Aims Surgery for infective endocarditis (IE) is associated with high mortality. Our objectives were to describe the experience with surgical treatment for IE in Spain, and to identify predictors of in-hospital mortality. Methods Prospective cohort of 1000 consecutive patients with IE. Data were collected in 26 Spanish hospitals. Results Surgery was performed in 437 patients (43.7%). Patients treated with surgery were younger and predominantly male. They presented fewer comorbid conditions and more often had negative blood cultures and heart failure. In-hospital mortality after surgery was lower than in the medical therapy group (24.3 vs 30.7%, p = 0.02). In patients treated with surgery, endocarditis involved a native valve in 267 patients (61.1%), a prosthetic valve in 122 (27.9%), and a pacemaker lead with no clear further valve involvement in 48 (11.0%). The most common aetiologies were Staphylococcus (186, 42.6%), Streptococcus (97, 22.2%), and Enterococcus (49, 11.2%). The main indications for surgery were heart failure and severe valve regurgitation. A risk score for in-hospital mortality was developed using 7 prognostic variables with a similar predictive value (OR between 1.7 and 2.3): PALSUSE: prosthetic valve, age ≥ 70, large intracardiac destruction, Staphylococcus spp, urgent surgery, sex [female], EuroSCORE ≥ 10. In-hospital mortality ranged from 0% in patients with a PALSUSE score of 0 to 45.4% in patients with PALSUSE score > 3. Conclusions The prognosis of IE surgery is highly variable. The PALSUSE score could help to identify patients with higher in-hospital mortality.

Unidades didácticas digitales adaptadas al currículo educativo de la Región de Murcia que sirven de apoyo al profesor en el aula, al tiempo que fomentan la interacción con los alumnos a través de la tecnología. Están concebidas como recursos complementarios al libro de texto tradicional. La publicación forma parte del proyecto AULA XXI cuyo objetivo es avanzar en la integración plena de las TIC en el ámbito educativo regional. Se incluyen actividades de aprendizaje dinámicas que favorecen una actitud más activa del alumno en clase, a la vez que le proporciona una guía para seguir los contenidos de repaso o ampliación a través de sistemas de autoevaluación.

El propósito de este estudio es analizar el cambio en magnitud, composición y distribución del gasto de los hogares en los años 1997 y 2003, usando las encuestas de calidad de vida. Este análisis encuentra una gran disminución del gasto per cápita (23.9%), especialmente en zonas urbanas y regiones más adelantadas, reflejo de la crisis económica que golpea al país desde 1998. Este decrecimiento desigual generó una mejor distribución en el 2003 y ocasionó un cambio en la estructura de gastos, llevando a un importante crecimiento de la proporción de los gastos en alimentos y una disminución de los gastos de educación y salud. En el documento se utilizó estadística descriptiva, discriminando por quintiles de cada región y clase (urbano rural). Posteriormente se realizaron estimaciones paramétricas y no paramétricas de las curvas de Engel, para cada rubro, con el fin de identificar su naturaleza.

El proyecto trata de concienciar a la comunidad escolar y autoridades para frenar la degradación de las zonas ajardinadas del instituto donde existen 52 especies de árboles y arbustos. Los objetivos son aportar materiales para que los alumnos conozcan y valoren su entorno; comprender y expresar mensajes utilizando lenguaje científico; fomentar la cooperación, responsabilidad y una conciencia más ecológica; estimular la capacidad de observación; y familiarizarlos con las nuevas tecnologías. Los alumnos observan los ejemplares del instituto que identifican y localizan, mediante planos y croquis, para después describirlos y fotografiarlos. También se realizan consultas bibliográficas sobre especies, se estructura la información y se adapta al sistema informático, y se elaboran materiales como la guía en papel y cd-rom Descubre, conoce y protege los árboles y arbustos del entorno, con ficha técnica descriptiva y fotografía de cada especie; clave dicotómica para su identificación; y los carteles identificación con nombre vulgar, científico y la familia. La evaluación del aprendizaje va incluida en la sección de actividades del cd-rom.

Plantea el estudio de algunos temas clásicos de Psicología dando importancia prioritaria a las experiencias personales. El libro es fundamentalmente una herramienta de trabajo, por lo que hace falta completarlo con aspectos teóricos. Cada tema comienza con una página-guía en la que consta el índice, el léxico y el esquema conceptual del tema, especificando los posibles aspectos teóricos que se podrían tratar. A continuación, en el desarrrollo, destaca principalmente la parte práctica, con gran número de actividades para las clases de iniciación a la Psicología.

Se presenta una programación y ejercicios para el teatro de colegio, incluyendo además tres piezas de teatro que corresponden a las tres modalidades de teatro infantil: el teatro para niños hecho por actores adultos, 'Marisabidilla', de Rafael de Mendizábal; el teatro hecho por los niños en su totalidad, 'Pastores y príncipes de Belén', y el teatro hecho en colectividad por actores adultos y por niños, 'Corona de Pasión, ambos de Manuel Muñoz Hidalgo. La programación y ejercicios se han realizado en los colegios 'San Agustín de Salamanca' y 'Santa María del Carmen' de Murcia. Se recoge y destaca, de manera muy especial, el valor de la representación escénica en la labor educativa de los niños. Los ejercicios tratan temas como el personaje, la voz, anális de un texto de autor, medios expresivos, puesta en escena, dirección escénica, etc.

This Doctoral Thesis entitled Contribution to the analysis, design and assessment of compact antenna test ranges at millimeter wavelengths aims to deepen the knowledge of a particular antenna measurement system: the compact range, operating in the frequency bands of millimeter wavelengths. The thesis has been developed at Radiation Group (GR), an antenna laboratory which belongs to the Signals, Systems and Radiocommunications department (SSR), from Technical University of Madrid (UPM). The Radiation Group owns an extensive experience on antenna measurements, running at present four facilities which operate in different configurations: Gregorian compact antenna test range, spherical near field, planar near field and semianechoic arch system. The research work performed in line with this thesis contributes the knowledge of the first measurement configuration at higher frequencies, beyond the microwaves region where Radiation Group features customer-level performance. To reach this high level purpose, a set of scientific tasks were sequentially carried out. Those are succinctly described in the subsequent paragraphs. A first step dealed with the State of Art review. The study of scientific literature dealed with the analysis of measurement practices in compact antenna test ranges in addition with the particularities of millimeter wavelength technologies. Joint study of both fields of knowledge converged, when this measurement facilities are of interest, in a series of technological challenges which become serious bottlenecks at different stages: analysis, design and assessment. Thirdly after the overview study, focus was set on Electromagnetic analysis algorithms. These formulations allow to approach certain electromagnetic features of interest, such as field distribution phase or stray signal analysis of particular structures when they interact with electromagnetic waves sources. Properly operated, a CATR facility features electromagnetic waves collimation optics which are large, in terms of wavelengths. Accordingly, the electromagnetic analysis tasks introduce an extense number of mathematic unknowns which grow with frequency, following different polynomic order laws depending on the used algorithmia. In particular, the optics configuration which was of our interest consisted on the reflection type serrated edge collimator. The analysis of these devices requires a flexible handling of almost arbitrary scattering geometries, becoming this flexibility the nucleus of the algorithmia’s ability to perform the subsequent design tasks. This thesis’ contribution to this field of knowledge consisted on reaching a formulation which was powerful at the same time when dealing with various analysis geometries and computationally speaking. Two algorithmia were developed. While based on the same principle of hybridization, they reached different order Physics performance at the cost of the computational efficiency. Inter-comparison of their CATR design capabilities was performed, reaching both qualitative as well as quantitative conclusions on their scope. In third place, interest was shifted from analysis - design tasks towards range assessment. Millimetre wavelengths imply strict mechanical tolerances and fine setup adjustment. In addition, the large number of unknowns issue already faced in the analysis stage appears as well in the on chamber field probing stage. Natural decrease of dynamic range available by semiconductor millimeter waves sources requires in addition larger integration times at each probing point. These peculiarities increase exponentially the difficulty of performing assessment processes in CATR facilities beyond microwaves. The bottleneck becomes so tight that it compromises the range characterization beyond a certain limit frequency which typically lies on the lowest segment of millimeter wavelength frequencies. However the value of range assessment moves, on the contrary, towards the highest segment. This thesis contributes this technological scenario developing quiet zone probing techniques which achieves substantial data reduction ratii. Collaterally, it increases the robustness of the results to noise, which is a virtual rise of the setup’s available dynamic range. In fourth place, the environmental sensitivity of millimeter wavelengths issue was approached. It is well known the drifts of electromagnetic experiments due to the dependance of the re sults with respect to the surrounding environment. This feature relegates many industrial practices of microwave frequencies to the experimental stage, at millimeter wavelengths. In particular, evolution of the atmosphere within acceptable conditioning bounds redounds in drift phenomena which completely mask the experimental results. The contribution of this thesis on this aspect consists on modeling electrically the indoor atmosphere existing in a CATR, as a function of environmental variables which affect the range’s performance. A simple model was developed, being able to handle high level phenomena, such as feed - probe phase drift as a function of low level magnitudes easy to be sampled: relative humidity and temperature. With this model, environmental compensation can be performed and chamber conditioning is automatically extended towards higher frequencies. Therefore, the purpose of this thesis is to go further into the knowledge of millimetre wavelengths involving compact antenna test ranges. This knowledge is dosified through the sequential stages of a CATR conception, form early low level electromagnetic analysis towards the assessment of an operative facility, stages for each one of which nowadays bottleneck phenomena exist and seriously compromise the antenna measurement practices at millimeter wavelengths.

Managing large medical image collections is an increasingly demanding important issue in many hospitals and other medical settings. A huge amount of this information is daily generated, which requires robust and agile systems. In this paper we present a distributed multi-agent system capable of managing very large medical image datasets. In this approach, agents extract low-level information from images and store them in a data structure implemented in a relational database. The data structure can also store semantic information related to images and particular regions. A distinctive aspect of our work is that a single image can be divided so that the resultant sub-images can be stored and managed separately by different agents to improve performance in data accessing and processing. The system also offers the possibility of applying some region-based operations and filters on images, facilitating image classification. These operations can be performed directly on data structures in the database.

En el campo de la biomedicina se genera una inmensa cantidad de imágenes diariamente. Para administrarlas es necesaria la creación de sistemas informáticos robustos y ágiles, que necesitan gran cantidad de recursos computacionales. El presente artículo presenta un servicio de cloud computing capaz de manejar grandes colecciones de imágenes biomédicas. Gracias a este servicio organizaciones y usuarios podrían administrar sus imágenes biomédicas sin necesidad de poseer grandes recursos informáticos. El servicio usa un sistema distribuido multi agente donde las imágenes son procesadas y se extraen y almacenan en una estructura de datos las regiones que contiene junto con sus características. Una característica novedosa del sistema es que una misma imagen puede ser dividida, y las sub-imágenes resultantes pueden ser almacenadas por separado por distintos agentes. Esta característica ayuda a mejorar el rendimiento del sistema a la hora de buscar y recuperar las imágenes almacenadas.
