The current epidemic of paediatric obesity is consistent with a myriad of health-related comorbid conditions. Despite the higher prevalence of orthopaedic conditions in overweight children, a paucity of published research has considered the influence of these conditions on the ability to undertake physical activity. As physical activity participation is directly related to improvements in physical fitness, skeletal health and metabolic conditions, higher levels of physical activity are encouraged, and exercise is commonly prescribed in the treatment and management of childhood obesity. However, research has not correlated orthopaedic conditions, including the increased joint pain and discomfort that is commonly reported by overweight children, with decreases in physical activity. Research has confirmed that overweight children typically display a slower, more tentative walking pattern with increased forces to the hip, knee and ankle during 'normal' gait. This research, combined with anthropometric data indicating a higher prevalence of musculoskeletal malalignment in overweight children, suggests that such individuals are poorly equipped to undertake certain forms of physical activity. Concomitant increases in obesity and decreases in physical activity level strongly support the need to better understand the musculoskeletal factors associated with the performance of motor tasks by overweight and obese children.

Induction therapy of promyelocytic leukemia with all-trans retinoic acid is a standard therapy despite significant side-effects. The most important, the "retinoic acid syndrome", consists of a hyperinflammatory reaction with capillary leakage (edema, pleural, and pericardial effusion), infiltration of myeloid cells into internal organs and systemic signs of inflammation. We describe here two cases of another hyperinflammatory reaction during all-trans retinoic acid therapy, the Sweet's syndrome, consisting of infiltrates of the skin and internal organs by neutrophilic granulocytes. Fever, painful erythematous cutaneous plaques, prominent musculoskeletal involvement (myositis, fasciitis), a sterile pulmonary infiltration and intercurrent proteinuria characterized the clinical course of all-trans retinoic acid-associated Sweet's syndrome. Treatment with glucocorticoids led to resolution of the syndrome within 48 h. Three other cases of all-trans retinoic acid-associated Sweet's syndrome without involvement of internal organs, prominent on our cases, were published previously. Recognition of ATRA-associated Sweet's syndrome is of practical importance.

Introduction: Technical literature shows high frequencies of injuries occurring in classical ballet dancers; however, only limited information about the permanent effects of chronic diseases are mentioned. Objective: To compare the presence of MSD among dancers who wear pointe shoes and those who do not. Methods: The research was conducted at the 27th Festival of Joinville in Santa Catarina. The study had the participation of 111 dancers, 88 of whom wore pointe shoes while 23 did not. Specific procedures were used to obtain information related to MSD and foot injuries caused by dancing. Results: The most affected parts were the knees (29.7% with pointe shoes versus 39% without), spine (26.4% with pointe shoes versus 22% without), and ankle/foot (20% with pointe shoes versus 12.2% without). Through odds ratio and respective confidence intervals (IC95%), the study identified protection factor in the knees (0.24; CI95% - 0.09-0.64) and legs (0.11; CI95% - 0.02-0.65) for dancers who wear pointe shoes. It was found that the risk of injuries in specific structures of the foot is significantly higher among those dancers. In this case, the appearance of bunions (9.74; CI95% - 1.25-75,99), calluses on the toes (3.46; CI95% - 1.29-9.27) and the association of the three (4.47; CI95% - 1.69-11.83) were those that showed an increased risk factor compared to dancers who do not stand en pointe. Conclusion: The use of pointe shoes in elite Brazilian dancers was associated to lower occurrence of MSD in the knee and leg, however it was strongly associated to foot injuries.

Two major difficulties facing widespread clinical implementation of existing Tissue Engineering (TE) strategies for the treatment of musculoskeletal disorders are (1) the cost, space and time required for ex vivo culture of a patient’s autologous cells prior to re-implantation as part of a TE construct, and (2) the potential risks and availability constraints associated with transplanting exogenous (foreign) cells. These hurdles have led to recent interest in endogenous TE strategies, in which the regenerative potential of a patient’s own cells is harnessed to promote tissue regrowth without ex vivo cell culture. This article provides a focused perspective on key issues in the development of endogenous TE strategies, progress to date, and suggested future research directions toward endogenous repair and regeneration of musculoskeletal tissues and organs.

Background: Brachial plexus birth palsy (BPBP) most often occurs as a result of foetal-maternal disproportion. The C5 and C6 nerve roots of the brachial plexus are most frequently affected. In contrast, roots from the C7 to Th1 that result in total injury together with C5 and C6 injury, are affected in fewer than half of the patients. BPBP was first described by Smellie in 1764. Erb published his classical description of the injury in 1874 and his name became linked with the paralysis that is associated with upper root injury. Since then, early results of brachial plexus surgery have been reasonably well documented. However, from a clinical point of view not all primary results are maintained and there is also a need for later follow-up results. In addition most of the studies that are published emanate from highly specialized clinics and no nation wide epidemiological reports are available. One of the plexus injuries is the avulsion type, in which the nerve root or roots are ruptured at the neural cord. It has been speculated whether this might cause injury to the whole neural system or whether shoulder asymmetry and upper limb inequality results in postural deformities of the spine. Alternatively, avulsion could manifest as other signs and symptoms of the whole musculoskeletal system. In addition, there is no available information covering activities of daily living after obstetric brachial plexus surgery. Patients and methods: This was a population-based cross-sectional study on all patients who had undergone brachial plexus surgery with at least 5 years of follow-up. An incidence of 3.05/1000 for BPBP was obtained from the registers for this study period. A total of 1706 BPBP patients needing hospital treatment out of 1 717 057 newborns were registered in Finland between 1971 and 1997 inclusive. Of these BPBP patients, 124 (7.3%) underwent brachial plexus surgery at a mean age of 2.8 months (range: 0.4―13.2 months). Surgery was most often performed by direct neuroraphy after neuroma resection (53%). Depending on the phase of the study, 105 to 112 patients (85-90%) participated in a clinical and radiological follow-up assessment. The mean follow up time exceeded 13 years (range: 5.0―31.5 years). Functional status of the upper extremity was evaluated using Mallet, Gilbert and Raimondi scales. Isometric strength of the upper limb, sensation of the hand and stereognosis were evaluated for both the affected and unaffected sides then the differences and their ratios were calculated and recorded. In addition to the upper extremity, assessment of the spine and lower extremities were performed. Activities of daily living (ADL), participation in normal physical activities, and the use of physiotherapy and occupational therapy were recorded in a questionnaire. Results: The unaffected limb functioned as the dominant hand in all, except four patients. The mean length of the affected upper limb was 6 cm (range: 1-13.5 cm) shorter in 106 (95%) patients. Shoulder function was recorded as a mean Mallet score of 3 (range: 2―4) which was moderate. Both elbow function and hand function were good. The mean Gilbert elbow scale value was 3 (range: -1―5) and the mean Raimondi hand scale was 4 (range:1―5). One-third of the patients experienced pain in the affected limb including all those patients (n=9) who had clavicular non-union resulting from surgery. A total of 61 patients (57%) had an active shoulder external rotation of less than 0° and an active elbow extension deficiency was noted in 82 patients (77%) giving a mean of 26° (range: 5°―80°). In all, expect two patients, shoulder external rotation strength at a mean ratio 35% (range: 0―83%) and in all patients elbow flexion strength at a mean ratio of 41% (range: 0―79%) were impaired compared to the unaffected side. According to radiographs, incongruence of the glenohumeral joint was noted in 15 (16%) patients, whereas incongruence of the radiohumeral joint was found in 20 (21%) patients. Fine sensation was normal for 34/49 (69%) patients with C5-6 injury, for 15/31 (48%) with C5-7 and for only 8/25 (32%) of patients with total injury. Loss of protective sensation or absent sensation was noted in some palmar areas of the hand for 12/105 patients (11%). Normal stereognosis was recorded for 88/105 patients (84%). No significant inequalities in leg length were found and the incidence of structural scoliosis (1.7%) did not differ from that of the reference population. Nearly half of the patients (43%) had asynchronous motion of the upper limbs during gait, which was associated with impaired upper limb function. Data obtained from the completed questionnaires indicated that two thirds (63%) of the patients were satisfied with the functional outcome of the affected hand although one third of all patients needed help with ADL. Only a few patients were unable to participate in physical activities such as: bicycling, cross-country skiing or swimming. However, 71% of the patients reported problems related to the affected upper limb, such as muscle weakness and/or joint stiffness during the aforementioned activities. Incongruity of the radiohumeral joints, extent of the injury, avulsion type injury, age less than three months of age at the time of plexus surgery and inexperience of the surgeon was related to poor results as determined by multivariate analyses. Conclusions: Most of the patients had persistent sequelae, especially of shoulder function. Almost all measurements for the total injury group were poorer compared with those of the C5-6 type injury group. Most of the patients had asymmetry of the shoulder region and a shorter affected upper limb, which is a probable reason for having an abnormal gait. However, BPBP did not have an effect on normal growth of the lower extremities or the spine. Although, participation in physical activities was similar to that of the normal population, two-thirds of the patients reported problems. One-third of the patients needed help with ADL. During the period covered by this study, 7.3% BPBP of patients that needed hospital treatment had a brachial plexus operation, which amounts to fewer than 10 operations per year in Finland. It seems that better results of obstetric plexus surgery and more careful follow-up including opportunities for late reconstructive procedures will be expected, if the treatment is solely concentrated on by a few specialised teams.

Trata-se de uma pesquisa documental, retrospectiva de fonte secundária,que adota uma abordagem quantitativa descritiva-exploratória. A partir da constatação de altos índices de absenteísmo nas unidades hospitalares, despertou-se o interesse em estudar os custos diretos das doenças ocupacionais que levam aos afastamentos e seu impacto econômico para o orçamento de recursos humanos de um hospital universitário do Rio de Janeiro. Neste contexto, definiu-se como objeto de estudo, o impacto econômico do absenteísmo por doença na equipe de enfermagem e, como objetivos: identificar as causas prevalentes de afastamentos no hospital universitário, de acordo com Classificação Internacional de Doenças e Problemas Relacionados a Saúde (CID-10); estimar os custos diretos mínimos das doenças que afastaram o trabalhador de enfermagem; estimar o custo real aproximado do absenteísmo relacionado a 1 (um) dia de trabalho prestado pelos trabalhadores de enfermagem, com projeção de 1 (um) mês e 1(um) ano numa visão operacional do Sistema Único de Saúde (SUS). Foi utilizada uma amostra estratificada de prontuários dos profissionais de saúde da equipe de enfermagem (enfermeiros e técnicos de enfermagem), a partir do seguinte critério de inclusão: profissionais de enfermagem concursados com afastamento no ano de 2010 e com diagnóstico médico determinante do afastamento, definido claramente. Para a coleta das informações foi feita a apreciação dos documentos arquivados no Serviço de Saúde do Trabalhador do hospital estudado e contou com a apreciação de especialistas médicos relativos aos grupos de diagnósticos estudados, orientados por roteiros criados pela pesquisadora. Os dados foram analisados e armazenados no programa Statistical Package for the Social Sciences (SPSS) versão 15 e no editor Microsoft excel 2003. Dentre os resultados obtidos tiveram destaque para as seguintes causas de afastamento, respectivamente, às doenças do sistema osteomuscular, os fatores que influenciam o estado de saúde e o contato com serviços de saúde, os transtornos mentais e comportamentais, as lesões, envenenamento e outras consequencias de causas externas e, as doenças do sistema circulatório, que representam um custo estimado aproximado de R$ 2,6 milhões. Pôde-se constatar que o impacto econômico do absenteísmo decorrentes dos agravos à saúde para o orçamento de recursos humanos do hospital universitário foi de aproximadamente 2,7%. O custo real aproximado do absenteísmo de enfermagem por dia, foi avaliado em R$ 92,50, tendo projeção mensal de R$ 2.775,00 e anual de R$ 33.300,00. Recomenda-se avaliar o absenteísmo dos profissionais regularmente para identificar as causas reais do absenteísmo por doença, a fim de definir metas para os programas de intervenção à saúde dos trabalhadores e promover uma Gestão participativa que favoreça uma análise do processo de trabalho no que concerne o atendimento das necessidades de saúde e operacionais da força de trabalho, determinantes do absenteísmo.

El absentismo laboral por causa medica afecta de manera global al trabajador, la empresa y a la sociedad ya que en cada uno de ellos tiene un efecto negativo para el desarrollo económico ,por esto resulta importante estudiar esta temática para avanzar en su prevención y adecuado control. Objetivo: caracterizar el absentismo laboral por causa médica de una empresa de telecomunicaciones. Métodos: se realizó un estudio descriptivo de corte transversal a partir de una base de datos correspondiente a los registros de las incapacidades que se presentaron entre el 1 de enero y el 30 de junio de los años 2012, 2013 y 2014. Resultados: Durante el periodo evaluado se presentaron 243 incapacidades que generaron un total de 1.759 días perdidos Las principales causas de absentismo de origen médico para los periodos estudiados fueron las patologías asociadas al sistema osteomuscular, como dorsalgias, lumbagos, fracturas y heridas musculares. La tasa global de absentismo (TGA) por periodo presenta un incremento progresivo pasando de 13.79% en el 2012 a 20.90% en el año 2014. Conclusiones y recomendaciones: Predominan durante todos los periodos evaluados las incapacidades menores a tres días, las generadas en los trabajadores con cargo operativo. Las principales causas de absentismo laboral por causa médicas fueron las patologías asociadas al sistema osteomuscular, como dorsalgias, lumbagos, fracturas y heridas musculares. El comportamiento de las tasas globales de absentismo por enfermedad general, accidente de trabajo y enfermedad laboral a meritan que la empresa realice evaluaciones de tiempo y movimientos así como de la, carga de trabajo asignada a sus colaboradores, con el propósito de determinar si estas variables están determinando el incremento en la frecuencia y la severidad el absentismo observadas en el presente estudio.

RESUMEN INTRODUCCION Los desórdenes musculoesqueléticos constituyen uno de los más comunes y costosos problemas de salud en el trabajo en todo el mundo. La detección a tiempo de síntomas iniciales debe ser una prioridad de los sistemas de vigilancia epidemiológica. El objetivo de este trabajo de investigación fue determinar la prevalencia de los desórdenes musculoesqueléticos (DME) en la población de funcionarios del cuerpo de policía judicial de Bogotá en el año 2014. MATERIALES Y METODOS Estudio de corte transversal con una muestra de 300 trabajadores a los que se les aplico el cuestionario Nórdico de Síntomas musculoesqueléticos. RESULTADOS Se encuesto al 33,9% de los trabajadores pertenecientes al cuerpo técnico de Policía Judicial de la Fiscalía General de la Nación en la ciudad de Bogotá. Los trabajadores evaluados tienen una edad promedio de 41,9 años, con un predominio del género femenino (54%), encontrándose que la mayoría de los que respondieron al cuestionario 87,66% (n=263) habían presentado algún síntoma musculoesqueléticos en los últimos 12 meses. Un hallazgo significativo fue la afectación cervical en el 53% (n=159) de los que reportaron sintomatología musculo esquelética. CONCLUSIONES La población perteneciente a la Policía Técnica Judicial en la ciudad de Bogotá presenta una alta prevalencia (87,66%) de síntomas que afectan al sistema musculoesquelético, principalmente comprometiendo las regiones de columna cervical, parte baja de la espalda y parte alta de la espalda, con una mayor afectación a las personas de género femenino.

Resumen: Introducción: El ausentismo laboral por causa médica es un problema por la afectación que genera en el trabajador y en la empresa. Objetivo: Caracterizar el ausentismo laboral por causas médicas de una empresa de alimentos de Bogotá. Materiales y métodos: Estudio de corte transversal con datos secundarios de registros de incapacidades de los años 2013 y 2014. El procesamiento de la información se realizó con el programa SPSS, se obtuvieron medidas de tendencia central y de dispersión. Se determinó el número y la duración de incapacidades, la duración media de estas, el sistema afectado, se realizó el análisis de frecuencia por centro de costo y género. Resultados: Se registraron un total de 575 incapacidades, 387 fueron por enfermedad de origen común y 188 por accidentes de trabajo. Se perdieron 3.326 días por ausentismo, de los cuales en 45,09% se presentó en 2013 y el 54,91% restante en 2014, de estos 1985 se generaron en eventos de origen común y 1341 por accidentes de trabajo. La principal causa de incapacidades por enfermedades de origen común fueron patologías asociadas al sistema músculo esquelético, y para las originadas en accidentes de trabajo fueron las lesiones en manos. Conclusiones: para el año 2014 los accidentes de trabajo disminuyeron con respecto al año 2013 y el sistema más afectado respecto a enfermedad común fue el osteomuscular. Es conveniente que se implemente un sistema o programa de vigilancia y análisis en puestos de trabajo para identificar los factores de riesgo asociados y minimizar los riesgos.

Disorders localized to the musculoskeletal system are a common problem among dental personnel. This study has the aim of surveying epidemiological studies reporting positive associations between dental practice and musculoskeletal disorders (MSKDs). The focus was to evaluate the size of reported risk increase and the extent to what alternative causal explanations were considered. Reports with significant links (p value .05) were systematically selected from 2 electronic databases. Twenty-five studies were identified. Risk measures were reported in 8 studies, and all of them presented weak associations. The impact of at least 1 competing explanations was analyzed in 32% of studies, but adjustment was considered not adequate in half of them. The evidence on dentistry as a profession with potential risk for development of MSKDs remains questionable. Further research is needed to more carefully elucidate the risk and the impact of MSKDs in this particular occupational group.

As doenças que afetam o sistema músculo-esquelético acometem centenas de milhões de pessoas no mundo e estão entre as causas mais comuns de invalidez e sofrimento crônico. A doença vertebral degenerativa constitui uma exacerbação do processo de envelhecimento, podendo estar envolvidos os fatores genéticos, ambientais ou lesões traumáticas, deformidades e doenças preexistentes. Muito se tem discutido sobre os múltiplos fatores envolvidos na degeneração discal, mas sua etiologia permanece indeterminada. Contudo, atualmente a participação da genética parece muito mais forte do que se suspeitava anteriormente. Neste artigo, é abordada a participação de alguns genes no processo discogênico, bem como o que isso representa para o melhor entendimento da etiopatogênese da doença e na melhora de seu tratamento.
