Couples with alcohol and relationship problems often report poor communication, yet little is known about the communication of maritally distressed couples in which the woman abuses alcohol (MDWA couples). Compared with maritally distressed couples without alcohol problems (MDNA) and couples with neither problem (NDNA), MDWA couples showed a distinctive pattern of negative communication. Similar to MDNA men, MDWA men spoke negatively to their partners but listened positively to their partners much like NDNA men. MDWA women listened negatively, much as MDNA women did, but spoke positively, like NDNA women did. The interactions of MDWA couples can be characterized as a male-demand-female-withdraw pattern, which is a gender reversal of the female-demand-male-withdraw pattern often observed in MDNA couples.

Background: Increasingly there is a call from clinicians and researchers for measures that document the impact of aphasia on a person's everyday communication. Do existing assessments of communication disability adequately sample communication activities relevant to our clients? Communication skills and networks change with age. A need exists to determine the everyday communication activities of older people and in particular those with aphasia. Aims: The primary aim of this study was to describe and compare the everyday communication activities of older people with aphasia and healthy older people who are living in the community. A secondary aim was to investigate the content validity of the American Speech-Language Hearing Association Functional Assessment of Communication Skills for Adults (ASHA FACS, 1997) for older Australians. Methods & Procedures: Naturalistic observation was the method of choice for detailing the everyday communication of 15 older people with chronic aphasia following stroke and a matched group of 15 healthy older people who were living in the community. Researchers, in the role of participant observer, took field notes for 8 hours, over three occasions within a week. A total of 240 hours of observation have been coded in terms of communication activity, topic, communication partners, and place of communication. A brief 5-day diary served to check the representativeness of the observational data. After each hour of observation, the researcher checked which ASHA FACS items had been observed. Outcomes & Results: Naturalistic observation provided a rich, rigorous, and systematic methodology for detailing the dynamics and complexities of authentic communication. The most common communication activities for both groups were conversations at home and in social groups. Real-life communication was revealed to serve the dual purposes of transaction and interaction. Results indicate that older people with aphasia engage in similar communication activities to healthy older people although differences were evident in the frequency of communication and in specific activities such as story telling, writing, commenting, and acknowledging. ASHA FACS items were generally relevant to older Australians living in the community. Conclusions: This study demonstrated that communication activity is multifaceted in terms of the type of communication and contextual factors. The observational data describe the effects of aphasia on a person's everyday communication activity and reveal the impact of aphasia on the social functions of communication including sharing information, maintaining and establishing relationships, and telling one's story. Functional communication assessment requires a greater focus on the interactional and uniquely interpersonal aspects of social communication.

The effect of number of samples and selection of data for analysis on the calculation of surface motor unit potential (SMUP) size in the statistical method of motor unit number estimates (MUNE) was determined in 10 normal subjects and 10 with amyotrophic lateral sclerosis (ALS). We recorded 500 sequential compound muscle action potentials (CMAPs) at three different stable stimulus intensities (10–50% of maximal CMAP). Estimated mean SMUP sizes were calculated using Poisson statistical assumptions from the variance of 500 sequential CMAP obtained at each stimulus intensity. The results with the 500 data points were compared with smaller subsets from the same data set. The results using a range of 50–80% of the 500 data points were compared with the full 500. The effect of restricting analysis to data between 5–20% of the CMAP and to standard deviation limits was also assessed. No differences in mean SMUP size were found with stimulus intensity or use of different ranges of data. Consistency was improved with a greater sample number. Data within 5% of CMAP size gave both increased consistency and reduced mean SMUP size in many subjects, but excluded valid responses present at that stimulus intensity. These changes were more prominent in ALS patients in whom the presence of isolated SMUP responses was a striking difference from normal subjects. Noise, spurious data, and large SMUP limited the Poisson assumptions. When these factors are considered, consistent statistical MUNE can be calculated from a continuous sequence of data points. Patients with severe forms of Guillain-Barré syndrome (GBS) require intensive care. Specific treatment, catheterization, and devices may increase morbidity in the intensive care unit (ICU). To understand the spectrum of morbidity associated with ICU care, the authors studied 114 patients with GBS. Major morbidity occurred in 60% of patients. Complications were uncommon if ICU stay was less than 3 weeks. Respiratory complications such as pneumonia and tracheobronchitis occurred in half of the patients and were linked to mechanical ventilation. Systemic infection occurred in one-fifth of patients and was more frequent with increasing duration of ICU admission. Direct complications of treatment and invasive procedures occurred infrequently. Life-threatening complications such as gastrointestinal bleeding and pulmonary embolism were very uncommon. Pulmonary morbidity predominates in patients with severe GBS admitted to the ICU. Attention to management of mechanical ventilation and weaning is important to minimize this complication of GBS. Other causes of morbidity in a tertiary center ICU are uncommon.

An equivalent unit cell waveguide approach (WGA) is described to obtain reflection coefficient phase curves for designing a microstrip patch reflectarray supported by a ground plane with periodic apertures or slots. Based on the presented theory, a computer algorithm for determining the reflection coefficient of a plane wave normally incident on a multi-layer structure of patches and apertures is developed. The validity of the developed algorithm is verified by comparing the obtained results with those published in the literature and the ones generated by Agilent High Frequency Structure Simulator (HFSS). A good agreement in all the presented examples is obtained, proving that the developed theory and computer algorithm can be an effective tool for designing multi-layer microstrip reflectarrays with a periodically perforated ground plane. An equivalent unit cell waveguide approach (WGA) to designing 4 multilayer microstrip reflectarray of variable size patches is presented. In this approach, a normal incidence of a plane wave on an infinite periodic array of radiating elements is considered to obtain reflection coefficient phase curves for the reflectarray's elements. It is shown that this problem is equivalent to the problem of reflection of the dominant TEM mode in a waveguide with patches interleaved by layers of dielectric. This waveguide problem is solved using a field matching technique and a method of moments (MoM). Based on this solution, a fast computer algorithm is developed to generate reflection coefficient phase curves for a multilayer microstrip patch reflectarray. The validity of the developed algorithm is tested against alternative approaches and Agilent High Frequency Structure Simulator (HFSS). Having confirmed the validity of the WGA approach, a small offset feed two-layer microstrip patch array is designed and developed. This reflectarray is tested experimentally and shows good performance.

In 2002, an integrated basic science course was introduced into the Bachelor of Dental Sciences programme at the University of Queensland, Australia. Learning activities for the Metabolism and Nutrition unit within this integrated course included lectures, problem-based learning tutorials, computer-based self-directed learning exercises and practicals. To support student learning and assist students to develop the skills necessary to become lifelong learners, an extensive bank of formative assessment questions was set up using the commercially available package, WebCT®. Questions included short-answer, multiple-choice and extended matching questions. As significant staff time was involved in setting up the question database, the extent to which students used the formative assessment and their perceptions of its usefulness to their learning were evaluated to determine whether formative assessment should be extended to other units within the course. More than 90% of the class completed formative assessment tasks associated with learning activities scheduled in the first two weeks of the block, but this declined to less than 50% by the fourth and final week of the block. Patterns of usage of the formative assessment were also compared in students who scored in the top 10% for all assessment for the semester with those who scored in the lowest 10%. High-performing students accessed the Web-based formative assessment about twice as often as those who scored in the lowest band. However, marks for the formative assessment tests did not differ significantly between the two groups. In a questionnaire that was administered at the completion of the block, students rated the formative assessment highly, with 80% regarding it as being helpful for their learning. In conclusion, although substantial staff time was required to set up the question database, this appeared to be justified by the positive responses of the students.

In this study, a combination of recA-based PCR assays and 16S rDNA restriction fragment length polymorphism (RFLP) analysis was used to determine the genomovar diversity of clinical Burkholderia cepacia complex isolates. Twenty-eight isolates were prospectively collected from patients attending a large Australian adult cystic fibrosis (CF) unit, 22 isolates were referred from other Australian CF units and a further eight isolates originated from patients without CF. The 28 prospectively collected isolates were distributed amongst the following genomovars: Burkholderia cepacia genomovar I (28.6%), Burkholderia multivorans (21.4%), Burkholderia cepacia genomovar III (39.3%), Burkholderia vietnamiensis (3.6%) and Burkholderia ambifaria (7.1%). The results of this study highlight the usefulness of 16S rDNA RFLP typing for the identification of other Burkholderia spp. and non-fermenting gram-negative bacteria.

Esta dissertação teve como objeto o Ensino Médio Integrado no Espírito Santo, sua trajetória e implantação a partir do Decreto 5.154/2004. O objetivo principal foi analisar a política do ensino médio integrado à educação profissional técnica de nível médio implementada no Espírito Santo. Investigar a educação pública pressupõe contextualizar a sociedade e as construções sociais que se projetam. O ensino médio no Brasil, enquanto etapa intermediária entre o ensino fundamental e a educação superior constitui-se de grande complexidade na estruturação de políticas públicas, no que tange à sua expansão qualitativa e quantitativa. Foram estabelecidos os seguintes objetivos específicos: identificar os pressupostos teóricos e pedagógicos contidos na Proposta Pedagógica da unidade escolar EEEFM Arnulpho Mattos; analisar a matriz curricular e a proposta pedagógica; relacionar o projeto pedagógico do curso com a prática educacional relatada nos encontros com os alunos; analisar a organização escolar e sua gestão política e pedagógica em face da proposta do EMI; relacionar a proposta do EMI, o princípio da politecnia e os relatos dos alunos; mapear a oferta do EMI no estado e o seu investimento financeiro. Nesse cenário, identificamos a relação entre trabalho e educação que contextualiza a relação da escola com a sociedade no sistema capitalista, em especial no ensino médio. Assim, nossa hipótese foi que esta política tem dificuldades de alcançar seus objetivos porque sua estrutura material segue a lógica da escola capitalista, assentada na perspectiva da Teoria do Capital Humano. A formação profissional no ensino médio é uma demanda social e histórica da classe trabalhadora, que necessita ingressar mais rápido no mercado de trabalho do que os filhos das elites brasileiras. A metodologia empregada foi uma pesquisa do tipo qualitativa, sendo um estudo de caso da EEEF Arnulpho Mattos. Os procedimentos metodológicos adotados para a investigação foram: observação, entrevistas semi-estruturadas, a análise documental, a revisão bibliográfica e os princípios metodológicos inscritos na sociologia da experiência (DUBET, 1994). Na articulação desses procedimentos, verificamos que a política do ensino médio integrado no ES teve, em seu início, um planejamento de implantação bem estruturado mas interrompido e, posteriormente, caracterizou-se pela descontinuidade das ações. Ademais, constatamos que os alunos ingressam e permanecem no curso sem compreenderem de fato o que significa a formação integrada entre a área geral e a técnica/profissional. Nos grupos de discussão percebemos que bastaram alguns encontros para que se pudesse estabelecer uma visão e relação diferenciadas com a experiência social do ensino médio integrado, principalmente por serem alunos trabalhadores. Acrescentamos às nossas conclusões que há iniciativas da escola acerca da integração entre as disciplinas de formação geral e técnicas, mas sem apoio ou orientação da unidade gestora, ou seja, a Sedu. A ampliação da oferta do EMI ocorreu no estado sem investimento na formação e valorização dos professores, sem concurso público e sem uma infra-estrutura necessária para a formação de jovens e adultos com capacidade de entender e de interferir no mundo do trabalho.

This article presents a research work, the goal of which was to achieve a model for the evaluation of data quality in institutional websites of health units in a broad and balanced way. We have carried out a literature review of the available approaches for the evaluation of website content quality, in order to identify the most recurrent dimensions and the attributes, and we have also carried out a Delphi method process with experts in order to reach an adequate set of attributes and their respective weights for the measurement of content quality. The results obtained revealed a high level of consensus among the experts who participated in the Delphi process. On the other hand, the different statistical analysis and techniques implemented are robust and attach confidence to our results and consequent model obtained.

This paper proposes a practical approach for profit-based unit commitment (PBUC) with emission limitations. Under deregulation, unit commitment has evolved from a minimum-cost optimisation problem to a profit-based optimisation problem. However, as a consequence of growing environmental concern, the impact of fossil-fuelled power plants must be considered, giving rise to emission limitations. The simultaneous address of the profit with the emission is taken into account in our practical approach by a multiobjective optimisation (MO) problem. Hence, trade-off Curves between profit and emission are obtained for different energy price profiles, in a way to aid decision-makers concerning emission allowance trading. Moreover, a new parameter is presented, ratio of change, and the corresponding gradient angle, enabling the proper selection of a compromise commitment for the units. A case study based on the standard IEEE 30-bus system is presented to illustrate the proficiency Of Our practical approach for the new competitive and environmentally constrained electricity supply industry.

This paper is on the unit commitment problem, considering not only the economic perspective, but also the environmental perspective. We propose a bi-objective approach to handle the problem with conflicting profit and emission objectives. Numerical results based on the standard IEEE 30-bus test system illustrate the proficiency of the proposed approach.

The production of MVOC by fungi has been taken into account especially from the viewpoint of indoor pollution with microorganisms but the relevance of fungal metabolites in working environments has not been sufficiently studied. The purpose of this study was to assess exposure to MVOCs in a waste-handling unit. It was used Multirae equipment (RAE Systems) to measured MVOCs concentration with a 10.6 eV lamps. The measurements were done near workers nose and during the normal activities. All measurements were done continuously and had the duration of 5 minutes at least. It was consider the higher value obtained in each measurement. In addition, for knowing fungi contamination, five air samples of 50 litres were collected through impaction method at 140 L/minute, at one meter tall, on to malt extract agar with the antibiotic chloramphenicol (MEA). MVOCs results range between 4.7 ppm and 8.9 ppm in the 6 locations consider. These results are eight times higher than normally obtained in indoor settings. Considering fungi results, two species were identified in air, being the genera Penicillium found in all the samples in uncountable colonies and Rhizopus only in one sample (40 UFC/m3). These fungi are known as MVOCs producers, namely terpenoids, ketones, alcohols and others. Until now, there has been no evidence that MVOCs are toxicologically relevant, but further epidemiological research is necessary to elucidate their role on human’s health, particularly in occupational settings where microbiological contamination is common. Additionally, further research should concentrate on quantitative analyses of specific MVOCs.

O objetivo desta investigação é compreender como é que alunos do 3.º ano de escolaridade desenvolvem o conceito de área com ênfase em conhecimentos sobre figuras e propriedades geométricas. Para melhor objetivar o estudo, formulei três questões que orientaram a investigação. Pretendo (1) compreender como é que os alunos mobilizam conhecimentos sobre as propriedades das figuras geométricas no estudo da área de uma figura plana? A que estratégias recorrem? Que dificuldades sentem? (2) que compreensão é que os alunos têm sobre o processo de medição? e (3) que ideias e/ou experiências são relevantes no desenvolvimento do conceito de área? Este é um estudo que se insere no paradigma interpretativo e que segue uma abordagem qualitativa, pelo que não visa generalização de resultados, mas uma descrição compreensiva dos processos vivenciados, tendo em conta os contextos pessoais e sociais. Optei por realizar dois Estudos de Caso. Os dados foram recolhidos através de fontes diversificadas, donde se destacam a observação participante e a análise de registos audiovisuais e escritos dos alunos. A análise dos dados teve por referência categorias criadas com base em contributos da literatura de referência. As conclusões desta investigação centram-se numa análise acerca das estratégias a que os alunos em estudo tendencialmente mais recorrem, visando também algumas das suas dificuldades mais relevantes e sobre as quais importa refletir. Destaca-se que ambos os alunos foram capazes de recorrer a raciocínios baseados e não baseados na medida, embora seja evidente uma tendência para raciocínios baseados na medida. É igualmente importante referir que os dois alunos revelaram um bom conhecimento acerca do processo de medição, mesmo quando ele implicava a definição de uma unidade de medida adequada e/ou mudança de unidade. É dado, também, destaque às experiências matemáticas que devem ser proporcionadas aos alunos no sentido do desenvolvimento de um conhecimento amplo e flexível do conceito de área. - Abstract The purpose of this investigation is to understand the development of 3rd year students’ concept of area based on their knowledge of geometrical properties and figures. As means to orientate the present study, the following investigation questions were formulated: 1) how do students use their knowledge about geometrical properties in order to understand geometrical shape’s areas? Which strategies do they use? What do they struggle with? (2) What is their understanding about measurement? And (3) what are the most relevant mathematical reasonings and classroom tasks in the development of the concept of area? This is a study that follows a qualitative approach: it does not intend to be a generalization, but a fully comprehensive description of the witnessed learning processes, bearing in mind the social and personal contexts. As such, two study-cases were developed. The data were collected through various sources, such as direct observation and analysis of audiovisual records or written worksheets. The scrutiny of the generated data was undertaken following criteria based on the literature of reference. The conclusions of this investigation are centered on the most used strategies as well as the students’ difficulties upon which it is important to discuss. It was noticed that the two students were able to use both measurement and non measurement thinking – although, preferably, they tend to use the first one. On the other hand, it is equally demonstrated that the two students have shown good acquaintance about measurement, even if obliged to the establishment of a suitable unit of measurement and/or change of units. At last, it is also referred which mathematical classroom tasks are important to develop a sustained and comprehensive concept of area.
