Improving public involvement in health system decision making stands as a primary goal in health systems reform. However, still limited evidence is found on how best to elicit preferences for health care programs. This paper examines a contingent choice technique to elicit preferences among health programs so called, willingness to assign (WTAS): Moreover, we elicited contingents rankings as well as the willingness to pay extra taxes for comparative purposes. We argue that WTAS reveals relative ( monetary-based) values of a set of competing public programmes under a hypothetical healthcare budget assessment. Experimental evidence is reported from a delibertive empirical study valuing ten health programmes in the context of the Catalan Health Services. Evidence from a our experimental study reveals that perferences are internally more consistent and slightly less affected by "preference reversals" as compared to values revealed from the willingness to pay (WTP) extra taxes approach. Consistent with prior studies, we find that the deliberative approach helped to avoid possible misunderstandings. Interestingly, although programmes promoting health received the higher relative valuation, those promoting other health benefits also ranked highly

The present article identifies, for children living in developing countries, the major causes of ill-health that are inadequately covered by established health programmes. Injuries and noncommunicable diseases, notably asthma, epilepsy, dental caries, diabetes mellitus and rheumatic heart disease, are growing in significance. In countries where resources are scarce it is to be expected that increasing importance will be attached to the development and implementation of measures against these problems. Their control may benefit from the application of elements of programmes directed against infectious, nutritional and perinatal disorders, which continue to predominate.

The report presents evidence on a range of factors affecting disparity between mental and physical health, and includes case studies and examples of good practice to illustrate some of the key issues and solutions. It should be seen as the first stage of an on-going process over the next 5"10 years that will deliver parity for mental health and make whole-person care a reality. It builds on the Implementation Framework for the Mental Health Strategy in providing further analysis of why parity does not currently exist, and the actions required to bring it about. A parity approach should enable NHS and local authority health and social care services to provide a holistic, whole person response to each individual, whatever their needs, and should ensure that all publicly funded services, including those provided by private organisations, give people's mental health equal status to their physical health needs. Central to this approach is the fact that there is a strong relationship between mental health and physical health, and that this influence works in both directions. Poor mental health is associated with a greater risk of physical health problems, and poor physical health is associated with a greater risk of mental health problems. Mental health affects physical health and vice versa. The report makes a series of key recommendations for the UK government, policy-makers and health professionals. Recommendations include: The government and the NHS Commissioning Board should work together to give people equivalent levels of access to treatment for mental health problems as for physical health problems, agreed standards for waiting times, and agreed standards for emergency/crisis mental healthcare. Action to promote good mental health and to address mental health problems needs to start at the earliest stage of a person's life and continue throughout the life course. Preventing premature mortality " there must be a major focus on improving the physical health of people with mental health problems. Public health programmes must include a focus on the mental health dimension of issues commonly considered as physical health concerns, such as smoking, obesity and substance misuse. Commissioners need to regard liaison doctors (who work across physical and mental healthcare) as an absolute necessity rather than an optional luxury. NHS and social care commissioners should commission liaison psychiatry and liaison physician services to drive a whole-person, integrated approach to healthcare in acute, secure, primary care and community settings, for all ages. Mental health services and mental health research must receive funding that reflects the prevalence of mental health problems and their cost to society. Mental illness is responsible for the largest proportion of the disease burden in the UK (22.8%), larger than that of cardiovascular disease (16.2%) or cancer (15.9%). However, only 11% of the NHS budget was spent on NHS services to treat mental health problems for all ages during 2010/11. Culture, attitudes and stigma " zero-tolerance policies in relation to discriminatory attitudes or behaviours should be introduced in all health settings to help combat the stigma that is still attached to mental illness within medicine. Political and managerial leadership is required at all levels. There should be a mechanism at national level for driving a parity approach to relevant policy areas across government; all local councils should have a lead councillor for mental health; all providers of specialist mental health services should have a board-level lead for physical health and all providers of physical healthcare services should have a board-level lead for mental health. The General Medical Council (GMC) and Nursing and Midwifery Council (NMC) should consider how medical and nursing study and training could give greater emphasis to mental health. BACKGROUND: Meticulous steps and procedures are proposed in planning guidelines for the development of comprehensive multiyear plans for national immunization programmes. However, we know very little about whether the real-life experience of those who adopt these guidelines involves following these procedures as expected. Are these steps and procedures followed in practice? We examined the adoption and usage of the guidelines in planning national immunization programmes and assessed whether the recommendations in these guidelines are applied as consistently as intended. METHODS: We gathered information from the national comprehensive multiyear plans developed by 77 low-income countries. For each of the 11 components, we examined how each country applied the four recommended steps of situation analysis, problem prioritization, selection of interventions, and selection of indicators. We then conducted an analysis to determine the patterns of alignment of the comprehensive multiyear plans with those four recommended planning steps. RESULTS: Within the first 3 years following publication of the guidelines, 66 (86%) countries used the tool to develop their comprehensive multiyear plans. The funding conditions attached to the use of these guidelines appeared to influence their rapid adoption and usage. Overall, only 33 (43%) countries fully applied all four recommended planning steps of the guidelines. CONCLUSIONS: Adoption and usage of the guidelines for the development of comprehensive multiyear plans for national immunization programmes were rapid. However, our findings show substantial variation between the proposed planning ideals set out in the guidelines and actual use in practice. A better understanding of factors that influence how recommendations in public health guidelines are applied in practice could contribute to improvements in guidelines design. It could also help adjust strategies used to introduce them into public health programmes, with the ultimate goal of a greater health impact.

In the past three decades, Brazil has undergone rapid changes in major social determinants of health and in the organisation of health services. In this report, we examine how these changes have affected indicators of maternal health, child health, and child nutrition. We use data from vital statistics, population censuses, demographic and health surveys, and published reports. In the past three decades, infant mortality rates have reduced substantially, decreasing by 5.5% a year in the 1980s and 1990s, and by 4.4% a year since 2000 to reach 20 deaths per 1000 livebirths in 2008. Neonatal deaths account for 68% of infant deaths. Stunting prevalence among children younger than 5 years decreased from 37% in 1974-75 to 7% in 2006-07. Regional differences in stunting and child mortality also decreased. Access to most maternal-health and child-health interventions increased sharply to almost universal coverage, and regional and socioeconomic inequalities in access to such interventions were notably reduced. The median duration of breastfeeding increased from 2.5 months in the 1970s to 14 months by 2006-07. Official statistics show stable maternal mortality ratios during the past 10 years, but modelled data indicate a yearly decrease of 4%, a trend which might not have been noticeable in official reports because of improvements in death registration and the increased number of investigations into deaths of women of reproductive age. The reasons behind Brazil`s progress include: socioeconomic and demographic changes (economic growth, reduction in income disparities between the poorest and wealthiest populations, urbanisation, improved education of women, and decreased fertility rates), interventions outside the health sector (a conditional cash transfer programme and improvements in water and sanitation), vertical health programmes in the 1980s (promotion of breastfeeding, oral rehydration, and immunisations), creation of a tax-funded national health service in 1988 (coverage of which expanded to reach the poorest areas of the country through the Family Health Program in the mid-1990s); and implementation of many national and state-wide programmes to improve child health and child nutrition and, to a lesser extent, to promote women`s health. Nevertheless, substantial challenges remain, including overmedicalisation of childbirth (nearly 50% of babies are delivered by caesarean section), maternal deaths caused by illegal abortions, and a high frequency of preterm deliveries.

The objective of this paper is to summarise epidemiological information about the distribution of dental caries among Indigenous peoples in Brazil. The authors also present a case study of a specific group of Xavante Indians, one of the most numerous of Brazil`s Indigenous peoples, describing how their oral health has deteriorated over recent decades, and showing how an oral health programme is attempting to reverse the present trend of increase in caries. The programme at Etenheritipa Xavante village incorporated three principal components: educational, preventive, and clinical. From the beginning, the programme included epidemiological record keeping for monitoring the level of caries in the population. Transversal studies of the condition of oral health among the Xavante of Etenheritipa were undertaken in 1999, 2004, and 2007. In the period from 2004 to 2007 the DMFS values in the 11-15 age cohort had a significant reduction in caries experience. The mean DMFS score fell from 4.95 in 2004 to 2.39 in 2007 (p<0.01). An increase in the percent of individuals who were free from caries was also noted: in 1999, 20% of adolescents 11-15 had no caries; in 2007, the proportion had risen to 47%. The Xavante case is a prime example of the transition in oral health that is taking place among the Indigenous peoples of the Americas, and it highlights the importance of oral health promotion through preventive measures such as access to fluoridation and basic care in reducing the inequality between Indians and non-Indians.

The interruption of vectorial transmission of Chagas disease in Venezuela is attributed to the combined effects of ongoing entomoepidemiological surveillance, ongoing house spraying with residual insecticides and the concurrent building and modification of rural houses in endemic areas during almost five decades. The original endemic areas which totaled 750,000 km², have been reduced to 365,000 km². During 1958-1968, initial entomological evaluations carried out showed that the house infestation index ranged between 60-80%, the house infection index at 8-11% and a house density index of 30-50 triatomine bugs per house. By 1990-98, these indexes were further reduced to 1.6-4.0%, 0.01-0.6% and 3-4 bugs per house respectively. The overall rural population seroprevalence has declined from 44.5% (95% C.I.: 43.4-45.3%) to 9.2% (95% C.I.: 9.0-9.4%) for successive grouped periods from 1958 to 1998. The annual blood donor prevalence is firmly established below 1%. The population at risk of infection has been estimated to be less than four million. Given that prevalence rates are stable and appropriate for public health programmes, consideration has been given to potential biases that may distort results such as: a) geographical differences in illness or longevity of patients; b) variations in levels of ascertainment; c) variations in diagnostic criteria; and d) variations in population structure, mainly due to appreciable population migration. The endemic areas with continuous transmission are now mainly confined to piedmonts, as well as patchy foci in higher mountainous ranges, where the exclusive vector is Rhodnius prolixus. There is also an unstable area, of which landscapes are made up of grasslands with scattered broad-leaved evergreen trees and costal plains, where transmission is very low and occasional outbreaks are reported.

Malaria and other arthropod born diseases remain a serious public health problem affecting the lives and health of certain social groups when the two basic strategies to control fail due to : (1) the lack of effective chemoprophylaxis/chemotherapy or the rapid development of drug resistance of the infectious agents and (2) the ineffectiveness of pesticides or the arthropod vectors develop resistance to them. These situations enhances the need for the design and implementation of other alternatives for sustainable health programmes. The application of the epidemiological methods is essential not only for analyzing the relevant data for the understanding of the biological characteristics of the infectious agents, their reservoirs and vectors and the methods for their control, but also for the assessment of the human behaviour, the environmental, social and economic factors involved in disease transmission and the capacity of the health systems to implement interventions for both changes in human behaviour and environmental management to purpose guaranteed prevention and control of malaria and other arthropod born diseases with efficiency, efficacy and equity. This paper discuss the evolution of the malaria arthropod diseases programmes in the American Region and the perspectives for their integration into health promotion programs and emphasis is made in the need to establish solid basis in the decision-making process for the selection of intervention strategies to remove the risk factors determining the probability to get sick or die from ABDs. The implications of the general planning and the polices to be adopted in an area should be analyzed in the light of programme feasibility at the local level, in the multisectoral context specific social groups and taking in consideration the principles of stratification and equity

The European Centre for Disease Prevention and Control (ECDC) and the European Respiratory Society (ERS) jointly developed European Union Standards for Tuberculosis Care (ESTC) aimed at providing European Union (EU)-tailored standards for the diagnosis, treatment and prevention of tuberculosis (TB). The International Standards for TB Care (ISTC) were developed in the global context and are not always adapted to the EU setting and practices. The majority of EU countries have the resources and capacity to implement higher standards to further secure quality TB diagnosis, treatment and prevention. On this basis, the ESTC were developed as standards specifically tailored to the EU setting. A panel of 30 international experts, led by a writing group and the ERS and ECDC, identified and developed the 21 ESTC in the areas of diagnosis, treatment, HIV and comorbid conditions, and public health and prevention. The ISTCs formed the basis for the 21 standards, upon which additional EU adaptations and supplements were developed. These patient-centred standards are targeted to clinicians and public health workers, providing an easy-to-use resource, guiding through all required activities to ensure optimal diagnosis, treatment and prevention of TB. These will support EU health programmes to identify and develop optimal procedures for TB care, control and elimination.

Uncontrolled peripheral urbanisation coupled with environmental degradation has affected the status of schistosomiasis in Pernambuco (PE), Brazil. This endemic disease continues to perpetuate its transmission in rural areas and has also become a cause for concern in coastal towns of the state. The lack of basic infrastructure (sanitation and health programmes) to support the new urban areas leads to faecal contamination of natural aquatic environments, resulting in consequent infection of vector snails and the emergence of new sources of schistosomiasis transmission. In the present paper, we discuss the current epidemiological status of schistosomiasis in PE. We have consolidated and analysed information from parasitological, malacological and morbidity surveys undertaken by the group of researchers at the Laboratory of Schistosomiasis, Centro de Pesquisas Aggeu Magalhães-Fiocruz. The results of our analysis show: (i) the maintenance of the levels of schistosomiasis in the rural Zona da Mata, PE, (ii) the record of the human cases of schistosomiasis and the foci of infected snails detected along the coast of PE through 2007, (iii) the high record of the severe clinical form of schistosomiasis in the metropolitan region of Recife (RMR) and (iv) new breeding sites of schistosomiasis vector snails that were identified in a 2008 survey covering the RMR and the coastal localities of PE.

En dépit de nombreuses interventions en santé reproductive en Afrique subsaharienne, la trilogie « IST/VIH/SIDA - grossesses précoces - avortements » persiste à des niveaux très élevés par rapport aux autres parties du monde. Cela indique que les nombreuses interventions en santé reproductive auprès des adolescents et des jeunes ont enregistré peu de succès en ce qui concerne le changement des comportements sexuels. Ces interventions se focalisent souvent sur l’individu, et négligent les environnements sociaux et culturels dans lesquels se forge le vécu de la sexualité chez les jeunes. Un de ces agents de socialisation est la famille, où les individus naissent, grandissent, et sont socialisés selon les valeurs et normes en vigueur. Fort de ce constat, l’objectif principal de la présente thèse est de resituer l’environnement familial au cœur des débats en santé reproductive chez les adolescents et les jeunes en Afrique subsaharienne. Trois questions spécifiques sont examinées dans cette thèse. Premièrement, elle aborde les associations entre les structures familiales et l’entrée en sexualité. Deuxièmement, elle analyse leurs influences sur les connaissances des modes de transmission et des moyens de prévention du VIH/SIDA. Troisièmement, elle cherche à déterminer les forces potentielles dans les familles dites « à risque » (ayant au plus un parent biologique) à partir de la théorie de résilience selon laquelle des facteurs familiaux et contextuels peuvent atténuer les comportements sexuels à risque chez les adolescents et jeunes. Cette thèse démontre substantiellement que vivre avec ses deux parents biologiques, la nature des relations entre parents/tuteurs et le jeune et un niveau élevé du contrôle parental sont significativement associés à de faibles risques des rapports sexuels prémaritaux. Par contre, les unions polygamiques, un statut socioéconomique élevé du ménage, et le fait d’être orphelin augmentent significativement le risque de rapports sexuels prémaritaux. L’étude démontre aussi que l’environnement familial et la communication sur la sexualité, aussi bien avec les parents/tuteurs qu’avec les pairs, jouent un rôle fondamental dans l’acquisition des connaissances correctes des modes de transmission et de prévention du VIH/SIDA. Néanmoins, le rôle des parents/tuteurs sur l’acquisition des connaissances sur le VIH/SIDA s’avère indirect puisqu’elle repose sur une hypothèse implicite. Seule une mesure directe des connaissances des parents sur les modes de transmission et les moyens de prévention peut mieux rendre compte de cette association. Les résultats obtenus à partir de la théorie de résilience indiquent, dans chaque type de familles, que la qualité des relations entre les parents/tuteurs et le jeune est significativement associée à une faible probabilité de comportement sexuel à risque, défini comme étant la cooccurrence de plusieurs partenaires sexuels au cours de 12 derniers mois et de non-utilisation du condom. Par contre, le contrôle parental est associé à une faible probabilité de comportement sexuel à risque seulement dans les familles à deux parents biologiques. Ce résultat suggère que l’influence du contrôle parental baisse une fois que les jeunes ont eu une expérience sexuelle. Les interventions en santé reproductive devraient promouvoir chez les parents/tuteurs les facteurs familiaux susceptibles de réduire les comportements sexuels à risque.

Introduction Chaque année, 289 000 femmes décèdent des complications reliées à la grossesse et à l’accouchement, et 2.9 millions de nouveau-nés décèdent avant d’atteindre 28 jours de vie. La quasi-totalité (99%) des décès maternels et néonataux ont cours dans les pays à revenu faible et intermédiaire (PRFI). L’utilisation des services obstétricaux essentiels, incluant l’assistance qualifiée à l’accouchement (AA) et les services postnataux, contribue largement à la réduction de la morbidité et de la mortalité maternelle et néonatale. Il est donc essentiel d’évaluer les déterminants et les inégalités de couverture de ces services, en vue d’informer l’élaboration de politiques et de programmes de santé dans les PRFI. Objectifs 1. Étudier systématiquement les déterminants et inégalités socioéconomiques, géographiques et démographiques dans l’utilisation des services de santé postnataux dans les PRFI. 2. Évaluer l’effet de la politique de subvention des frais aux usagers introduite au Burkina Faso en 2007 sur les taux d’utilisation de l’assistance qualifiée à l’accouchement, en fonction du statut socioéconomique (SSE). Méthodes 1. Nous avons réalisé une revue systématique sur l’utilisation des services postnataux dans les PRFI, en fonction des déterminants socioéconomiques, géographiques et démographiques. Notre étude incluait une méta-analyse de l’utilisation des services selon les quintiles de SSE et le milieu de vie (urbain vs. rural). 2. Nous avons utilisé un devis quasi-expérimental. Les sources de données consistaient en deux sondages représentatifs (n=1408 et n=1403), conduits respectivement en 2008 et 2010 auprès de femmes des districts sanitaires de Houndé et de Ziniaré au Burkina Faso, en plus d’une enquête sur la qualité structurelle des soins offerts dans les centres de santé primaire. Nous avons utilisé des modèles de régression de Poisson, multi-niveaux et segmentés, afin d’évaluer l’effet de la politique de subvention sur les taux d’AA. Nous avons estimé des ratios et différences de taux d’incidence ajustés, en fonction du SSE et du temps écoulé depuis l’introduction de la subvention. Résultats 1. Les estimés de ratio de cotes (RC) agrégés (IC 95%) pour les femmes de SSE élevé (5e quintile ou Q5), Q4, Q3 et Q2 (référence : quintile le plus pauvre, Q1) étaient respectivement : 2.27 (1.75 – 2.93); 1.60 (1.30-1.98); 1.32 (1.12-1.55); et 1.14 (0.96-1.34). La méta-analyse a aussi démontré un gradient d’utilisation des services postnataux entre les femmes urbaines et rurales : RC (IC 95%) = 1.36 (1.01-1.81). L’évaluation narrative a par ailleurs identifié une différence dans la couverture de services selon le niveau d’éducation. 2. Pour les femmes de faible SSE, le taux d’AA était 24% plus élevé (IC 95% : 4-46%) immédiatement après l’introduction de la subvention, en comparaison au taux attendu en l’absence de ladite subvention. L’ampleur de l’effet a diminué dans le temps, correspondant à des estimés (IC 95%) de 22% (3-45%) à 6 mois, 20% (1-43%) à 12 mois, et 17% (-4-42%) à 24 mois après l’introduction de la subvention. La force d’association variait selon les strates de SSE, l’effet le plus prononcé étant observé au sein du SSE le plus faible. Conclusions 1. L’utilisation des services postnataux demeure inéquitable selon le SSE et l’accessibilité géographique aux formations sanitaires dans les PRFI. 2. Notre étude suggère que l’introduction de la subvention des frais aux usagers au Burkina Faso résulte en une augmentation soutenue dans le taux d’assistance qualifiée à l’accouchement, particulièrement chez les femmes de faible SSE. Cette évidence scientifique devrait alimenter l’élaboration de programmes de santé materno-infantile, en plus de guider la planification de politiques et le renforcement des systèmes de santé des PRFI.

Objetivo: Determinar la ocurrencia de reacciones adversas a medicamentos (RAM) como causa de ingreso a una unidad de cuidado intermedio de un hospital universitario. Materiales y Métodos: Se revisaron las historias clínicas de los pacientes admitidos a la Sala de Emergencias – Cuidado Intermedio (SALEM) entre septiembre y diciembre de 2012 que cumplieron los criterios de inclusión y se detectaron los casos sospechosos de reacción adversa a medicamento (RAM) que posteriormente fueron evaluados por cuatro investigadores respecto a la causalidad a través del Algoritmo de Naranjo, prevenibilidad usando los criterios de Shumock y Thornton y la clasificación clínica mediante el empleo del sistema DoTS. Resultados: Se encontraron 96 pacientes que presentaron 108 casos de RAM. Las RAM más frecuentes fueron las arritmias y la hemorragia de vías digestivas altas (12.04%), 20.3% de los casos correspondieron a fallos terapéuticos, y, los medicamentos mayormente asociados fueron el ácido acetil salicílico (15.74%) y el losartán (10.19%). 46 casos fueron catalogados como posibles y uno solo como definitivo. Usando la clasificación DoTS se estableció que en el 82.4% de los casos la dosis era colateral (dentro del rango de dosis terapéutica), 89.8% fueron independientes del tiempo, y entre los factores mayormente asociados a susceptibilidad a la RAM estuvieron las comorbilidades (41.7%) y la edad (49%). 44% de las RAM fueron prevenibles. Conclusión: Las RAM son una causa de ingreso no despreciable en una unidad de cuidado intermedio para las cuales existen diferentes sistemas de evaluación, y una cantidad significativa de ellas es prevenible. Se requieren más estudios a nivel nacional para evaluar la incidencia de estas y establecer estándares de clasificación y medidas para mitigar su efecto.
