Introdução: A artroplastia total do ombro é largamente aceite como tratamento para uma variedade de patologias do ombro com dor persistente e perda acentuada da funcionalidade. Em termos globais, os resultados obtidos após a intervenção cirúrgica são bastante bons, parecendo estar, no entanto, condicionados com a patologia subjacente e a qualidade da coifa dos rotadores. Objectivo: Pretende-se com este trabalho descrever e avaliar a eficácia de um plano de intervenção numa prótese total do ombro com integridade da coifa dos rotadores. Metodologia: Paciente sexo feminino, 61 anos, reformada, com dor acentuada no ombro esquerdo desde há três anos, submetida a uma cirurgia de colocação de prótese total do ombro. Apresentava limitação da amplitude articular, diminuição da força, alteração do ritmo escapulo-umeral e ligeira incapacidade funcional. Cinco semanas após a cirurgia, iniciou sessões diárias de fisioterapia durante 12 semanas. A intervenção incluiu a aplicação de modalidades anti-álgicas, mobilização articular, fortalecimento muscular e reeducação neuromuscular. Foi avaliada em quatro momentos: antes da intervenção (M1), 4 (M2), 7 (M3) e 12 (M4) semanas após. Resultados: A dor diminuiu de 4 para 0 (END) no final de 7 semanas e as amplitudes articulares passivas aproximaram-se dos valores definidos pelo protocolo com a flexão/elevação anterior a evoluir de 90º para 130º; a abdução/elevação lateral de 80º para 110º e a rotação medial de 35º para 70º. A força muscular inicial era de 3 nos grupos musculares flexores, extensores e rotadores do ombro atingindo no final grau 4. O ritmo escapulo-umeral demonstrou assincronia durante a elevação do ombro na primeira avaliação, ficando quase normalizado no final das sessões. Quanto à funcionalidade, antes da intervenção o score obtido foi de 30.8 atingindo um valor de 11.6 no final das sessões de tratamento. Conclusão: Após 12 semanas de tratamento, os objectivos delineados foram praticamente atingidos, uma vez que a utente já não refere dor no ombro e apresenta uma boa capacidade funcional, apesar de apresentar ligeiras diferenças nas amplitudes articulares e força muscular esperadas, apontando para o sucesso do protocolo de tratamento aplicado.

Introdução: Existem diversas causas e/ou estruturas responsáveis pela dor lombar, com ou sem irradiação para o membro inferior, pelo que o sucesso no tratamento deste problema depende do diagnóstico diferencial e, consequentemente, da escolha do tratamento mais adequado a cada situação. Objectivo: avaliar os efeitos da terapia manual ortopédica num caso de lombociatalgia, cujo mecanismo dominante é a sensibilização nervosa periférica, no final do tratamento e 8 semanas após a conclusão do mesmo, e descrever a avaliação e intervenção realizadas. Métodos: foi realizado um estudo de caso acerca dum utente do sexo masculino, com 35 anos de idade, e com um quadro clínico compatível com uma situação de sensibilização nervosa periférica. A intervenção consistiu em 5 sessões distribuídas ao longo de 3 semanas. Como instrumentos de avaliação foram utilizados o 'Índice de incapacidade de Oswestry para a lombalgia’ e a Escala Numérica da Dor (END), que foram aplicados na primeira e na última sessões de tratamento e 8 semanas após esta. Resultados: Após o tratamento, verificou-se uma diminuição da incapacidade (de 74 para 2/100) e da intensidade da dor (dor matinal: de 4 para 1/10; dor associada a actividades/posturas em flexão: de 7-8 para 2/10; dor ao fim do dia: de 3-4 para 1-2/10). Oito semanas após a conclusão do tratamento estes valores mantinham-se praticamente inalterados. Conclusão: O processo de raciocínio clínico utilizado na avaliação e intervenção deste caso foi demonstrado. Os resultados obtidos sugerem que a terapia manual ortopédica poderá ser eficaz na diminuição da dor e da incapacidade em casos de lombociatalgia por sensibilização nervosa periférica, e que os ganhos se poderão manter 8 semanas após a conclusão do tratamento. Este estudo pode constituir uma evidência preliminar a favor da terapia manual ortopédica no tratamento deste subgrupo de utentes.

Introdução: Nas crianças/jovens com Paralisia Cerebral (PC), as limitações motoras repercutem-se em limitações funcionais e, consequentemente, na diminuição da participação em ocupações. Sendo as manifestações da PC diferentes de indivíduo para indivíduo, estas vão refletir, dependendo da gravidade, quadro motor, ambiente físico e social, diferentes níveis de participação. Objetivo: O objetivo deste estudo foi avaliar a relação entre a idade, sexo e grau de comprometimento motor e a participação em crianças/jovens com diagnóstico de paralisia cerebral com idades compreendidas entre os 5 e os 18 anos na ilha de São Miguel. Amostra e Métodos: 25 crianças de ambos os sexos (5- 18 anos), sinalizadas em instituições especializadas de reabilitação e em Centros de Atividades Ocupações (CAO’s) na Ilha de São Miguel – Açores. Foram aplicados dois instrumentos de avaliação às crianças/jovens, Gross Motor Function Measure e Quality of Upper Extremity Skills Test, e foram entregues aos pais os outros dois instrumentos para autopreenchimento, Assessment of Life Habits e Child Health Questionnaire – Parent- Form 50. Na análise estatística, recorreu-se a testes como o Kolmogorov-Smirnov, Tstudent ou Mann-Whitney, teste de Fisher, teste de Spearman e ANOVA. Resultados: Não foram encontradas relações significativas entre a idade e o sexo e o nível de participação das crianças/jovens com PC. Contrariamente, ao avaliarmos a relação entre o grau de participação e o grau de afetação verificamos que esta é significativa (p=0,004). Conclusão: Na nossa amostra não se encontrou uma influência da idade e do sexo com a frequência da participação (relações não foram significativas). Contudo, pode-se concluir que as crianças/jovens que apresentam menos limitações motoras, como as que se enquadram no nível I/II da Gross Motor Function Classification System, apresentam níveis de participação maiores do que as que apresentam níveis de afetação motora maiores (Nível V)

During February of 1974, the western extremity of St. Jorge's Island, one of the Azores Islands group, was severely affected by intense earthquakes, causing heavy damage. One of the authors of the present work (T. P. R.) who at the time was employed in the Rosais Lighthouse, registered day after day all the phenomena he could observe, writting down a journal that is published herein. An introduction on the geology, tectonic's, and the seismology of St. Jorge’s Island, is presented. As it can be verified, this seismic crisis was associated to a short submarine vulcanic eruption.

Trophozoites of the Giardia muris group from hamsters, domestic rats and mice and of the Giardia duodenalis group from hamsters and domestic rats were examined by transmission electron microscopy. The basic ultrastructure of the trophozoites was similar. Differences were shown in the morphology of the ventrolateral flange of the trophozoites of Giardia muris and Giardia duodenalis groups. Marginal plates are less developed in the species of the Giardia duodenalis group. In this group, the distal extremity of the lateral flange is short and thick and the marginal plate does not penetrate into the distal extremity of the flange. In the Giardia muris group, the ventro-lateral flange is well developed and narrow and the marginal plate penetrates the distal extremity of the flange. The osmiophilic lamella, which accompanies the dorsal surface of the marginal plate is seen only in the Giardia muris group.

The thoracic outlet syndrome is a changeable clinical syndrome caused by compression of the neurovascular bundle of the upper extremity, within the cervicoaxillary channel. From April 1980 through May 1995, 24 patients with clinical thoracic outlet syndrome were evaluated by selective arteriography. The diagnosis was confirmed in seven patients, in 14 the exam was normal and in the last three cases another arterial pathology was detected--subclavian artery occlusion, subclavian artery kinking and vertebral steal syndrome. The authors' aim is to emphasize arteriography as a diagnostic exam for thoracic outlet syndrome, very useful in the detection and localization of arterial compression. It also allows the diagnosis of other arterial entities.

The authors report neuromuscular manifestations in a 45-year-old woman after consuming octopus meat (Octopus sp.). The patient presented malaise, paresthesias in perioral and extremity areas, intense muscular weakness and arterial hypotension, followed by severe itch and disseminated cutaneous rash. Gastrointestinal manifestations and fever were not observed, reducing the probability of alimentary poisoning. The presence of muscular and neurological symptoms suggests neurotoxin action, which could have been ingested by the victim from the octopus salivary glands or from an accumulation of toxins in the meat, or by an unknown mechanism. There is little known about toxins of the Octopus genus and this communication is important alert to the possibility of poisoning in humans that eat octopus and its differentiation from alimentary poisonings arising from incorrect conservation of seafood.

Introdução: As soluções reconstrutivas das queimaduras dos membros inferiores com exposição óssea (transferência de tecidos vascularizados) e dos defeitos do escalpe, não susceptíveis de encerramento com retalhos locais (expansão tecidual), são complexas, morosas e, por vezes, inviáveis. O recurso a substitutos cutâneos biológicos, como a matriz de regeneração dérmica, constitui uma importante alternativa às soluções reconstrutivas tradicionais. Relato dos casos: Estudo retrospectivo de 246 doentes com queimaduras dos membros inferiores, admitidos na Unidade de Queimados/Serviço de Cirurgia Plástica do nosso Centro Hospitalar, entre Janeiro de 2007 e Dezembro de 2008. Os autores apresentam 2 casos clínicos com queimaduras bilaterais das pernas e com exposição óssea. Nos dois casos, realizou-se desbridamento tangencial e encerramento das áreas cruentas com um substituto cutâneo, a matriz de regeneração dérmica. Na 4ª semana após a aplicação da matriz, o componente externo da membrana bilaminar foi substituído por enxerto de pele parcial. Obteve-se cobertura estável das áreas cruentas, sem necessidade de procedimentos cirúrgicos adicionais. Conclusões: A utilização de substitutos dérmicos expande as opções reconstrutivas nos casos de queimaduras com exposição óssea dos membros inferiores. A utilização da matriz biológica permitiu a preservação dos membros nos dois pacientes. A matriz de regeneração dérmica possibilitou uma cobertura estável de estruturas vitais, sem necessidade de transferência ou expansão tecidual e com morbilidade mínima.

We sought to describe the bone bridge technique in adults, and present a variation for use in children, as well as to present its applicability as an option in elective transtibial amputations. This paper presents a prospective study of 15 transtibial amputations performed between 1992 and 1995 in which the bone bridge technique was employed. The patients' ages ranged from 8 to 48 years, with an average of 22.5 years. This technique consisted of the preparation of a cylinder of periosteum extracted from the tibia and with cortical bone fragments attached to it to promote a tibiofibular synostosis on the distal extremity of the amputation stump. We noted that the cortical bone fragments were dispensable when the technique was employed in children, due to the increased osteogenic capacity of the periosteum. This led to a variation of the original technique, a bone bridge without the use of the cortical bone fragments. RESULTS: The average time spent with this procedure, without any significant variation between adults and children, was 171 minutes. The adaptation to the definitive prosthesis was accomplished between 20 and 576 days, with an average of 180 days. Revision of the procedure was necessary in 3 amputations. CONCLUSIONS: This technique may be employed in transtibial amputations in which the final length of the stump lies next to the musculotendinous transition of the gastrocnemius muscle, as well as in the revision of amputation stumps in children, where the procedure has been shown to be effective in the prevention of lesions due to excessive bone growth.

Two new species of Bucephalidae (Trematoda) are described from fishes of the Brazilian Amazon. Glandulorhynchus turgidus gen. et sp. n. from Hydrolycus sp. differs from all others in the family by having conspicuous glands near the anterior extremity and an expanded uterus used for storing eggs. Prosorhynchus piranhus sp. n. from Serrasalmus rhombeus is characterized as having a small rhynchus and pharynx, and a large cirrus sac.

In this paper an account is given of the principal facts observer in the meiosis of Euryophthalmus rufipennis Laporte which afford some evidence in favour of the view held by the present writer in earlier publications regarding the existence of two terminal kinetochores in Hem ip ter an chromosomes as well as the transverse division of the chromosomes. Spermatogonial mitosis - From the beginning of prophase until metaphase nothing worthy of special reference was observed. At anaphase, on the contrary, the behavior of the chromosomes deserves our best attention. Indeed, the chromoso- mes, as soon as they begin to move, they show both ends pronouncedly turned toward the poles to which they are connected by chromosomal fibres. So a premature and remarkable bending of the chromosomes not yet found in any other species of Hemiptera and even of Homoptera points strongly to terminally localized kinetochores. The explanation proposed by HUGHES-SCHRADER and RIS for Nautococcus and by RIS for Tamalia, whose chromosomes first become bent late in anaphase do not apply to chromosomes which initiate anaphase movement already turned toward the corresponding pole. In the other hand, the variety of positions assumed by the anaphase chromosomes of Euryophthalmus with regard to one another speaks conclusively against the idea of diffuse spindle attachments. First meiotic division - Corresponding to the beginning of the story of the primary spermatocytes cells are found with the nucleus entirelly filled with leptonema threads. Nuclei with thin and thick threads have been considered as being in the zygotente phase. At the pachytene stage the bivalents are formed by two parallel strands clearly separated by a narrow space. The preceding phases differ in nothing from the corresponding orthodox ones, pairing being undoubtedly of the parasynaptic type. Formation of tetrads - When the nuclei coming from the diffuse stage can be again understood the chromosomes reappear as thick threads formed by two filaments intimately united except for a short median segment. Becoming progressively shorter and thicker the bivalents sometimes unite their extremities forming ring-shaped figures. Generally, however, this does not happen and the bivalents give origin to more or less condensed characteristic Hemipteran tetrads, bent at the weak median region. The lateral duplicity of the tetrads is evident. At metaphase the tetrads are still bent and are connected with both poles by their ends. The ring-shaped diakinesis tetrads open themselves out before metaphase, showing in this way that were not chiasmata that held their ends together. Anaphase proceeds as expected. If we consider the median region of the tetrads as being terminalized chiasmata, then the chromosomes are provided with a single terminal kinetochore. But this it not the case. A critical analysis of the story of the bivalents before and after the diffuse stage points to the conclusion that they are continuous throughout their whole length. Thence the chromosomes are considered as having a kinetochore at each end. Orientation - There are some evidences that Hemipteran chromosomes are connected by chiasmata. If this is true, the orientation of the tetrads may be understood in the following manner: Chiasmata being hindered to scape by the terminal kinetochores accumulate at the ends of the tetrads, where condensation begins. Repulsion at the centric ends being prevented by chiasmata the tetrads orient themselves as if they were provided with a single kinetochore at each extremity, taking a position parallelly to the spindle axis. Anaphase separation - Anaphase separation is consequently due to a transverse division of the chromosomes. Telophase and secund meiotic division - At telophase the kinetochore repeli one another following the moving apart of the centosomes, the chiasmata slip toward the acentric extremities and the chromosomes rotate in order to arrange themselves parallelly to the axis of the new spindle. Separation is therefore throughout the pairing plane. Origin of the dicentricity of the chromosomes - Dicentricity of the chromosomes is ascribed to the division of the kinetochore of the chromosomes reaching the poles followed by separation and distension of the chromatids which remain fused at the acentric ends giving thus origin to terminally dicentric iso-chromosomes. Thence, the transverse division of the chromosomes, that is, a division through a plane perpendicular to the plane of pairing, actually corresponds to a longitudinal division realized in the preceding generation. Inactive and active kinetochores - Chromosomes carrying inactive kinetochore is not capable of orientation and active anaphasic movements. The heterochromosome of Diactor bilineatus in the division of the secondary spermatocytes is justly in this case, standing without fibrilar connection with the poles anywhere in the cell, while the autosomes are moving regularly. The heterochromosome of Euryophthalmus, on the contrary, having its kinetochores perfectly active ,is correctly oriented in the plane of the equator together with the autosomes and shows terminal chromosomal connection with both poles. Being attracted with equal strength by two opposite poles it cannot decide to the one way or the other remaining motionless in the equator until some secondary causes (as for instances a slight functional difference between the kinetochores) intervene to break the state of equilibrium. When Yiothing interferes to aide the heterochromosome in choosing its way it distends itself between the autosomal plates forming a fusiform bridge which sometimes finishes by being broken. Ordinarily, however, the bulky part of the heterochromosome passes to one pole. Spindle fibers and kinetic activity of chromosomal fragments - The kinetochore is considered as the unique part of the chromosome capable of being influenced by other kinetochore or by the poles. Under such influence the kinetochore would be stimulated or activited and would elaborate a sort of impulse which would run toward the ends. In this respect the chromosome may be compared to a neüròn, the cell being represented by the kinetochore and the axon by the body of the chromosome. Due to the action of the kinetochore the entire chromosome becomes also activated for performing its kinetic function. Nothing is known at present about the nature of this activation. We can however assume that some active chemical substance like those produced by the neuron and transferred to the effector passes from the kinetochore to the body of the chromosome runing down to the ends. And, like an axon which continues to transmit an impulse after the stimulating agent has suspended its action, so may the chromosome show some residual kinetic activity even after having lost its kinetochore. This is another explanation for the kinetic behavior of acentric chromosomal fragmehs. In the orthodox monocentric chromosomes the kinetic activity is greater at the kinetochore, that is, at the place of origin of the active substance than at any other place. In chromosomes provided with a kinetochore at each end the entire body may become active enough to produce chromosomal fibers. This is probably due to a more or less uniform distribution and concentration of the active substance coming simultaneously from both extremities of the chromosome.

Spermatogonial chromosomes of Pachylis laticornis and Pachylis pharaonis begin anaphasic movement with both ends turned toward the same pole, maintaining this form util they reach the poles. This is a proof that they are provided with one kinetochore at each end. Additional proof for a longitudinal division of each longitudinal half of the anaphase chromosomes of the primary sper- matocytes is presented against the idea of a previous end-toend pairing at metaphase. The longitudinal split of the chromosomes of the secondary spermatocytes which used to be considered as tertiary split is therefore a true secondary split. The heterochromosome in both species passes undivided to one pole in the first division of the spermatocyte. In Pachylis laticornis it appears connected with the poles by means of two fibrils detached from each extremity, what may be considered as indicating a rather premature longitudinal spliting. The behavior of the heterochromosome of Pachylis pharaonis is highly interesting and affords one of the most beautiful evidences in favour of the dicentricity of the chromosomes. Really, in metaphase the heterochromosome appears at the equator of the cell with a more or less round shape. In the beginning of anaphase it becomes fusiform. As anaphase proceeds it distends itself between the autosomal plates forming a long fusiform bridge or sends toward the plates a thick chromosomal thread. The bulky part of the heterochromosome as it passes to one side it reincorporates the substance of the thread in this side. The thread in the other side, which becomes generally thiner, is left with its kinetochore in the cell at this side. The heterochromosome therefore becomes terminally monocentric in the first division of the spermatocyte. Some figures, however, suggest that the heterochromossome from time to time may pass with both kinetochores to one of the cells, as ordinarily happens in the case of Pachylis laticornis. Summing up, other things apart the behavior of the heterochromosome in both species studied here puts out of doubt the question of the existence of two terminally located kinetochores.
