The objective of the research was to evaluate the effect of a training program of physical exercises mixed (aerobic / anaerobic conditioning circuit-break with active), including exercises, walking, weight training and Swiss ball, caused about possible changes in their metabolic system, as changes acute and chronic, individuals bearers of DM2. The methodology used is a model of experimented design of pretest and post-test applied to the group. The blood glucose levels were analyzed in fasting pre-test and post-test, and capillary glycemia in three periods of physical exercise: pre, during and post training, and the statistical processing done by the use of Statistical Software for PC-For SPSS ® Windows ®, V. 12.0 .. With the result has been a drop in the values significant glucometers between 6 weeks of training, with a negative delta average of 68 mg / dl. Furthermore, similar results were also found variations in the daily glycemic between the moments before and after the training session acute. Thus we find that, regardless of the biological mechanism responsible, the program mixed aerobic and anaerobic conditioning circuit-break with active was effective in reducing and controlling chronic and acute glycemic of subjects type 2 diabetic.

A Diabetes Mellitus Gestacional (DMG) pode ser definida como intolerância a carboidrato durante a gravidez e estima-se que pode afetar 10-22% de todas as pacientes grávidas. Durante a gravidez podem surgir diversas complicações para o feto como risco elevado de aborto espontâneo, anormalidades congênitas e morbidade e mortalidade neonatal. Entretanto, podem surgir também alterações morfofuncionais em diversos órgãos da mãe diabética, porém isso não é bem estabelecido. Investigar se haverá ou não alterações bioquímicas e histopatológicas em diversos órgãos, como hipófise, útero, placenta e pâncreas de ratas grávidas com diabetes mellitus durante e no final da gravidez e compará-las . Além disso, investigar se há alteração na matriz extracelular (MEC) da hipófise desses animais. No 5 dia de vida, ratas Wistar foram divididas em dois grupos: um tratado com estreptozotocina (Grupo Diabético / DIAB), na dose de 90 mg/kg, subcutâneo e outro grupo, que foi tratado com veículo (tampão citrato/CTR). Aos 90 dias de vidas, foram submetidas ao cruzamento. Após isso, foram sacrificadas no 11 e 21 dia de gravidez. Foram avaliados glicemia e bioquímica maternal e número de implantes .O pâncreas, útero, placenta e hipófises foram coradas com Hematoxilina e Eosina e somente as hipófises foram coradas com Massom e Picrosirius, para avaliação da MEC.Os animais diabéticos tanto do 11 quanto do 21 dia apresentaram uma redução no número de implantes, menor peso e maior glicemia e colesterol total, em relação aos animais controle independente do dia da gravidez. Não foi verificada diferença dos níveis de triglicerídeos entre os grupos não diabéticos e diabéticos, independente dos dias. Entretanto, os animais diabéticos que finalizaram o período de gestação apresentaram uma maior glicemia maternal em relação ao grupo diabético do 11 dia. Pâncreas de ratas diabéticas do 21 dia apresentaram vacuolização intracitoplasmática das ilhotas, insulite,migração de células inflamatórias, espessamento da parede do vaso e fibrose periductal e vascular. Essas alterações foram verificadas com bem menor intensidade nos animais diabéticos do 11 dia. Foi verificado que a placenta de animais diabéticos apresenta congestão na interface materno-fetal, migração celular, maior concentração de vasos maternos e fetais, mas em forma irregular , necrose e vacuolização. A hipófise de animais diabéticos mostraram células cromófobas agregadas, aumento da espessura de fibras de colágeno vermelhas da MEC, em contraste com o controle, que foi visualizado fibras em verde e em formato de feixe. A diabetes desempenhou um total remodelamento da hipófise. Gravidez de animais diabeticos mostraram maior dano ao pâncreas e placenta, especialmente no final da gravidez. Em consequência dessa alterações, esses animais diabéticos apresentaram hiperglicemia, maior colesterol total, porém menor peso materno, número de implantes e sem alterações nos triglicerídeos. Esse é o primeiro estudo a demonstrar remodelamento tecidual em alguns elementos da MEC na hipófise, como espessamento da camada da MEC e fibras de colágeno em verde. Alterações da MEC da hipófise são provavelmente devido ao processo de diabetes na gestação.

BACKGROUND: Diabetics have a significantly higher percentage of sperm with nuclear DNA (nDNA) fragmentation and increased levels of advanced glycation end products (AGEs), in their testis, epididymis and sperm. As the receptor for AGEs (RAGE) is important to oxidative stress and cell dysfunction, we hypothesise, that it may be involved in sperm nDNA damage. METHODS: Immunohistochemistry was performed to determine the presence of RAGE in the human testis and epididymis. A comparison of the receptor's incidence and localisation on sperm from 10 diabetic and 11 non-diabetic men was conducted by blind semi-quantitative assessment of the immunostaining. ELISA analysis ascertained RAGE levels in seminal plasma and sperm from 21 diabetic and 31 non-diabetic subjects. Dual labelling immunolocalisation was employed to evaluate RAGE's precise location on the sperm head. RESULTS: RAGE was found throughout the testis, caput epididymis, particularly the principle cells apical region, and on sperm acrosomes. A Diabetes Mellitus (DM) compreende um conjunto de desordens metabólicas comuns caracterizadas por hiperglicemia, que afeta diferentes órgãos do organismo. Ao longo do tempo, ocorrem danos microvasculares no glomérulo renal, retina e nervos periféricos, bem como doença macrovascular nas artérias. A composição da saliva também é afetada pela DM, com consequências na homeostasia oral. No entanto, o proteoma e o peptidoma salivar têm sido pouco explorados na DM tipo 1 e nas suas complicações crónicas. Tendo em conta o crescente interesse na saliva como fluido diagnóstico, o objetivo principal deste trabalho foi avaliar os eventos proteolíticos subjacentes à DM tipo 1 e às suas complicações microvasculares, bem como, caracterizar as alterações induzidas pela DM tipo 1 no proteoma e peptidoma salivar. A DM tipo 1 e particularmente as complicações microvasculares associadas modulam o perfil proteolítico dos fluidos biológicos, com diferenças significativas de atividade observadas na urina e saliva, atribuídas principalmente ao complexo Metaloproteinase da Matriz (MMP)-9/lipocalina associada à gelatinase de neutrófilos, aminopeptidase N, azurocidina e calicreína 1. O aumento da atividade proteolítica observado na saliva total dos diabéticos resultou no aumento da percentagem de péptidos, principalmente de um número acrescido de fragmentos de colagénio do tipo I, refletindo possivelmente um estado inflamatório crónico dos tecidos orais e periodontais. O peptidoma também corrobora uma maior suscetibilidade das proteínas salivares, especificamente, das proteínas ricas em prolina básicas (bPRP) 1, bPRP2 e proteínas ricas em prolina ácidas (aPRP) à proteólise, evidenciando a geração de fragmentos de proteínas associadas à ligação a bactérias. A análise do proteoma salivar baseada em iTRAQ mostrou uma sobre-expressão de L-plastina, fator do adenocarcinoma do pâncreas e das proteínas S100-A8 e S100-A9, enfatizando a importância do sistema imune inato na patogénese da DM tipo 1 e das complicações microvasculares associadas. A análise integrada de todas as proteínas expressas diferencialmente entre os pacientes diabéticos com ou sem complicações microvasculares e indivíduos saudáveis foi realizada com o STRING, onde se observam três conjuntos funcionalmente ligados, um compreende a interação entre o colagénio tipo I, colagénio tipo II e MMP-9, um segundo conjunto envolve a MMP-2 e o colagénio de tipo I e um terceiro conjunto composto por proteínas salivares e inflamatórias. Estes conjuntos estão associados com as vias Kegg de interação recetor-matriz extracelular, de adesão focal e migração transendotelial dos leucócitos. Por outro lado, a análise do proteoma e peptidoma salivar destacou potenciais biomarcadores para o diagnóstico e prognóstico da DM tipo 1 e das suas complicações.

RESUMO - A (não) adesão à terapêutica é um problema mundial que, para além de afetar diretamente a saúde dos indivíduos, afeta também os recursos económicos e sociais. Apesar da importância em aderir à terapêutica, largamente fundamentada na evidência científica, facto é que, em muitos casos, os doentes não o fazem. Assim, estudar as questões relacionadas com a adesão tem vindo a ganhar cada vez mais relevância e atualmente constitui um desafio para os sistemas de saúde, pois os mecanismos envolvidos no comportamento de adesão dos indivíduos são complexos. Deste modo, com este estudo pretendeu-se caracterizar uma amostra de indivíduos com diabetes mellitus tipo 2 e relacionar o seu nível de adesão à terapêutica medicamentosa com os fatores de não adesão. Este estudo foi desenvolvido no Agrupamento de Centros de Saúde (ACES) Almada e Seixal e nele participaram 151 diabéticos tipo 2. Para a recolha de dados utilizou-se o Questionário de Identificação de Fatores de Não Adesão adaptado de Cabral e Silva (2010) e a Medida de Adesão aos Tratamentos de Delgado e Lima (2001). Os resultados mostraram um perfil de participantes maioritariamente idosos, reformados, do género feminino, casados, com o ensino primário completo, rendimento mensal entre 301-1000 euros e tendencialmente aderentes à terapêutica medicamentosa. O (i) esquecimento, o (ii) preço da medicação, o (iii) número elevado de medicamentos para tomar de uma vez só, não perceber bem o que deve tomar e como e (v) adormecer antes da toma foram os fatores de não adesão relatados com mais frequência. Foram encontradas relações significativas entre o nível de adesão e o esquecimento, o preço da medicação o número elevado de medicamentos para tomar de uma vez só e adormecer antes da toma. Não encontrámos relações significativas entre o nível de adesão e os dados sociodemográficos, os fatores terapêuticos e o fator de não adesão “não perceber bem o que deve tomar e como”. No presente estudo são discutidos os resultados obtidos, consideradas algumas limitações e efetuadas propostas de investigações futuras.

To investigate the prevalence and risk factors of perceived diabetes-related discrimination in the workplace and in work-related insurances in persons with diabetes mellitus in Switzerland. 509 insulin-treated diabetic subjects representative of the northwestern Swiss population responded to a self-report questionnaire on perceived diabetes-related discrimination in the workplace and in work-related insurances (salary loss insurance, supplementary occupational plan). Discrimination was defined as being treated differently at least once in relation to diabetes. The reported rates of different aspects of discrimination in the workplace and in work-related insurances ranged between 5-11% and 4-15% respectively. Risk factors that independently increased the risk of not being hired due to diabetes were the presence of at least two severe hypoglycaemic events/year and relevant diabetic complications (OR 5.6 and OR 2.6 respectively; both<0.05). The presence of at least two severe hypoglycaemic events/year was also associated with an increased risk of losing one's job (OR 6.5, <0.01). Overweight or obesity were related to increased discrimination in work-related insurances (OR for denial 2.1-2.4; OR for reserve 3.9-4.4; all<0.05). Perceived diabetes-related discrimination in the workplace and by work-related insurances is a common problem. In the light of our findings the introduction of effective non-discrimination legislation for patients with chronic illnesses appears to be desirable.

To examine the association between sleep disorders, obesity status, and the risk of diabetes in adults, a total of 3668 individuals aged 40+ years fromtheNHANES 2009-2010 withoutmissing information on sleep-related questions,measurements related to diabetes, and BMI were included in this analysis. Subjects were categorized into three sleep groups based on two sleep questions: (a) no sleep problems; (b) sleep disturbance; and (c) sleep disorder. Diabetes was defined as having one of a diagnosis from a physician; an overnight fasting glucose > 125 mg/dL; Glycohemoglobin > 6.4%; or an oral glucose tolerance test > 199mg/dL. Overall, 19% of subjects were diabetics, 37% were obese, and 32% had either sleep disturbance or sleep disorder. Using multiple logistic regression models adjusting for covariates without including BMI, the odds ratios (OR, (95% CI)) of diabetes were 1.40 (1.06, 1.84) and 2.04 (1.40, 2.95) for those with sleep disturbance and with sleep disorder, respectively. When further adjusting for BMI, the ORs were similar for those with sleep disturbance 1.36 (1.06, 1.73) but greatly attenuated for those with sleep disorders (1.38 [0.95, 2.00]). In conclusion, the impact of sleep disorders on diabetes may be explained through the individuals’ obesity status.

Comparar la función pulmonar de pacientes con diabetes mellitus tipo 2 reciente, con diabetes mellitus tipo 2 de mayor evolución. Luego de ajustar por variables determinantes de la función pulmonar. Estudio observacional analítico de corte transversal. Obtención de variables espirométricas, como lo son el Volumen Espiratorio Forzado del primer segundo [VEF1], Relación entre el VEF1 y la Capacidad vital forzada, (VEF1/CVF), y residuales de Volumen Espiratorio Forzado del primer segundo (rVEF1) y de la Capacidad vital forzada y (rCVF). 495 pacientes diabéticos que consultaron a la Asociación Colombiana de Diabetes (ACD), entre julio 2005 y septiembre de 2007. Análisis entre duración de la diabetes (años), y el deterioro en la función pulmonar (variables espirométricas). Los pacientes con duración mayor de 8 años de la diabetes, tuvieron menor VEF1 comparado con los pacientes con duración menor de 8 años (2,62lts y 2,78lts, P 0.0139), así como de CVF (3.3lts y 3.5lts, P 0.0164). Después de ajustar por determinantes conocidos de la función pulmonar (sexo, edad, talla, tabaquismo, exposición a humo de leña), y por control de la diabetes (HbA1c) y tratamiento hipoglucemiante, estas diferencias persistieron; Los pacientes con duración mayor de 8 años de diabetes, tuvieron mayores residuales de VEF1 en promedio (rVEF1 -176,6mL y -115.7mL, con una diferencia de 60.8ml P<0.0001), así como de residuales de CVF (rCVF-261.9mL, y 160.5mL,con una diferencia de 100,4ml P<0.0001). Este estudio sustenta las observaciones clínicas que la duración de la diabetes es factor de riesgo independiente para deterioro de la función pulmonar.

The aim of this study was to investigate oral yeast colonization, antifungal susceptibility and strain diversity in insulin-dependent diabetes mellitus patients (175), as well as to evaluate the influence of dental prostheses. Oral rinse samples were cultured on selective media, in order to isolate, count and identify the yeasts recovered. More than half of the diabetic subjects (53%) carried significant amounts of Candida cells in the buccal cavity and these organisms were recovered at higher densities in diabetics wearing dentures. A total of 93 yeast strains were isolated from these patients, including: Candida spp. (n = 89); Pichia (n = 02); Trichosporon (n = 1), and Geotrichum (n = 1). C. albicans represented 56% of these strains, non-albicans Candida 39.8%, and other genera of yeast 4.3%. C. albicans was prevalent, followed by C. parapsilosis, C. tropicalis, C. glabrata, C. krusei, C. rugosa and C. guilliermondii. Agar disk-diffusion tests of the susceptibility of non-albicans Candida and other genera of yeast to fluconazole showed resistance in 21.9%, mainly in C. rugosa (100%), C. glabrata (57%) and C. krusei (50%). Local oral factors, such as the presence of dentures, in association with diabetes, seemed to have the effect of increasing the amount and variety of Candida species in the oral cavities, mainly those with lower drug susceptibilities.

Popular Health Education in its emancipatory dimension refers to individuals and groups to exchange knowledge and experiences, allowing them to associate health to the outcomes of their living conditions. Under this view, health workers and health users are subjects of the educative process. Thus, this study aims to identify the key clinical and socio sanitary attributes and promote educational activities with patients with Diabetes Mellitus (DM) in a Family Health Care Unit of the Western Sanitary District, in the city of Natal / RN. It is an action research which uses the references of the Theory of Liberating Education, which is based on a problem-solving pedagogy and that values dialogue in the process of understanding oneself and the world. Thirty-six diabetics, who are residents of the area covered by the health care unit, and thirty health workers participated in the survey. Each group had an average of twelve participants, and the meetings took place at the Unit´s hall, using conversation wheels, group dynamics, life narratives, experiences telling, movie exhibition and discussions, music, knowledge telling, desires, limitations, beliefs and values socially constructed. Data collection took place during the second half of two thousand and thirteen through Free Word Association Technique (FWAT), recordings of conversation wheels, participative observation, group dynamics, testimonies, questionnaires, life narratives and photographs. The empirical material was organized and subjected to three analyzes: thematic content (Bardin), textual statistics analysis by software IRAMUTEQ (Ratinaud), and photographic analysis (Edmund Feldman). The data analyses originated words, expressions, categories, themes and creative situations showing that popular health education is in process of construction, but still very incipient in primary care. The National Policy on Popular Health Education shows us the necessary ways for the transformation of health practices and the build of a more shared and solidary society. The meetings could be place to reverse that normative logic that has been happening over the years in primary care, but that by itself is not enough. It is possible to conclude that the use of active practices, increasing of listening and training on Popular Health Education will enable changes in the scenario where users and health workers deal with diabetes mellitus. Thus we see the popular health education is being timidly incorporated to the educational process of the subjects involved in this study, and far away from the principles of participation, organization of political work, increase opportunities for dialogue, respect, solidarity and tolerance among different actors involved in addressing the health problems that are fundamental to the improvement in building healthy practices of primary care

The present study was performed to assess the rate of hypertensive complications in diabetic pregnant patients and the influence of White's classification and the quality of the diabetic control. This study included 169 diabetic pregnant women who had delivered at the University Hospital of Botucatu Brazil from 1980 to 1981. The hypertensive disorders occurred in 29.8% of the cases. The incidence of the hypertensive process was the same in all classes of diabetic patients, and it was independent of the glycemic control. In patients with gestational diabetes (classes A and AB), chronic hypertension was the commnest type found; in patients with short-term diabetes (classes B and C) pregnancy-induced hypertension (PIH) and chronic hypertension with superimposed PIH was the most frequent type, and diabetic patients with vasculopathies (classes D-R) had preeclampsia and chronic hypertension with superimposed preeclampsia as the commonest type found.

The aim of this study was to analyze the risk factors related to the cardiovascular diseases (CVRF) in elderly type 2 diabetics. A cross-sectional observational study was carried out on 100 elderly patients attending the Rehabilitation Center of Araraquara (CRRA), São Paulo State, Brazil, from March to December, 2004. The majority were married, female, white, with a low income and low educational level. Regarding habits and style of life, the subjects had an adequate diet, were sedentary, non-smoking and non-drinking. In the population of 100, 42% were overweight, 42% obese, 71% had above-normal waist measurements and 84% high waist-to-hip ratios. Concerning the CVRF, it was observed that more than half had hypertension, hypercholesterolaemia and hypertriglyceridaemia. 84% had high values of LDL-cholesterol and 59% HDL-cholesterol levels below the reference values, 78% high levels of fasting glycemia, 76% glycohemoglobin and 57% fibrinogen and thus subject to cardiovascular risk. The results showed a high frequency of cardiovascular risk factors, differing according to sex and the age.

Several studies have shown that diabetics are more susceptible to the development of severe periodontal disease. Currently, the use of animal models can be considered a feasible alternative in radiographic assessments of these two pathologies. The purpose of this radiographic study was to evaluate the effect of induced diabetes mellitus on alveolar bone loss after 30 days of ligature-induced periodontal disease. Sixty-four Wistar rats were randomly distributed into four experimental groups. Diabetes was induced in Groups II and IV, while periodontal disease was induced in Groups III and IV; Group I was used as control. In order to perform the radiographic assessment of the specimens, the rats were killed on the 3rd and 30th days of the study. Radiographic measurements were assessed with ANOVA and Tukey's test to determine statistically significant differences (p < 0.05). It was observed that Groups III and IV featured greater bone loss when compared to Groups I and II. Only the diabetic group with periodontal disease (Group IV) featured statistically significant greater bone loss when compared to the other groups. These results suggested that the alveolar bone loss resulting from the periodontal disease installation is greater when associated to the diabetes mellitus.
