Een validatie studie teneinde inzicht te geven in 1) de inhoudsvaliditeit, de concurrente validiteit en de interne consistentie van de Geriatric Depression Scale (GDS-30) en de Depressielijst (DL) voor het meten van depressie bij verpleeghuisbewoners en 2) de invloed van cognitief functioneren op het aantal missing items op de DL en de GDS-30. Een cross-sectionele studie met data afkomstig uit de effectstudie ‘Act in Case of Depression’. De steekproefomvang bestond uit verpleeghuisbewoners (N=432) van PG en somatische afdelingen. Meetinstrumenten: DL, GDS-30 en de Mini Mental State Examination (MMSE) voor cognitief functioneren. Experts beoordeelden de DL en de GDS-30 als geschikt voor afname bij verpleeghuisbewoners zonder cognitieve problemen. Echter ongeschikt voor afname bij verpleeghuisbewoners met ernstige cognitieve problemen. Experts vonden de items relevant voor de somatische cliënten, echter was er minder consensus over de itemrelevantie voor PG cliënten. De DL en de GDS-30 vertoonden een sterke interne consistentie en een sterke onderlinge correlatie. Er was geen associatie tussen de stemmingssymptomen van de DL en de GDS-30 en motivatie symptomen van de DL aan de ene kant en de MMSE aan de andere kant. De motivatiesymptomen gemeten met de GDS-30 vertoonden een middelmatige associatie met cognitief functioneren. Cliënten van PG afdelingen hadden meer missing items op de DL en op de GDS-30 dan cliënten van somatische afdelingen. Cognitief functioneren vertoonde een zwakke associatie met het aantal missing items op de DL en de GDS-30.

The aim of this work is to empirically generate a shortened version of the Geriatric Depression Scale (GDS), with the intention of maximising the diagnostic performance in the detection of depression compared with previously GDS validated versions, while optimizing the size of the instrument. A total of 233 individuals (128 from a Day Hospital, 105 randomly selected from the community) aged 60 or over completed the GDS and other measures. The 30 GDS items were entered in the Day Hospital sample as independent variables in a stepwise logistic regression analysis predicting diagnosis of Major Depression. A final solution of 10 items was retained, which correctly classified 97.4% of cases. The diagnostic performance of these 10 GDS items was analysed in the random sample with a receiver operating characteristic (ROC) curve. Sensitivity (100%), specificity (97.2%), positive (81.8%) and negative (100%) predictive power, and the area under the curve (0.994) were comparable with values for GDS-30 and higher compared with GDS-15, GDS-10 and GDS-5. In addition, the new scale proposed had excellent fit when testing its unidimensionality with CFA for categorical outcomes (e.g., CFI=0.99). The 10-item version of the GDS proposed here, the GDS-R, seems to retain the diagnostic performance for detecting depression in older adults of the GDS-30 items, while increasing the sensitivity and predictive values relative to other shortened versions.

Objectives: To identify the prevalence of neuropsychiatric disorders, especially DP and CD, on a sample of nursing home residents, relating this prevalence with some aspects of the demographics and psycho pharmacotherapy. Methods: 48 elders from two different nursing homes were selected. The collection of demographic and pharmacological data was made utilizing medical records. The medication was classified according to the Anatomical Therapeutic Chemical Code (ATC) criteria. The Geriatric Depression Scale (GDS 30) and the Mini Mental State Examination (MMSE) tests were utilized to determine the prevalence of DP and CD. Results: It was observed in the sample a high incidence of DP and CD among the researched elders. More schooling individuals tend to present less CD. Individuals with less CD indicatives present less symptomatology for DP. Of all the researched elders, 54,2% are submitted to psycho pharmacotherapy. Of all the consumed medicine, 16,5% belonged to the class of neuropsychiatric medicine. The medicated elders present, in average, a larger symptomatology for DP (12 points/average/GDS) than the non-medicated elders (9,9 points/average/GDS). The inverse occurs in relation to the CD indicatives. The use of psychotropics, especially in association, can have negative effects related to depression and cognition. Discussion: The pharmacotherapy, characterized for the polymedication and chronicity, especially of neuropsychiatric medicines, deserves special attention among elders, because the data suggest a significant relation between the utilization of medicines, singly or in association, and the increase of CD and DP. In addition, the data suggest that DP is a risk factor for CD and DM.

Introduction: Parkinson`s disease (PD) is a neurodegenerative disorder with prominent motor manifestations and many other non-motor symptoms that significantly decrease quality-of-life and are frequently under-recognized, for example depression. Objective: To study the validity of a Brazilian version of the Zung Self-rating Depression Scale (SDS) for the diagnosis of depression in patients with PD. Methods: We evaluated 78 consecutive non demented patients over the age of 40 with diagnosis of PD at a Movement Disorders Outpatient Clinic, who could read and understand questionnaires. They completed the SIDS and the Geriatric Depression Scale with 15 items (GDS-15). The diagnosis of depression was made after a structured clinical interview based on DSM-IV criteria for the diagnosis of major depression (SCID-CV). Results: The prevalence of major depression was 23.1%. Cronbach`s alpha was 0.73 and the area under the ROC curve was 0.93 for the SDS. The score index of 55 had a sensitivity of 88.9% and a specificity of 83.3% for the diagnosis of depression. The total scores of the SDS and GDS-15 were highly correlated (0.652, p < 0.0001) and correlated weakly with the scores of a motor scale. Discussion: The SIDS is a valid too] for screening depression in patients with PD since the specific SDS index of 55 is adopted. Two shortened versions could be used with good results. ABSTRACT OBJECTIVE To estimate life expectancy with and without depressive symptoms in older adults for the years 2000 and 2010. METHODS We evaluated individuals aged 60 years or older (n = 1,862 in 2000 and n = 1,280 in 2010), participants of the Saúde, Bem-Estar e Envelhecimento (SABE – Health, Wellbeing and Aging) study in in Sao Paulo, Southeastern Brazil. Depression was measured using the shorter version of the Geriatric Depression Scale (GDS-15); respondents scoring ≥ 6 were classified as having depression. Estimates of life expectancy with and without depression were obtained using the Sullivan method. RESULTS Data from 2000 indicate that 60-year-old men could expect to live, on average, 14.7 years without depression and 60-year-old women could expect to live 16.5 years without depression. By 2010, life expectancy without depression had increased to 16.7 years for men and 17.8 years for women. Expected length of life with depression differed by sex, with women expected to live more years with depression than men. CONCLUSIONS Between 2000 and 2010, life expectancy without depression in Sao Paulo increased. However, older adults in Brazil, especially older women, still face a serious burden of mental illness.

OBJECTIVE: To determine the psychometric properties of an adapted version of the Falls Efficacy Scale (FES) in older rehabilitation patients. DESIGN: Cross-sectional survey. SETTING: Postacute rehabilitation facility in Switzerland. PARTICIPANTS: Seventy elderly persons aged 65 years and older receiving postacute, inpatient rehabilitation. INTERVENTIONS: Not applicable. MAIN OUTCOME MEASURES: FES questions asked about subject's confidence (range, 0 [none]-10 [full]) in performing 12 activities of daily living (ADLs) without falling. Construct validity was assessed using correlation with measures of physical (basic ADLs [BADLs]), cognitive (Mini-Mental State Examination [MMSE]), affective (15-item Geriatric Depression Scale [GDS]), and mobility (Performance Oriented Mobility Assessment [POMA]) performance. Predictive validity was assessed using the length of rehabilitation stay as the outcome. To determine test-retest reliability, FES administration was repeated in a random subsample (n=20) within 72 hours. RESULTS: FES scores ranged from 10 to 120 (mean, 88.7+/-26.5). Internal consistency was optimal (Cronbach alpha=.90), and item-to-total correlations were all significant, ranging from .56 (toilet use) to .82 (reaching into closets). Test-retest reliability was high (intraclass correlation coefficient, .97; 95% confidence interval, .95-.99; P<.001). Subjects reporting a fall in the previous year had lower FES scores than nonfallers (85.0+/-25.2 vs 94.4+/-27.9, P=.054). The FES correlated with POMA (Spearman rho=.40, P<.001), MMSE (rho=.37, P=.001), BADL (rho=.43, P<.001), and GDS (rho=-.53, P<.001) scores. These relationships remained significant in multivariable analysis for BADLs and GDS, confirming FES construct validity. There was a significant inverse relationship between FES score and the length of rehabilitation stay, independent of sociodemographic, functional, cognitive, and fall status. CONCLUSIONS: This adapted FES is reliable and valid in older patients undergoing postacute rehabilitation. The independent association between poor falls efficacy and increased length of stay has not been previously described and needs further investigations.

Para avaliar o efeito da suplementação de fosfatidilserina nos resultados obtidos na Escala de Queixas de Memória (EQM) e na Escala de Depressão Geriátrica (EDG), um grupo de adultos e idosos saudáveis (n= 915) com idade 65,65 ± 11,10 anos, e com um nível de scolaridade de 7,56 ± 4,56 anos participaram do estudo. Os participantes foram submetidos a Avaliação Breve do Estado Mental ABEM), Escala de Queixas de Memória (EQM) e Escala de Depressão Geriátrica (EDG). Quatro indivíduos apresentaram disfunção cognitiva, 874 queixas de memória apresentadas e 436 estavam acima do ponto de corte para a EDG. Subsequentemente, um grupo de mulheres (n= 48) utilizaram a suplementação de fosfatidilserina durante 4 semanas. A suplementação de fosfatidilserina, ao longo de 4 semanas reduziu significativamente os valores EQM (p=0,000) e de EDG (p=0,009) neste grupo de mulheres.

This case study objectified investigating the symptomatology prevalence for depression (DP) and cognitive decline (CD) on institutionalized elderly’s; verifying the existence of correlations between DP and CD with age, gender and scholarity; and analyzing the possible correlations between symptomatology for DP and CD. For the realization of this research were selected, in two Elderly’s Long-Permanence Institutions (ELPIs), 24 subjects, which were classified according to gender, scholarity and age group. To verify the occurrence of CD, the Mini Mental State Examination (MMSE) was applied and, for the incidence of DP, the Geriatric Depression Scale (GDS) was applied. The data were tabulated and descriptively analyzed. For the study of the relations between quantitative variables, the Pearson correlation test was utilized and also the Spearmann test when necessary; for the comparison between two independent groups, the Student's t-test was utilized; for the verification of the association between gender, scholarity, DP and CD indicatives, the Pearson's X2 test and, due to theoretical restriction, the Fisher's exact test were utilized. The level of 5% probability was adopted for the rejection of nullity's hypothesis on all the tests. The research demonstrated that 50% of the research' subjects presented DP indicatives and 54.2% presented CD indicatives. Associations between: gender and DP (p = 0.414), gender and CD (p = 0.219), scholarity and CD (p = 0.527) were not observed. Positive regular correlation was verified between age and DP (r = 0.557; p = 0.005) and negative regular correlation was verified between DP and CD (r = -0.406; p = 0.049). The data suggest that DP might be a reaction to CD perceived by the subject. However, the hypothesis that indicates DP as a risk factor for CD and DM cannot be overruled, which suggests the importance of monitoring and treating depressive episodes on elderly populations.

Objectives: To evaluate the possibility of combining cognitive training to an educational intervention composed by eight sessions about hypertension for a better management of the disease among the elderly. Methods: 64 older adults who reported having hypertension, divided into experimental group (EG, n=35) and control group (CG, n=29) participated in the study. Control participants received training after the post-test. The protocol contained socio-demographic and clinical data, Mini-Mental State Examination (MMSE), Geriatric Depression Scale (GDS), Rivermead Behavioral Memory Test (RBMT), Verbal Fluency Animal Category (VF) and Short Cognitive Test (SKT). Results: The EG showed better cognitive performance when compared with the CG, at post-test. Conclusion: Cognitive gains may occur after psychoeducational interventions for older adults with hypertension.

OBJETIVOS: Avaliar a possibilidade de aliar o treino cognitivo à intervenção psicoeducativa de oito sessões sobre hipertensão visando melhor manejo desta condição crônica em idosos. MÉTODOS: Participaram 64 idosos que se declararam como hipertensos, divididos em grupo experimental (GE, n=35) e grupo controle (GC, n=29). O grupo controle recebeu treino após o pós teste. O protocolo incluiu dados sócio-demográficos e clínicos, o Mini-Exame do Estado Mental (MEEM), Escala de Depressão Geriátrica (EDG), Teste Comportamental de Memória de Rivermead (RBMT), Fluência Verbal categoria animais (FV) e Short Cognitive Test (SKT). RESULTADOS: Pôde-se observar que o GE apresentou melhor desempenho cognitivo, quando comparado ao GC, após a intervenção. CONCLUSÕES: Podem ocorrer ganhos cognitivos associados a uma intervenção psicoeducativa para idosos hipertensos.

Objective: To assess the reliability and validity of a brief measure of quality of life recently developed by the World Health Organization, the WHOQOL-BREF, and to examine its association with a variety of clinical and sociodemographic factors in older depressed patients. Design: Cross-sectional study. Methods: Older depressed patients (N=41) underwent diagnostic assessment using the Composite International Diagnostic Interview (CIDI) and were independently assessed on a variety of measures including the WHOQOL-BREF (a 26-item self-report questionnaire generating four domain scores), Hamilton Depression Rating Scale (HAM-D); Geriatric Depression Scale (GDS); Mini-mental State Examination (MMSE); Modified Barthel Index (MBI); Instrumental activities of daily living (IADL), and measures of physical health status and social relationships. Estimates of inter-rater and test-retest reliability, and concurrent validity were made. Results: 39 subjects completed the study. The majority of subjects (94.9%) received a diagnosis of DSM-IV Major Depressive Disorder. Levels of comorbidity were high. Three of the four domains of the WHOQOL-BREF (Physical, Psychological and Environment domains) demonstrated satisfactory reliability and validity. However, the Social Relationships domain exhibited poor validity. Quality of life scores were strongly correlated with severity of depression, number of self-reported physical symptoms and self-assessed general health status. There was no relationship between diagnostic comorbidity and quality of life scores. Conclusions: The WHOQOL-BREF was successfully administered to older depressed patients although the concurrent validity of one of its four domains was poor. Quality of life scores were strongly correlated with severity of depression, raising the issue of measurement redundancy.

Introdução: Os sintomas neuropsiquiátricos (SNP) afetam a maioria das pessoas que sofrem de Declínio Cognitivo e da doença de Alzheimer (DA). O nosso principal objetivo passa por relatar a prevalência dos SNP avaliados através do Neuropsychiatric Inventory Questionnaire (NPI-Q) numa amostra de idosos recolhida em instituições do Concelho de Coimbra. o nosso principal objetivo passa por relatar a prevalência e gravidade dos sintomas neuropsiquiátricos avaliados através do NPI-Q, numa amostra de idosos recolhida em instituições do Concelho de Coimbra. Estudaremos também a título exploratório a prevalência e gravidade do declínio cognitivo, bem como a gravidade dos sintomas depressivos e dos sintomas ansiosos. Pretendemos, ainda, verificar se existem associações estatisticamente significativas entre a gravidade e grau de perturbação associado aos SNP (tal como avaliados pelos cuidadores formais) (NPI-Q)e o declínio cognitivo (avaliado com o MoCA, administrado junto dos idosos). É igualmente nosso objectivo averiguar se existem correlações estatisticamente significativas entre a sintomatologia depressiva (GDS) / a ansiosa (GAI) avaliado junto dos idosos e as variáveis avaliadas junto dos cuidadores formais . — . a gravidade e o grau de perturbação associados aos SPN . — ., assim como verificar se existem correlações estatisticamente significativas entre o declínio cognitivo (MoCA) e as sintomatologias depressiva e ansiosa. Queremos igualmente observar se existem eventuais associações entre as variáveis gravidade e grau de perturbação (NPI-Q) e as variáveis sociodemográficas em estudo. Na medida em que a literatura refere associações significativas entre o declínio cognitivo, a sintomatologia depressiva e ansiosa e as diferentes variáveis sociodemográficas, pretendemos explorar se estas associações estão presentes na nossa amostra. Não deixaremos de explorar também eventuais associações entre as variáveis sociodemográficas e os diferentes SNP(item por item) e por fim pretendemos constatar se na nossa amostra temos associações estatisticamente significativas entre cada item do NPI-Q e as pontuações do MoCA, GDS e GAI. Metodologia: A amostra incluiu 40 idosos(média de idades, M = 79,80; Desvio-padrão, DP = 6,42; variação = 67-90) sob resposta social em instituições do Concelho de Coimbra que aceitaram responder voluntariamente ou cujos cuidadores concederam o consentimento a uma bateria de teste(incluindo questões sociodemográficas, o MoCA, o Geriatric Anxiety Inventory/GAI, a Geriatric Depression Scale/GDS e o NPI-Q. Resultados: Os SNP mais prevalentes foram a Ansiedade (30%) e a Irritabilidade/Labilidade (27,5%). Não se verificaram associações estatisticamente significativas entre a gravidade dos SNP e grau de perturbação associado (NPI-Q) e o declínio cognitivo nos idosos (MoCA). Não se encontraram associações estatisticamente significativas entre a gravidade dos sintomas o grau de perturbação dos mesmos (NPI-Q) e as variáveis sociodemográficas, nem entre a sintomatologia depressiva e ansiosa e a gravidade e grau de perturbação associado aos SNP. As mulheres idosas manifestavam maior sintomatologia ansiosa face aos homens; bem como os idosos que se encontram na valência de Centro de dia manifestavam maior sintomatologia ansiosa do que os idosos residindo em Centro de noite/Lar de idosos. Os idosos que não frequentaram a escola apresentavam um valor inferior no MoCA por comparação com os que frequentaram a escola.Discussão/Conclusão: Ao contrário do esperado, não se observou uma associação entre a gravidade e grau de perturbação associados aos SNP avaliados junto dos cuidadores formais e o declínio cognitivo, avaliado junto dos idosos. Entre os motivos possíveis para este facto salientamos o tamanho reduzido da amostra e o facto dos SNP terem sido avaliados por funcionários da instituição que podiam ou não possuir a formação necessária para os avaliar, ou não os conhecer há tempo suficiente, ou ainda não estar em contacto directo com os idosos. Outro dado relevante foi a ausência de associações significativas entre a gravidade/grau de perturbação associado aos SNP, a sintomatologia depressiva/ansiosa e as variáveis sociodemográficas. Importa, em estudos futuros, com uma amostra maior, verificar se os resultados se mantêm e comparar os obtidos quando o NPI-Q é respondido por clínicos, familiares e auxiliares dos idosos.

OBJETIVO Analisar a prevalência e fatores associados a sintomas depressivos em idosos. MÉTODOS Estudo epidemiológico transversal e de base domiciliar (inquérito EpiFloripa Idoso) com 1.656 idosos, realizado por conglomerados em dois estágios, setores censitários e domicílios, em Florianópolis, SC. A prevalência de sintomas depressivos (desfecho) foi obtida por meio da Geriatric Depression Scale (GDS-15), e testadas associações segundo variáveis sociodemográficas, de saúde, comportamentais e sociais. Foram calculadas razões de prevalências brutas e ajustadas com intervalo de 95% de confiança por regressão de Poisson. RESULTADOS A prevalência de sintomas depressivos foi de 23,9% (IC95% 21,84;26,01). Os fatores de risco associados no modelo final foram: escolaridade de cinco a oito anos (RP = 1,50; IC95% 1,08; 2,08), um a quatro anos (RP = 1,62; IC95% 1,18; 2,23) e nenhum ano de estudo (RP = 2,11; IC95% 1,46;3,05); situação econômica pior quando comparada com a que tinha aos 50 anos (RP = 1,33; IC95% 1,02;1,74); déficit cognitivo (RP = 1,45; IC95% 1,21;1,75); percepção de saúde regular (RP = 1,95; IC95% 1,47;2,60) e ruim (RP = 2,64; IC95% 1,82;3,83); dependência funcional (RP = 1,83; IC95% 1,43; 2,33); e dor crônica (RP = 1,35; IC95% 1,10;1,67). Grupo etário de 70 a 79 anos (RP = 0,77; IC95% 0,64;0,93); atividade física de lazer (RP = 0,75; IC95% 0,59;0,94); participação em grupos de convivência ou religiosos (RP = 0,80; IC95% 0,64;0,99); e ter relação sexual (RP = 0,70; IC95% 0,53;0,94) mostraram-se fatores protetores ao aparecimento dos sintomas depressivos. CONCLUSÕES Situação clínica adversa, desvantagem socioeconômica e pouca atividade social e sexual mostraram-se associadas aos sintomas depressivos em idosos.
