This study investigated whether Negative Affectivity (NA) causes bias in self-report measures of activity limitations or whether NA has a real, non-artifactual association with activity limitations. The Symptom Perception Hypothesis (NA negatively biases self-reporting), Disability Hypothesis (activity limitations cause NA) and Psychosomatic Hypothesis (NA causes activity limitations) were examined longitudinally using both self-report and objective activity limitations measures. Participants were 101 stroke patients and their caregivers interviewed within two weeks of discharge, six weeks later and six months post-discharge. NA and self-report, proxy-report and observed performance activity (walking) limitations were assessed at each interview. NA was associated with activity limitations across measures. Both the Disability and Psychosomatic Hypotheses were supported: initial NA predicted objective activity limitations at six weeks but, additionally, activity limitations at six weeks predicted NA at six months. These results suggest that NA both affects and is affected by activity limitations and does not simply influence reporting.

Objectifs : Cette thèse porte sur l’association entre les caractéristiques socioenvironnementales des voisinages (milieux locaux) et la prévalence des limitations d’activités (ou handicap) dans la population québécoise. Elle a trois objectifs principaux : (1) clarifier les enjeux conceptuels et méthodologiques relatifs à l’étude des déterminants socioenvironnementaux des limitations d’activités; (2) décrire les contributions respectives de la composition socioéconomique des voisinages et de facteurs contextuels à la variabilité locale de la prévalence des limitations d’activités; (3) évaluer la présence d’interactions entre la santé fonctionnelle des personnes (incapacité) et des caractéristiques des voisinages en lien avec la prévalence des limitations d’activités. Méthodes : Une analyse de la littérature scientifique a été effectuée en lien avec le premier objectif de la thèse. En lien avec le deuxième objectif, des données pour le Québec du recensement canadien de 2001 (échantillon de 20% de la population) ont été utilisées pour estimer l’association entre la prévalence des limitations d’activités et des caractéristiques des voisinages : classification urbain-rural, composition socioéconomique (défavorisation matérielle et sociale) et facteurs contextuels (qualité des habitations, stabilité résidentielle et utilisation des transports actifs et collectifs). En lien avec le troisième objectif, des données pour la population urbaine du Québec issues de l’Enquête sur la santé dans les collectivités canadiennes (2003, 2005 et 2007/2008) ont permis de tester la présence d’interaction entre la santé fonctionnelle des personnes et des caractéristiques des voisinages (défavorisation matérielle et sociale, qualité des habitations, stabilité résidentielle et densité des services). Pour les analyses associées aux deux derniers objectifs, l’analyse des corrélats de la prévalence des limitations d’activités a été effectuée à l’aide de régressions logistiques multiniveaux. Résultats : Différents éléments conceptuels et opérationnels limitent la possibilité de faire une synthèse des analyses épidémiologiques portant sur les influences socioenvironnementales sur les limitations d’activités. Les résultats des analyses empiriques suggèrent que : (1) la variation géographique de la prévalence des limitations d’activités s’explique en grande partie par la composition socioéconomique des voisinages; (2) des facteurs contextuels sont associés à cette variation géographique; (3) les mesures relatives d’inégalités sous-estiment les disparités contextuelles dans la distribution des nombres absolus de personnes ayant une limitation d’activités; et (4) l’association entre la prévalence des limitations d’activités et la défavorisation sociale pourrait varier selon la santé fonctionnelle des personnes. Conclusions : Différentes caractéristiques socioenvironnementales sont potentiellement associées aux variations géographiques des limitations d’activités au Québec. Le développement d’indicateurs socioenvironnementaux favoriserait une connaissance plus précise de l’influence de ces caractéristiques socioenvironnementales sur les limitations d’activités et des mécanismes par lesquels s’exerce cette influence. L’établissement d’un système national de surveillance des aménagements territoriaux est proposé afin de soutenir la recherche et la prise de décision. Des indicateurs locaux d’accessibilité aux transports, aux espaces publics ainsi qu’aux services de proximité devraient être priorisés. Ces aspects de l’aménagement du territoire sont susceptibles de rejoindre plusieurs enjeux de santé publique et ils ont comme autre avantage d’être inclus dans différentes orientations québécoises ciblant le vieillissement en santé et la réduction des limitations d’activités.

Objective: Between-participant research has shown that high negative affectivity predicts greater activity limitations and vice versa. This study examined both between- and within-participant associations of negative and positive affectivity with activity levels using ecological momentary assessment. Method: Participants were 25 people who had undergone joint replacement surgery 12 months previously. Participants made multiple reports of their activity and positive and negative affectivity over a single day using, a computerized diary. Activity was also objectively recorded using an activity monitor. The following day, participants made a self-report of their activity over the measurement day and general positive and negative affectivity levels were recorded. Results: Higher self-reported walking time over the whole measurement day was associated with higher general positive affectivity but not negative affectivity. However, using ecological momentary assessment, higher diary reports of negative affectivity predicted increased activity levels while positive affectivity neither predicted nor was predicted by activity. Conclusion: These findings demonstrate the importance of within-participant methodology in detecting subtle and immediate effects of individuals' mood on behavior that may differ from findings investigating between-participant effects over longer time periods.

Objectivo: Avaliar o impacto qualitativo de um programa de intervenção em fisioterapia, segundo o Conceito de Bobath, nas actividades e participação de dois indivíduos com lesão do Sistema Nervoso Central. Avaliar as modificações no comportamento da actividade muscular ao longo da fase de apoio do ciclo da marcha, na força de reacção ao solo e respectiva contribuição muscular. Metodologia: A avaliação realizou-se antes e após um programa de intervenção, segundo a abordagem do Conceito de Bobath, através da Classificação Internacional de Funcionalidade, Incapacidade e Saúde, electromiografia, plataforma de forças e máquina fotográfica. Resultados: Obteve-se melhorias na restrição da participação e na limitação da actividade. Verifica-se uma tendência de modificação do comportamento muscular ao longo da fase de apoio e na componente antero-posterior (Fy), mais evidente no mecanismo de aceleração. A mudança na contribuição muscular para a este mecanismo é mais evidente. Conclusão: O programa de intervenção, segundo o Conceito de Bobath, induziu mudanças positivas quanto à funcionalidade dos indivíduos, reflectindo-se na possibilidade de reorganização dos componentes neuro-motores em indivíduos com lesão do Sistema Nervoso Central.

COPD is a major cause of morbidity and mortality worldwide, representing a major public health problem due to the high health and economic resource consumption. Pulmonary rehabilitation is a standard care recommendation for these patients, in order to control the symptoms and optimize the functional capacity, reducing health care costs associated with exacerbations and activity limitations and participation. However, in patients with severe COPD exercise performance can be difficult, due to extreme dyspnea, decreased muscle strength and fatigue. In addition, hypoxemia and dyspnea during efforts and daily activities may occur, limiting their quality of life. Thus, NIV have been used as adjunct to exercise, in order to improve exercise capacity in these patients. However, there is no consensus for this technique recommendation. Our objective was to verify whether the use of NIV during exercise is effective than exercise without NIV in dyspnea, walked distance, blood gases and health status in COPD patients, through a systematic review and meta-analysis.

O crescente reconhecimento das limitações das crianças com multideficiência e deficiência visual, quer nas interacções com os parceiros quer de uma forma geral nos ambientes em que se inserem, motivou este estudo, que pretendeu analisar o nível de participação destas crianças em actividades na escola. Considerando a importância de contribuir com informação para orientações na intervenção educativa de crianças com MDVI, realizou-se um estudo que analisa o seu comportamento e envolvimento em actividades da escola. Para a realização deste estudo, observaram-se os comportamentos de três crianças com MDVI, com idades compreendidas entre os 9 e os 10 anos, em três ambientes da escola, nomeadamente a sala de aula, o refeitório e o recreio, e em três actividades (pintura, jogos, almoço, saltar à corda, andar de baloiço e subir escadas) de forma a analisar o seu envolvimento e limitações nas actividades. Na análise dos dados das observações foram identificadas quatro categorias de participação: Inicia, Perde Oportunidade, Inicia com Apoio e Comportamento Potencialmente Comunicativo, registando-se valores que permitiram encontrar características dos comportamentos das crianças observadas, assim como o seu nível de participação em actividades na escola. Os resultados do estudo permitiram verificar que a participação das crianças em actividades está condicionada pelos ambientes em que estão envolvidas, e não pelas problemáticas que cada criança apresenta.----------------------------------------ABSTRACT: The motivation of this study is the increasing knowledge and awareness of children who have multiple disabilities and a visual impairment (MDVI) and the limitation with their peer interactions and in general. The purpose of this study was to analyze the participation level of children with MDVI in school activities. Considering the importance of contributing with guidelines for educational intervention with children with MDVI, we did a study that analyzes the behavior and the level of participation of MDVI children in school activities. In this research study we observed the behavior of three children with MDVI, of 9/10 years old, in three different environments at school; the classroom, the canteen and the playground, and in different activities (painting, playing games, having lunch, skipping rope, etc), in order to analyze their participation and their activity limitations in the activities referred. Data analysis identified four categories of participation: Initiation; Missed Opportunities; Initiation with support and Potentially communicative behavior. Results of data analysis allowed us to find out characteristics of children´s behavior, as well as their level of participation in activities. The main findings of this research allowed us to verify that the child’s engagement in activities depends on the environments where they are located and not on their disability.

On assistive technology targeted for people with activity limitations and participation, usability issues becomes an essential tool to ensure that the product has the appropriate ergonomics characteristics, in other words, ensure that it fits the specific user´s needs. The aim of this study was to analyze the usability of an adaptive seating device for children with neuromotor impairments, by using kinematic indicators of the reaching movement. The study sample consisted of 13 children with associated neurologic conditions. The tests were developed by using a wooden bench height adjustable, integrated with the adaptive seating device under study, and a system to capture three-dimensional image, called Qualisys Track Manager. The following reaching kinematics variables were measured: maximum reaching velocity, movement duration, index of curvature, and unit movements. It was found that the use of the adaptive seating device had a positive impact on upper limb function in children with neuromotor impairments. It was also noticed an improvement in the reaching movement kinematics, which was statistical significant for the index of curvature and unit movements. As main conclusions, it is possible to point out some positive effects that the product under study seems to have on users' movements, such as the improved movement quality of the upper limb, which could mean a better postural adjustments and higher trunk postural control. By identifying new measures of usability in terms of effectiveness and efficiency for the analyzeddevice, the results obtained may serve also as performance indicators, providing new data that may help to improve the product and eventually modifying it, in order to turn it more compatible with the needs of the considered target population.

Les troubles musculo-squelettiques du membre supérieur (TMS-MS) peuvent avoir un impact négatif sur l'autonomie d’une personne. À ce jour, il n’existe pas d’outils en français évaluant les limitations d'activités et les restrictions de participation, disposant de bonnes propriétés métrologiques et d'applicabilité dans un contexte clinique marqué par des contraintes de temps. L’Upper Limb Functional Index (ULFI), qui présente de bonnes qualités métrologiques et applicabilité clinique adéquate, s'avère un outil intéressant pour les ergothérapeutes de par son approche centrée sur la personne. Toutefois, il n'était disponible qu'en anglais et en espagnol. La présente recherche, constituée de deux études, visait à adapter l’ULFI à la population canadienne française et à évaluer ses propriétés métrologiques et son applicabilité clinique. La première étude a examiné sa cohérence interne, sa validité convergente et son applicabilité clinique auprès de 50 patients bilingues atteints de TMS-MS. La seconde étude visait à analyser sa fidélité test-retest et sa sensibilité au changement auprès de 60 patients francophones. Les résultats ont révélé que l'ULFI-CF possède des propriétés métrologiques solides : une cohérence interne élevée (α de Cronbach = 0,93), une excellente fidélité test-retest (CCI = 0,87-0,95), une excellente validité convergente (r = 0,70-0,85) et une sensibilité au changement de bonne à excellente (tailles d'effet = 0,49-0,88 et r = 0,64 pour la section 1). L'ULFI-CF démontre également une bonne applicabilité clinique. En conclusion, l’ULFI-CF s’avère un outil pertinent pour les cliniciens œuvrant auprès d’une clientèle canadienne française souffrant d'un TMS-MS dans un contexte clinique marqué par des contraintes de temps.

Introduction Human immunodeficiency virus (HIV) is a serious disease which can be associated with various activity limitations and participation restrictions. The aim of this paper was to describe how HIV affects the functioning and health of people within different environmental contexts, particularly with regard to access to medication. Method Four cross-sectional studies, three in South Africa and one in Brazil, had applied the International Classification of Functioning, Disability and Health (ICF) as a classification instrument to participants living with HIV. Each group was at a different stage of the disease. Only two groups had had continuing access to antiretroviral therapy. The existence of these descriptive sets enabled comparison of the disability experienced by people living with HIV at different stages of the disease and with differing access to antiretroviral therapy. Results Common problems experienced in all groups related to weight maintenance, with two-thirds of the sample reporting problems in this area. Mental functions presented the most problems in all groups, with sleep (50%, 92/185), energy and drive (45%, 83/185), and emotional functions (49%, 90/185) being the most affected. In those on long-term therapy, body image affected 93% (39/42) and was a major problem. The other groups reported pain as a problem, and those with limited access to treatment also reported mobility problems. Cardiopulmonary functions were affected in all groups. Conclusion Functional problems occurred in the areas of impairment and activity limitation in people at advanced stages of HIV, and more limitations occurred in the area of participation for those on antiretroviral treatment. The ICF provided a useful framework within which to describe the functioning of those with HIV and the impact of the environment. Given the wide spectrum of problems found, consideration could be given to a number of ICF core sets that are relevant to the different stages of HIV disease. The Global Initiative Against Asthma (GINA) was developed to meet the global challenge of asthma. GINA has been adopted in most countries and comparison of asthma management in different parts of the world may be of help when assessing the global dissemination of the guideline. The overall goals in GINA include that asthma patients should be free of symptoms, acute asthma attacks and activity limitations. The aim of the present study was to compare asthma management and asthma control in Sao Paulo, Brazil and Uppsala, Sweden. Information was collected from asthmatics in Sao Paulo and Uppsala with a questionnaire. The questionnaire dealt with the following issues: symptoms, smoking, self-management, hospital visits, effect on school/work and medication. The Sao Paulo patients were more likely to have uncontrolled asthma (36% vs 13%, P < 0.001), having made emergency room visits (57% vs 29%, P < 0.001) and having lost days at school or work because of their asthma (46% vs 28%, P = 0.03) than the asthmatics from Uppsala. There were no difference in the use of inhaled corticosteroids, but the Brazilian patients were more likely to be using theophylline (18% vs 1%, P = 0.001) and less likely to be using long-acting beta-2 agonists (18% vs 37%, P < 0.001). We conclude that the level of asthma control was lower among the patients from Sao Paulo than Uppsala. Few of the patients in either city reached the goals set up by GINA. Trata-se de um estudo descritivo realizado com cinquenta mulheres em pós-parto vaginal com episiotomia. Objetivou-se mensurar e caracterizar a dor perineal em primíparas submetidas ao parto normal com episiotomia e verificar as atividades limitadas pela dor. Para avaliação da dor foi utilizada a Escala Numérica e o Questionário McGill, bem como um formulário para analisar as atividades que estavam limitadas. A média de dor perineal encontrada foi cinco. As categorias sensorial e avaliação subjetiva foram as mais selecionadas no Questionário McGill. A dor perineal foi caracterizada como latejante, que repuxa, que esquenta, ardida, dolorida, chata, incômoda, que prende e que deixa tensa. Sentar, deitar e deambular foram as atividades mais limitadas. Em conclusão, foi possível verificar a presença de dor perineal nas puérperas causada pela episiotomia e identificar que as atividades de sentar, deitar e deambular estavam limitadas por este sintoma.

Goal: The Halex is an indicator of health status that combines self-rated health and activity limitations, which has been used by NCHS to predict future years of healthy life. The scores for each health state were developed based on strong assumptions, notably that a person in excellent health with ADL disabilities is as healthy as a person in poor health with no disabilities. Our goal was to examine the performance of the Halex as a longitudinal measure of health for older adults, and to improve the scoring if necessary. Methods: We used data from the Cardiovascular Health Study (CHS) to compare the relationship of baseline health to health 2 years later. Subject ages ranged from 65 to 103 (mean age 75). A total of 40,827 transitions were available for analysis. We examined whether Halex scores at time 0 were related monotonically to scores two years later, and iterated the original scores to improve the fit over time. Findings: The original Halex scores were not consistent over time. Persons in excellent health with ADL limitations were much healthier 2 years later than people in poor health with no limitations, even though they had been assumed to have identical health. People with ADL limitations had higher scores than predicted. The assumptions made in creating the Halex were not upheld in the data. Conclusions: The new iterated scores are specific to older adults, are appropriate for longitudinal data, and are relatively assumption-free. We recommend the use of these new scores for longitudinal studies of older adults that use the Halex health states.
