Objective: To present the clinical and radiographic results of a case series of patients with acute acromioclavicular dislocation (MD) treated by arthroscopic coracoclavicular fixation with suture anchors. Method: Twenty patients with AAD with less than 30 days since the injury were submitted to a coracoclavicular stabilization procedure using 2 suture anchors placed at the base of the coracoid process. Each suture anchor was connected to 2 strands of No.2 nonabsorbable-braided sutures, which were passed through the holes drilled in the clavicle and tied to the upper surface of the clavicle. The coracoclavicular distance was measured and compared to the opposite side using radiographic evaluation. Constant and UCLA scores were used to determine clinical and functional evaluation after 6 months. Results: Of the initial twenty cases, six were submitted to a new surgical procedure and were excluded from the study. Of the fourteen patients remaining, only two maintained the initial reduction, while the remainder presented some degree of reduction loss. The Constant and UCLA score averages were 94.79 (82-100) and 32.64 (26-35) respectively. Conclusion: The technique had a high incidence of reduction loss after 6 months of follow up. The clinical and functional evaluation was satisfactory, with a high average score. Level of Evidence: Level III, retrospective study.

The purpose of this study was to assess the results of acute grade I and II acromioclavicular (AC) joint sprains treated by conservative measures. Between 1993 and 1997, 37 consecutive patients were treated conservatively for AC joint sprains, grade I and II in the Tossy classification. Of these patients, 4 were excluded (three lost to follow-up and one sustained a further AC injury), leaving a series of 33 patients. Among them, in 9 (27%), chronic AC joint pathology that required subsequent surgery developed at a mean of 26 months after injury. The remaining 24 were reviewed clinically and radiologically at a mean of 6.3 years (range, 4-8 years) after injury. At the latest follow-up, 17 of the 33 patients (52%) remained asymptomatic. Of the 24 patients reviewed, 7 complained of activity-related pain. Eight patients presented with residual anteroposterior instability. Tenderness at the AC joint as well as a positive cross-body test was observed in 12 patients. The mean Constant score at follow-up was 82 points. The x-ray films showed degenerative changes in 13 patients, ossification of the coracoclavicular ligaments in 2, an association of degenerative changes with ossification of the coracoclavicular ligaments in 3, and distal clavicular osteolysis in 3. Only 4 cases had no radiographic changes after this kind of AC injury. On the basis of these results, we conclude that the severity of the consequences after grade I and II AC sprains is underestimated.

OBJECTIVE: (1) To analyse the imaging appearances of nine patients with acromioclavicular joint cysts presenting as shoulder masses for tumor staging with operative, histopathological and joint aspiration findings.DESIGN AND PATIENTS: Retrospective review of imaging and correlation with clinical, operative and surgical notes. Images were reviewed by two musculoskeletal radiologists by consensus. Nine patients who presented clinically with a shoulder mass were evaluated by radiographs (n=9), ultrasound (n=1), conventional arthrography (n=3), MRI (n=6; with direct MR arthrography n=2, indirect MR arthrography n=4).RESULTS: All patients had a focal mass superior to the AC joint, with a size ranging from 1.5 cm to 6 cm and a mean of 3.27 cm. Correlation was available with surgery (n=7), histopathology (n=2) and cyst aspiration (n=2). Two patients were managed conservatively. Geyser sign was positive in all three arthrograms. All MRIs revealed extensive rotator cuff tears with a column of fluid extending from the glenohumeral joint through the rotator cuff tear into the acromioclavicular joint and acromioclavicular cyst. Chondrocalcinosis was seen in the acromioclavicular joint cyst (n=2) and in the glenohumeral joint (n=1). Aspirate in two patients contained calcium pyrophosphate dihydrate crystals.CONCLUSION: Acromioclavicular joint cysts may present as a tumor mass. They are associated with extensive rotator cuff tears and there is usually communication of the cyst with the joint space. This feature excludes a diagnosis of tumor. AC joint cysts may be associated with calcium pyrophosphate dihydrate deposition disease.

The different approaches used in arthroscopic stabilisation of the acromioclavicular joint are well known. However, and despite a great incidence of ectopic pectoralis minor insertion, an alternative choice for the use of arthroscopic portal has not being sufficiently described. Here, we describe a case of acute acromioclavicular dislocation grade III. The arthroscopic stabilisation was achieved using the TightRope (Arthrex, Naples, USA) implant. Through this technique, the approach to the articular portion of the coracoid process can be made intra-articularly or from the subacromial space. We accessed intra-articularly, by opening the rotator interval to reach the coracoid process from the joint cavity. After opening the rotator interval, an ectopic insertion of the pectoralis minor was observed. The choice of approach of the coracoid process from the subacromial space would have complicated the intervention, making it necessary to sever the ectopic tendon to complete the technique, lengthening the surgical time and increasing the chance of complications. For this reason, the use of a standard posterior portal providing intra-articular arthroscopic access through the rotator interval is recommended since the aforementioned anatomical variation is not infrequent. ANTECEDENTES: La luxación acromioclavicular es la lesión de hombro más prevalencia en quienes practican deportes de contacto, se relaciona con una alta tasa de accidentes de tránsito, por ello es importante conocer su prevalencia, factores de riesgo, para realizar un tratamiento adecuado. OBJETIVO GENERAL: Determinar la prevalencia de la luxación acromioclavicular y tratamientos en pacientes del Hospital José Carrasco Arteaga en el periodo enero 2010 a diciembre 2014. METODOLOGÍA: Se realizó un estudio descriptivo transversal, cuyo universo fueron las historias clínicas de pacientes que recibieron atención en el Hospital José Carrasco Arteaga entre enero del año 2010 hasta diciembre del 2014. Se recolectó la información de la base de datos (AS 400) en un formulario específico, el cual se analizó mediante el paquete estadístico SPSS 15.0 y Epi-Info 7, utilizando distribuciones de frecuencia y porcentajes. Los resultados se presentaron en gráficos, tablas simples y combinadas. USO DE RESULTADOS: De 120 pacientes con diagnóstico de luxación acromioclavicular se presentó una prevalencia de 3,7 casos por cada 1000 personas que se atendieron en emergencia del hospital José Carrasco Arteaga. Se incluyeron 86 casos en el estudio. Los hombres resultaron el grupo más afectado (91,9%). El tipo de luxación más frecuente fue el III (59,3%). La lesión que más asociada fue la fractura de clavícula (4,7%). El mecanismo de lesión directo (91,9%) fue el más prevalente. En 41.9 % de los casos la luxación se asoció con accidentes de tránsito

A retrospective cohort. To report the incidence rates of shoulder injuries diagnosed with magnetic resonance imaging (MRI) in tetraplegic athletes and sedentary tetraplegic individuals. To evaluate whether sport practice increases the risk of shoulder injuries in tetraplegic individuals. Campinas, Sao Paulo, Brazil. Ten tetraplegic athletes with traumatic spinal cord injury were selected among quad rugby athletes and had both the shoulders evaluated by MRI. They were compared with 10 sedentary tetraplegic individuals who were submitted to the same radiological protocol. All athletes were male with a mean age of 32.1 years (range 25-44 years, s.d.=6.44). Time since injury ranged from 6 to 17 years, with a mean value of 9.7 years and s.d. of 3.1 years. All sedentary individuals were male with a mean age of 35.9 years (range 22-47 years, s.d.=8.36). Statistical analysis showed a protective effect of sport in the development of shoulder injuries, with a weak correlation for infraspinatus and subscapularis tendinopathy (P=0.09 and P=0.08, respectively) and muscle atrophy (P=0.08). There was a strong correlation for acromioclavicular joint (ACJ) and labrum injuries (P=0.04), with sedentary individuals at a higher risk for these injuries. Tetraplegic athletes and sedentary individuals have a high incidence of supraspinatus tendinosis, bursitis and ACJ degeneration. Statistical analysis showed that there is a possible protective effect of sport in the development of shoulder injuries. Weak evidence was encountered for infraspinatus and subscapularis tendinopathy and muscle atrophy (P=0.09, P=0.08 and P=0.08, respectively). Strong evidence with P=0.04 suggests that sedentary tetraplegic individuals are at a greater risk for ACJ and labrum injuries.Spinal Cord advance online publication, 17 March 2015; doi:10.1038/sc.2014.248.

Com o presente estudo pretendeu-se: (1) verificar se a ligadura funcional para a lesão da articulação Acrómio-Clavicular (AC), descrita por Chanussot e Danowski (2001), contribui para o restabelecimento da força muscular num paciente com lesão grau III; e, (2) observar se a ligadura funcional altera o padrão electromiográfico dos principais músculos superficiais do complexo articular do ombro: infra-espinhoso, deltóide anterior, grande peitoral, trapézio superior e trapézio inferior. Efectuou-se assim um estudo de caso do tipo observacional, descritivo transversal. Para avaliação da força muscular foi utilizado um dinamómetro isocinético e para a avaliação do nível de activação de fibras musculares foi utilizada a EMG de superfície. Os movimentos em análise foram os de rotação interna e externa, a 60º/s e 180º/s. Os resultados revelaram, na presença de ligadura, um aumento do Peak Torque dos rotadores internos, assim como da actividade do grande peitoral e trapézio inferior. Constatou-se ainda uma diminuição do Peak Torque e da percentagem do Root Mean Square do trapézio inferior, trapézio superior e grande peitoral, do ombro esquerdo, sem ligadura, quando comparado com o ombro contralateral. A ligadura funcional, numa entorse da acromio-clavicular grau III, poderá contribuir para a recuperação de força muscular e activação de fibras musculares.

Paciente masculino de 24 años, ingresa a urgencias por trauma contundente en clavícula derecha durante accidente de tránsito. Presenta dolor en articulación esternoclavicular que se exacerba con la movilización del hombro y disfagia. Al examen físico, leve depresión del extremo medial de la clavícula y limitación de los arcos de movimiento del hombro. Las imágenes confirmaron el diagnostico de LEC posterior. El paciente fue llevado a cirugía para reducción y cerclaje, sin complicaciones.
