Apoptosis is a well-known specific process of cell death that normally occurs in physiological situations such as tissue or organ development and involution. During tumor growth there is a balance between proliferation and cell death which involves apoptotic mechanisms. In the present study genomic DNAs from 120 breast tumor biopsies were analyzed by agarose gel electrophoresis and none of them presented the fragmentation pattern characteristic of the apoptosis process. However, 33% of the 105 breast cancer patients clearly showed the apoptotic pattern when DNA from blood cells was analyzed. None of the DNAs from healthy volunteer blood cells showed any trace of apoptosis. Since the breast cancer patients were not receiving chemo- or hormone therapy, the possible relationship between blood cortisol levels and the apoptotic pattern found in patient blood cells was investigated. Using a chemoluminescence immunodetection assay, similar cortisol levels were observed in breast cancer patient sera presenting or not apoptotic blood cells and in healthy volunteer sera. Analysis of the clinical data obtained from 60 of these patients showed that patients bearing tumors of smaller size (under 20 mm) were more susceptible to the apoptotic effect in blood cells. According to the Elston grade, it was observed that 7 of 12 patients with grade III tumors (58%) presented apoptotic peripheral blood cells, in contrast to 10 of 48 patients with grade I and grade II tumors. These observations may reflect the immunosuppression characteristic of some breast cancer patients, which may contribute to tumor growth. Therefore, further studies are necessary to elucidate the factor(s) involved in such massive blood cell death.

Eighty micrograms red blood cell (RBC) ghosts from patients who had previously exhibited the cutaneous form of loxoscelism (presenting localized dermonecrosis) and the viscerocutaneous form of loxoscelism (presenting dermonecrosis, hemoglobinuria, hematuria, and jaundice) and from controls were incubated with 2.5 µg crude Loxosceles gaucho venom in 5 mM phosphate buffer, pH 7.4, at 37ºC. Among all membrane proteins, quantitative proteolysis of the important integral transmembrane protein 3 increased with venom dose and with incubation time from 30 to 120 min, as demonstrated by gel densitometry. Similar quantitative data were obtained for RBC ghosts from patients and from control subjects, a fact that argues against the possibility of genetic factors favoring the hemolytic viscerocutaneous form. These data suggest that the clinical forms may be different types of the same disease, with the viscerocutaneous form being the result of large amounts of intravascularly injected venom and the superficial form being the result of in situ venom action. Since protein 3 is a housekeeping integral membrane protein, whose genetic deficiency leads to hemolytic anemia, it is reasonable to relate it to the hemolysis which occurs in the viscerocutaneous form of loxoscelism. The venom protease responsible for the process was not inhibited after 120-min incubation by 0.2 mM paramethylsulfonyl fluoride or by 0.2 mM N-ethylmaleimide but was inhibited by 25 mM ethylenediaminetetraacetic acid (a calcium-chelating agent) in 5 mM phosphate buffer at pH 7.4, which suggests that the enzyme is a calcium-dependent metalloprotease.

Two different levels of control for bone marrow hematopoiesis are believed to exist. On the one hand, normal blood cell distribution is believed to be maintained in healthy subjects by an "innate" hematopoietic activity, i.e., a basal intrinsic bone marrow activity. On the other hand, an "adaptive" hematopoietic state develops in response to stress-induced stimulation. This adaptive hematopoiesis targets specific lineage amplification depending on the nature of the stimuli. Unexpectedly, recent data have shown that what we call "normal hematopoiesis" is a stress-induced state maintained by activated bone marrow CD4+ T cells. This T cell population includes a large number of recently stimulated cells in normal mice whose priming requires the presence of the cognate antigens. In the absence of CD4+ T cells or their cognate antigens, hematopoiesis is maintained at low levels. In this review, we summarize current knowledge on T cell biology, which could explain how CD4+ T cells can help hematopoiesis, how they are primed in mice that were not intentionally immunized, and what maintains them activated in the bone marrow.

L’anémie est fréquente chez les patients pédiatriques en postopératoire de chirurgie cardiaque. Malgré le nombre important de patients transfusés, le taux d’hémoglobine (Hb) pour lequel les bénéfices surpassent les risques est inconnu chez ces patients. Récemment, Lacroix et al. ont démontré qu’une stratégie transfusionnelle restrictive n’était pas inférieure à une stratégie libérale en ce qui concerne le développement ou la progression du syndrome de défaillance multiviscérale (SDMV) et la mortalité chez les patients de soins intensifs pédiatriques (SIP).Devant le manque d’évidence, une analyse de sous-groupes des patients en postopératoire de chirurgie cardiaque de l’étude Transfusion Requirements in Pediatric Intensive Care (TRIPICU) a été réalisée. L’objectif de cette étude était de déterminer l’impact d’une stratégie transfusionnelle restrictive comparée à une stratégie libérale sur l’acquisition ou l’aggravation du syndrome de défaillance multiviscérale (SDMV) chez les enfants en postopératoire de chirurgie cardiaque. Cette étude n’a pas démontré de différences statistiquement, ni cliniquement significatives du nombre de patients ayant acquis ou aggravés un SDMV, ni des issues secondaires entre les stratégies transfusionnelles restrictive et libérale. L’analyse de sous-groupes permet de générer une hypothèse de recherche et les résultats devraient être confirmés par un essai randomisé contrôlé.

Les transfusions de culots érythrocytaires (CE) sont un traitement fréquent en soins intensifs pédiatriques. Des études chez l’adulte suggèrent qu’une durée prolongée d’entreposage des CE est associée à une mauvaise évolution clinique. Aucune étude prospective n’a été conduite en pédiatrie. Notre objectif était d’évaluer l’effet clinique de la durée d’entreposage des CE chez des patients de soins intensifs pédiatriques. Nous avons donc conduit une étude observationnelle prospective dans 30 centres de soins intensifs pédiatriques en Amérique du Nord, chez tous les patients consécutifs de moins de 18 ans, séjournant aux soins intensifs pendant plus de 48 heures. Le critère de jugement primaire était l’incidence de cas de syndrome de défaillance multiviscérale après transfusion. Les critères de jugement secondaire étaient la mortalité à 28 jours et la durée d’hospitalisation aux soins intensifs. En utilisant un modèle de régression logistique, les risques relatifs furent ajustés pour le sexe, l’âge, la sévérité de la maladie à l’admission, le nombre total de transfusions et la dose totale de transfusion. L’étude a montré que les patients recevant des CE entreposés pendant 14 jours ou plus avaient un risque relatif ajusté de 1.87 (IC 95% 1.04 :3.27, p=0.03) de contracter ou de détériorer un syndrome de défaillance multiviscérale après transfusion. Ces mêmes patients avaient une durée d’hospitalisation aux soins intensifs prolongée (+3.7 jours, p<0.001), mais pas de risque augmenté de mortalité. En conclusion, chez les patients de soins intensifs pédiatriques, la transfusion de CE entreposés 14 jours ou plus est associée avec une augmentation de l’incidence de syndrome de défaillance multiviscérale et une durée d’hospitalisation prolongée aux soins intensifs.

Les transfusions de globules rouges (GR) sont fréquentes aux soins intensifs pédiatriques (SIP). Cependant, il n’y a pas de donnée récente sur les pratiques transfusionnelles aux SIP. Les objectifs de notre étude étaient 1) de décrire les pratiques transfusionnelles aux SIP du CHU Sainte-Justine en y déterminant la fréquence des transfusions de GR et en caractérisant les déterminants de ces transfusions, 2) de comparer ces pratiques avec celles observées il y a dix ans, et 3) d’évaluer le degré d’adhérence à la recommandation principale d’une large étude randomisée contrôlée, l’étude TRIPICU, laquelle proposait une pratique précise chez les patients stabilisés. Nous avons réalisé une étude monocentrique prospective observationnelle d’une durée d’un an. L’information requise a été extraite des dossiers médicaux. Les déterminants des transfusions ont été recherchés quotidiennement jusqu’à la première transfusion pour les cas transfusés, ou jusqu’à la sortie des SIP pour les cas non transfusés. Les justifications des transfusions déclarées par les médecins traitants ont été compilées à l’aide d’un questionnaire. Il y a eu 913 admissions consécutives durant la période d’étude, dont 842 ont été retenues. Au moins une transfusion a été donnée à 144 patients (17.1%). Le taux moyen d’hémoglobine avant la première transfusion était de 77.3±27.2 g/L. Les déterminants d’un premier événement transfusionnel à l’analyse multivariée étaient le jeune âge (< 12 mois), la présence d’une cardiopathie congénitale, un nadir d’hémoglobine ≤ 70 g/L, la gravité de la maladie, et certaines dysfonctions d’organe. Les trois justifications de transfusions les plus fréquemment évoquées par les médecins étaient une hémoglobine basse, un transport en oxygène insuffisant et une instabilité hémodynamique. La recommandation principale de l’étude TRIPICU a été appliquée dans 96.4% des premiers événements transfusionnels. En conclusion, les transfusions de GR sont fréquentes aux SIP. Jeune âge, cardiopathie congénitale, hémoglobine basse, gravité de la maladie et certaines dysfonctions d’organes sont des déterminants significatifs de transfusions de GR aux SIP. La plupart des premiers événements transfusionnels furent prescrits en accord avec les récentes recommandations.

Introducción: Uno de los problemas que afrontan los hospitales públicos de Bogotá es la insuficiencia de unidades de glóbulos rojos- UGR “O”; este trabajo pretendió evaluar a través de indicadores, el comportamiento del manejo de inventarios de sangre en un Hospital público de tercer nivel. Método: Estudio descriptivo restrospectivo, a partir de registros de Red Distrital de Sangre, banco de sangre Hemocentro Distrital y del servicio de transfusión del Hospital. La evaluación de gestión del inventario y eliminación se hizo con el universo de UGR; para la obtención de: reserva/transfusión, tiempo/reserva y la transfusión no isogrupo, se utilizó una muestra del universo de pruebas cruzadas completas (6.575), Resultados: El Hospital recibió 4.644 UGR, con 4.719 transfundidas, reflejando inconsistencias en estos datos. La participación de transfusiones por servicio fue: Urgencias 22,6%(n=1072), cirugía 21,6%(n=1025), UCI adulto 19,2%(n=912), medicina interna del 17,6%(n=836), ginecología del 6,8%(n=323). El indice WAPI fue de 0,19%(n=9) UGR incineradas, el tiempo de reserva a 24 horas o menos del 65,7%, mayor de 24 horas y menor de 48 horas el 4,2%, mayor de 48 horas y menor de 72 horas el 0,6% y sin información disponible 29,5%; la reserva transfusión del 73%; la transfusión no Isogrupo correspondió 5,1% (n=18), distribuida: prueba cruzada incompatible 1,7%(n=6), protocolo pediatría/neonatología 2%(n=7); Urgencia vital 1,4%(n=5). Conclusiones: Comparado con otros estudios se encontró buen desempeño de los indicadores Wapi (0.19%) y Reserva/trasfusión (73%). No se evaluó demanda satisfecha y utilización componente UGR por registros inconsistentes; en tiempo/reserva la información es crítica para validar el resultado.

Os avanços na área da Medicina transfucional, nomeadamente a descoberta de nova informação sobre os grupos sanguíneos de várias espécies, a introdução rotineira da tipificação sanguínea e das provas de compatibilidade eritrocitária, o estudo das reacções transfusionais adversas, o despiste de doenças infeciosas no dador e a aplicação da terapia por componentes, têm contribuído para aumentar a segurança da transfusão sanguínea em Medicina Veterinária e, por consequência, a sua utilização é cada vez mais frequente. O presente trabalho é constituído por três objectivos: perspectivar a medicina transfusional em Portugal através da análise dos resultados de um inquérito, dirigido aos CAMV, nomeadamente sobre o uso da terapia por componentes, as principais indicações de transfusão e a ocorrência de reacções transfusionais; caracterizar a população de gatos e cães receptores de transfusões sanguíneas, com enfoque na prevalência dos diferentes grupos sanguíneos, na indicação para a realização da transfusão e tipo de produto administrado; determinar a ocorrência de reacções transfusionais através da monotorização do doente antes, durante e após a administração das transfusões. No presente estudo, 86% dos Centros de Atendimento Médico Veterinário (CAMV) inquiridos recorrem à transfusão sanguínea como terapia complementar. Destes, 54.7% utiliza sangue total e produtos do sangue, 41.3% apenas sangue total e 4% apenas produtos do sangue. Os produtos do sangue mais utilizados são o concentrado de glóbulos vermelhos e o plasma fresco congelado (34.2% e 31.6% respectivamente). A anemia constitui o principal motivo para a realização de transfusões sanguíneas e a hipertermia a reacção transfusional mais frequente. Relativamente à caracterização da população de cães e gatos que receberam transfusão sanguínea conclui-se que 77.8% dos cães pertenciam ao grupo DEA 1.1 negativo e 22.2% ao grupo DEA 1.1 positivo. Todos os gatos incluídos neste estudo pertenciam ao grupo sanguíneo A. A anemia por hemorragia foi a indicação predominante para a administração de sangue nos cães (54.3%). Nos gatos a anemia por não produção de eritrócitos prevalece (60%). No que respeita às reacções transfusionais, das 61 transfusões realizadas apenas se registou uma dispneia num gato.

The dog is considered to be the natural host of Rhipicephalus sanguineus and is unable to develop appreciable resistance even after repeated feedings. The guinea pig develops strong resistance after one infestation with adult ticks. Antibody (IgG) titres against tick salivary gland antigens (SGAs) and blood leukocyte numbers in dogs and guinea pigs undergoing experimental R. sanguineus tick infestations were measured to detect a possible correlation with susceptibility or resistance of hosts. Since infested dogs develop an immediate hypersensitivity reaction to R. sanguineus antigens, total and anti-R. sanguineus SGA IgE levels were also measured in this host species. IgG and IgE antibody levels were determined by an enzyme-linked immunosorbent assay (ELISA) along three consecutive infestations of both hosts. Most dogs and guinea pigs displayed low IgG levels against R. sanguineus SGAs, though marked differences in individual response were observed. Although dog's total serum IgE levels increased significantly after infestations, no change in the amount of anti-salivary gland IgE was detected. Total and differential blood cell counts were determined in dogs and guinea pigs during primary and secondary infestation. In dogs, a tertiary infestation and a subsequent higher infestation level were also evaluated. Infested dogs did not display any alteration in blood leukocyte counts throughout the experiment. Guinea pigs, on the other hand, developed a significant basophilia during primary infestation which increased further during secondary infestation. These data reveal similarities and differences in the reactions of resistant and non-resistant hosts to ticks. They contribute for the understanding of such host-parasite relationships and will hopefully aid in the development of immune control of ticks. (C) 2003 Elsevier B.V. All rights reserved.

The experiment was conducted to evaluate the effect of feed restriction and different sodium levels in the diet on the performance and hematological values (erythrocyte, hemoglobin, hematocrit, mean cell volume, mean cell hemoglobin, mean cell hemoglobin concentration and white blood cell) of broilers from 22 to 42 days of age. A completely randomized design was used in a 4 x 2 x 2 factorial (levels of sodium: 0.16%, 0.19%, 0.22% and 0.25%; with or without feed restriction; male and female), and two replicates of thirty broilers each. Sodium levels did not affect performance (p>0.05). Feed restriction resulted in increased feed intake and affected weight gain and feed conversion negatively (p<0.05). Better weight gain and feed conversion (p<0.05) were seen in male broilers. Hematological values were not affected by sodium levels, feed restriction or sex (p>0.05).

JUSTIFICATIVA E OBJETIVOS: A plasmaféresis é a técnica de tratamento de escolha para pacientes com anemia hemolítica grave. Uma de suas conseqüências é a depleção de colinesterase plasmática, o que interfere na metabolização de alguns bloqueadores neuromusculares de uso corrente na prática anestesiológica. RELATO do CASO: Paciente com 26 anos, estado físico ASA IV, gestação de 30 semanas e 3 dias, portadora de anemia falciforme, traço talassêmico e alo-imunização para antígenos de alta freqüência. Apresentou crise de falcização, sendo transfundida com derivado sangüíneo incompatível. Evoluiu com hemólise maciça, sendo admitida com hemoglobina de 3 g/dL e hematócrito de 10%, icterícia intensa, taquicardia, apatia e descoramento. Na avaliação hematológica concluiu-se ser situação de inexistência de sangue compatível para transfusão. Foi tratada com corticoterapia, imunoglobulinas e plasmaféresis. No segundo dia de internação, evoluiu com insuficiência renal aguda e edema pulmonar agudo, piora do estado geral e instabilidade hemodinâmica. Indicada a resolução da gestação em decorrência do quadro clínico da paciente e do sofrimento fetal agudo que se sobrepôs. A paciente foi admitida na sala de operações consciente, dispnéica, pálida, ictérica, SpO2 de 91% em ar ambiente, freqüência cardíaca de 110 bpm e pressão arterial de 110 x 70 mmHg, em uso de dopamina (1 µg.kg-1.min-1) e dobutamina (10 µg.kg-1.min-1). Optou-se por anestesia geral balanceada, com alfentanil (2,5 mg), etomidato (14 mg) e atracúrio (35 mg) e isoflurano. Não se observou intercorrências anestésico-cirúrgicas. Ao final, a paciente foi encaminhada à UTI, sob intubação orotraqueal, e em uso de drogas vasoativas, tendo sido extubada após 3 horas. CONCLUSÕES: Este caso mostrou-se um desafio para a equipe, visto que a paciente apresentava instabilidade hemodinâmica e alteração do coagulograma, condições que contra-indicam a anestesia regional; além disto, a plasmaféresis potencialmente depleta os estoques de colinesterases plasmáticas, o que interfere na anestesia. Entretanto, o arsenal medicamentoso disponível permitiu o manuseio seguro desta situação.

The ability of rattlesnake (Crotalus durissus terrificus) red blood cells to volume regulate in vitro has been investigated. Blood was drawn through a catheter inserted in the dorsal aorta and equilibrated to gas mixtures of different composition. Cells shrunken osmotically by increasing the extracellular osmolarity from approximate to 291 mosm l(-1) (n = 3) to approximate to 632 mosm l(-1) (calculated) only partially regulated their volume back towards the original volume either at pH 7.51 +/- 0.05 (mean +/- S.D., n = 5) or pH 7.20 +/- 0.06 (mean +/- S.D., n = 3), There was no improvement of the regulatory volume increase at low haemoglobin oxygen saturation. The limited volume restoration was inhibited by separate additions of amiloride (10(-4) M) or DIDS (10(-4) M) suggesting involvement of the Na+/H+ and Cl-/HCO3- exchangers. Cells that were swollen osmotically by an approximate to 30% dilution of the extracellular medium also exhibited a limited ability to recover their volume. Therefore, these cells show little ability to volume regulate when exposed to in vitro conditions that shrink or swell the cell. (C) 2000 Elsevier B.V. All rights reserved.
