RESUMO - A avaliao de necessidades de cuidados crucial no planeamento, monitorizao e avaliao de servios de psiquiatria e sade mental, bem como na investigao e na clnica. Este princpio obviamente aplicvel aos servios responsveis por populaes de pessoas mais velhas. O instrumento CANE Camberwell Assessment of Need for the Elderly possibilita uma avaliao consistente das necessidades de utentes idosos, nomeadamente em situaes de patologia neuropsiquitrica. Procede-se a uma avaliao cruzada, entrevistando a pessoa em questo, o seu cuidador informal e o tcnico responsvel. Esta avaliao multidimensional abrange domnios da esfera biolgica, psicolgica e social, sendo aplicvel na comunidade ou em internamento (regime parcial ou completo). A utilidade do CANE tem sido evidenciada em contextos clnicos, de investigao e de avaliao de servios. Existem mltiplas tradues a nvel internacional, a maioria das quais validada. Na rea da epidemiologia psiquitrica nem sempre esto disponveis os dados relativos qualidade das adaptaes de instrumentos, pelo que se apresenta o processo de desenvolvimento da verso portuguesa (de acordo com as regras para validao transcultural, no processo de traduo-retroverso). A aplicabilidade da verso portuguesa foi satisfatria neste estudo-piloto, representando a primeira fase de um trabalho multicntrico nacional. Nesta fase inicial, foram considerados casos de idosos com patologia neuropsiquitrica (maioritariamente demncia 71,4%), em dois centros (Lisboa e Porto) (n = 21). A mdia de idades foi 73,9 ( 6,3) anos, sendo 76,2% do sexo feminino. A maioria vivia em casa, apresentava co-morbilidade somtica e estava em contacto com um cuidador informal (em geral, familiares do sexo feminino). Os avaliadores identificaram necessidades, nem sempre cobertas, nas seguintes dimenses: cuidados com a casa, alimentao, actividades dirias, memria, sade fsica, sofrimento psicolgico, companhia e dinheiro/economias. Nem sempre a perspectiva de doentes, cuidadores, tcnicos e avaliadores foi inteiramente coincidente. Estes resultados preliminares da aplicao da verso portuguesa do CANE so consistentes quanto sua validade ecolgica, facial e de contedo, estando em curso contributos adicionais para a validao efectiva numa amostra de maior dimenso.

The introduction of engineered nanostructured materials into a rapidly increasing number of industrial and consumer products will result in enhanced exposure to engineered nanoparticles. Workplace exposure has been identified as the most likely source of uncontrolled inhalation of engineered aerosolized nanoparticles, but release of engineered nanoparticles may occur at any stage of the lifecycle of (consumer) products. The dynamic development of nanomaterials with possibly unknown toxicological effects poses a challenge for the assessment of nanoparticle induced toxicity and safety.In this consensus document from a workshop on in-vitro cell systems for nanoparticle toxicity testing11Workshop on 'In-Vitro Exposure Studies for Toxicity Testing of Engineered Nanoparticles' sponsored by the Association for Aerosol Research (GAeF), 5-6 September 2009, Karlsruhe, Germany. an overview is given of the main issues concerning exposure to airborne nanoparticles, lung physiology, biological mechanisms of (adverse) action, in-vitro cell exposure systems, realistic tissue doses, risk assessment and social aspects of nanotechnology. The workshop participants recognized the large potential of in-vitro cell exposure systems for reliable, high-throughput screening of nanoparticle toxicity. For the investigation of lung toxicity, a strong preference was expressed for air-liquid interface (ALI) cell exposure systems (rather than submerged cell exposure systems) as they more closely resemble in-vivo conditions in the lungs and they allow for unaltered and dosimetrically accurate delivery of aerosolized nanoparticles to the cells. An important aspect, which is frequently overlooked, is the comparison of typically used in-vitro dose levels with realistic in-vivo nanoparticle doses in the lung. If we consider average ambient urban exposure and occupational exposure at 5mg/m3 (maximum level allowed by Occupational Safety and Health Administration (OSHA)) as the boundaries of human exposure, the corresponding upper-limit range of nanoparticle flux delivered to the lung tissue is 310-5-510-3μg/h/cm2 of lung tissue and 2-300particles/h/(epithelial) cell. This range can be easily matched and even exceeded by almost all currently available cell exposure systems.The consensus statement includes a set of recommendations for conducting in-vitro cell exposure studies with pulmonary cell systems and identifies urgent needs for future development. As these issues are crucial for the introduction of safe nanomaterials into the marketplace and the living environment, they deserve more attention and more interaction between biologists and aerosol scientists. The members of the workshop believe that further advances in in-vitro cell exposure studies would be greatly facilitated by a more active role of the aerosol scientists. The technical know-how for developing and running ALI in-vitro exposure systems is available in the aerosol community and at the same time biologists/toxicologists are required for proper assessment of the biological impact of nanoparticles.

The Andalusian Public Health System Virtual Library (Biblioteca Virtual del Sistema Sanitario Pblico de Andaluca, BV-SSPA) provides access to health information resources and services to healthcare professionals through its Website. This virtual environment demands higher users knowledge in order to satisfy of the need of information of our users, as digital natives as digital immigrants, improving at the same time the communication with all of them. 1. To collect clients' views and expectations according to their nature of digital natives and immigrants. 2. To know our online reputation. A Collecting User Expectation Questionnaire will be built, taking into account the segmentation of the BV-SSPA users professional groups of the Andalusian Public Health System. A pilot test will be run to check the survey dimensions and items about practices, attitudes and knowledge of our users. Two Quality Function Deployment (QFD) matrices will enable the BV-SSPA services to be targeted to our digital natives or digital immigrants, according to their nature, finding the best way to satisfy their information needs. We provide feedback on BV-SSPA: users can have the opportunity to post feedback about the site via the 'Contact us' section and comment about their experience. And Web 2.0 is a shop window, providing the opportunity to show the comments; and through time, our online reputation will be built, but the BV-SSPA must manage its own personal branding. Web 2.0 tools are a driver of improvement, because they provide a key source of insight into people's attitudes. Besides, the BV-SSPA digital identity will be analyzed through indicators like major search engine referrals breakdown, top referring sites (non search engines), or top search engine referral phrases, among others. Definition of digital native and digital immigrant profiles of the BV-SSPA, and their difference, will be explained by their expectations. The design of the two QFD matrices will illustrate in just one graph the requirements of both groups for tackling digital abilities and inequalities. The BV-SSPA could deliver information and services through alternative channels. On the other hand, we are developing a strategy to identify, to measure and to manage a digital identity through communication with the user and to find out our online reputation. With the use of different tools from quantitative and qualitative methodology, and the opportunities offered by Web 2.0 tools, the BV-SSPA will know the expectations of their users as a first step to satisfy their necessities. Personalization is pivotal to the success of the Site, delivering tailored content to individuals based on their recorded preferences. The valuable user research can be used during new product development and redesign. Besides positive interaction let us build trust, show authenticity, and foster loyalty: we improve with effort, communication and show.

OBJECTIVE: To evaluate parents' and nurses' opinions regarding the adequacy of an educational program on shaken baby syndrome: the Perinatal Shaken Baby Syndrome Prevention Program (PSBSPP). DESIGN: Qualitative and quantitative assessments in the form of interviews and questionnaires administered in French. SETTING: Two birthing institutions in Montral, QC, Canada: a university hospital and a regional center. PARTICIPANTS: Two hundred and sixty-three parents (73.8% mothers, 26.2% fathers) received the intervention after the birth of their child, and 69 nurses administered it. METHODS: Parents' and nurses' assessments of the adequacy and relevance of the program and nurses' assessments of the training they received to administer the program were evaluated. RESULTS: Both parents and nurses supported this initiative. Most parents appreciated the usefulness of the information. Nurses believed the program was adequate, and their training to deliver the program was satisfactory. All participants reported that the program was highly relevant, especially for new parents. CONCLUSION: The Perinatal Shaken Baby Syndrome Prevention Program achieves the goals of (a) increasing parents' knowledge about infant crying, anger, and shaken baby syndrome and (b) helping parents identify coping strategies. The relevance of introducing the PSBSPP in all birthing institutions is supported. Future studies should focus on vulnerable and culturally diverse populations, and longitudinal follow-up could help determine if the PSBSPP reduces the incidence of shaken baby syndrome.

The 2002 Iowa Youth Survey (IYS) State of Iowa report was designed to help state-level planners identify youth development-related needs, develop relevant programs, and assess the outcomes of those programs. These data can help us better understand our youth and their needs. They can help us assess the strengths and weaknesses of our schools, families and communities from the young persons perspective. In addition, the data in this report help the state obtain funds for a wide variety of programs. At every step in the process from needs identification, to program development and implementation, to program assessment the 2002 IYS data will provide a valuable resource. The state report can also help Iowas schools, area education agencies and counties assess their relative strengths and weaknesses. The grades 6, 8, and 11, as well as male and female percentages reported in each of these reports can be compared with the respective state report percentages. The higher the proportion of students in each of these columns that completed a usable IYS questionnaire, the more likely the comparisons with the state report percentages will be unbiased. Such comparisons should be considered exploratory, but for the most part are likely to prove useful.

The historically-reactive approach to identifying safety problems and mitigating them involves selecting black spots or hot spots by ranking locations based on crash frequency and severity. The approach focuses mainly on the corridor level without taking the exposure rate (vehicle miles traveled) and socio-demographics information of the study area, which are very important in the transportation planning process, into consideration. A larger study analysis unit at the Transportation Analysis Zone (TAZ) level or the network planning level should be used to address the needs of development of the community in the future and incorporate safety into the long-range transportation planning process. In this study, existing planning tools (such as the PLANSAFE models presented in NCHRP Report 546) were evaluated for forecasting safety in small and medium-sized communities, particularly as related to changes in socio-demographics characteristics, traffic demand, road network, and countermeasures. The research also evaluated the applicability of the Empirical Bayes (EB) method to network-level analysis. In addition, application of the United States Road Assessment Program (usRAP) protocols at the local urban road network level was investigated. This research evaluated the applicability of these three methods for the City of Ames, Iowa. The outcome of this research is a systematic process and framework for considering road safety issues explicitly in the small and medium-sized community transportation planning process and for quantifying the safety impacts of new developments and policy programs. More specifically, quantitative safety may be incorporated into the planning process, through effective visualization and increased awareness of safety issues (usRAP), the identification of high-risk locations with potential for improvement, (usRAP maps and EB), countermeasures for high-risk locations (EB before and after study and PLANSAFE), and socio-economic and demographic induced changes at the planning-level (PLANSAFE).

Traumatic Brain Injury (TBI) impacts the lives of thousands of Iowans every year. TBI has been described as the Silent Epidemic because so often the scars are not visible to others. The affects of brain injury are cognitive, emotional, social, and can result in physical disability. In addition to the overwhelming challenges individuals with brain injury experience, families also face many difficulties in dealing with their loved ones injury, and in navigating a service delivery system that can be confusing and frustrating. In 1998, the Iowa Department of Public Health (IDPH) conducted a comprehensive statewide needs assessment of brain injury in Iowa. This assessment led to the development of the first Iowa Plan for Brain Injury, Coming Into Focus. An updated state plan, the Iowa Plan for Brain Injuries 2002 2005, was developed, which reported on progress of the previous state plan, and outlined gaps in service delivery in Iowa. Four areas of focus were identified by the State Plan for Brain Injuries Task Force that included: 1) Expanding the Iowa Brain Injury Resource Network (IBIRN); 2) Promoting a Legislative and Policy Agenda, While Increasing Legislative Strength; 3) Enhancing Data Collection; and, 4) Increasing Funding. The IDPH utilized Coming Into Focus as the framework for an application to the federal TBI State Grant Program, which has resulted in more than $900,000 for plan implementation. Iowa continues to receive grant dollars through the TBI State Grant Program, which focuses on increasing capacity to serve Iowans with brain injury and their families. Highlighting the success of this grant project, in 2007 the IDPH received the federal TBI Programs Impacting Systems Change Award. The Iowa Brain Injury Resource Network (IBIRN) is the product of nine years of TBI State Grant Program funding. The IBIRN was developed to ensure that Iowans got the information and support they needed after a loved one sustained a TBI. It consists of a hospital and service provider pre-discharge information and service linkage process, a resource facilitation program, a peer-to-peer volunteer support network, and a service provider training and technical assistance program. Currently over 90 public and private partners work with the IDPH and the Brain Injury Association of Iowa (BIA-IA) to administer the IBIRN system and ensure that families have a relevant and reliable location to turn for information and support. Further success was accomplished in 2006 when the Iowa legislature created the Brain Injury Services Program within the IDPH. This program consists of four components focusing on increasing access to services and improving the effectiveness of services available to individuals with TBI and their families, including: 1) HCBS Brain Injury Waiver-Eligible Component; 2) Cost Share Component; 3) Neuro-Resource Facilitation; and, 4) Enhanced Training. The Iowa legislature appropriated $2.4 million to the Brain Injury Services Program in state fiscal year (SFY) 2007, and increased that amount to $3.9 million in SFY 2008. The Cost Share Component models the HCBS Brain Injury Waiver menu of services but is available for Iowans who do not qualify functionally or financially for the Waiver. In addition, the Neuro-Resource Facilitation program links individuals with brain injury and their families to needed supports and services. The Iowa Plan for Brain Injury highlights the continued need for serving individuals with brain injury and their families. Additionally, the Plan outlines the paths of prevention and services, which will expand the current system and direct efforts into the future.

Human biomonitoring (HBM) is an effective tool for assessing actual exposure to chemicals that takes into account all routes of intake. Although hair analysis is considered to be an optimal biomarker for assessing mercury exposure, the lack of harmonization as regards sampling and analytical procedures has often limited the comparison of data at national and international level. The European-funded projects COPHES and DEMOCOPHES developed and tested a harmonized European approach to Human Biomonitoring in response to the European Environment and Health Action Plan. Herein we describe the quality assurance program (QAP) for assessing mercury levels in hair samples from more than 1800 mother-child pairs recruited in 17 European countries. To ensure the comparability of the results, standard operating procedures (SOPs) for sampling and for mercury analysis were drafted and distributed to participating laboratories. Training sessions were organized for field workers and four external quality-assessment exercises (ICI/EQUAS), followed by the corresponding web conferences, were organized between March 2011 and February 2012. ICI/EQUAS used native hair samples at two mercury concentration ranges (0.20-0.71 and 0.80-1.63) per exercise. The results revealed relative standard deviations of 7.87-13.55% and 4.04-11.31% for the low and high mercury concentration ranges, respectively. A total of 16 out of 18 participating laboratories the QAP requirements and were allowed to analyze samples from the DEMOCOPHES pilot study. Web conferences after each ICI/EQUAS revealed this to be a new and effective tool for improving analytical performance and increasing capacity building. The procedure developed and tested in COPHES/DEMOCOPHES would be optimal for application on a global scale as regards implementation of the Minamata Convention on Mercury.

The purpose of my research was to examine how community-based organizations in the Niagara region provide programs for children with Autism Spectrum Disorder (ASD), who are considered to represent extreme or severe cases. A qualitative, comparative case study was conducted that focused on three organizations who provide summer recreation and activity programs, in order to examine the issues these organizations face when determining program structure and staff training; and to understand what the threshold for physical activity is in this type of setting, and how the unique needs surrounding these severe cases are met while attending the program. Purposeful sampling was employed to select a supervisor and senior staff member from each organization to discuss the training process, program development and implementation, and the resources and strategies used within their organizations community-based program. A confirming comparative analysis was comparative analysis of a parents survey with six mothers whose children are considered severe indicated that camp staffs expectations are unrealistic where as the parents and supervisors have more realistic expectations within the real world of camp. There is no definition of severe or extreme and therefore severity is dependent upon the context.

The purpose of this project was to raise awareness surrounding child and adolescent mental health in an effort to reduce preconceived stigmas in relation to this specialized field. This project presented a literature review of the current state of child and adolescent mental health in Canada today, including the prevalence and several treatment options for young people confronting mental health challenges. Consideration of the powerful role of the education system upon youth with mental health issues became evident, specifically regarding early identification and prevention. A needs assessment was conducted to gather feedback from the clinical practitioners of a Section 23 classroom within a Southern Ontario hospital. This assessment was used to develop an informational and pedagogical workshop resource to extend practitioner understanding of this pertinent issue and support the social and emotional needs of young people confronting mental heath challenges. Results of the assessment indicated the significant need for such a workshop resource, and these responses were used to guide the development of Group Chat: A Workshop to Support the Emotional and Social Needs of Youth. The latter was subsequently presented to participants, whereby evaluative questionnaires indicated the efficacy and usefulness of this workshop resource to both practitioners and students alike.

Alors que la thorie de lautodtermination (Deci & Ryan, 1985, 2000, 2008) propose un bassin de connaissances pouvant tre dune relle utilit pour aider les travailleurs et les organisations crotre et prosprer, peu dtudes ont port, ce jour, sur lvaluation dinterventions visant implanter ces connaissances sur le terrain en milieu organisationnel. En ce sens, la prsente thse a pour but de vrifier limpact dune formation visant le dveloppement des habilets de soutien aux besoins psychologiques des gestionnaires sur les comportements en emploi de ces gestionnaires ainsi que sur la satisfaction des besoins psychologiques, la motivation et la sant psychologique de leurs employs. Pour ce faire, 17 gestionnaires provenant de deux organisations canadiennes du secteur public ont particip un programme de formation dune dure de trois journes non conscutives. Ils ont galement rpondu des questionnaires un mois avant la formation ainsi que trois quatre mois aprs le dbut de celle-ci afin dvaluer leurs apprentissages ainsi que leurs comportements de soutien aux besoins psychologiques en emploi. Par ailleurs, 55 de leurs employs ont rpondu des questionnaires prformation et postformation au sujet de leur perception lgard des comportements de soutien aux besoins psychologiques de leur suprieur immdiat ainsi que de leur propre niveau de satisfaction des besoins psychologiques, de motivation au travail et de sant psychologique. Ltude a t mene laide dun devis de recherche de type prtest et post-test groupes non-quivalents, sans groupe contrle. Afin de renforcer ce devis, la stratgie de rfrence interne a t utilise auprs des employs (Haccoun & Hamtiaux, 1994). Les rsultats suggrent que, dans la mesure o des gains taient possibles par le biais dune formation plus traditionnelle (c.--d., que le potentiel des gestionnaires samliorer davantage par rapport leur niveau de base tait suffisant et que les habilets vises se dveloppaient bien par le biais dune telle formation), les gestionnaires ont dvelopp leurs habilets de soutien aux besoins psychologiques et, selon leur perspective, transfr leurs apprentissages au travail. Toutefois, les employs de ces gestionnaires nont pas peru de changement statistiquement significatif entre les deux temps de mesure quant aux habilets de soutien aux besoins psychologiques de leur suprieur immdiat en emploi et les retombes escomptes de la formation chez les employs nont pas t observes. la lumire de ces rsultats, le dernier chapitre de la thse discute de limportance de lanalyse des besoins, de limplication de lenvironnement organisationnel dans les initiatives de formation ainsi que de lajout de mthodes dapprentissage complmentaires aux formations plus traditionnelles afin den assurer lefficacit. Par ailleurs, les contributions, limites et pistes de recherches futures de la thse y seront exposes.

This article aims to analyse the progress taken by Brazil towards the accomplishment of a sustainable development, mainly highlighting the success of the Zero Hunger social programme in achieving the elimination of starvation. As one of the essential pillars to the sustainable development, the States social performance demands actions directed to the elimination of extreme poverty and hunger, establishing a basis for equitable growth. It is necessary to clarify the juridical and constitutional framework of Brazil, aiming to emphasise the Zero Hunger importance in achieving the Brazilian Republics goals and reaching an international pattern on sustainable development. Also, it will be stressed that the international order influences Brazilian domestic law and is one of the main aspects for the Zero Hunger program development.
