assistance and 66.7% of real emergencies and 60% of false emergencies took longer than 7 days to reach the emergency room of University of Campinas. This shows the poor infrastructure of secondary services regarding excellence of emergency care and education of patients. Conclusions: We recommend education of general physicians and ophthalmologists for emergency eye care and also the supply of both secondary and tertiary public services or medicare, strategically setup in the whole state of Sao Paulo.

Descreve-se o hábito de fumar de mulheres internadas no Serviço de Obstetrícia de um hospital do município de São Paulo, Brasil, assim como conhecimentos a respeito dos efeitos do fumo durante a gestação e orientação recebida durante esse período. Apresentam-se algumas implicações para a educação em saúde, considerando-se diferentes níveis de programas anti-fumo.

Foi realizado estudo com a finalidade de verificar as condições de profilaxia da raiva humana, em sub-regiões administrativas do Estado de São Paulo com vistas a obter melhor orientação quanto ao uso de vacinas e soros anti-rábicos. Foram analisadas 4.121 fichas contendo dados de pessoas que procuraram 31 Unidades Sanitárias dos Distritos de Marília, Assis e Tupã, no período de 1982/83. Os resultados evidenciaram: má qualidade do preenchimento das fichas; 84,1% dos acidentes foram provocados por animais da espécie canina; o grupo etário mais atingido foi o de 5 a 14 anos; 65,7% dos acidentes foram no próprio domicílio; vários casos deixariam de ser tratados, caso houvesse maior e melhor observação durante o período de 10 dias; o índice de abandono de tratamento foi de 10,4%; falta maior entrosamento entre as Instituições que, direta ou indiretamente, têm compromissos com o controle e a profilaxia da raiva.

Quando alguém é afectado por uma doença crónica tem que alterar o seu estilo de vida de modo a poder viver o melhor possível com a doença que o vai acompanhar, se não durante toda a vida, pelo menos durante grande parte da vida. Em função das características pessoais e da interacção como o meio envolvente, social e físico, alguns ajustar-se-ão melhor e mais facilmente do que outros. Estas alterações denominam-se Ajustamento ou Adaptação. Embora estes termos sejam sinónimos, por vezes a denominação expressa orientação teórica diferente: a primeira, mais interactiva, para designar o comportamento resultante dessa interacção, momento a momento, com o meio envolvente, a segunda mais estrutural. “Ajustamento” é um termo do senso comum, utilizado na linguagem de todos os dias. O dicionário diz que ajustamento e adaptação são sinónimos e, em contexto de psicologia, saúde e doenças, os dois termos também são utilizados como sinónimos. No entanto, na psicologia, por vezes, estes termos podem expressar conceitos diferentes, consoante a orientação teórica que os utiliza. O interesse pela mudança subjacente ao ajustamento ou à adaptação considera dois aspectos: a estrutura e o processo. A estrutura refere-se a factores estáveis tais como a traços de personalidade ou estrutura cognitiva e, também, a características estáveis do meio ambiente, enquanto o processo refere-se às mudanças que vão ocorrendo em consequência das interacções, momento a momento, com as situações que surgem, explicam Lazarus e Folkman (1985).

Foram colhidos 1815 líquidos cefalorraquidianôs de doentes internados com meningite, no Hospital Emílio Ribas, São Paulo, durante os meses de maio a outubro de 1989. Neisseria meningitidis 56%, dos quais 44% do tipo B; Haemophilus influenzae 17%, sendo 72 % isoladas de neonatos até 3 anos; Streptococcus pneumoniae 14%, dos quais 60% isolados de recém nascidos a 1 ano de idade. O estudo citoquímico liquórico mostrou: celularidade: global > 500cel/mm³ e específica > 70%; proteínas > 90mg/dl e glicose < 45mg/dl em 90% e celularidade < 500cel/mm³ entre 2 a 6% nos três tipos de meningites avaliadas. A bacterioscopia e o citoquímico Uquóricos foram decisivos na orientação da cultura epreditivos no diagnóstico etiológico dessas meningites (Teste de Goodman).
