OBJECTIVE To describe the response to rituximab in patients with treatment-resistant chronic inflammatory demyelinating polyneuropathy (CIDP) with antibodies against paranodal proteins and correlate the response with autoantibody titers. METHODS Patients with CIDP and IgG4 anti-contactin-1 (CNTN1) or anti-neurofascin-155 (NF155) antibodies who were resistant to IV immunoglobulin and corticosteroids were treated with rituximab and followed prospectively. Immunocytochemistry was used to detect anti-CNTN1 and anti-NF155 antibodies and ELISA with human recombinant CNTN1 and NF155 proteins was used to determine antibody titers. RESULTS Two patients had a marked improvement; another patient improved slightly after 10 years of stable, severe disease; and the fourth patient had an ischemic stroke unrelated to treatment and was lost to follow-up. Autoantibodies decreased in all patients after rituximab treatment. CONCLUSIONS Rituximab treatment is an option for patients with CIDP with IgG4 anti-CNTN1/NF155 antibodies who are resistant to conventional therapies. CLASSIFICATION OF EVIDENCE This study provides Class IV evidence that rituximab is effective for patients with treatment-resistant CIDP with IgG4 anti-CNTN1 or anti-NF155 antibodies.

Substantial proportion of Crohn's disease (CD) patients shows no response or a limited response to treatment with infliximab (IFX) and to identify biomarkers of response would be of great clinical and economic benefit. The expression profile of five genes (S100A8-S100A9, G0S2, TNFAIP6, and IL11) reportedly predicted response to IFX and we aimed at investigating their etiologic role through genetic association analysis. Patients with active CD (350) who received at least three induction doses of IFX were included and classified according to IFX response. A tagging strategy was used to select genetic polymorphisms that cover the variability present in the chromosomal regions encoding the identified genes with altered expression. Following genotyping, differences between responders and nonresponders to IFX were observed in haplotypes of the studied regions: S100A8-S100A9 (rs11205276* G/rs3014866* C/rs724781* C/rs3006488* A; P = 0.05); G0S2 (rs4844486* A/rs1473683* T; P = 0.15); TNFAIP6 (rs11677200* C/rs2342910* A/rs3755480* G/rs10432475* A; P = 0.10); and IL11 (rs1126760* C/rs1042506* G; P = 0.07). These differences were amplified in patients with colonic and ileocolonic location for all but the TNFAIP6 haplotype, which evidenced significant difference in ileal CD patients. Our results support the role of the reported expression signature as predictive of anti-TNF outcome in CD patients and suggest an etiological role of those top-five genes in the IFX response pathway.

Behçet's disease (BD) is universally recognized as a multisystemic inflammatory disease of unknown etiology with chronic course and unpredictable exacerbations: its clinical spectrum varies from pure vasculitic manifestations with thrombotic complications to protean inflammatory involvement of multiple organs and tissues. Treatment has been revolutionized by the progressed knowledge in the pathogenetic mechanisms of BD, involving dysfunction and oversecretion of multiple proinflammatory molecules, chiefly tumor necrosis factor- (TNF-) α, interleukin- (IL-) 1β, and IL-6. However, although biological treatment with anti-TNF-α agents has been largely demonstrated to be effective in BD, not all patients are definite responders, and this beneficial response might drop off over time. Therefore, additional therapies for a subset of refractory patients with BD are inevitably needed. Different agents targeting various cytokines and their receptors or cell surface molecules have been studied: the IL-1 receptor has been targeted by anakinra, the IL-1 by canakinumab and gevokizumab, the IL-6 receptor by tocilizumab, the IL12/23 receptor by ustekinumab, and the B-lymphocyte antigen CD-20 by rituximab. The aim of this review is to summarize all current experiences and the most recent evidence regarding these novel approaches with biological drugs other than TNF-α blockers in BD, providing a valuable addition to the actually available therapeutic armamentarium.

Individualized treatment regimens may reduce patient burden with satisfactory patient outcomes in neovascular age-related macular degeneration. Intravitreal anti-VEGF drugs are the current gold standard. Fixed monthly injections offer the best visual outcome but this regimen is not commonly followed outside clinical trials. A PRN regimen requires monthly visits where the patient is treated in the presence of signs of lesion activity. Therefore, an early detection of reactivation of the disease with immediate retreatment is crucial to prevent visual acuity loss. Several trials suggest that "treat and extend" and other proactive regimens provide a reasonable approach. The rationale of the proactive regimens is to perform treatment anticipating relapses or recurrences and therefore avoid drops in vision while individualizing patient followup. Treat and extend study results in significant direct medical cost savings from fewer treatments and office visits compared to monthly treatment. Current data suggest that, for one year, PRN is less expensive, but treat and extend regimen would likely be less expensive for subsequent years. Once a patient is not a candidate to continue with treatment, he/she should be sent to an outpatient unit with adequate resources to follow nAMD patients in order to reduce the burden of specialized ophthalmologist services.
Natalizumab-related anaphylactoid reactions in MS patients are associated with HLA class II alleles.

OBJECTIVES We aimed to investigate potential associations between human leukocyte antigen (HLA) class I and class II alleles and the development of anaphylactic/anaphylactoid reactions in patients with multiple sclerosis (MS) treated with natalizumab. METHODS HLA class I and II genotyping was performed in patients with MS who experienced anaphylactic/anaphylactoid reactions and in patients who did not develop infusion-related allergic reactions following natalizumab administration. RESULTS A total of 119 patients with MS from 3 different cohorts were included in the study: 54 with natalizumab-related anaphylactic/anaphylactoid reactions and 65 without allergic reactions. HLA-DRB1*13 and HLA-DRB1*14 alleles were significantly increased in patients who developed anaphylactic/anaphylactoid reactions (p M-H = 3 × 10(-7); odds ratio [OR]M-H = 8.96, 95% confidence interval [CI] = 3.40-23.64), with a positive predictive value (PPV) of 82%. In contrast, the HLA-DRB1*15 allele was significantly more represented in patients who did not develop anaphylactic/anaphylactoid reactions to natalizumab (p M-H = 6 × 10(-4); ORM-H = 0.2, 95% CI = 0.08-0.50), with a PPV of 81%. CONCLUSIONS HLA-DRB1 genotyping before natalizumab treatment may help neurologists to identify patients with MS at risk for developing serious systemic hypersensitivity reactions associated with natalizumab administration.

BACKGROUND Despite the evidence that Lactoferrin (Lf) is involved in allergic asthma processes, it is unknown whether neutrophils can be one of the main cellular sources of this key inflammatory mediator directly in response of an IgE mediated stimulus. The present study was undertaken to analyze this question. METHODS Neutrophils from healthy subjects (n = 34) and neutrophils from allergic asthmatic patients (n = 102) were challenged in vitro with specific allergens to which the patients were sensitized, PAF, or agonist mAbs against IgE-receptors, and the levels of Lf were measured in the culture supernatant. The levels of serum IgE together with the severity of symptoms were also analyzed. RESULTS Lf was released into the culture supernatant of neutrophils from allergic asthmatic patients in response to allergens and PAF. This response was highly allergen-specific, and did not happen in neutrophils from healthy donors. Allergen effect was mimicked by Abs against FcεRI and galectin-3 but not by FcεRII. The levels of released Lf correlated well with the levels of serum specific IgE and severity of asthma symptoms. These observations represent a novel view of neutrophils as an important source of Lf in allergic asthma. Importantly, the levels of released Lf by neutrophils could therefore be used to evaluate disease severity in allergic asthmatic patients.

A single-step PCR assay with genus-specific primers for the amplification of a 223-bp region of the sequence encoding a 31-kDa immunogenetic Brucella abortus protein (BCSP31) was used for the rapid diagnosis of human brucellosis. We examined peripheral blood from 47 patients, with a total of 50 cases of brucellosis, and a group of 60 control subjects, composed of patients with febrile syndromes of several etiologies other than brucellosis, asymptomatic subjects seropositive for Brucella antibodies, and healthy subjects. Diagnosis of brucellosis was established in 35 cases (70%) by isolation of Brucella in blood culture and in the other 15 cases (30%) by clinical and serological means. The sensitivity of our PCR assay was 100%, since it correctly identified all 50 cases of brucellosis, regardless of the duration of the disease, the positivity of the blood culture, or the presence of focal forms. The specificity of the test was 98.3%, and the only false-positive result was for a patient who had had brucellosis 2 months before and possibly had a self-limited relapse. In those patients who relapsed, the results of our PCR assay were positive for both the initial infection and the relapse, becoming negative once the relapse treatment was completed and remaining negative in the follow-up tests at 2, 4, and 6 months. In conclusion, these results suggest that the PCR assay is rapid and easy to perform and highly sensitive and specific, and it may therefore be considered a useful tool for diagnosis of human brucellosis.

Chronic hepatitis B virus (HBV) infection is responsible for up to 30% of cases of liver cirrhosis and up to 53% of cases of hepatocellular carcinoma. Liver transplantation (LT) is the best therapeutic option for patients with end-stage liver failure caused by HBV. The success of transplantation, though, depends on receiving prophylactic treatment against post-transplant viral reactivation. In the absence of prophylaxis, liver transplantation due to chronic hepatitis B (CHB) is associated with high rates of viral recurrence and poor survival. The introduction of treatment with hepatitis B immunoglobulins (HBIG) during the 1990s and later the incorporation of oral antiviral drugs have improved the prognosis of these patients. Thus, LT for CHB is now a universally accepted option, with an estimated 5 years survival of around 85% vs the 45% survival seen prior to the introduction of HBIG. The combination of lamivudine plus HBIG has for many years been the most widely used prophylactic regimen. However, with the appearance of new more potent oral antiviral agents associated with less resistance (e.g., entecavir and tenofovir) for the treatment of CHB, new prophylactic strategies are being designed, either in combination with HBIG or alone as a monotherapy. These advances have allowed for more personalized prophylaxis based on the individual risk profile of a given patient. In addition, the small pool of donors has required the use of anti-HBc-positive donors (with the resulting possibility of transmitting HBV from these organs), which has been made possible by suitable prophylactic regimens.

Tanto el asma ocupacional como la neumonitis por hipersensibilidad, como es el pulmón del cuidador de aves, son patologías respiratorias que se pueden prevenir o disminuir su aparición mediante la evitación de la fuente antigénica. Para poder actuar de forma preventiva es de utilidad el disponer de ensayos rápidos que sean capaces de estimar la presencia de alérgeno de forma inmediata. En el marco de este proyecto de dos años de duración tenemos por objeto el desarrollar y estandarizar dos métodos rápidos, inmunocromatográficos, para la determinación de alérgenos de soja y de proteínas séricas de paloma. Alérgenos que han sido seleccionados por su importancia en el medio como agentes causales de asma y neumonitis por hipersensibilidad, respectivamente. También tenemos por objeto determinar la carga de alérgeno de soja en la fracción de partículas menores de 10 micrómetros (PM10) en los alrededores del puerto de Barcelona y comprarla con los niveles en los filtros de partículas suspendidas totales (TSP). Como pasos previos al desarrollo de los ensayos rápidos se han producido anticuerpos específicos frente al extracto de cáscara de soja de bajo peso molecular y frente al suero de paloma, se ha desarrollado un ELISA tipo sándwich para cada alérgeno y parte de los anticuerpos se ha conjugado con oro coloidal. El ensayo inmunocromatográfico para la soja presenta un límite de detección de 6.25ng/ml y ha sido validado mediante el análisis de 119 muestras ambientales, presentando una elevada especificidad y sensibilidad. El ensayo inmunocromatográfico para la determinación de antígenos séricos de paloma requiere ser validado. Mediante un métodos de ELISA de inhibición se han determinado los niveles de alérgeno de soja en filtros PM10 y TSP. A pesar de la buena correlación entre los niveles de alérgeno en ambos filtros, se observó una amplia variación en la proporción PM10/TSP entre días.

El hecho de que las alteraciones inmunitarias están implicadas en la enfermedad periodontal está ampliamente recogido en la literatura. No obstante la naturaleza exacta y la implicación de las mismas no está clara. Por otro lado el empleo de la citometría de flujo ha posibilitado un avance importante en las técnicas de investigación biomédica y en el caso concreto que nos ocupa la posibilidad de determinar diferentes marcadores monoclonales en la línea celular linfocítica. Estudiamos los marcadores monoclonales (CD4, CDB, CD3 y CD19) en 15 pacientes afectados de enfermedad periodontal avanzada. Los resultados obtenidos se encuentran dentro de los límites de normalidad. Si bien no hemos obtenido datos significativos creemos que la muestra es pequeña y sería útil realizar estudios más amplios y a ser posible a nivel local. No obstante pensamos que lo novedoso es el estudio de anticuerpos monoclonales en relacióncon la enfermedad periodontal, datos que no hemos encontrado en la literatura.

El objetivo es comprobar si el efecto residual inmunodepresor de la ciclofosfamida contribuye a la evocación de la respuesta de anticuerpos condicionada. Se condicionaron ratones no consanguíneos, machos, con el estímulo condicionado sacarina al 0,15% y con el estímulo incondicionado ciclofosfamida, a 50 y 250 mg/kg. A los 4 días del condicionamiento con 50 mg/kg de ciclofosfamida, o a los 14 días del condicionamiento con 250 mg/kg de ciclofosfamida, los ratones se inmunizaron con glóbulos rojos de rata y, a continuación, bebieron la disolución de sacarina. Se comprobó que i) la dosis más baja de ciclofosfamida no generó efecto residual, mientras que la dosis más alta si, y ii) en ningún caso, la presentación de sacarina disminuyó la respuesta de anticuerpos anti-glóbulos rojos de rata. Se concluye que no se produjo condicionamiento de la respuesta de anticuerpos, tanto si el efecto residual de la ciclofosfamida está presente, como si está ausente.

OBJETIVO: Verificar a prevalência de anticorpos anticardiolipinas IgG e IgM em pacientes com úlcera de perna e se os seus portadores podem ser identificados clinicamente. MÉTODOS: Estudaram-se 151 pacientes com úlcera de perna (81 venosas, 50 diabéticas e 20 arteriais) e 150 controles. Pesquisou-se, nos dois grupos, a presença de anticorpos anticardiolipina IgG e IgM pelo método de ELISA. No grupo úlcera foram coletados dados demográficos dos pacientes, de tamanho e número de úlceras e gravidade da dor medido por escala visual analógica. Os dados obtidos foram agrupados em tabelas de frequência e contingência. Adotou-se significância de 5%. RESULTADOS: Encontrou-se prevalência de anticorpos anticardiolipina de 7.2% (n=12) no grupo com úlceras e de 1.3% (n=2) no controle (p=0.01). Comparando-se a prevalência dos anticorpos anticardiolipina nos diferentes tipos de úlcera verificou-se aumento nas de origem venosa (p=0.02) e diabéticas (p=0.01), mas não nas arteriais (p=0.31) em relação à população controle. As úlceras de perna anticardiolipinas positivas não diferiram daquelas sem anticardiolipinas quanto a tamanho da ferida (p=0.6); gravidade da dor (p=0.67), número médio de úlceras (p=0.38), tempo de duração de doença (p= 0.59), gênero do paciente (p=0.98) e história de trombose prévia (p=0.69). CONCLUSÃO: Existe aumento de prevalência de anticorpos anticardiolipinas nos portadores de úlceras de perna venosas e diabéticas, mas não nas arteriais. As características clínicas das úlceras anticardiolipinas positivas não auxiliam na identificação desses pacientes.

OBJETIVO: Estudar a prevalência de anticorpos anticardiolipinas em pacientes com úlceras venosas, diabéticas e arteriais e verificar se a contagem de plaquetas, antecedentes obstétricos e de trombose venosa profunda e achados de livedo reticularis ao exame físico servem como marcadores para os casos positivos. MÉTODOS: Estudaram-se 151 pacientes com úlcera de perna (81 com úlceras venosas, 50 com úlceras diabéticas e 20 com úlceras arteriais) e 150 controles. Pesquisou-se, nos dois grupos, a presença de anticorpos anticardiolipina IgG e IgM e contagem de plaquetas. No grupo úlcera foram coletados dados de antecedentes de trombose venosa profunda e de abortamentos e os pacientes foram examinados para presença de livedo reticularis. Os dados obtidos foram agrupados em tabelas de frequência e contingência utilizando-se dos testes de Fisher e qui-quadrado para variáveis nominais e de Mann-Whitney e Kruskall-Wallis para as numéricas. Adotou-se significância de 5%. RESULTADOS: Encontrou-se prevalência de anticorpos anticardiolipina de 7.2% (n=12) no grupo com úlceras e de 1.3% (n=2) no controle (p=0.01). As úlceras de perna anticardiolipinas positivas não diferiram daquelas sem anticardiolipinas quanto ao gênero do paciente (p=0.98) e história de trombose prévia (p=0.69), abortamentos anteriores (p=0.67) e contagens de plaquetas (p=0.67). Só dois pacientes tinham livedo reticularis não permitindo inferências estatísticas a respeito deste dado. CONCLUSÃO: Existe aumento de prevalência de anticorpos anticardiolipinas nos portadores de úlceras de perna em relação à população geral. As características clínicas das úlceras anticardiolipinas positivas e a contagem de plaquetas não auxiliam na identificação desses pacientes.

OBJETIVO: identificar e descrever a prevalência de anticorpos antifosfolípides (anticorpo anticardiolipina e anticoagulante lúpico) em gestantes diabéticas. MÉTODOS: estudo prospectivo de prevalência, realizado no período de julho de 2003 a março de 2004. Foram estudadas 56 gestantes diabéticas gestacionais e pré-gestacionais que ingressaram ao pré-natal e aceitaram participar do estudo. Nenhuma gestante foi excluída. Se um ou outro anticorpo estivesse presente, a gestante seria tratada com heparina e ácido acetilsalicílico. Foram caracterizados os perfis da gestante, a evolução da gestação e o recém-nascido. RESULTADOS: foram diagnosticados anticorpos antifosfolípides em apenas quatro gestantes das 56 estudadas, o que representou prevalência de 7% (IC 95% - 0,1-13,9). Nas gestantes diabéticas com anticorpos antifosfolípides a duração do diabetes foi de cinco anos ou mais. A idade variou de 27 a 38 anos, sendo uma primigesta, outra secundigesta e as outras duas multíparas. As gestantes com anticorpos antifosfolípides, que foram tratadas, tiveram recém-nascidos vivos, de termo, cujos pesos variaram entre 2.650 g e 4.000 g. CONCLUSÃO: a prevalência de anticorpos antifosfolípides em gestantes diabéticas gestacionais é baixa, e similar à população geral de grávidas. Está prevalência aumenta quando a gestante é diabética pré-gestacional.

OBJETIVO: determinar a prevalência dos fatores trombofílicos em mulheres inférteis. MÉTODOS: estudo de corte transversal, no qual foram admitidas mulheres inférteis atendidas em clínica privada e submetidas à investigação de trombofilia, conforme protocolo da referida clínica, no período de março de 2003 a março de 2005, após aprovação do Comitê de Ética e Pesquisa da Universidade Estadual de Campinas (UNICAMP). Foram incluídas mulheres com história de infertilidade, definida como um ano de coito desprotegido sem concepção. Foram excluídas mulheres com hepatopatia e dados incompletos em prontuário, obtendo-se uma amostra de 144 mulheres. Os fatores trombofílicos avaliados foram: o anticorpo anticardiolipina (ACL), o anticoagulante lúpico (ACGL), a deficiência de proteína C (DPC), a deficiência de proteína S (DPS), a deficiência de antitrombina III (DAT), a presença do fator V de Leiden, uma mutação no gene da protrombina e a mutação do metileno tetrahidrofolato redutase (MTHFR). Resultados: os valores de prevalência obtidos para ACL e ACGL foram de 2%. A prevalência dos fatores trombofílicos hereditários foram: DPC=4%, DPS=6%, DAT=5%, fator V de Leiden=3%, mutação da protrombina=3% e mutação MTHFR=57%. CONCLUSÕES: das 144 pacientes selecionadas, 105 mulheres, ou seja, 72,9% apresentavam pelo menos um fator trombofílico presente. Isto reforça a importância e justifica a necessidade da investigação destes fatores neste grupo de mulheres.
