PURPOSE: To investigate the facial symmetry of rats submitted to experimental mandibular condyle fracture and with protein undernutrition (8% of protein) by means of cephalometric measurements. METHODS: Forty-five adult Wistar rats were distributed in three groups: fracture group, submitted to condylar fracture with no changes in diet; undernourished fracture group, submitted to hypoproteic diet and condylar fracture; undernourished group, kept until the end of experiment, without condylar fracture. Displaced fractures of the right condyle were induced under general anesthesia. The specimens were submitted to axial radiographic incidence, and cephalometric mensurations were made using a computer system. The values obtained were subjected to statistical analyses among the groups and between the sides in each group. RESULTS: There was significative decrease of the values of serum proteins and albumin in the undernourished fracture group. There was deviation of the median line of the mandible relative to the median line of the maxilla, significative to undernutrition fracture group, as well as asymmetry of the maxilla and mandible, in special in the final period of experiment. CONCLUSION: The mandibular condyle fracture in rats with proteic undernutrition induced an asymmetry of the mandible, also leading to consequences in the maxilla.

OBJETIVO: Avaliar a utilização do fio de polipropileno de autossustentação de tripla convergência para a correção do desvio de hemiface que se apresenta como sequela tardia da paralisia facial periférica. MÉTODOS: Realizou-se um estudo retrospectivo e observacional em 34 indivíduos portadores de paralisia facial tardia. Utilizou-se fio de polipropileno de tripla convergência para a correção do desvio da hemiface paralisada. Trata-se de fio monofilamentar, sintético, transparente e inabsorvível que possui garras que tracionam os tecidos ptosados mantendo-os na posição correta, em simetria com o lado são. Os resultados foram avaliados através de análise subjetiva, quanto ao grau de satisfação dos pacientes, submetidos a um questionário próprio. RESULTADOS: Analisou-se 34 pacientes, 73,52% deles relataram grande melhora, 20,58% relataram melhora moderada e 5,88%, uma melhora leve; encontrou-se um alto índice de satisfação, 94,1%. Dois pacientes apresentaram extrusão parcial do fio. Nenhum paciente desenvolveu infecção. CONCLUSÃO: O emprego do fio de polipropileno de autossustentação de tripla convergência para a correção do desvio da hemiface, consequente à paralisia facial, ocasionou acentuada melhora da assimetria facial e a recuperação da autoestima dos pacientes.

The condylar hyperplasia is an acquired development anomaly, rare, characterized by an excessive and progressive growing, affecting neck, condilar head, body and the mandible bough, provoking an important facial asymmetry. In the article we present a case of male patient, 22-years-old, reclaiming of painful sintomatology in the region of temporomandibular joint and severe facial asymmetry. It was instituted an orthodontic-surgical treatment by means of orthognathic combined surgery and high condilectomy. After six years of post-surgical controlling, the patient is now in a good shape, without recurrence of facial asymmetry and condylar hyperplasia.

Osteochondroma (OC) is the most common benign tumor of long bones. However it is rarely found in the facial skeleton, being the coronoid process and mandibular condyle the most affected sites in this region. It basically consists in bone growth covered by cartilage. The etiology is still controversial: neoplastic, developmental, reparative and traumatic origins have been discussed in literature. The treatments of these lesions include total condylectomy or local resection of the lesion. This paper aims to report a case of a patient with history of trauma and possible fracture of the mandibular condyle in childhood, which in youth developed dentofacial deformity with severe facial asymmetry. The treatment consisted of resection of lesion both with maxillary and mandibular osteotomies associated with graft from the iliac crest bone. Actually, the patient is with a favorable aesthetic, without functional deficit and absence of lesion’s recurrence.

Class III skeletal malocclusion may present several etiologies, among which maxillary deficiency is the most frequent. Bone discrepancy may have an unfavorable impact on esthetics, which is frequently aggravated by the presence of accentuated facial asymmetries. This type of malocclusion is usually treated with association of Orthodontics and orthognathic surgery for correction of occlusion and facial esthetics. This report presents the treatment of a patient aged 15 years and 1 month with Class III skeletal malocclusion, having narrow maxilla, posterior open bite on the left side, anterior crossbite and unilateral posterior crossbite, accentuated negative dentoalveolar discrepancy in the maxillary arch, and maxillary and mandibular midline shift. Clinical examination also revealed maxillary hypoplasia, increased lower one third of the face, concave bone and facial profiles and facial asymmetry with mandibular deviation to the left side. The treatment was performed in three phases: presurgical orthodontic preparation, orthognathic surgery and orthodontic finishing. In reviewing the patient's final records, the major goals set at the beginning of treatment were successfully achieved, providing the patient with adequate masticatory function and pleasant facial esthetics.

Osteochondroma of the mandibular condyle has been found in the oral and maxillofacial region rarely. This paper describes a case of osteochondroma of the mandibular condyle in a 20-year-old woman, who was referred to our service with facial asymmetry, prognathic deviation of chin, cross-bite to the contralateral side, changes in condylar morphology, limited mouth opening, and malocclusion. Computed tomography (CT) was performed for better evaluation to the pathological conditions on the temporomandibular joint. Based on the clinical examination, patient history, and complementary exams, the hypothesis of osteochondroma was established. Condylectomy was performed using a preauricular approach with total removal of the lesion. After 3 years of postoperative follow up and orthodontic therapy, the patient is symptom-free, and has normal mouth opening with no deviation in the opening pattern.

PURPOSE: To report an uncommon case of osteochondroma affecting the mandibular condyle of a young patient and to illustrate the important contributions of different imaging resources to the diagnosis and treatment planning of this lesion. CASE DESCRIPTION: A 24-year-old female patient with the chief complaint of an increasing facial asymmetry and pain in the left pre-auricular region, revealing a reduced mouth opening, mandibular deviation and posterior cross-bite over a period of 18 months. Panoramic radiography revealed an enlargement of the left condyle, whereas computed tomography (CT) sections and three-dimensional CT showed a well-defined bone growth arising from condylar neck. The scintigraphy exam showed an abnormal osteogenic activity in the left temporomandibular joint. The condyle was surgically removed and after 18 months follow-up the panoramic radiography and CT scans showed no signs of recurrence. CONCLUSION: Although osteochondroma is a benign bone tumor that rarely arises in cranial and maxillofacial region, it should be considered in the differential diagnosis of slow-growing masses of the temporomandibular area and the use of different imaging exams significantly contribute to the correct diagnosis and treatment planning of this pathological condition.

O osteossarcoma é um tumor mesenquimal maligno, no qual as células cancerosas produzem matriz óssea. É o tumor maligno primário mais comum do osso, responsável por aproximadamente 20% dos sarcomas, sendo que 5% destes ocorrem nos maxilares. Possuem variados aspectos não só clínicos e histopatológicos como também no curso e prognóstico. Este artigo apresenta um relato de caso de paciente do sexo feminino, com 20 anos de idade, que nos foi encaminhada apresentando aumento de volume na região de pré-molar inferior esquerdo. Após diagnóstico clínico de lesão do periápice dental, a paciente foi submetida previamente a tratamento endodôntico do dente envolvido, a partir do qual, em um período de 11 dias, pode-se observar um considerável aumento da lesão provocando visível assimetria facial. A radiografia oclusal mostrava imagem compatível com área de destruição óssea e formação de osso anormal na região, com cortical externa exibindo evidente radiopacidade semelhante a raios de sol, sugerindo o diagnóstico de osteossarcoma. A lesão foi biopsiada e obteve-se o diagnóstico histopatológico de osteossarcoma. A paciente foi então submetida à mandibulectomia parcial e uma reconstrução da área, usando osso de costela e enxerto de pele da nádega, para o revestimento da mucosa oral envolvida. Aos 8 meses após a cirurgia houve recorrência local da lesão e a paciente foi a óbito aproximadamente 1 ano depois da recidiva.

A hipertrofia idiopática do músculo masseter (HIM) é uma patologia pouco freqüente e de causa desconhecida. Alguns autores correlacionam tal condição com hábitos de mascar gomas, disfunção da articulação temporomandibular (ATM), hipertrofias congênitas e funcionais, e distúrbios emocionais (nervosismo e ansiedade). A maioria dos pacientes queixa-se da alteração estética decorrente da assimetria facial, também chamada "face quadrada", no entanto, sintomas como trismo, protrusão e bruxismo também podem ocorrer. Os objetivos deste estudo foram: relatar um caso de HIM e descrever a sintomatologia e o tratamento realizado. O paciente relatava aumento bilateral na região do ângulo da mandíbula de evolução lenta e progressiva. Negava dor ou desconforto, porém se queixava de otalgia bilateral, trismo noturno e ansiedade. Ao exame físico, observou-se hipertrofia bilateral de masseter sem alterações inflamatórias no local. Foi indicado tratamento cirúrgico com abordagem extra-oral. Exames complementares são indicados na dúvida diagnóstica. A conduta terapêutica varia de conservadora a cirúrgica, sendo que esta depende principalmente da experiência e da habilidade do cirurgião.

OBJETIVO: analisar a habilidade de ortodontistas e leigos para perceber as assimetrias faciais causadas por desvios mandibulares. MÉTODOS: foram obtidas fotografias frontais da face de dois indivíduos, sendo um do sexo masculino e outro do sexo feminino. As fotografias foram tiradas em Posição Natural de Cabeça (PNC) com desvios mandibulares progressivos — em 2, 4 e 6mm —, partindo-se da posição de Máxima Intercuspidação Habitual (MIH). Para testar a reprodutibilidade do método, utilizaram-se os Coeficientes de Correlação Intraclasse (ICC) e o teste de Kappa ponderado.As diferenças entre os examinadores leigos e ortodontistas foram investigadas através do teste de Mann-Whitney, enquanto a análise de Friedman foi utilizada para investigar as diferenças nos escores para os progressivos avanços mandibulares. Todas as estatísticas foram executadas com nível de confiabilidade de 95%. RESULTADOS: os ortodontistas foram hábeis em perceber os desvios somente a partir de 4mm, quando comparados à posição de MIH (p≤0,05), enquanto os leigos tiveram o mesmo padrão para o indivíduo do sexo feminino. Porém, ao examinar o sujeito do sexo masculino, os leigos não observaram nenhuma alteração executada a partir de MIH (p>0,05). De modo geral, apesar de as medianas atribuídas pelos ortodontistas terem sido menores que as dos leigos, essa diferença foi significativa apenas para o desvio de 6mm, em ambos os pacientes. CONCLUSÕES: ortodontistas e leigos avaliaram a assimetria mandibular de modo diferente, visto que ortodontistas tendem a ser mais críticos quando as assimetrias são mais severas. Conclui-se, ainda, que existe variação na avaliação das assimetrias faciais dependendo do paciente examinado, principalmente entre os examinadores leigos.

Eye loss results on facial asymmetry compromising its aesthetics. Ocular prostheses are important to re-establish aesthetics; protect the anophthalmic cavity; recover function such as the redirection of lachrymatory liquid; and reintegrate the patient to society. The aim of this study was to describe a case report, demonstrating clinical and laboratorial procedures for confection of ocular prostheses and highlighting their advantages. A female patient was afflicted by toxoplasmosis, which led to the loss of her eye. An eye visceration was performed. The patient reported no previous use of ocular prosthesis. A PMMA ocular prosthesis was manufactured restoring patient’s aesthetic and facial contour. The patient was satisfied with the treatment. Therefore the present study has shown that PMMA ocular prosthesis is able to restore facial aesthetics and can be a safe and satisfactory alternative for patients that have lost their eyes due to toxoplasmosis.

The Peripheral ossifying fibroma is a reactive proliferative lesion, non neoplastic, slow growth, which can produce recurrence after removal. It´s etiology is uncertain, but is associated with local irritants, and is found mostly in the anterior maxilla. Clinically it is characterized by an asymptomatic increase in volume, which may, over time, facial asymmetry. The aim of this paper is to describe a case of peripheral ossifying fibroma in a patient 40, female, exophytic lesion in the jaw, unusual for its large dimensions and with a history of three recurrences, leading to facial asymmetry. She underwent surgery to remove the lesion along with the likely irritants, and the pathological diagnosis of peripheral ossifying fibroma. We conclude that it is fundamentally important for complete removal of the lesion to reduce the tendency to relapse, including the periosteum and the periodontal ligament, in addition to possible causes.
