Tibolone, a synthetic steroid, is effective in the treatment of postmenopausal symptoms. Its cardiovascular safety profile has been questioned, because tibolone reduces the levels of high-density lipoprotein (HDL) cholesterol. Soy-derived isoflavones may offer health benefits, particularly as regards lipids and also other cardiovascular disease (CVD) risk factors. The soy-isoflavone metabolite equol is thought to be the key as regards soy-related beneficial effects. We studied the effects of soy supplementation on various CVD risk factors in postmenopausal monkeys and postmenopausal women using tibolone. In addition, the impact of equol production capability was studied. A total of 18 monkeys received casein/lactalbumin (C/L) (placebo), tibolone, soy (a woman s equivalent dose of 138 mg of isoflavones), or soy with tibolone in a randomized order for 14 weeks periods, and there was a 4-week washout (C/L) in between treatments. Postmenopausal women using tibolone (N=110) were screened by means of a one-week soy challenge to find 20 women with equol production capability (4-fold elevation from baseline equol level) and 20 control women, and treated in a randomized cross-over trial with a soy powder (52 g of soy protein containing 112 mg of isoflavones) or placebo for 8 weeks. Before and after the treatments lipids and lipoproteins were assessed in both monkeys and women. In addition, blood pressure, arterial stiffness, endothelial function, sex steroids, sex hormone-binding globulin (SHBG), and vascular inflammation markers were assessed. A 14% increase in plasma low-density lipoprotein (LDL) + very low-density lipoprotein (VLDL) cholesterol was observed in tibolone-treated monkeys vs. placebo. Soy treatment resulted in a 18% decrease in LDL+VLDL cholesterol, and concomitant supplementation with tibolone did not negate the LDL+VLDL cholesterol-lowering effect of soy. A 30% increase in HDL cholesterol was observed in monkeys fed with soy, whereas HDL cholesterol levels were reduced (48%) after tibolone. Interestingly, Soy+Tibolone diet conserved HDL cholesterol levels. Tibolone alone increased the total cholesterol (TC):HDL cholesterol ratio, whereas it was reduced by Soy or Soy+Tibolone. In postmenopausal women using tibolone, reductions in the levels of total cholesterol and LDL cholesterol were seen after soy supplementation compared with placebo, but there was no effect on HDL cholesterol, blood pressure, arterial stiffness or endothelial function. Soy supplementation decreased the levels of estrone in equol producers, and those of testosterone in the entire study population. No changes were seen in the levels of androstenedione, dehydroepiandrosterone sulfate, or SHBG. The levels of vascular cell adhesion molecule-1 increased, and platelet-selectin decreased after soy treatment, whereas C-reactive protein and intercellular adhesion molecule-1 remained unchanged. At baseline and unrelated to soy treatment, equol producers had lower systolic, diastolic and mean arterial pressures, less arterial stiffness and better endothelial function than non-producers. To conclude, soy supplementation reversed the tibolone-induced fall in HDL cholesterol in postmenopausal monkeys, but this effect was not seen in women taking tibolone. Equol production capability was associated with beneficial cardiovascular changes and thus, this characteristic may offer cardiovascular benefits, at least in women using tibolone.

Bone mass accrual and maintenance are regulated by a complex interplay between genetic and environmental factors. Recent studies have revealed an important role for the low-density lipoprotein receptor-related protein 5 (LRP5) in this process. The aim of this thesis study was to identify novel variants in the LRP5 gene and to further elucidate the association of LRP5 and its variants with various bone health related clinical characteristics. The results of our studies show that loss-of-function mutations in LRP5 cause severe osteoporosis not only in homozygous subjects but also in the carriers of these mutations, who have significantly reduced bone mineral density (BMD) and increased susceptibility to fractures. In addition, we demonstrated for the first time that a common polymorphic LRP5 variant (p.A1330V) was associated with reduced peak bone mass, an important determinant of BMD and osteoporosis in later life. The results from these two studies are concordant with results seen in other studies on LRP5 mutations and in association studies linking genetic variation in LRP5 with BMD and osteoporosis. Several rare LRP5 variants were identified in children with recurrent fractures. Sequencing and multiplex ligation-dependent probe amplification (MLPA) analyses revealed no disease-causing mutations or whole-exon deletions. Our findings from clinical assessments and family-based genotype-phenotype studies suggested that the rare LRP5 variants identified are not the definite cause of fractures in these children. Clinical assessments of our study subjects with LPR5 mutations revealed an unexpectedly high prevalence of impaired glucose tolerance and dyslipidaemia. Moreover, in subsequent studies we discovered that common polymorphic LRP5 variants are associated with unfavorable metabolic characteristics. Changes in lipid profile were already apparent in pre-pubertal children. These results, together with the findings from other studies, suggest an important role for LRP5 also in glucose and lipid metabolism. Our results underscore the important role of LRP5 not only in bone mass accrual and maintenance of skeletal health but also in glucose and lipid metabolism. The role of LRP5 in bone metabolism has long been studied, but further studies with larger study cohorts are still needed to evaluate the specific role of LRP5 variants as metabolic risk factors.

Tutkimuksessa selvitetään oksidatiivisen stressin yhteyttä akuuttiin sepelvaltimotautioireyhtymään. Tämä tehdään vertaamalla veren hapettuneen LDL-kolesterolin pitoisuuksia kolmessa potilasryhmässä. Siinä pohditaan myös LDL-kolesterolin käyttöä biologisena merkkisuureena diagnostiikassa. Tällä hetkellä ei ole biomarkkeria, joka osoittaisi akuutin sydrooman vaarassa olevat potilaat. Hapettuneen LDL-kolesterolin pitoisuus määritettiin potilaiden plasmasta ELISA-menetelmää käyttäen. Tulokset tukevat työhypoteesia ja tutkimus antaa arvokasta tietoa suomalaisten statiinilääkitystä käyttävien sepelvaltimotautipotilaiden oksidatiivisestä stressistä.

Familial hypercholesterolemia (FH) is a common autosomal codominant disease with a frequency of 1:500 individuals in its heterozygous form. The genetic basis of FH is most commonly mutations within the LDLR gene. Assessing the pathogenicity of LDLR variants is particularly important to give a patient a definitive diagnosis of FH. Current studies of LDLR activity ex vivo are based on the analysis of I-125-labeled lipoproteins (reference method) or fluorescent-labelled LDL. The main purpose of this study was to compare the effectiveness of these two methods to assess LDLR functionality in order to validate a functional assay to analyse LDLR mutations. LDLR activity of different variants has been studied by flow cytometry using FITC-labelled LDL and compared with studies performed previously with I-125-labeled lipoproteins. Flow cytometry results are in full agreement with the data obtained by the I-125 methodology. Additionally confocal microscopy allowed the assignment of different class mutation to the variants assayed. Use of fluorescence yielded similar results than I-125-labeled lipoproteins concerning LDLR activity determination, and also allows class mutation classification. The use of FITC-labelled LDL is easier in handling and disposal, cheaper than radioactivity and can be routinely performed by any group doing LDLR functional validations.

Estudos demonstram a associação de alterações da apolipoproteína E (ApoE) e do receptor do LDL (RLDL) com a ocorrência de doenças cardiovasculares e dislipidemia. O objetivo principal deste trabalho foi investigar a associação entre genótipos diferenciais da ApoE e do RLDL com a persistência de alterações de variáveis lipídicas em indivíduos jovens acompanhados há 28 anos no Estudo do Rio de Janeiro (ERJ). Através de um estudo longitudinal, tipo coorte, investigou-se 56 indivíduos (35M) em três avaliações. Em A1 (13.301.53 anos), A2 (22.091.91 anos) e A3 (31.231.99). Nas três ocasiões foi realizada avaliação clínica. Em A2 e A3 foram dosados colesterol total, HDL, LDL e triglicerídeos. Em A3 acrescentou-se o estudo dos polimorfismos genéticos da ApoE e do RLDL. Os fragmentos de interesse neste estudo foram amplificados por PCR (polymerase chain reaction) e os genótipos foram identificados através de reações de restrição. As frequências genotípicas de ApoE foram ε3/ε3 (62,5%), ε3/ε4 (25%), ε2/ε3 (5,4%) ,ε2/ε4 (5,4%) e ε4/ε4 (1,8%) e para os genótipos de RLDL foram AA (85,7%), AT (12,5%) e TT (1,8%). O genótipo ε2/ε2 não foi observado. A análise da distribuição dos genótipos de ApoE segundo a permanência de dislipidemia mostrou que todos os indivíduos com genótipo de ApoE dos tipos ε2/ε4 e ε4/ε4 mantiveram pelo menos um lípide alterado em A2 e A3 entretanto, todos os indivíduos com genótipo de ApoE do tipo ε2/ε3 não apresentaram lípides alterados em A2 e A3. Para o genótipo do RLDL não houve diferença significativa. Quando analisadas isoladamente, não foi identificado nenhum resultado significativo em A2 e/ou A3 associado a estes genótipos. O polimorfismo do gene da ApoE esteve associado à permanência de dislipidemia em indivíduos jovens acompanhados em estudo de seguimento longitudinal

O exercício físico pode promover o desequilíbrio oxidativo e na secreção e ação das adipocinas, leptina e adiponectina, que desempenham papel fundamental no desenvolvimento de doenças cardiometabólicas, incluindo a síndrome metabólica, disfunção endotelial, desordens inflamatórias e vasculares. Militares são submetidos a exercícios físicos intensos e podem apresentar maior risco de doenças cardiovasculares. A identificação de parâmetros capazes de detectar o risco cardiovascular precoce em grupos muito ativos fisicamente é complexa. A razão leptina/adiponectina (L/A) e a concentração de LDL eletronegativa (LDL (-)) vem sendo considerados bons indicadores de risco cardiovascular precoce porém não há estudos que investiguem a utilidade destes marcadores em militares. Os polifenóis, presentes na uva (Vitis vinífera), apresentam efeitos cardioprotetores como inibição da oxidação das lipoproteínas e controle glicêmico. Na presente tese objetivou-se identificar o risco cardiovascular em jovens militares altamente treinados, utilizando diferentes parâmetros, incluindo a razão L/A e investigar a influência do exercício militar e do uso de dose única de Vitis vinifera sobre parâmetros cardiometabólicos de militares fisicamente treinados. O primeiro estudo contou com a participação de 54 militares d gênero masculino, condicionados fisicamente, após descanso de 24h. No segundo estudo, participaram 54 militares do gênero masculino distribuídos aleatoriamente, de forma duplo-cego, para receber única dose contendo 200 mg de extrato seco de Vitis vinifera (GS, n = 27) ou 200 mg de placebo (amido) (GP, n = 27). Considerados em conjunto, os resultados sugerem que militares treinados poderiam ser classificados como limítrofes quanto ao risco cardiovascular, quando somente o perfil lipídico é empregado como marcador. As medidas de CC e CC/E são parâmetros de fácil obtenção e podem ser empregados na identificação do potencial risco cardiovascular, enquanto outros estudos não confirmam a eficácia da razão L/A para este fim. A comparação das concentrações plasmáticas de adiponectina e leptina apresentou diferença significativa apenas intragrupo para ambos os grupos. As concentrações plasmáticas das adipocinas foram influenciadas pelo treinamento. Os indicadores de homeostase glicêmica, glicemia de jejum e insulina plasmática, foram semelhantes entre os grupos, não sendo sendo influenciada pelo treinamento ou suplementação. A suplementação de polifenóis reduziu as concentrações de LDL(-) do GS em relação ao GP em T24h e T48h (p<0,05). Estes achados sugerem que o treinamento físico afeta a secreção das adipocinas independentemente do uso da Vitis vinífera. Entretanto, a dose única de polifenóis pode reduzir o risco cardiometabólico proveniente da oxidação da LDL (-)

Bacterial lipoproteins (LP) are a family of cell wall components found in a wide variety of bacteria. In this study, we characterized the response of HUCL, a telomerase-immortalized human corneal epithelial cell (HCEC) line, to LP isolated from Staphylococcus (S) aureus. S. aureus LP (saLP) prepared by Triton X-114 extraction stimulated the activation of NF-kappa B, JNK, and P38 signaling pathways in HUCL cells. The extracts failed to stimulate NF-kappa B activation in HUCL cells after lipoprotein lipase treatment and in cell lines expressing TLR4 or TLR9, but not TLR2, indicating lipoprotein nature of the extracts. saLP induced the up-regulation of a variety of inflammatory cytokines and chemokines (IL-6, IL-8, ICAM-1). antimicrobial molecules (hBD-2, LL-37, and iNOS), and homeostasis genes (Mn-SOD) at both the mRNA level and protein level. Similar inflammatory response to saLP was also observed in primarily cultured HCECs using the production of IL-6 as readout. Moreover, TLR2 neutralizing antibody blocked the saLP-induced secretion of IL-6, IL-8 and hBD2 in HUCL cells. Our findings suggest that saLP activates TLR2 and triggers innate immune response in the cornea to S. aureus infection via production of proinflammatory cytokines and defense molecules. This paper proposes that atherosclerosis is initiated by a signaling event that deposits calcium hydroxyapatite (Ca-HAP). This event is preceded by a loss of mechanical structure in the arterial wall. After Ca-HAP has been deposited, it is unlikely that it will be reabsorbed because the solubility product constant (K sp) is very small, and the large stores of Ca +2 and PO 4-3 in the bones oppose any attempts to dissolve Ca-HAP by decreasing the common ions. The hydroxide ion (OH -) of Ca-HAP can be displaced in nature by fluoride (F -) and carbonate (CO 3-2) ions, and it is proposed that anions associated with cholesterol ester hydrolysis and, in very small quantities, the enolate of 7-ketocholesterol could also displace the OH -of Ca-HAP, forming an ionic bond. The free energy of hydration of Ca-HAP at 310 K is most likely negative, and the ionic radii of the anions associated with the hydrolysis of cholesterol ester are compatible with the substitution. Furthermore, examination of the pathology of atherosclerotic lesions by Raman and NMR spectroscopy and confocal microscopy supports deposition of Ca-HAP associated with cholesterol. Investigating the affinity of intermediates of cholesterol hydrolysis for Ca-HAP compared to lipoproteins such as HDL, LDL, and VLDL using isothermic titration calorimetry could add proof of this concept and may lead to the development of a new class of medications targeted at the deposition of cholesterol within Ca-HAP. Treatment of acute ischemic events as a consequence of atherosclerosis with denitrogenation and oxygenation is discussed. © the author(s), publisher and licensee Libertas Academica Ltd.
