OBJECTIVES: To assess the prevalence of musculoskeletal symptoms and their association with sociodemographic risk factors among female garment factory workers in Sri Lanka. METHODS: 1058 randomly selected female garment factory workers employed in the free trade zone of Kogalla, Sri Lanka were recruited to complete two interviewer-administered questionnaires assessing musculoskeletal symptoms and health behaviors. DISCUSSION: Musculoskeletal complaints among female garment workers in the FTZ of Kogalla are less common than expected. Sociocultural factors may have resulted in underreporting and similarly contribute to the low rates of healthcare utilization by these women. RESULTS: 164 (15.5%) of workers reported musculoskeletal symptoms occurring more than 3 times or lasting a week or more during the previous 12-month period. Back (57.3%) and knee (31.7%) were the most common sites of pain. Although most symptomatic women reported that their problems interfered with work and leisure activities, very few missed work as a result of their pain. Prevalence correlated positively with increased age and industry tenure of less than 12 months. Job type, body mass index, and education were not significant predictors of musculoskeletal symptoms.

Musculoskeletal (MSK) complaints are common within primary care (1) (2) (3) but some General Practitioners (GPs)/family physicians do not feel comfortable managing these symptoms (3), preferring to refer onto hospital specialists or Integrated Clinical Assessment and Treatment Services (ICATs). Long waiting times for hospital outpatient reviews are a major cause of patient inconvenience and complaints (4). We therefore aimed to establish a GP-ran MSK and sport and exercise medicine (SEM) clinic based within a Belfast GP surgery that would contribute to a sustainable improvement in managing these common conditions within primary care as well as reducing waiting times for patients with these conditions to see a specialist. This shift from hospital-based to community-based management is in-keeping with recent policy changes within the UK health-system, including Transforming Your Care within Northern Ireland (NI) (5). The GP-ran MSK and SEM clinic was held monthly within a Belfast GP practice, staffed by one GP with a specialist interest in MSK and SEM conditions and its performance was reviewed over a three month period. Parameters audited included cases seen, orthopaedic and x-ray referral rates and secondary care referrals comparing the GP practice’s performance to the same time period in the previous year as well as patient satisfaction questionnaires.

Two studies among college students were conducted to evaluate appropriate measurement methods for etiological research on computing-related upper extremity musculoskeletal disorders (UEMSDs). ^ A cross-sectional study among 100 graduate students evaluated the utility of symptoms surveys (a VAS scale and 5-point Likert scale) compared with two UEMSD clinical classification systems (Gerr and Moore protocols). The two symptom measures were highly concordant (Lin's rho = 0.54; Spearman's r = 0.72); the two clinical protocols were moderately concordant (Cohen's kappa = 0.50). Sensitivity and specificity, endorsed by Youden's J statistic, did not reveal much agreement between the symptoms surveys and clinical examinations. It cannot be concluded self-report symptoms surveys can be used as surrogate for clinical examinations. ^ A pilot repeated measures study conducted among 30 undergraduate students evaluated computing exposure measurement methods. Key findings are: temporal variations in symptoms, the odds of experiencing symptoms increased with every hour of computer use (adjOR = 1.1, p < .10) and every stretch break taken (adjOR = 1.3, p < .10). When measuring posture using the Computer Use Checklist, a positive association with symptoms was observed (adjOR = 1.3, p < 0.10), while measuring posture using a modified Rapid Upper Limb Assessment produced unexpected and inconsistent associations. The findings were inconclusive in identifying an appropriate posture assessment or superior conceptualization of computer use exposure. ^ A cross-sectional study of 166 graduate students evaluated the comparability of graduate students to College Computing & Health surveys administered to undergraduate students. Fifty-five percent reported computing-related pain and functional limitations. Years of computer use in graduate school and number of years in school where weekly computer use was ≥ 10 hours were associated with pain within an hour of computing in logistic regression analyses. The findings are consistent with current literature on both undergraduate and graduate students. ^

This research is a secondary data analysis of the CUPID-INCA Nicaragua study, a cross-sectional study comparing psychosocial and physical factors on musculoskeletal symptoms among nurses, office workers and maquiladoras in Nicaragua. There were three objectives for this thesis. (1) To describe the study population according to their socio-demographic, psychosocial (i.e. work organization and health beliefs) and physical factors. (2) To estimate the prevalence of musculoskeletal disorders (MSDs) in the study population (nurses, office workers and maquilas). (3) To analyze and compare the trends of association between psychosocial factors and MSDs to that of physical factors and MSDs in the study population. Trends of association between MSDs and psychosocial factors were also compared between nurses, office workers and maquilas. ^ Majority of the total study population were females, middle aged, non smokers and had been on the job for more than five years. Prevalence rates of low back pain and upper extremity pain were 28% and 37% respectively in nurses, 17% and 34% in office workers and 18% and 31% in maquilas. Workers' health belief was significantly associated with MSDs in all three occupational groups. Psychosocial factors were not consistently associated more with MSDs than physical factors. Maquilas had more psychosocial factors statistically significantly associated with musculoskeletal symptoms than nurses and office workers. ^ The findings of this research suggest that both psychosocial and physical risk factors play a role on the prevalence of musculoskeletal symptoms in the three working populations in Nicaragua. Future research in this area should explore further, the risk of developing MSDs from workers' exposure to psychosocial factors as well as physical factors.^

The aim of this study was to evaluate factors associated with reported work-related musculoskeletal symptoms among aircraft assembly workers. Population consisted of 552 (491 men/61 women) workers who performed tasks related to the work of aircraft assembly. Participants completed a comprehensive questionnaire, including socio-demographic information, habits/lifestyles, working conditions, and work organization. Workers also answered the Nordic Musculoskeletal Questionnaire to obtain data on musculoskeletal symptoms. Multivariate logistic regression was performed to analyze factors associated with musculoskeletal reported symptoms. Results showed that body regions with the highest prevalence of reported musculoskeletal symptoms were similar when referred the past twelve months and the past seven days. Significant factors associated with musculoskeletal symptoms included variables related to conflicts at work, sleep problems, mental fatigue, and lack of time for personal care and recovery. Working time in the industry was associated only with reports for the last seven days and regular physical activity off-work seems to be a positive factor in preventing musculoskeletal symptoms for the past twelve months. The results highlight the multi-factorial nature of the problem. Actions to prevent musculoskeletal diseases at the aircraft assembly work should consider multiple interventions that would promote better recovery between work shifts.

This study investigated the relationship between psychosocial risk factors and (1) neck symptoms and (2) neck pain and disability as measured by the neck disability index (NDI). Female office workers employed in local private and public organizations were invited to participate, with 333 completing a questionnaire. Data were collected on various risk factors including age, negative affectivity, history of previous neck trauma, physical work environment, and task demands. Sixty-one percent of the sample reported neck symptoms lasting greater than 8 days in the last 12 months. The mean NDI of the sample was 15.5 out of 100, indicating mild neck pain and disability. In a hierarchical multivariate logistic regression, low supervisor support was the only psychosocial risk factor identified with the presence of neck symptoms. Similarly, low supervisor support was the only factor associated with the score on the NDI. These associations remained after adjustment for potential confounders of age, negative affectivity, and physical risk factors. The interaction of job demands, decision authority, and supervisor support was significantly associated with the NDI in the final model and this association increased when those with previous trauma were excluded. Interestingly, and somewhat contrary to initial expectations, as job demands increased, high decision authority had an increasing effect on the NDI when supervisor support was low.

This study investigated the relative contribution of individual, workplace, psychosocial and physiological features associated with neck pain in female office workers towards developing appropriate intervention programs. Workers without disability (Neck Disability Index (NDI) score≤8, n=33); workers with neck pain and disability (NDI≥9/100, n=52) and 22 controls (women who did not work and without neck pain) participated in this study. Two logistic regression models were constructed to test the association between various measures in (1) workers with and without disability, and (2) workers without disability and controls. Measures included those found to be significantly associated with higher NDI in our previous studies: psychosocial domains; individual factors; task demands; quantitative sensory measures and measures of motor function. In the final model, higher score on negative affectivity scale (OR=4.47), greater activity in the neck flexors during cranio-cervical flexion (OR=1.44), cold hyperalgesia (OR=1.27) and longer duration of symptoms (OR=1.19) remained significantly associated with neck pain in workers. Workers without disability and controls could only be differentiated by greater muscle activity in the cervical flexors and extensors during a typing task. No psychosocial domains remained in either regression model. These results suggest that impairments in the sensory and motor system should be considered in any assessment of the office worker with neck pain and may have stronger influences on the presenting symptoms than workplace and psychosocial features.

RESUMO - As lesões musculoesqueléticas ligadas ao trabalho (LMELT) são frequentes em contexto hospitalar e particularmente entre os enfermeiros (Estryn-Béhar, 1991; NIOSH, 1997). Em meio ocupacional o diagnóstico destas patologias é, com frequência, realizado com recurso a questionários de sintomas. A aplicação de questionários de auto-referência de sintomas de LMELT tem permitido recolher informação sobre a prevalência europeia destas lesões, registando-se valores que oscilam entre os 17 e os 44% (Buckle, 1999) e entre nós, designadamente numa indústria de componentes automóveis, os valores apresentam dimensão semelhante ou superior (Serranheira et al., 2003). O presente estudo pretendeu identificar a sintomatologia musculoesquelética auto-referida pelos enfermeiros em meio hospitalar, procurando relações com a actividade de enfermagem. Utilizou-se um instrumento de recolha de informação construído a partir de uma adaptação do questionário nórdico musculoesquelético (QNM) (Kuorinka et al., 1987) que se aplicou a 899 enfermeiros de diferentes serviços em cinco hospitais da região do grande Porto, no ano de 2004 (responderam 507 enfermeiros). Os resultados evidenciam uma prevalência elevada de sintomas musculoesqueléticos em diferentes zonas anatómicas nos últimos 12 meses (84%), particularmente atingindo a região lombar (65%), cervical (55%), dorsal (37%), ombros (34%) e punhos/mãos (30%). Não se encontram associações significativas entre algumas actividades que os enfermeiros realizam e a presença de níveis de desconforto, incómodo ou dor com origem no sistema musculoesquelético, eventualmente devido ao elevado número de subactividades desempenhadas pelos enfermeiros. Apesar disso, existem algumas relações: (1) com a tipologia de actividades realizadas (por exemplo levantamento e transporte de cargas) nos diferentes serviços (destaca-se a Medicina e a Neonatologia), (2) com o aumento do número de horas de trabalho semanais. Desse modo, observa-se que a prevalência de sintomas de LMELT neste grupo profissional é elevada o que pode condicionar a actividade dos enfermeiros e, por consequência, o bem-estar dos utentes. Sugere-se que a actividade de enfermagem e a organização do trabalho deste grupo profissional sejam objecto de uma análise mais detalhada no sentido da identificação dos elementos determinantes da sintomatologia musculoesquelética e sua consequente prevenção.

Objetivo: Establecer la prevalencia de síntomas osteomusculares cervicobraquiales y su relación con el proceso de pasteurización de leche, en una comercializadora de lácteos en Nemocón, Cundinamarca. Metodología: Se realizó un estudio descriptivo de corte transversal en una pasteurizadora de leche, con una población de 18 trabajadores, a los cuales se les aplicó una encuesta basada en el Cuestionario Nórdico y el sugerido por NIOSH en busca de morbilidad sentida osteomuscular cervicobraquial, así como de información personal incluyendo edad, actividades extralaborales, tiempo de vida laboral, grado de satisfacción laboral, peso y talla realizada para toda la población con los mismos instrumentos de medición. Se utilizaron frecuencias, porcentajes y unidades de tendencia central con base en promedios y porcentajes. Se utilizaron test de Fisher y T de student. Resultados: La población estudiada fue en su totalidad de género masculino, con una mediana de 26 años de edad. El tiempo laborado en la empresa tuvo una mediana de 3,5 años. La prevalencia de síntomas por segmentos fue muy alta, siendo menor en cuello (16,7%) y mayor en mano y muñeca (88,9%). No se encontró diferencia entre la presencia de síntomas y la realización de actividades extralaborales ni con factores individuales como edad e índice de masa corporal. Tampoco se relacionó con la satisfacción laboral ni el tiempo de vida laboral. Discusión: Los resultados de este estudio muestran una alta prevalencia de síntomas osteomusculares en trabajadores de una pasteurizadora de leche, con exposición a factores de riesgo por carga física tanto postural, por movimiento repetitivo, carga física, manipulación de cargas y exposición a frío, con la presencia de morbilidad sentida cervicobraquial osteomuscular. Dado que no se encontraron asociaciones significativas con los factores extralaborales e individuales evaluados, la alta prevalencia de sintomatología puede ser explicada por la exposición a carga física laboral. RESUMEN Objetivo: Analizar el comportamiento de la sintomatología de los Desórdenes Músculo-esqueléticos (DME) de miembros superiores y determinar si la dinamometría es predictiva y sirve como soporte para el diagnostico de los mismos. Pacientes y Métodos: Se tomó una cohorte de 400 trabajadores de tres empresas del sector floricultor de la sabana de Bogotá en el año 2006, quienes fueron evaluados clínicamente por un médico especialista en salud ocupacional. Se seleccionaron 105 trabajadores que no presentaban síntomas osteomusculares de miembros superiores, a quienes se realizó seguimiento clínico y dinamometría anual hasta el año 2009. En cada evaluación se realizaron tres mediciones con el dinamómetro Dynatron® midiendo la fuerza de agarre en cada mano, tomando como dato final el promedio obtenido. Los datos fueron digitados en un base de Excel 2003 (Office®) y la depuración y el procesamiento fueron realizados en el programa estadístico de sobrevida. Resultados: El grupo de estudio estaba compuesto por 73 mujeres (69.5%) y 32 hombres (32.5%). La mano dominante fue la derecha en 99 trabajadores (94.3%). El índice de masa corporal (IMC) se encontró alterado revelando sobrepeso en 50 trabajadores (47.6%). Tenían una media de antigüedad en la empresa de 6,48 años y de 14.12 años de exposición en el sector floricultor. El promedio de la fuerza de agarre manual fue mayor para los hombres con respecto a las mujeres. Los primeros presentaron cifras; para la mano derecha de 105 libras y para la izquierda de 102 libras; en las mujeres fue de 62 libras para la mano derecha y de 61 libras para la izquierda. El 38.1% de la población estudiada desarrolló patologías osteomusculares de miembros superiores a lo largo de los 4 años de seguimiento, dentro de las cuales predominó la epicondilitis de miembro superior derecho, presentándose con mayor frecuencia en las mujeres. La probabilidad de supervivencia global para las mujeres fue 46,3% mientras que para los hombres fue de 81,6% a lo largo del tiempo de estudio con una p=0.0107. En el periodo de estudio las mujeres necesitaron aproximadamente de 38 meses para alcanzar el 50% de sobrevida. Para el género femenino el promedio de la fuerza de la mano derecha de 28 libras tiene una sensibilidad del 100% con una especificidad del 1%. En la mano izquierda el promedio de la fuerza de 30,0 libras tiene una sensibilidad del 100% y una especificidad del 1%. En los hombres se encontró que el promedio de fuerza de la mano derecha de 65 libras tiene una sensibilidad del 100% y una especificidad del 1% y de la mano izquierda de 66 libras tiene una sensibilidad del 100% y una especificidad del 1%. Conclusiones: Las lesiones osteomusculares presentaron altas incidencias en la población estudiada que no se relacionan con aspectos como IMC y tiempo de vida en el sector floricultor y las mujeres presentaron una probabilidad de supervivencia significativamente menor con respecto a los hombres. La dinamometría en valores bajos es un estudio que se correlaciona con la presencia de DME pero no tiene valor predictivo.
