Ferreira de Castro (1898-1974) e Miguel Torga (1907-1995) viveram ambos, no início da adolescência, a dura experiência de emigração para o Brasil. O primeiro partiu com apenas doze anos, em 1911, o segundo, com treze, em 1920. Ambos procuraram o ―Eldorado‖, cruzaram o Atlântico num vapor, cresceram, amadureceram, regressaram a Portugal, revisitaram novamente o Brasil e escreveram sobre essas vivências, como é corroborado por Emigrantes e A Selva de Ferreira de Castro, o Diário, A Criação do Mundo, Traço de União de Miguel Torga que constituem o nosso corpus de trabalho. No presente artigo, analisaremos à luz da imagologia, um dos métodos da literatura comparada, que visa precisamente o estudo das imagens, as representações do Brasil que emergem da obra destes dois escritores. Nesta sequência, analisaremos, numa óptica comparatista, a ficcionalização das vivências dos autores, a trajectórias das suas personagens, contemplando, na configuração do espaço estrangeiro, as primeiras impressões e sua evolução, as descrições da paisagem, do povo, da vida e da cultura brasileiras. Além disso, seguiremos os caminhos da alteridade para desvendarmos igualmente o modo como é visto o ―outro‖, e a forma como se inscreve no discurso. Em suma, analisaremos o impacto da vivência da emigração, a importância desempenhada pelo país de acolhimento na obra dos dois escritores supramencionados, atendendo às ressonâncias da luso-brasilidade, alicerçadoras de uma maior abertura e dum diálogo mais próximo com o Brasil.

O presente estudo avaliou os padrões de consumo alimentar entre adultos brasileiros e a sua associação com o Índice de Massa Corporal (IMC). Em 1996/1997, o Instituto Brasileiro de Geografia e Estatística (IBGE), coletou dados antropométricos, de consumo alimentar e socioeconômicos de 9351 indivíduos entre 20 e 60 anos, moradores das áreas urbanas e rurais das regiões Nordeste e Sudeste do Brasil. Através da análise de componentes principais foram identificados os padrões de consumo alimentar. A associação entre os padrões de consumo alimentar e o IMC foi avaliado através de regressão linear. A prevalência de obesidade (IMC >= 30 kg/rn2) foi de 2,7% entre homens moradores das áreas rurais e em torno de 8,0% para os moradores da área urbana, em ambas as regiões. Entre as mulheres, esta prevalência na área rural foi de 6,5% no Nordeste, 14,3% no Sudeste e em torno de 12% nas áreas urbanas. A prevalência de sobrepeso (IMC>= 25 kg/m2) na área rural foi, aproximadamente, 20,0%para o sexo masculino e 24,0% para o sexo feminino, ficando ao redor de 30% nas áreas urbanas. Identificou-se três padrões de consumo: o padrão 1 (misto), com o consumo de quase todos os alimentos, o padrão 2, um padrão a base de arroz, farinha e feijo, composição característica da dieta tradicional do brasileiro, e o padrão 3, onde poucos alimentos explicaram a variação de consumo, contudo, estes alimentos variaram nas quatro áreas. Ajustando-se para idade, renda, escolaridade e atividade fsica, o padrão misto associou-se positivamente com o IMC (p<0,003), exceto no Sudeste rural (p=016). A dieta tradicional no Sudeste rural (p=0,007) e o padrão 3, composto por tubérculos, farinha e carne, no Nordeste urbano (p=O,0004), associaram-se negativamente com o IMC. Concluiu-se que o padrão misto se associou positivamente ao IMC, sugerindo que o consumo calórico total, mais do que o padrão da dieta, explicaria o aumento da obesidade observado no Brasil.

The development of a compact gamma camera with high spatial resolution is of great interest in Nuclear Medicine as a means to increase the sensitivity of scintigraphy exams and thus allow the early detection of small tumours. Following the introduction of the wavelength-shifting fibre (WSF) gamma camera by Soares et al. and evolution of photodiodes into highly sensitive silicon photomultipliers (SiPMs), this thesis explores the development of a WSF gamma camera using SiPMs to obtain the position information of scintillation events in a continuous CsI(Na) crystal. The design is highly flexible, allowing the coverage of different areas and the development of compact cameras, with very small dead areas at the edges. After initial studies which confirmed the feasibility of applying SiPMs, a prototype with 5 5 cm2 was assembled and tested at room temperature, in an active field-of-view of 10 10 mm2. Calibration and characterisation of intrinsic properties of this prototype were done using 57Co, while extrinsic measurements were performed using a high-resolution parallel-hole collimator and 99mTc. In addition, a small mouse injected with a radiopharmaceutical was imaged with the developed prototype. Results confirm the great potential of SiPMs when applied in a WSF gamma camera, achieving spatial resolution performance superior to the traditional Anger camera. Furthermore, performance can be improved by an optimisation of experimental conditions, in order to minimise and control the undesirable effects of thermal noise and non-uniformity of response of multiple SiPMs. The development and partial characterisation of a larger SiPM WSF gamma camera with 10 10 cm2 for clinical application are also presented.

Background: Bilateral mammaplasty or mastopexy is frequently used for oncoplastic objectives. However, little information has been available regarding outcome following immediate and delayed reconstruction. Method: Patients were divided into Group I (immediate reconstruction) and Group II (delayed reconstruction). Retrospective review was performed to compare complications, length of hospital stay, revision surgeries, and satisfaction. The associations between the complications with potential risk factors (timing, age, body mass index, smoking, and comorbid medical conditions) were analyzed. Results: There were a total of 144 patients with a mean follow-up of 47 months. Of the 106 patients in Group I, complications occurred in 24 (22.6%), skin necrosis was observed in 7.5%, fat necrosis in 5.6%, and 6.6% patients developed local recurrence. Mean period of hospitalization was 1.89 days. Of the 38 patients of the Group II, complications occurred in 12 (31.5%), skin necrosis was observed in 7 (18.4%), fat necrosis in 4 (10.5%), and 5.2% patients developed local recurrence. Mean period of hospitalization was 1.35 days. Increased length of hospital stay greater than 1 day (P < 0.001) and the number of revision surgeries (P = 0.043) were associated with the timing of the reconstruction. In univariate analysis, no difference between groups was found with respect to complication incidence (P = 0.275); however, after adjusting for other risk factors, the probability of complications tend to be higher for Group II (OR = 2.65; 95% confidence interval - 1.01-7.00; P = 0.049). Conclusions: On the basis of the results of our study, the probability of complications tends to be higher for delayed reconstructions, and it is demonstrated that obesity and smoking are risk factors for complications. Ultimately, these data may facilitate the provision of individualized risk information for shared medical decision-making.

Introduction: Although the use of local flaps in conservative breast surgery (CBS) reconstruction is a reliable technique, little information has been available regarding outcome following the use of perforator flaps. The purpose of this study is to analyze the feasibility, surgical planning and outcome following CBS reconstruction with intercostal artery perforator (ICAP) flap. Patients/methods: Thirteen patients underwent CBS reconstructions with an ICAP flap. These flaps were raised from adjacent tissue located on the lateral and thoracic region and based on perforators originating from the costal and muscular segment of the intercostal vessels. The technique was indicated in patients with small/moderate volume breasts. Results: Mean time of follow-up was 32 months. Flap complications were evaluated and information on patient satisfaction were collected. 61.5 percent had tumors located in the lower-outer quadrants and 69.2 percent had tumors measuring 2 cm or less (T1). Complications occurred in 3 patients (23%), including wound dehiscence in 2 patients and fat necrosis in one. All cases were treated by conservative approach with a good result. No flap loss or wound infection were reported. 90 percent were either satisfied or very satisfied with their result. Conclusion: The ICAP flap is a reliable technique for immediate CBS reconstruction. The technique is advantageous because it does not require the use of muscle transfer, with minimum donor site morbidity. Success depends on patient selection, coordinated planning with the oncological surgeon and careful intra-operative management. (C) 2011 Elsevier Ltd. All rights reserved.

Although the biodimensional anatomical expander-implant system (BEIS) is a reliable technique, little information has been available regarding outcome following nipple-areola sparing mastectomy (NSM). To perform the resection of glandular tissue, while improving the surgical access and maintaining the nipple-areola vascularization we have developed a new approach for NSM based on the double concentric periareolar incision (DCPI). The purpose of this study is to analyze the feasibility, surgical planning and its outcome following NSM. 18 patients underwent NSM reconstructions. Mean time of follow-up was 29 months. The technique was indicated in patients with small/moderate volume breasts. Flap complications were evaluated and information on aesthetic results and patient satisfaction were collected. 83.3% had tumors measuring 2 cm or less (T1) and 72.1% were stage 0 and 1. All patients presented peripherally tumors located (at least 5 cm from the nipple). Skin complications occurred in 11.1%. One patient (5.5%) presented small skin necrosis and a wound dehiscence was observed in one patient (5.5%). The aesthetic result was good/very good in 94.4 percent and the majority of patients were very satisfied/satisfied. No local recurrences were observed. All complications except one were treated by a conservative approach. DCPI-BEIS is a simple and reliable technique for NSM reconstruction. The success depends on patient selection, coordinated planning with the oncologic surgeon and careful intra-operative and post-operative management. (C) 2009 Elsevier Ltd. All rights reserved.

Introduction: Nerve allografting is regarded as a treatment of choice in large neural tissue losses preventing repair by primary anastomosis. In these cases, a synthetic polyglycolic acid tube is an alternative for nerve grafting. On the other hand, several studies have emphasized the importance of neurotrophic factors on neural regeneration, including substances with potential to optimize neural regeneration, especially the GM1, an neurotrophic enhancer factor. Objective: to compare, in rats, the neural regeneration degree using histological analysis, regenerated myelinized axons count, and functional analysis with the use of neurotube and GM1. Methods: This assessment was performed by interposing allograft (group A), polyglycolic acid tube (group B) and polyglycolic acid tube associated to GM1 (group C) on 5-mm sciatic nerve defects. Results: Neuroma formation was found only on group A. Groups A and C showed similar histological patterns, except for the regenerated axons on group C, which were shown to be better organized and myelinized than in group A. Conclusion: on functional recovery, no statistically significant difference was found for the three groups, despite of qualitative and quantitative histological differences found.

Relato de experiência ocorrida na Escola de Lajes, em Piraí, Rio de Janeiro, patrocinada pela Light Serviços de Eletricidade S.A., através de convênio com a Prefeitura. Analisada do ponto de vista financeiro e pedagógico, a experiência destaca-se como um exemplo de melhoria da qualidade do ensino público sustentado por recursos empresariais, remetendo à necessidade de revisão da legislação sobre salário-educação para viabilizar iniciativas semelhantes.

Este estudo busca examinar a PoIítica de Educação Básica da Nova República mediante a análise de suas propostas fundamentais - formulação e implementação - nos dois primeiros anos desse governo ~ 1985 e ·1986. Faz urna análise das decisões políticas governamentais de educação básica no processo sócio-politico-econômico que as bem como das implicações dessas decisões no quadro das deficiências que intencionava-se mudar. Baseia-se, " dessa forma, n o discurso do governo de proporcionar "uma educação voltada para o social e o econômico do Pais" e de se atribuir à educação o significado de "instrumento eficaz para o resgate da cidadania e redução do fosso de injustiça social e das desigualdades existentes na sociedade brasileira".

Background: Health-related quality of life (HRQOL) measurements provide valuable information about the psychological and social impact of treatment on patients with cystic fibrosis (CF). This study evaluated the HRQOL of Brazilian patients with CF and assessed the changes in HRQOL domains over 1 year after dornase alfa (Pulmozyme) introduction. Patients and Methods: One hundred fifty-six stable patients with CF and 89 caregivers answered the Portuguese-validated version of the Cystic Fibrosis Questionnaire-Revised (CFQ-R) at baseline (T 0), and at 3 (T 1), 6 (T 2), 9 (T 3), and 12 (T 4) months of follow-up. Eighteen patientswere excluded because they did not fulfill the inclusion criteria. The patients were analyzed in two groups: those aged 6-11 years and those aged 14 years and older. ANOVA for observed repeated results and the last observation carried forward (LOCF) method for missing data were used for the statistical analysis. Results: After 1 year of follow-up, there was significant improvement in respiratory symptoms (T 4-T 0=8.1; 95% confidence interval (95% CI)=[2.1;14.0]; effect size (ES)=0.35; P<0.001), Emotional Functioning (T 4-T 0=5.6; 95% CI=[1.1;10.1]; ES=0.31; P<0.05), Social Functioning (T 4-T 0=6.0; 95% CI=[1.3;11.7]; ES=0.31; P<0.05), Body Image (T 4-T 0=11.9; 95% CI=[4.1;19.7]; ES=0.42; P<0.05), and Treatment Burden (T 4-T 0=5.3; 95% CI=[0.3;10.3]; ES=0.24; P<0.05) domains in the younger group. A significant improvement in Role Functioning (T 4-T 0=6.1; 95% CI=[1.1;11.1]; ES=0.40; P<0.05), Body Image (T 4-T 0=12.6; 95% CI=[3.5;21.7]; ES=0.46; P<0.05), and Weight (T 4-T 0=11.7; 95% CI=[1.8;21.6]; ES=0.40; P<0.05) was obtained in the older group. The caregivers' CFQ-R showed improvements in the Digestive Symptoms (T 4-T 0=5.5; 95% CI=[1.5;9.4]; ES=0.30; P<0.05), Respiratory Symptoms (T 4-T 0=7.6; 95% CI=[3.9;11.4]; ES=0.48; P<0.05), and Weight (T 4-T 0=10.1; 95% CI=[1.6;18.6]; ES=0.26; P<0.05) domains. Conclusion: The introduction of dornase alfa improved the HRQL of the patients with CF during the first year of treatment. © 2010 Wiley-Liss, Inc.
