Despite abundant research on work meaningfulness, the link between work meaningfulness and general ethical attitude at work has not been discussed so far. In this article, we propose a theoretical framework to explain how work meaningfulness contributes to enhanced ethical behavior. We argue that by providing a way for individuals to relate work to one's personal core values and identity, work meaningfulness leads to affective commitment - the involvement of one's cognitive, emotional, and physical resources. This, in turn, leads to engagement and so facilitates the integration of one's personal values in the daily work routines, and so reduces the risk of unethical behavior. On the contrary, anomie, that is, the absence of meaning and consequently of personal involvement, will lead to lower rational commitment rather than affective commitment, and consequently to disengagement and a-morality. We conclude with implications for the management of ethical attitudes.

Background: This paper details contemporary ethical dilemmas encountered by Queensland dentists. Methods: An age-stratified sample of 499 dentists resident in Queensland was surveyed. The questionnaire contained scenarios of five common ethical dilemmas. In addition, open-ended questions sought the respondent's most frequent, difficult and recent ethical dilemmas, and where they would seek guidance in dealing with ethical problems. Results: Respondents acknowledged the patient's rights in treatment decisions and the dentist's right to refuse demands for inappropriate treatment. However, responses varied in the extent to which dentists may influence treatment decisions. Few respondents would ignore evidence of poor dental treatment but they are evenly divided in choosing to inform the patient, the dentist or both. Poor quality treatment is the most frequent and difficult dilemma, and half have experienced this problem recently. Requests by patients for fraudulent receipts occur in a third of responses. Dentists develop ethical values from multiple sources but for help with dental ethical problems, 90 per cent of respondents would consult another dentist. Conclusions: Of the ethical dilemmas discussed in this survey, those relating to poor quality treatment confronted most respondents. Also the actions of dentists in dealing with these dilemmas were most varied.

Due to actual demographic evolution, emergency departments have to face a dramatic increase in admissions of elderly people. The peculiar medical and socio-demographic characteristics of these old patients emphasize the need of specific decision processes and resources allocation. An individual-based approach, related to significant ethical values, should allow better diagnostic and therapeutic attitudes. Such a way to admit, evaluate and treat older patients implies an active collaboration with patients and their relatives, but also with all medical interveners, including in particular primary care physicians.

Na concepção deste trabalho, que tem como tema O texto literário na aula de Português L2 – Uma proposta de didactização: Vida e Morte de João Cabafume, de Gabriel Mariano, procurámos investigar e reflectir sobre a abordagem do texto literário no 3º ciclo do ensino secundário cabo-verdiano, especificamente na aula de Português L2, nas áreas de estudos Científica e Tecnológica e Económica e Social, do 11º ano de escolaridade, seguida da apresentação, a título de exemplo, de uma proposta de didactização do texto acima mencionado, na perspectiva da utilização do texto literário como material em que o uso estético da língua potencia a dimensão plural da significação. Para a consecução dos objectivos propostos, procurámos dar visibilidade, na perspectiva da leitura do texto literário, às orientações do programa, às potenciais práticas pedagógicas no ensino de narrativa literária, incluindo os métodos de ensino, os recursos utilizados pelos professores, a forma como os alunos encaram todo o processo de leitura do texto literário em que está envolvido. No entanto, para a concepção da proposta de didactização, procurámos cruzar propostas metodológicas de modelos de leitura e métodos de ensino do texto narrativo e de línguas defendidos por vários autores do campo da didáctica da literatura e das línguas, cujos princípios assentam em bases que valorizam a interacção entre o texto e o leitor. Partindo de um conto de Gabriel Mariano, escritor cabo-verdiano, Vida e Morte de João Cabafume, tentámos propor um roteiro de leitura adequada do texto narrativo, de forma a conduzir o aluno não só na leitura do contexto narrativo, como também na compreensão da dimensão plural da língua enquanto material estético, procurando desenvolver, fundamentalmente, as suas atitudes, os valores éticos e morais e, num âmbito complementar, as suas competências da escrita e da oralidade. PURPOSE OF REVIEW: This update reviews the concepts underlying ethical issues in various contexts and countries, highlighting the evolution in the use of the core values underpinning the field and practice of bioethics as applied to healthcare. RECENT FINDINGS: It stresses the specific position of the adolescent as being a unique individual searching for autonomy and, most of the time, being competent to make decisions regarding the adolescent's own health. It briefly outlines the principles of a 'deliberative' approach in which the practitioner, while keeping in mind the legal context of the country where the practitioner is working, assesses to what extent the adolescent can be considered as competent, and then discusses with the adolescent the medical and psychosocial aspects of the various actions to be taken in a situation, as well as the basic ethical values linked with each of the various options available. The deliberation can involve relevant stakeholders, provided the issues concerning confidentiality have been fully discussed with the adolescent. SUMMARY: This process forces the practitioner, the adolescent patient and those who care for the adolescent patient to look outside their usual frameworks and make a decision that is in the best interest of the young person, and is informed by various ethical values.

The competence of graduating nursing students is an important issue in health care as it is related to professional standards, patient safety and the quality of nursing care. Many changes in health care lead to increased demand with respect to nurses’ competence as well the number of nurses. The purpose of this empirical study was to i) describe the nurse competence areas of nursing students in Europe, ii) evaluate the nurse competence of graduating nursing students, iii) identify factors related to the nurse competence, and to iv) assess the congruence between graduating nursing students’ self-assessments and their mentors’ assessments of students’ nurse competence. The study was carried out in two phases: descriptive phase and evaluation phase. The descriptive phase focused on describing the nurse competence areas of nursing students in Europe with the help of a literature review (n=10 empirical studies and n=4 additional documents). Thematic analysis was used as the analysis method. In the evaluation phase, the nurse competence with particular focus on nursing skills of graduating nursing students (n=154) was assessed. In addition, factors related to the nurse competence were examined. Also, the congruence between graduating nursing students’ self-assessments and their mentors’ assessments of students’ nurse competence was evaluated by comparing graduating nursing students’ self-assessments with the assessments by their mentors (n=42) in the final clinical placement in four university hospitals. Descriptive statistics and inferential statistics were used to analyse the data. Based on the results, the nurse competence of nursing students in Europe consists of nine main competence areas: (1) professional/ethical values and practice, (2) nursing skills and interventions, (3) communication and interpersonal skills, (4) knowledge and cognitive ability, (5) assessment and improving quality in nursing, (6) professional development, (7) leadership, management and teamwork, (8) teaching and supervision, and (9) research utilization. Graduating nursing students self-assessed their nurse competence as good. However, when graduating nursing students’ nurse competence was assessed by their mentors, the results were poorer. Readiness for practice based on nurse education, pedagogical atmosphere on the ward, supervisory relationship between student and mentor and being in paid work in health care at the moment of the study were the most significant factors related to the nurse competence. Conclusions: Nurse competence can be evaluated with a scale based on self-assessment, but other evaluation methods could be used alongside to ensure that nurse competence can be completed and evaluated critically. Practical implications are presented for nurse education and nursing practice. In future, longitudinal research is needed in order to understand the development of nurse competence during nurse education and the transition process from a nursing student to a professional nurse.

The study seeks answer to the following questions: 1. What is ethos in an occupational healthcare context? 2. What does it mean for occupational healthcare nurses to fulfill their inner ethos in a healthcare context controlled by economic demands of gain and efficiency? The main concept in this study is ethos as Eriksson describes it in her caritative theory of caring. Ethos is associated with ethics and reflects the fundamental assumptions that we have about the human being´s holiness and dignity and about the inviolability of life. The empirical part of the study consists of focus interviews with four occupational healthcare nurses. The study uses hermeneutical reading as an interpretation method, and presents the results of the study in six theses reflecting these against both recent research and the theoretical background. The results of the study show that ethos in occupational healthcare has to do with justice, honesty and faithfulness. These concepts are common to nurses in different nursing contexts. Ethos is not primarily profession-bound but is something universal, and eternal in the human being´s way of being and becoming. The study shows that ethos is a way of being, openness and a way of existing in love. To fulfill ethos in an occupational health context means to choose ethos continuously and courageously for the sake of the patient and the good, even if it involves a struggle or a sacrifice.

Background: In Honduras, research capacity strengthening (RCS) has not received sufficient attention, but an increase in research competencies would enable local scientists to advance knowledge and contribute to national priorities, including the Millennium Development Goals (MDGs). Objective: This project aimed at strengthening research capacity in infectious diseases in Honduras, focusing on the School of Microbiology of the National Autonomous University of Honduras (UNAH). The primary objective was the creation of a research-based graduate program for the continued training of researchers. Parallel objectives included institutional strengthening and the facilitation of partnerships and networks. Methods: Based on a multi-stakeholder consultation, an RCS workplan was designed and undertaken from 2007 to 2012. Due to unexpected adverse circumstances, the first 2 years were heavily dedicated to implementing the project's flagship, an MSc program in infectious and zoonotic diseases (MEIZ). In addition, infrastructure improvements and demand-driven continuing education opportunities were facilitated; biosafety and research ethics knowledge and practices were enhanced, and networks fostering collaborative work were created or expanded. Results: The project coincided with the peak of UNAH's radical administrative reform and an unprecedented constitutional crisis. Challenges notwithstanding, in September 2009, MEIZ admitted the first cohort of students, all of whom undertook MDG-related projects graduating successfully by 2012. Importantly, MEIZ has been helpful in expanding the School of Microbiology's traditional etiology-based, disciplinary model to infectious disease teaching and research. By fulfilling its objectives, the project contributed to a stronger research culture upholding safety and ethical values at the university. Conclusions: The resources and strategic vision afforded by the project enhanced UNAH's overall research capacity and its potential contribution to the MDGs. Furthermore, increased research activity and the ensuing improvement in performance indicators at the prime Honduran research institution invoke the need for a national research system in Honduras.

Cette recherche constitue une incursion dans la bioéthique de l’Asie. Elle vise à vérifier s’il existe une approche de la bioéthique spécifique pour l’Asie et, s’il s’avère que tel est le cas, d’identifier les particularités les plus apparentes de la nature de cette ‘bioéthique asiatique’. La recherche a été réalisée à même des sources documentaires constituées d’un corpus d’articles publiés entre 2001 et 2005, dans les versions électroniques de revues scientifiques reconnues. La recherche conclut à l’existence d’une ‘bioéthique asiatique’ qui accorde à certaines valeurs humaines une prédominance spécifique, de même qu’un sens différent qui lui est propre. Le lien, entre la culture et les traditions morales locales d’une part, et l’ordre de prédominance et l’interprétation des valeurs éthiques d’autre part, doit être reconnu et pris en compte pour que la bioéthique soit pertinente et applicable aux personnes touchées par les enjeux évalués en Asie. La recherche identifie également quatre particularités de la ‘bioéthique asiatique’ qui constitue le début d’une tentative pour en cerner la nature : le rapport entre ‘soi’ et les autres, la compassion, l’harmonie, de même qu’une conception spirituelle du monde dont la base est l’interdépendance et l’interconnexion d’un ordre universel entrelacé et participatif.

À l’instar de nombreux pays en développement, la prévention et la prise en charge du paludisme présente occupent une place centrale dans les politiques publiques en Côte d’ivoire. En vue de fournir des outils d’aide à la décision aux acteurs politiques, plusieurs études sur l’évaluation de l’efficacité et des coûts des programmes sont régulièrement engagées. Toutefois, très peu d’études se sont penchées sur les dimensions éthiques de ces programmes. En conséquence, nous avons choisi d’explorer dans une perspective éthique les programmes de prévention et de prise en charge du paludisme en Côte d’Ivoire à travers une analyse documentaire descriptive. La méthode qualitative fut retenue à cet effet. Ce mémoire vise à démontrer que la lutte contre le paludisme est une entreprise qui intègre des valeurs éthiques implicites notamment de solidarité et de promotion de la vie en santé. En outre, des mesures particulières notamment de gratuité d’accès aux services de prévention et de prise en charge sont prévues dans le but de prendre en compte les populations vulnérables face au paludisme. Le chapitre d’analyse nous permettra de montrer comment ces dispositions présentent des limites en ce qui concerne l’équité. Enfin, nous allons démontrer que ces dispositions spécifiques sont susceptibles in fine de générer voir d’accentuer les risques d’inégalités sociales.
