Os autores apresentam um caso de rotura de aneurisma do seio de Valsalva, para a aurícula direita, com um quadro clínico atípico que levou a dificuldades de diagnóstico diferencial com insuficiência aórtica. Após confirmação hemodinâmica, atendendo à rápida deterioração do estado geral, procedeu-se a correcção cirúrgica de urgência. A propósito deste caso os autores procederam a uma revisão de patologia, etiopatogenia e clínica da entidade em causa.

A 66-year-old female with Streptococcus viridans aortic and tricuspid infective endocarditis develops, during the course of antibiotic therapy, rupture of a right coronary sinus of Valsalva aneurysm to the right ventricle. An urgent cardiac surgery is preformed with implantation of a mechanical aortic prosthesis and a right coronary sinus plasty. Six months later a huge aortic pseudoaneurysm is diagnosed and she is submitted to a second uneventful surgery. A review is done for the significant features with discussion of diagnosis and therapy.

Isolated anterior mitral leaflet cleft (not associated with atrio-ventricular septal defect) is a rare cause of congenital mitral regurgitation, and the treatment consists of direct suturing of the cleft. We present a clinical case with this entity.

Apresenta-se o caso de um doente avaliado por sopro cardíaco contínuo, em que o estudo ecocardiográfico permitiu diagnosticar ruptura de aneurisma congénito do seio de Valsalva (SV) para a aurícula direita. Os autores fazem uma breve revisão sobre os aspectos clínicos desta patologia.

Apresenta-se o caso de um homem de 62 anos de idade com um episódio de dispneia súbita que motivou estudo ecocardiográfico, o qual revelou uma massa na aurícula direita. A avaliação posterior demonstrou tratar-se de extensão de neoplasia do rim direito. A massa tumoral foi removida através duma abordagem multidisciplinar, com cirurgia torácica e abdominal. O estudo anatomopatológico mostrou tratar-se de um carcinoma de células renais.

INTRODUCTION: Transthoracic echocardiography is the method of choice for the diagnosis of cardiac myxomas, but the transesophageal approach provides a better definition of the location and characteristics of the tumor. The authors review their thirteen years' experience on the echocardiographic diagnosis of this pathology. METHODS: From 1994 to 2007, 41 cardiac tumors were diagnosed in our echocardiographic laboratory, of which 27 (65.85%) were cardiac myxomas. The exams and the patients' clinical files were retrospectively reviewed. RESULTS: Of the 27 patients, 22 (81.5%) were female, with a mean age of 62.1 +/- 13.6 years (25-84 years). The predominant clinical features were due to the obstruction caused by the tumor in more than two thirds of the patients, followed by constitutional symptoms in one third and embolic events in 30%. In the lab results, anemia was found in three patients and elevated sedimentation rate and CRP in two. In two patients the myxoma was found by chance. All the cases were of the sporadic type, although we found a prevalence of thyroid disease of 14% (4 patients). All patients underwent urgent surgical resection except one, in whom surgery was refused due to advanced age and comorbidities. The myxomas followed a typical distribution with 24 (88.8%) located in the left atrium, 18 of them attached to the atrial septum (AS) and two to the mitral valve. In one patient, the tumor involved both atria. The other two cases originated in the right atrium at the AS. Embolic phenomena were more frequent in small tumors (p = 0.027) and in those with a villous appearance (p = 0.032). Obstructive manifestations were associated with larger tumors (p = 0.046) and larger left atria (p = 0.048). In our series, there were no deaths during hospitalization or in the follow-up period of 5.2 +/- 3.7 years in 19 patients. There were two recurrences, both patients being successfully reoperated. CONCLUSION: Myxoma is the most common cardiac tumor. Transesophageal echocardiography provides excellent morphologic definition, aiding in diagnosis and follow-up. Most clinical manifestations are obstructive and are associated with larger tumors. Small tumors with a friable appearance have a higher chance of embolization. Surgical resection is usually curative and the long-term prognosis is excellent.

Os tumores de células germinativas (TCG) do mediastino, são neoplasias raras dentro das lesões com esta localização. Classificam-se em seminomas puros, TCG não seminomatosos malignos e teratomas. A transformação maligna de um teratoma à custa do seu componente somático com degenerescência em sarcoma ou carcinoma é uma entidade ainda mais rara. Descrevemos um caso clínico de um homem de 32 anos de idade com quadro de toracalgia direita intensa. Os exames de imagem revelaram a existência de uma lesão expansiva com 7.7 cm de diâmetro, heterogénea, com áreas de densificação lipomatosa e imagem cálcica de configuração dentária no mediastino anterior projectado à direita, adjacente aos grandes vasos, sendo os aspectos radiológicos compatíveis com teratoma. A biopsia transtorácica guiada por TC, revelou aspectos morfológicos sugestivos de sarcoma. Foi submetido a cirurgia de ressecção em bloco da massa mediastínica, pulmão direito, segmento de pericárdio e timo. O exame histológico revelou tratar-se de um teratoma com transformação maligna do componente mesenquimatoso, com diferenciação muscular em leiomiosarcoma e rabdomiosarcoma. Fez quimioterapia com doxorrubicina e ifosfamida. Os aspectos essenciais desta entidade clínica, bem como o seu tratamento, nomeadamente cirúrgico, são, neste artigo, objecto de discussão, apoiada numa revisão da literatura mais recente dedicada ao tema.

A 75-year old female patient, with previous inferior acute myocardial infarction (AMI) in December 2000, was admitted in April 2001 with angina and heart failure. Transthoracic echocardiography (TTE) was suggestive of a postero-inferior pseudoaneurysm (PA) of the left ventricle (LV), with 61x49 mm. of size and mitral regurgitation. Cardiac catheterization was suspected of a PA of the LV and revealed a three vessels coronary artery disease. On 20th April she was submitted to cardiac surgery with resection of a large LV aneurysm (AN) and triple coronary artery bypass surgery. Afterwards, she was on NYHA class III and subsequent TTE and transesophagic echocardiography (TEE) were suggestive of a 90x60 mm LV posterior PA (confirmed by nuclear magnetic resonance) and severe mitral regurgitation, with good LV systolic function. She underwent a new cardiac surgery on 31st May 2002, with resuturing of the LV postero-inferior wall patch and removal of the PA. The patient is in good condition and on NYHA functional class I-II.

Nos doentes com carcinoma hepatocelular a incidência de metástases cardíacas é de 0.67-3%. Mesmo na ressecção com intuito curativo, o prognóstico é reservado, sendo a sobrevida aos 5 anos de 12-39%. Descrevemos um caso clínico, pouco habitual, de um indivíduo do sexo masculino, de 51 anos de idade, que apresentava uma massa localizada à aurícula direita, diagnosticada por exame histopatológico, como sendo metástase de um carcinoma hepatocelular, tendo sido necessária a ressecção cirúrgica urgente, devido a instabilidade cardiovascular.

Os autores apresentam um caso de tumor fibroso localizado na pleura, maligno, com dimensões gigantes: 26x14x12 cm e P=1,25 Kg, de um doente de sexo feminino de 56 anos. O tumor ocupava praticamente os 2/3 inferiores do campo pulmonar direito com uma inserção invulgar na pleura parietal diafragmática não pediculado e com características histológicas malignas. O diagnóstico de tumor fibroso localizado na pleura, foi feito no material obtido por punção aspirativa por agulha fina. O estudo imunocitoquímico realizado posteriormente na peça operatória confirmou o diagnóstico feito na citologia aspirativa. A avaliação da malignidade só foi feita no exame histológico da peça operatória, utilizando os critérios histológicos de alta celularidade, pleomorfismo celular, actividade mitótica (mais de quatro mitoses em dez campos de grande ampliação do microscópio), áreas de hemorragia e de necrose.
