Infants and young children are at particular risk of iron deficiency and its associated consequences for growth and development. The main objectives of this thesis were to quantify iron intakes, status and determinants of status in two year olds; explore determinants of neonatal iron stores; investigate associations between iron status at birth and two years with neurodevelopmental outcomes at two years and explore the influence of growth on iron status in early childhood, using data from the Cork BASELINE (Babies after SCOPE: Evaluating Longitudinal Impact using Neurological and Nutritional Endpoints) Birth Cohort Study (n=2137). Participants were followed prospectively with interviewer-led questionnaires and clinical assessments at day 2 and at 2, 6, 12 and 24 months. At two years, there was a low prevalence of iron deficiency and iron deficiency anaemia in this cohort, representing the largest study of iron status in toddlers in Europe, to date. The increased consumption of iron-fortified products and compliance with recommendations to limit unmodified cows’ milk intakes in toddlers has contributed to the observed improvements in status. Low serum ferritin concentrations at birth, which reflect neonatal iron stores, were shown to track through to two years of age; delivery by Caesarean section, being born small-for-gestational age and maternal obesity and smoking in pregnancy were all associated with significantly lower neonatal iron stores. Despite a low prevalence of iron deficiency in this cohort, both a mean corpuscular volume <74fl and ferritin concentrations <20μg/l were associated with lower neurodevelopmental outcomes at two years. An inverse association between growth in the second year of life and iron status at two years was also observed. This thesis has presented data from one of the largest, extensively-characterised cohorts of young children, to date, to explore iron and its associations with growth and development.

De nombreuses études empiriques ont démontré que la qualité des relations parent-enfant est importante pour le développement des fonctions exécutives (FE) des enfants. Cependant, la majorité des études ont porté sur des échantillons de mères ou de pères, mais non des deux. Le présent mémoire contient un article empirique qui poursuit deux buts. Premièrement, l’article a examiné la contribution unique de la qualité des interactions mère-enfant et père-enfant avec leur bambin (toddler) à la prédiction des FE en milieu scolaire. Deuxièmement, l’article a investigué les effets d'interactions entre la qualité des relations mère-enfant et père-enfant. L’étude a été menée auprès de 46 familles intactes (mère-père-enfant). Lorsque les enfants avaient 18 mois, la qualité des interactions mère-enfant et père-enfant a été mesurée par observation de séquences indépendantes de jeu avec le Mutually Responsive Orientation scale. À la maternelle, les problèmes exécutifs des enfants furent rapportés par le professeur à l’aide du Behavior Rating Inventory of Executive Function. Les résultats indiquent que les enfants qui ont des interactions de meilleure qualité avec leur père à 18 mois sont ensuite considérés par leur professeur de maternelle comme ayant moins de déficits exécutifs. Cela suggère que la relation père-enfant peut être un facteur important à considérer en ce qui concerne le développement des FE des enfants. Les implications théoriques et empiriques ainsi que les implications pratiques, notamment celles concernant les professeurs, sont abordées lors de la conclusion de ce mémoire.

I denna studie har vi undersökt fyra förskolepedagogers förhållningssätt, normer och tankar kring genus samt om deras uppfattning stämmer överens med deras agerande i den praktiska verksamheten. Syftet med denna studie var att få mer kunskap om hur genusmedvetenheten kan se ut på förskolan idag samt att ta reda på om alla barn ges samma förutsättningar i vår studerade leksituation; bygg och konstruktionsleken. En framträdande frågeställning har varit att få fatt vilka normer som råder bland pedagogerna och hur de tar sig uttryck i verksamheten. Som teoretisk ansats har vi använt oss utav feministisk poststrukturalism och data har vi samlat in genom observationer av verksamheten samt intervjuer med pedagogerna. Resultatet visar på att det finns en motsättning mellan pedagogernas egna uppfattningar om sitt förhållningssätt och deras agerande i det praktiska arbetet. I resultatet framkom även att pedagogerna såg sig själva som genusneutrala, trots detta framträdde en viss könstraditionell syn på föreställningen kring barns agerande och fysiska uttryck. Gällande alla barns likvärdiga förutsättningar visar ett av våra huvudresultat på en tydlig diskriminering gentemot de yngsta barnen på förskolan.

Die Betreuungsfrage für Kleinstkinder unter drei Jahren ist bislang unbeantwortet. Ein Ausbau der Krippenbetreuung wird häufig mit Hinweis auf positive Forschungsbefunde aus dem Ausland favorisiert. In einer Übersicht wurden die Vorbedingungen für eine solche Übertragung geprüft: methodische Zuverlässigkeit der Krippenstudien, Konsistenz der Ergebnisse sowie eindeutige Kenntnisse über die Rahmenbedingungen der international untersuchten Krippen. Diese Minimalvoraussetzungen für eine Übertragung erwiesen sich als nicht gegeben. Der Verfasser plädiert für individuellere Betreuungsformen, solange die Qualitätsstandards deutscher Krippen nicht auf einem Niveau sind, auf dem Entwicklungsrisiken ausgeschlossen werden können. OBJECTIVE: To comprehensively review observational and experimental studies examining the relationship between sedentary behavior and cognitive development during early childhood (birth to 5years). METHOD: Electronic databases were searched in July, 2014 and no limits were imposed on the search. Included studies had to be peer-reviewed, published, and meet the a priori determined population (apparently healthy children aged birth to 5years), intervention (duration, types, and patterns of sedentary behavior), comparator (various durations, types, or patterns of sedentary behavior), and outcome (cognitive development) study criteria. Data extraction occurred in October and November 2014 and study quality and risk of bias were assessed in December 2014. RESULTS: A total of 37 studies, representing 14,487 participants from nine different countries were included. Thirty-one studies used observational study designs and six studies used experimental study designs. Across study designs, increased or higher screen time (most commonly assessed as television viewing (TV)), reading, child-specific TV content, and adult-specific TV content had detrimental (negative) associations with cognitive development outcomes for 38%, 0%, 8%, and 25% of associations reported, respectively, and beneficial (positive) associations with cognitive development outcomes for 6%, 60%, 13%, and 3% of associations reported, respectively. Ten studies were moderate quality and 27 studies were weak quality. CONCLUSIONS: The type of sedentary behavior, such as TV versus reading, may have different impacts on cognitive development in early childhood. Future research with reliable and valid tools and adequate sample sizes that examine multiple cognitive domains (e.g., language, spatial cognition, executive function, memory) are needed. To explore the relationship between caregiver characteristics and the adequacy of domestic swimming pool fencing.A typical metropolitan area of a large Australian capital city, Brisbane.From a reanalysis of the dataset of the 1989 Brisbane Home Safety Survey of 1050 householders, associations between 10 caregiver factors, pool ownership, and quality of pool fencing, were analysed. Household characteristics relating to toddlers (children < or = 4 years), and socioeconomic measures were also included in the analyses. Pool fencing quality was measured on an ordinal scale derived from Australian Standards Association guidelines, confirmed through home visits by trained inspectors.Caregiver factors did not distinguish households with a swimming pool from those without, nor were they associated with adequacy of pool fencing among pool owners. Pool owners, with or without children, were less likely to perceive having a childproof fence as being important. Strongest correlates of adequacy of pool fencing were socioeconomic indicators of surrounding districts.These results do not support the arguments of opponents of compulsory pool fencing that caregiver factors are adequate to prevent toddler drownings and obviate the need for a pool fence. Pool owners do not appear to perceive their pool as a hazard for young children, and complacency about the adequacy of pool fencing needs to be replaced by increased caregiver health beliefs, skills, and perceptions. BACKGROUND: Almost half of young children do not achieve minimum recommendations of 60 daily minutes in physical activity. Physical activity is potentially an important determinant of the development of motor competence in children. This study is one of very few longitudinal studies in this area and the first to investigate early childhood physical activity as a predictor of subsequent motor skill competence. METHODS: Children were assessed as part of the Melbourne InFANT Program longitudinal cohort study at 19 months, 3.5 years and 5 years. Moderate-to-vigorous physical activity (MVPA) (accelerometry) was assessed at each time point. At age 5, children were also assessed in actual (Test of Gross Motor Development-2) and perceived motor competence (Pictorial Scale of Perceived Movement Skill Competence). General linear models were performed with all 12 skills (six object control and six locomotor skills), both actual and perceived, at age 5 as the respective outcome variables. Predictor variables alternated between MVPA at 19 months, 3.5 years and 5 years. RESULTS: Based on standardized TGMD-2 scores most children were average or below in their skill level at age 5. MVPA at 19 months was not a predictor of actual or perceived skill at age 5. MVPA at 3.5 years was associated with actual locomotor skill (B = 0.073, p = 0.033) and perceived total skill at 5 years of age (B = 0.059, p = 0.044). MVPA was not a predictor of actual or perceived object control skill at any age. CONCLUSION: Parents and preschool staff should be informed that more time in MVPA as a preschool child contributes to locomotor skill and to perceptions of skill ability in a child of school starting age. Understanding this relationship will assist in intervention development.

• Introducción: El síndrome de abstinencia (SA) es el conjunto de síntomas y signos que se producen al suspender bruscamente la administración de un fármaco una vez se haya establecido dependencia física. • Objetivos: Caracterizar los pacientes que presentan SA secundario a opiodes (OP) y/o benzodiacepinas(BZ) durante la hospitalización en las unidades de cuidados intensivos pediátricos de la Clínica Infantil Colsubsidio (CIC) y Hospital del Niño de Panamá (HDN) del 1 de abril al 30 de septiembre del 2016. • Materiales y métodos: se realizó un estudio descriptivo, longitudinal, prospectivo. Incluimos 189 pacientes en la CIC y 144 pacientes en el HDN. Se utilizó la escala SOPHIA para el diagnóstico de SA, las escalas COMFORT para evaluar la sedación en pacientes ventilados no relajados y la escala FLACC para evaluar la analgesia. Se utilizó software StataV12® para el análisis estadístico. • Resultados: se reportó una incidencia global de SA de 6.1/100 días personas. La incidencia acumulada de SA fue de 56.08% y 29.86% para la CIC y el HDN respectivamente. En la CIC el 69.81% de los pacientes que requirieron infusión de OP y BZ desarrollaron SA. Se reportó una dosis acumulada de fentanyl de 530.34 ± 276.49 mcg/kg. Con respecto al HDN, de los pacientes que recibieron opioides y benzodiacepinas el 53.49 % desarrollaron SA. • Conclusión: El SA secundario a opioides y/o benzodiacepinas es frecuente en nuestras unidades con una incidencia variable, es mayor la presentación del SA al usar ambos fármacos, mayores dosis acumuladas y más días de infusión continua.
