Objectives To determine whether a home based exercise programme can improve outcomes in patients with knee pain. Design Pragmatic, factorial randomised controlled trial of two years' duration. Setting Two general practices in Nottingham. Participants 786 men and women aged >45 years with self reported knee pain. Interventions Participants were randomised to four groups to receive exercise therapy, monthly telephone contact, exercise therapy plus telephone contact, or no intervention. Patients in the no intervention and combined exercise and telephone groups were randomised to receive or not receive a placebo health food tablet. Main outcome measures Primary outcome was self reported score for knee pain on the Western Ontario and McMaster universities (WOMAC) osteoarthritis index at two years. Secondary outcomes included knee specific physical function and stiffness (scored on WOMAC index), general physical function (scored on SF36 questionnaire), psychological outlook (scored on hospital anxiety and depression scale), and isometric muscle strength. Results 600 (76.3%) participants completed the study. At 24 months, highly significant reductions in knee pain were apparent for the pooled exercise groups compared with the nonexercise groups (mean difference –0.82, 95% confidence interval –1.3 to –0.3). Similar improvements were observed at 6, 12, and 18 months. Regular telephone contact alone did not reduce pain. The reduction in pain was greater the closer patients adhered to the exercise plan. Conclusions A simple home based exercise programme can significantly reduce knee pain. The lack of improvement in patients who received only telephone contact suggests that improvements are not just due to psychosocial effects because of contact with the therapist.

One in five adults 65 years and older has diabetes. Coping with diabetes is a lifelong task, and much of the responsibility for managing the disease falls upon the individual. Reports of non-adherence to recommended treatments are high. Understanding the additive impact of diabetes on quality of life issues is important. The purpose of this study was to investigate the quality of life and diabetes self-management behaviors in ethnically diverse older adults with type 2 diabetes. The SF-12v2 was used to measure physical and mental health quality of life. Scores were compared to general, age sub-groups, and diabetes-specific norms. The Transtheoretical Model (TTM) was applied to assess perceived versus actual behavior for three diabetes self-management tasks: dietary management, medication management, and blood glucose self-monitoring. Dietary intake and hemoglobin A1c values were measured as outcome variables. Utilizing a cross-sectional research design, participants were recruited from Elderly Nutrition Program congregate meal sites (n = 148, mean age 75). ^ Results showed that mean scores of the SF-12v2 were significantly lower in the study sample than the general norms for physical health (p < .001), mental health (p < .01), age sub-group norms (p < .05), and diabetes-specific norms for physical health (p < .001). A multiple regression analysis found that adherence to an exercise plan was significantly associated with better physical health (p < .001). Transtheoretical Model multiple regression analyses explained 68% of the variance for % Kcal from fat, 41% for fiber, 70% for % Kcal from carbohydrate, and 7% for hemoglobin A 1c values. Significant associations were found between TTM stage of change and dietary fiber intake (p < .01). Other significant associations related to diet included gender (p < .01), ethnicity (p < .05), employment (p < .05), type of insurance (p < .05), adherence to an exercise plan (p < .05), number of doctor visits/year ( p < .01), and physical health (p < .05). Significant associations were found between hemoglobin A1c values and age ( p < .05), being non-Hispanic Black (p < .01), income (p < .01), and eye problems (p < .05). ^ The study highlights the importance of the beneficial effects of exercise on quality of life issues. Furthermore, application of the Transtheoretical Model in conjunction with an assessment of dietary intake may be valuable in helping individuals make lifestyle changes. ^

On June 24, 2010, the Rebuild Iowa Office (RIO) held a discussion-based tabletop exercise for the purpose of creating a framework to support disaster recovery coordination within the State of Iowa. The purpose of this report is to provide an overview of the tabletop exercise, summarize and analyze exercise results, identify strengths which should be maintained and built upon, identify areas for further improvement, and support development of recommendations and corrective actions.

On June 24, 2010, the Rebuild Iowa Office (RIO) held a discussion-based tabletop exercise for the purpose of creating a framework to support disaster recovery coordination within the State of Iowa. The exercise design team was composed of RIO and Iowa Homeland Security and Emergency Management Division (HSEMD) staff. Initial planning for the Iowa Disaster Recovery Tabletop Exercise began in February 2010, in response to recommendations by the Rebuild Iowa Advisory Commission and the Rebuild Iowa Coordinating Council. Both of these groups were organized in direct response to the 2008 Iowa disasters to help establish a path for Iowa’s recovery and monitor progress toward meeting established goals. Recommendations included formalizing the responsibilities of the RIO as they relate to 2008 disasters, capturing lessons learned, and determining what a recovery model should look like for Iowa’s future disasters.

In March 2015, over 80 people, representing food banks, churches, advice services, community organisations, statutory agencies and universities attended the ‘Enough is Enough’ launch event in City Church, Belfast to examine the rising demand for emergency food across the city.The ‘Enough is Enough’ project aims to harness the expertise of health and social care professionals, city councillors, advice workers, food banks, community and faith based organisations and strategic bodies across Belfast to collectively address the issue of food poverty. This scoping study lays the foundation for developing an action plan to tackle food poverty in Belfast in collaboration with the community, voluntary and statutory sectors.The Belfast Food Network (BFN) commissioned the project with funding from the Public Health Agency. The research was carried out by Jenny McCurry, who also wrote the report, on behalf of Advice NI. The project was initiated and developed by Dr Elizabeth Mitchell, Institute of Public Health in Ireland, in her role as convener of the BFN Food Poverty Working Group (BFN/FPWG). Thanks are due to Kevin Higgins, Head of Policy, Advice NI, and Kerry Melville, Co-ordinator, BFN, for their involvement in the project.The BFN is a founding member of the pioneering Sustainable Food Cities Network (SFC). The scientific evidence, demonstrates that the obesity reduces the possibility that a woman, conceive naturally and diminishes the success of the treatment for the fertility. Nurses of the Unit of Attended Reproduction and of the Units of Nutrition and Dietary, they consider necessary to design an educational program of Formation and Information, to change attitudes related with the overweight and the obesity, in even not fertile. The strategy of this Program is based in the control and reduction of the weight, assuring the adhesion to the treatment of fertility, with interactive works shops for the change of alimentary habits, giving emotional support and indications to practice adapted physical exercise. This Nursing Intervention is applied like one tool that develops abilities for the learning and training; never using diets or products of the denominated miracle. The narrow collaboration and investigating participation among the nurses of these Units, bear the design of a methodology channeled to the obtaining of some prospective results that can be evaluated regarding the obtained results.

A través del uso de distintas metodologías para la elaboración de un plan exportador, y con la ayuda y guía de nuestro director Andrés Castro, se ha elaborado un análisis integral que le permitirá a la empresa Improquisa S.A., tener algunos elementos clave a su disposición en el momento en que deseen aprobar su plan de exportación o elaborar uno propio. Se inicia entonces con un análisis del sector de plásticos en Colombia, identificando las empresas que hacen parte del sector y que se destacan por tener ingresos operacionales por encima del nivel de las otras empresas. Así se encuentra que hay pocas empresas en pocos lugares de Colombia que conservan estas características. De la misma manera se identifican cuales son las empresas que exportan y hacia que destinos en años anteriores. Luego, pasamos a observar la empresa en su totalidad, pasando desde un planteamiento de su estrategia competitiva hasta un análisis financiero, pasando por todas las áreas de la empresa puesto que una exportación, al ser una actividad integral, involucra a la empresa en su totalidad y por esto se debía analizar completamente. El siguiente paso es entonces decidir a qué mercado exportar y las razones por las cuales se le da prioridad a ese destino por encima de los demás. Dicho ejercicio es realizado con la metodología de Andrés Castro en la que se toman tres potenciales mercados y, a través de la calificación y análisis de variables, se identifica cuál se adapta mejor a las necesidades y capacidades de la empresa. Finalmente se identifican clientes potenciales en el mercado objetivo y se mencionan estrategias claves que pueden ser utilizadas por la empresa para la consecución del plan exportador. Esperamos entonces que este ejercicio sea de gran utilidad para la empresa Improquisa S.A.
