OBJETIVO: Verificar os desempenhos perceptual-auditivo e ortográfico de escolares no que se refere à identificação de contrastes entre as fricativas do Português Brasileiro, e investigar em que medida esses dois tipos de desempenhos se relacionam. MÉTODOS: Foram analisados dados de desempenho perceptual-auditivo e de desempenho ortográfico extraídos de 20 crianças das duas primeiras séries do ensino fundamental de uma escola pública do município de Mallet (PR). A coleta de dados de percepção auditiva foi feita com base no Instrumento de Avaliação da Percepção de Fala (PERCEFAL), com o uso do software Perceval. Já a coleta de dados de ortografia foi feita por meio de um ditado das mesmas palavras que compõem o instrumento PERCEFAL. RESULTADOS: Foram observadas: maior acurácia perceptual-auditiva do que ortográfica; tendência de menor tempo de resposta e de menor variabilidade nos acertos perceptuais-auditivos do que nos erros; não correspondência de erros de percepção-auditiva e ortografia, já que, na percepção, o maior percentual de erros envolveu o ponto de articulação das fricativas, enquanto que, na ortografia, o maior percentual envolveu o vozeamento. CONCLUSÃO: Embora se mostrem relacionados, os desempenhos perceptual-auditivo e ortográfico não apresentam correspondência termo a termo. Portanto, na prática clínica, a atenção deve-se voltar não apenas para os aspectos que aproximam esses dois desempenhos, mas, também, para os aspectos que os diferenciam.

BACKGROUND: One of the great difficulties in evaluating a voice is the judgment of quality through the perceptual auditive analysis--although frequently used--, as it is influenced by socioeconomic and cultural aspects as well as individual preferences. Many are the adjectives and methods used in this assessment, especially because of the subjectivity involved in the process, leading to incompatibilities between listeners and difficulties in reaching a consensus on the use of this or that terminology. In such a context, the voice laboratory and more specifically the acoustic computerized analysis, has guided and complemented speech-language treatments. Among the several possibilities of spectrographic analysis, the (Long-Term Average Spectrum--LTAS) quantifies the quality of voices, pointing differences between gender, age, professional--spoken and sang--and dysphonic voices. The LTAS has been used a lot in researches that investigate voice. As it evidences the contribution of the glottic source and of resonance to the quality of voice, it provides objective parameters for the evaluation of this aspect which usually depends on our auditive perception. AIM: to demonstrate how LTAS can be applied in voice research and in the speech-language therapy practice, describing both the technical aspects required for the production and interpretation of results, and its limitations. CONCLUSION: The area of voice research has developed a lot in these last two decades especially because of the advent of the voice and speech laboratory. For this reason, the knowledge about the applicability of more tools for voice analysis, as the LTAS, as well as the existing need for more studies in this area, will most certainly contribute for the creation of new research areas not only in the field of professional voice but also in the field of therapy.

Background: familial dyslexia. Aim: to characterize and compare the phonological awareness, working memory, reading and writing abilities of individuals whose family members are also affected. Method: in this study 10 familial nuclei of natural family relationship of individuals with dyslexia were analyzed. Families of natural individuals living in the west region of the state of São Paulo were selected. Inclusion criteria were: to be a native speaker of the Brazilian Portuguese language, to have 8 years of age or more, to present positive familial history for learning disabilities, That is, to present at least one relative with difficulties in learning. Exclusion criteria were: to present any neurological disorder genetically caused or not, in any of the family members, such as dystonia, extra pyramidal diseases, mental disorder, epilepsy, attention deficit and hyperactivity disorder (ADHA); psychiatric symptoms or conditions; or any other pertinent conditions that could cause errors in the diagnosis. As for the diagnosis of developmental dyslexia, information about the familial history of the adolescents and children was gathered with the parents, so that a detailed pedigree could be delineated. Neurological, psychological, speech-language, and school performance evaluations were made with the individuals and their families. Results: the results of this study suggest that the dyslexic individuals and their respective relatives, also with dyslexia, presented lower performances than the control group in terms of rapid automatic naming, reading, writing and phonological awareness. Conclusion: deficits in phonological awareness, working memory, reading and writing seem to have genetic susceptibility that possibly determine, when in interaction with the environment, the manifestation of dyslexia.

Background: program for phonological remediation in developmental dyslexia. Aim: to verify the efficacy of a program for phonological remediation in students with developmental dyslexia. Specific goals of this study involved the comparison of the linguistic-cognitive performance of students with developmental dyslexia with that of students considered good readers; to compare the results obtained in pre and post-testing situations of students with dyslexia who were and were not submitted to the program; and to compare the results obtained with the phonological remediation program in students with developmental dyslexia to those obtained in good readers. Method: participants of this study were 24 students who were divided as follows: Group I (GI) was divided in two other groups - Gle with 6 students with developmental dyslexia who were submitted to the program; and Glc with 6 students with developmental dyslexia who were not submitted to the program; Group II (GII) was also divided in two other groups - GIIe with 6 good readers who were submitted to the program, and GIIc with 6 good readers who were not submitted to the program. The phonological remediation program (Gonzalez & Rosquete, 2002) was developed in three stages: pre-testing, training and post-testing. Results: results indicate that GI presented a lower performance in phonological skills, reading and writing when compared to GII in the pre-testing situation. However, GIe presented a similar performance to that of GII in the post-testing situation, indicating the effectiveness of the phonological remediation program in students with developmental dyslexia. Conclusion: this study made evident the effectiveness of the phonological remediation program in students with developmental dyslexia.

The main impact of hearing loss, in childhood, is the difficult to speech perception. The hearing aids and the cochlear implant are resources used to decrease a result of hearing loss. After the adjustment of these resources, with the therapeutic process, is possible that children with profound hearing impairment develop their hearing abilities, so they will have possibilities to notice phonetic and phonological characteristics of the language. Different speech perception procedures investigate which phonetic and phonological characteristics of the speech segment can be noticed by children with hearing loss. This study aims at analyzing the audiology profile and the speech perception in deaf children and adolescent.

To report the audiological outcomes of cochlear implantation in two patients with severe to profound sensorineural hearing loss secondary to superficial siderosis of the CNS and discuss some programming peculiarities that were found in these cases. Retrospective review. Data concerning clinical presentation, diagnosis and audiological assessment pre- and post-implantation were collected of two patients with superficial siderosis of the CNS. Both patients showed good hearing thresholds but variable speech perception outcomes. One patient did not achieve open-set speech recognition, but the other achieved 70% speech recognition in quiet. Electrical compound action potentials could not be elicited in either patient. Map parameters showed the need for increased charge. Electrode impedances showed high longitudinal variability. The implants were fairly beneficial in restoring hearing and improving communication abilities although many reprogramming sessions have been required. The hurdle in programming was the need of frequent adjustments due to the physiologic variations in electrical discharges and neural conduction, besides the changes in the impedances. Patients diagnosed with superficial siderosis may achieve limited results in speech perception scores due to both cochlear and retrocochlear reasons. Careful counseling about the results must be given to the patients and their families before the cochlear implantation indication.

The benefits of cochlear implants (CI) for communication skills are obtained over the years. There are but a few studies regarding the long-term outcomes in postlingual deaf children who grew up using the electronic device. Aim: To assess the functional results in a group of postlingual children, 10 years after using a CI. Methods: Ten postlingual deaf children, implanted before 18 years of age, participated in this study. We assessed: sentence recognition and speech intelligibility. We documented: device use and function and the patient's academic/occupational status. Study design: series. Results: The mean scores were 73% for sentence recognition in silence and 40% in noise. The average write-down intelligibility score was 92% and the average rating-scale intelligibility score was 4.15. There were no cases of device failure. Regarding educational/vocational status, three subjects graduated from the University. Five quit education after completing high school. Eight subjects had a professional activity. Conclusion: This study showed that cochlear implantation is a safe and reliable procedure. The postlingual profoundly hearing-impaired children after 10 years of CI use developed satisfactory levels regarding speech perception and intelligibility, and completed at least high school and were inserted in the labor market. Objectives: To report the results of cochlear implantation via the middle fossa approach in 4 patients, discuss the complications, and present a detailed description of the programming specifications in these cases. Study Design: Retrospective case review. Setting: Tertiary-care referral center with a well-established cochlear implant program. Patients: Four patients with bilateral canal wall down mastoid cavities who underwent the middle fossa approach for cochlear implantation. Interventions: Cochlear implantation and subsequent rehabilitation. A middle fossa approach with cochleostomy was successfully performed on the most superficial part of the apical turn in 4 patients. A Nucleus 24 cochlear implant system was used in 3 patients and a MED-EL Sonata Medium device in 1 patient. The single electrode array was inserted through a cochleostomy from the cochlear apex and occupied the apical, middle, and basal turns. Telemetry and intraoperative impedance recordings were performed at the end of surgery. A CT scan of the temporal bones was performed to document electrode insertion for all of the patients. Main Outcome Measures: Complications, hearing thresholds, and speech perception outcomes were evaluated. Results: Neural response telemetry showed present responses in all but 1 patient, who demonstrated facial nerve stimulation during the test. Open-set speech perception varied from 30% to 100%, despite the frequency allocation order of the MAP. Conclusion: Cochlear implantation via the middle cranial fossa is a safe approach, although it is a challenging procedure, even for experienced surgeons.

The harm upon the central auditory pathways of workers exposed to occupational noise has been scarcely studied. Objective: To assess the central auditory pathways by testing the long latency auditory evoked potentials (P300) of individuals exposed to occupational noise and controls. Method: This prospective study enrolled 25 individuals with normal hearing thresholds. The subjects were divided into two groups: individuals exposed to occupational noise (13 subjects; case group) and individuals not exposed to occupational noise (12 subjects; control group). The P300 test was used with verbal and non-verbal stimuli. Results: No statistically significant differences were found between ears for any of the stimuli or between groups. The groups had no statistically significant difference for verbal or non-verbal stimuli. Case group subjects had longer latencies than controls. In qualitative analysis, a greater number of altered P300 test results for verbal and non-verbal stimuli was seen in the case group, despite the absence of statistically significant differences between case and control subjects. Conclusion: Individuals exposed to high sound pressure levels had longer P300 latencies in verbal and non-verbal stimuli when compared to controls.
Auditory brainstem implant outcomes and MAP parameters: Report of experiences in adults and children

The auditory brainstem implant (ABI) was first developed to help neurofibromatosis type 2 patients. Recently, its use has been recently extended to adults with non-tumor etiologies and children with profound hearing loss who were not candidates for a cochlear implant (Cl). Although the results has been extensively reported, the stimulation parameters involved behind the outcomes have received less attention. Objective: The aim of this study is to describe the audiologic outcomes and the MAP parameters in ABI adults and children at our center. Methods: Retrospective chart review. Five adults and four children were implanted with the ABI24M from September 2005 to June 2009. In the adult patients, four had Neurofibromatosis type 2, and one had postmeningitic deafness with complete ossification of both cochleae. Three of the children had cochlear malformation or dysplasia, and one had complete ossified cochlea due to meningitis. Map parameters as well as the intraoperative electrical auditory brainstem responses were collected. Evaluation was performed with at least six months of device use and included free-field hearing thresholds, speech perception tests in the adult patients and for the children, the Infant-Toddler Meaningful Auditory Integration Scale (IT-MAIS) and (ESP) were used to evaluate the development of auditory skills, besides the MUSS to evaluate. Results: The number of active electrodes that did not cause any non-auditory sensation varied from three to nineteen. All of them were programmed with SPEAK strategy, and the pulse widths varied from 100 to 300 mu s. Free-field thresholds with warble tones varied from very soft auditory sensation of 70 dBHL at 250 Hz to a pure tone average of 45 dBHL. Speech perception varied from none to 60% open-set recognition of sentences in silence in the adult population and from no auditory sensation at all to a slight improvement in the IT-MAIS/MAIS scores. Conclusion: We observed that ABI may be a good option for offering some hearing attention to both adults and children. In children, the results might not be enough to ensure oral language development. Programming the speech processor in children demands higher care to the audiologist. (C) 2011 Elsevier Ireland Ltd. OBJETIVO: Avaliar a percepção da fala de crianças deficientes auditivas com o aparelho de amplificação sonora individual (AASI) e sistema de frequência modulada (FM) em situações de ruído em campo livre e em sala de aula. MÉTODOS: Participaram 13 crianças deficientes auditivas entre 7 e 17 anos. Foi aplicado o Hearing in Noise Test (HINT) com AASI e com o FM. Também foi aplicado o questionário Avaliação do Sistema FM, respondido pelos professores das crianças, com o intuito de avaliar, individualmente, o desempenho da criança em diferentes situações auditivas somente com AASI e com o AASI e o sistema FM. RESULTADOS: Houve diferença para todas as situações com e sem FM no teste HINT. O mesmo aconteceu com os resultados do questionário, sendo que sem FM a pontuação foi sempre menor do que com FM, independentemente da condição. CONCLUSÃO: O uso de medidas subjetivas, como o questionário, é fundamental para determinar a eficácia da indicação dos dispositivos auxiliares para o deficiente auditivo. A efetividade do sistema FM pode ser observada pela "vantagem FM", que é a diferença média mínima de 10 dB encontrada nas avaliações de percepção da fala com e sem FM nas diferentes situações de ruído. Os benefícios encontrados na presente pesquisa com o uso do sistema FM na melhora da percepção da fala podem ser extrapolados não só para a sala de aula e para a legislação da educação inclusiva, mas também para atividades sociais e de lazer.

As novas tecnologias do processador Freedom® foram criadas para proporcionar melhorias no processamento do som acústico de entrada, não apenas para novos usuários, como para gerações anteriores de implante coclear. OBJETIVO: Identificar a contribuição da tecnologia do processador de fala Freedom® para implante coclear multicanal, Nucleus22®, no desempenho de percepção de fala no silêncio e no ruído, e nos limiares audiométricos. MATERIAL E MÉTODO: A forma de estudo foi de coorte histórico com corte transversal. Dezessete pacientes preencheram os critérios de inclusão. Antes de iniciar os testes, o último mapa em uso com o Spectra® foi revisto e otimizado e o funcionamento do processador foi verificado. Os testes de fala foram apresentados a 60dBNPS em material gravado: monossílabos; frases em apresentação aberta no silêncio; e no ruído (SNR = 0dB). Foram realizadas audiometrias em campo livre com ambos os processadores de fala. A análise estatística utilizou testes não-paramétricos. RESULTADOS: Quando analisada a contribuição do Freedom® para pacientes com Nucleus22®, observa-se diferença estatisticamente significativa em todos os testes de percepção de fala e em todos os limiares audiométricos. CONCLUSÃO: A tecnologia contribuiu no desempenho de percepção de fala e nos limiares audiométricos dos pacientes usuários de Nucleus22®.

Apesar dos avanços tecnológicos empregados nas estratégias de processamento do sinal, um dos obstáculos ainda existentes são as lacunas de detalhes espectrais na informação elétrica transmitida. Considerando a sua importância na percepção da fala, pesquisadores investigaram mecanismos para otimizar o detalhamento espectral, por meio dos canais espectrais virtuais. A aplicação clínica desta técnica resultou em uma nova estratégia de processamento de sinal - a HiRes 120. OBJETIVO: Avaliar o desempenho auditivo de usuários de Implante Coclear com a HiRes 120. METODOLOGIA: Levantamento bibliográfico conduzido em base eletrônica de dados, com busca padronizada até o ano de 2011, utilizando-se palavras-chave específicas. Para a seleção e avaliação dos estudos científicos levantados na busca, foram estabelecidos critérios, contemplando os aspectos: tipo de estudo, participantes, intervenção adotada e avaliação dos resultados. CONCLUSÃO: As evidências científicas apontam uma melhora do desempenho auditivo nas situações de ruído com a estratégia HiRes 120, mas tal fato não ocorre nas situações de silêncio. A otimização da percepção da fala com o uso da estratégia está intimamente relacionada com a idade do usuário de implante coclear, com o tempo de privação sensorial e o tempo de aclimatização necessário para o aproveitamento das informações espectrais da estratégia.

This study had the aim to investigate the auditory and communicative abilities of children diagnosed with Auditory Neuropathy Spectrum Disorder due to mutation in the Otoferlin gene. It is a descriptive and qualitative study in which two siblings with this diagnosis were assessed. The procedures conducted were: speech perception tests for children with profound hearing loss, and assessment of communication abilities using the Behavioral Observation Protocol. Because they were siblings, the subjects in the study shared family and communicative context. However, they developed different communication abilities, especially regarding the use of oral language. The study showed that the Auditory Neuropathy Spectrum Disorder is a heterogeneous condition in all its aspects, and it is not possible to make generalizations or assume that cases with similar clinical features will develop similar auditory and communicative abilities, even when they are siblings. It is concluded that the acquisition of communicative abilities involves subjective factors, which should be investigated based on the uniqueness of each case.
