[EN] The objective of this study was to test the hypothesis that cooperative learning strategies will help to increase nutrition knowledge of nurses and nursing assistants caring for the elderly in different institutional communities of the Basque Country, Spain. The target population was a sample of volunteers, 16 nurses and 28 nursing assistants. Training consisted of 12 nutrition education sessions using cooperative strategies conducted over a period of 3 consecutive weeks. The assessment instruments included two pretest and two posttest questionnaires with questions selected in multiplechoice format. The first questionnaire was about general knowledge of applied nutrition (0-88 point scale) and the second one on geriatric nutrition knowledge (0-18 point scale). Data were analyzed using SPSS vs. 11.0. The outcomes indicated a significant increase in general nutrition knowledge (difference between the pre- and posttest mean score: 14.5±10.1; P<0.001) and in geriatric nutrition knowledge for all participants (difference between the pre- and post-test mean score: 4.6±4.6; P<0.001). So the results indicated that cooperative learning strategies could improve the nutrition knowledge of nursing staff. Additionally, the results of this study provide direction to continuing nutrition education program planners regarding appropriate content and methodology for programs.

BACKGROUND: The number of nonagenarians and centenarians is rising dramatically, and many of them live in nursing homes. Very little is known about psychiatric symptoms and cognitive abilities other than memory in this population. This exploratory study focuses on anosognosia and its relationship with common psychiatric and cognitive symptoms. METHODS: Fifty-eight subjects aged 90 years or older were recruited from geriatric nursing homes and divided into five groups according to Mini-Mental State Examination scores. Assessment included the five-word test, executive clock-drawing task, lexical and categorical fluencies, Anosognosia Questionnaire-Dementia, Neuropsychiatric Inventory, and Charlson Comorbidity Index. RESULTS: Subjects had moderate cognitive impairment, with mean ± SD Mini-Mental State Examination being 15.41 ± 7.04. Anosognosia increased with cognitive impairment and was associated with all cognitive domains, as well as with apathy and agitation. Subjects with mild global cognitive decline seemed less anosognosic than subjects with the least or no impairment. Neither anosognosia nor psychopathological features were related to physical conditions. CONCLUSIONS: Anosognosia in oldest-old nursing home residents was mostly mild. It was associated with both cognitive and psychopathological changes, but whether anosognosia is causal to the observed psychopathological features requires further investigation.

Exercise has reported benefits for those with dementia. In the current study we investigated the feasibility of delivery and the physical and functional benefits of an innovative aquatic exercise program for adults with moderate to severe dementia living in a nursing home aged care facility. Ten adults (88.4 years, inter quartile range 12.3) participated twice weekly for 12 weeks. Anthropometric and grip strength data, and measures of physical function and balance were collected at baseline and post-intervention. Feasibility was assessed by attendance, participation, enjoyment and recruitment. Following exercise, participant's left hand grip strength had improved significantly (p = .017). Small to moderate effect sizes were observed for other measures. A number of delivery challenges emerged, but participant enjoyment, benefits and attendance suggest feasibility. Aquatic exercise shows promise as an intervention among those with dementia who live in a nursing home aged care facility. Greater program investigation is warranted.

The purpose of the study was to evaluate the influence of a geriatric nursing education workshop on nursing staff competency. Forty-three nurses participated in the study, which used an intervention and comparison group research design with pretest and posttest measures. The results indicated that participation in the workshop increased nurses' knowledge of gerontologic issues and improved nurses' ability to assess patients and to plan and document nursing interventions in patient charts. The intervention did not have a significant impact on collaborative practice, role ambiguity, or job satisfaction.

Q-sort is a research method which allows defining profiles of attitudes toward a set of statements, ordered in relation to each other. Pertaining to the Q Methodology, the qualitative analysis of the Q-sorts is based on quantitative techniques. This method is of particular interest for research in health professions, a field in which attitudes of patients and professionals are very important. The method is presented in this article, along with an example of application in nursing in old age psychiatry.

Geriatric nursing competency in the acute care setting is a social mandate for the 21st century. This article reports on the content validation of an Australian research instrument, the Older Patients in Acute Care Survey (OPACS) that examines the attitudes, the knowledge, and the practices of nurses working with acute care patients. The OPACS tool was developed primarily to assist nurse educators to assess attitudes, knowledge, and practices of nursing staff in caring for older patients in the acute care setting; to evaluate the implementation of institution-specific educational interventions; and to improve quality of care given to older patients. An overall content validity index (CVI) for the OPACS was calculated (CVI = .918), revealing high content validity. Opinions (CVI = .92) and practices (CVI = .97) subconstructs revealed high content validity as well. Therefore, results indicate that the OPACS has high content validity in the U.S. acute care setting and could assist nurse educators in establishing and enhancing nurse competency in the care for geriatric patients in the future.

This paper describes a process undertaken to develop and review five clinical vignettes to be used in geriatric nursing educational research. The purpose of this process was to provide valid depictions of delirium and its subtypes and distinguish delirium from dementia. Five vignettes depicting hospital bedside interactions between nursing staff, family, and an older patient who displayed signs of one of the following conditions: delirium (hyper and hypo-active types respectively), dementia, or delirium (both types) superimposed on dementia were constructed. Vignette accuracy and reliability was established using a multistage process that culminated in formal review by a group of ten international nursing and medical delirium experts. The final five vignettes accurately depicted the given scenario as agreed by the experts and were at an appropriate level of simplicity and clarity. Given the increased interest in vignettes for both nursing research and educational purposes, the described method of vignette development and review has the ability to assist other vignette developers in creating reliable representations of their desired clinical scenarios.

A incontinência urinária gera implicações negativas nos âmbitos emocional, social e econômico tanto para o indivíduo incontinente, como para seus cuidadores. A terapia comportamental é uma das abordagens não-invasivas para a incontinência urinária. A terapia comportamental é realizada durante as consultas de enfermagem e a atuação do enfermeiro consiste na aplicação de um protocolo de orientações sobre hábitos de vida, medidas de controle da micção, treinamento para realização do diário miccional, treinamento de exercícios perineais e avaliação da resposta da paciente à terapia. O estudo tem como base teórica a Teoria do autocuidado de Dorothea Elisabeth Orem, pois a terapia comportamental visa instrumentalizar o indivíduo a realizar práticas de autocuidado a partir do protocolo de atendimento do ambulatório. O objetivo da pesquisa é avaliar a efetividade da terapia comportamental aplicada pelo enfermeiro para o controle miccional e melhora da qualidade de vida da mulher idosa. Trata-se de um ensaio clínico não-controlado.40 Foram incluídas no estudo mulheres acima de 60 anos que participam do Ambulatório do Núcleo de Atenção ao Idoso com a queixa clínica de perda involuntária de urina encaminhadas para o ambulatório de urogeriatria. A população estudada foi composta por 13 participantes. Os dados da pesquisa foram coletados a partir dos instrumentos de avaliação do ambulatório de urogeriatria que foram arquivados nos prontuários das pacientes: o diário miccional, avaliação de enfermagem na terapia comportamental e o questionário sobre qualidade de vida em mulheres com incontinência urinária chamado de Kings Health Questionnaire. Estes instrumentos foram aplicados antes e depois da terapia comportamental. Foram colhidos dados das pacientes acompanhadas no ambulatório durante o período de abril de 2011 a junho de 2012. Os resultados foram que após a terapia comportamental todas as idosas responderam que ingerem líquidos no período diurno, 92,30% das idosas responderam que estabeleceram um ritmo miccional de 2/2 horas ou de 3/3 horas. Sobre o parâmetro miccional perda de urina ao final da terapia comportamental 75% das idosas apresentaram ausência de perda de urina. Além disso, após a terapia comportamental nenhuma das pacientes teve perda de urina durante a realização dos exercícios e 92,30% apresentaram contração eficiente dos músculos perineais. Deste modo, esta pesquisa demonstrou que as idosas que participaram da terapia comportamental obtiveram melhora do controle urinário e da qualidade de vida. A terapia é um sistema que sofre retroalimentação à medida que o paciente adere às práticas de autocuidado e o enfermeiro reforça as orientações a fim de atingir o objetivo maior que é a sensação de bem estar. A teoria de Dorothea Orem se adequou bem ao estudo, pois a terapia comportamental permitiu aos idosos a assumirem responsabilidade com o seu corpo e se empenharem efetivamente para melhorar a sua condição de saúde e qualidade de vida.

A transição demográfica e epidemiológica da população portuguesa tem e terá um enorme impacto na utilização dos recursos de saúde. Atualmente, as pessoas idosas representam um dos grupos etários com taxas de internamento hospitalar mais significativos. Contudo, os dados sobre a hospitalização destas pessoas têm demonstrado resultados de saúde negativos, nomeadamente, o declínio funcional e cognitivo e o risco elevado de eventos adversos. Os/as enfermeiros/as têm um papel crucial na mudança desta realidade. Deste modo, a associação entre o contexto no qual decorre o cuidado de enfermagem geriátrica e os resultados deste cuidado relativos a/os utentes, enfermeiros/as e organizações têm sido proficuamente documentados. Algumas estratégias para promover a qualidade do cuidado geriátrico e a segurança das pessoas idosas hospitalizadas consistem em avaliar e (re)criar o ambiente de trabalho geriátrico dos/as enfermeiros/as (AGTE) e capacitar e treinar estes/as profissionais no cuidado à pessoa idosa. Embora, internacionalmente, os dados demonstrem a associação entre as características de hospitais e/ou enfermeiros/as e o AGTE, não existem estudos em Portugal nesta área, bem como sobre o conhecimento e as atitudes destes profissionais no contexto hospitalar. Por conseguinte, este estudo teve como objetivos: 1) traduzir, adaptar culturalmente e validar as escalas que compõem o questionário Geriatric Institucional Assessment Proflie (GIAP) para a população portuguesa; 2) analisar o AGTE (fatores intrínsecos e extrínsecos) que apoiam ou dificultam a adoção das melhores práticas geriátricas em hospitais portugueses; 3) analisar as atitudes e conhecimento de enfermeiros/as acerca de quatro síndromes geriátricas (úlceras de pressão, distúrbio do sono, contenção física e incontinência), destacando as boas práticas e os problemas encontrados nos hospitais portugueses; 4) analisar a relação entre as variáveis demográficas, profissionais e as características dos hospitais e as escalas que compõem o GIAP – versão portuguesa; 5) conhecer as perceções de enfermeiros/as acerca do cuidado às pessoas idosas hospitalizadas e dos obstáculos enfrentados para desenvolver um cuidado de boa qualidade; e 6) analisar a relação entre a perceção de enfermeiros/as sobre o AGTE e o conhecimento e atitudes geriátricas destes profissionais em função da região e unidade de internamento. Este estudo foi desenvolvido com base num método quantitativo do tipo exploratório-descritivo, transversal, prospetivo e correlacional. A amostra foi constituída por 1.068 enfermeiros/as de cinco hospitais da região norte e centro do país. A recolha de dados foi desenvolvida através de autopreenchimento do GIAP – versão portuguesa. De entre os principais resultados destacam-se: 1) a obtenção de um instrumento válido e fiável para avaliar o AGTE e conhecimentos e atitudes geriátricas; 2) a perceção de enfermeiros/as sobre o cuidado às pessoas idosas como sendo predominantemente negativa; 3) a perceção de enfermeiros/as sobre o apoio insuficiente dos líderes hospitalares para promover um AGTE favorável; 4) o cuidado a pessoas idosas com comportamentos inadequados e o uso de recursos geriátricos como os principais fatores que influenciam a eficácia e a qualidade do cuidado geriátrico; 5) a lacuna de conhecimento e atitudes negativas de enfermeiros/as acerca das quatro síndromes geriátricas; 6) a conceptualização de um modelo sobre a associação das características de enfermeiros/as, dos hospitais do estudo e das perceções destes/as profissionais sobre o cuidado geriátrico com o AGTE e o conhecimento e atitudes geriátricos; 7) a falta de apoio familiar, a descontinuidade e a escassez de tempo para o cuidado como principais obstáculos no cuidado à pessoa idosa hospitalizada; e 8) o perfil de cuidado geriátrico nos hospitais da região norte e centro de Portugal como tendencialmente homogéneo. Os resultados deste estudo sustentam a necessidade de um maior investimento dos decisores políticos, administradores hospitalares e docentes de Enfermagem na capacitação dos/as enfermeiros/as para o cuidado geriátrico e na promoção de um AGTE mais favorável. Também oferece recomendações significativas nos domínios da decisão política, da gestão institucional e da prática profissional que devem ser alvo de uma discussão alargada entre os vários agentes com responsabilidade nestes domínios. Espera-se que este estudo possa contribuir para a promoção de um contexto favorável ao desenvolvimento de um cuidado de enfermagem geriátrica de boa qualidade às pessoas hospitalizadas.

The first purpose of this study was to describe the deaths of fifteen nursing home residents with late-stage dementia. The devastating effect of dementia on a person has been called a ‘living death’ (Woods, 1989). The caring which occurs when someone is going through this process in a nursing home was recorded and analysed. In analysing this act of caring, the second purpose was to look for the origins of the structures and the sources of pressure that shaped the context and therefore helped determine the behaviour of the various groups under observation. These groups were residents and their relatives, the staff of the nursing home and the treating doctors. Before commencing observations and carrying out this study, an understanding needed to be developed of: the condition of dementia as it is perceived by health professionals and presented in the media; the institutions in which the majority of people with dementia end their days; the background and conditions of the staff who nurse in them; the models of care that guide and determine policies; and the conceptions of life and death which underpin relevant laws and moral standpoints. Accordingly, in part 1. chapter 1 the history, causes, pathology and effects of dementing conditions are examined. Relevant medical and lay literature including media influences are examined which pertain to the subject of death and dementia and nursing home care. The history of this institutional care is briefly examined together with the growth of the discipline of gerontological nursing. Chapter 2 discusses some of the effects of this history on present day care and the concept of emotional work being carried out within the present day aged care public policy regulations. The moral arguments surrounding illness and dying in Australian society today are briefly discussed. Chapter 3 describes the conceptual framework for the study, the ethnographic method that has been employed and an outline of critical theory as the basis for analysis. The chapter concludes by recounting the practical steps taken to arrange the field work, secure the consent of participants, record data and gather documents, and outlines the ethical considerations given to the undertaking of the study. Chapter 4 describes the context in which the study took place and the first tentative exploration of the culture from an etic perspective. Part 2 describes the death trajectories observed and raises some questions about alternative strategies that may have been considered within a different paradigm of caring. In chapter 5 the death trajectories of each of the fifteen residents in the study are described, each written with a particular emphasis to illustrate aspects of the culture of care that emerged through thematic analysis. Observations, comments and feelings from staff and family are wound in and around these case studies. Chapter 6 looks more closely at the impact of policy and institutional pressure on the milieu in which these deaths took place. Part 3 draws conclusions from the observations and makes suggestions for emancipatory change as viewed from the author's standpoint of critical ethnographic analysis. In the final chapter an argument is presented for policy change that leads a movement towards palliative care practices for people with late-stage dementia. Approaches to implementation of palliative care will need to take account of any expression by the resident such as an advanced directive, indicating a preferred approach to treatment in the period prior to death; a need for a better understanding of such issues as the significance of body breakdown, the manifestations of pain and electrolyte imbalances; the surrounding ethical complexities and shift in public opinion, and perhaps, most of ail, the culture of the institutions in which this dying will take place. A definition of late-stage dementia which might be used in determining patterns of care is set out. A discussion about changes in practice which relate lo communication with treating doctors, the administering of antibiotics, the relief of pain, the mobilisation of residents and the provision of food and water takes place in light of the evidence found. The discussion of these issues is raised in the form of debate. Each aspect needs more rigorous analysis and information so that evidence-based practice, rather than care which is value-laden and emotional, can be used when treatment decisions are made for people with late-stage dementia.

This qualitative study was developed with 117 healthcare professionals and undergraduate students who were participants of a non-verbal communication training program in gerontology, with the objective to identify the perception and understanding of healthcare undergraduates and graduates regarding aging and the elderly. The results allowed the construction of the following categories: an evolution of life that brings a variety of experiences; natural, mysterious and with accumulated experiences; a decrease in overall happiness due to an increase in isolation; a time of valuing affection and respect; an inevitable stage including distress, prejudice, and limitations worthy of attention; a natural consequence of life; and the presence of disease and the global physiological process. Regarding perceptions of the elderly, the answers were classified as positive, negative, mixed and neutral. We considered that the understanding regarding the elderly and aging was limited and pessimist, indicating a need to become aware that one's views affect the way we relate to others.

The objective of this study was to describe, from the nurses' perspective, the meaning of effective/affective care, the interference factors, and the learning promoted from dealing with elderly inpatients, as well as their perception of feeling ready or not to engage in caring. This qualitative study was performed with nurses from a hospital located in the Midwestern region of the State of Sao Paulo. The findings show that the meaning of providing effective/affective care involves knowing the client within his/her social framework, while it crosses the borders of sheer technical care and encompasses fulfilling the client's needs and requirements. Interferences in dealing with the elderly refer to those situations connected with the patients' conditions, work dynamics, and environmental and management adaptations. All surveys showed that despite feeling well prepared, nurses are aware of the need to study fields such as geriatrics and gerontology to a deeper extent. Complete and quality care is only possible if it includes techniques, knowledge and the ability to deal with others with patience and attention to detail.

O objetivo deste estudo foi descrever, na visão da enfermeira, o significado do cuidado efetivo/afetivo, os fatores de interferência e o aprendizado promovido pela convivência com o idoso hospitalizado, bem como a percepção de sentir-se ou não preparada para cuidar. Estudo qualitativo desenvolvido com enfermeiras de um hospital do interior paulista. Entre as descobertas, identificou-se que o significado sobre a promoção do cuidado efetivo/afetivo envolve o conhecimento do cliente em seu contexto social; extrapola o cuidado técnico e envolve o atendimento das necessidades do cliente. As interferências na convivência com os idosos foram as ligadas à própria condição do idoso, à dinâmica do trabalho e às adaptações ambientais e administrativas. Todas as entrevistadas informaram que, embora se sintam preparadas, percebem a necessidade de estudar melhor a área de geriatria e gerontologia. O cuidado percebido como adequado e de qualidade é o que engloba técnica, conhecimento e o saber conviver com paciência e atenção.
