A single-base substitution within an intronic repetitive element causes dominant retinitis pigmentosa with reduced penetrance.

We report the study of a large American family displaying autosomal dominant retinitis pigmentosa with reduced penetrance, a form of hereditary retinal degeneration. Although the inheritance pattern and previous linkage mapping pointed to the involvement of the PRPF31 gene, extensive screening of all its exons and their boundaries failed in the past to reveal any mutation. In this work, we sequenced the entire PRPF31 genomic region by both the classical Sanger method and ultrahigh throughput (UHT) sequencing. Among the many variants identified, a single-base substitution (c.1374+654C>G) located deep within intron 13 and inside a repetitive DNA element was common to all patients and obligate asymptomatic carriers. This change created a new splice donor site leading to the synthesis of two mutant PRPF31 isoforms, degraded by nonsense-mediated mRNA decay. As a consequence, amounts of PRPF31 mRNA derived from the mutant allele were very reduced, with no evidence of mutant proteins being synthesized. Our results indicate that c.1374+654C>G causes retinitis pigmentosa via haploinsufficiency, similar to the vast majority of PRPF31 mutations described so far. We discuss the potential of UHT sequencing technologies in mutation screening and the continued identification of pathogenic splicing mutations buried deep within intronic regions.

Processus et fonctions psychiques de l'écriture : de la création littéraire au groupe à médiation

Ce travail porte sur l'étude des processus et fonctions psychiques de la création littéraire et de l'écriture dans une perspective psychanalytique, à partir d'une méthodologie de recherche qualitative exploratoire. Un premier volet de la recherche s'attache à étudier les processus et fonctions psychiques dans la création littéraire à partir de six entretiens semi-structurés avec des écrivains romands publiés. Les entretiens sont analysés en profondeur à partir de la méthodologie qualitative Interpretative Phenomenological Analysis (Smith, Flowers & Larkin, 2009). Un second volet de la recherche étudie les processus et fonctions psychiques de l'écriture dans un atelier de groupe à médiation écriture prenant place dans un Centre de Jour pour adultes, à partir de l'élaboration de deux cas. L'articulation des analyses de ces expériences d'écriture dans les deux contextes étudiés permet de mettre en évidence des enjeux de continuité/discontinuité dans le passage du premier mouvement créateur de l'écriture - le « miroir-papier » - vers le mouvement de la publication/lecture - le « miroir-lecteur ». Ces enjeux de partage invitent à penser le travail créateur de l'écriture à partir de la notion de « Moi- peau » (Anzieu, 1995) et à préciser cinq fonctions psychiques de l'écriture : transformation ; protection ; échange ; réassurance narcissique ; partage. Un accent particulier est porté sur la fonction de protection, dans la tension qu'elle entretient avec celle de transformation, au travers de la mise en évidence de six formes d'enveloppes d'écriture protectrice. Finalement, les analyses mettent en évidence la manière dont cette tension entre un investissement défensif de l'écriture et un investissement de transformation se déploie par des voies singulières en vue d'une tentative de résolution du conflit subjectif interne entre les exigences du « public intérieur » (De M'Uzan, 1964) et la rencontre avec l'objet réel. Ces pistes d'analyses permettent de préciser les modalités de contribution de l'écriture aux processus de symbolisation et d'enrichir la pratique du champ des médiations thérapeutiques de repères d'appréciation clinique. - Based on an exploratory qualitative research methodology, this work focuses on the psychical processes and functions of literary creation and writing from a psychoanalytic perspective. The first part of the research investigates psychical processes and functions in creative writing through the analysis of six semi-structured interviews with published Swiss writers. The interviews are analyzed according to the exploratory qualitative methodology Interpretative Phenomenological Analysis (Smith, Flowers & Larkin, 2009). The second area of research examines the processes of writing in the context of an art-therapy writing group that took place in an adult day center. The observations lead to the elaboration of two case studies. The joint analysis of these writing experiences highlights issues of continuity/discontinuity in passing from the first creative writing movement - the « mirror-paper » - to the final publication/reading of the work - the «mirror-reader». These issues enable us to understand the creative writing processes from the concept of the « skin-ego » (Anzieu, 1995). Five functions of writing are proposed: transformation, protection, exchange, narcissistic reassurance, and sharing. An emphasis on the tension between the protection and the transformation functions is highlighted through the identification of six forms of protective writing psychical envelopes. Finally, the analysis shows how this tension between a defensive and a transformative investment in writing takes place in an attempt to resolve the internal conflict between the « inner audience » (De M'Uzan, 1964) and the encounter with the real object. This study clarifies the way creative writing can contribute to symbolization and can enrich clinical assessment in the field of art-therapy.

La substitution hormonale et ses dérivés dans la prévention et le traitement de l'ostéoporose [Hormone replacement therapy and its derivatives in the prevention and treatment of osteoporosis]

Hormone replacement therapy (HRT) is an established approach for the treatment and the prevention of osteoporosis. Many studies with bone mineral density as primary outcome have shown significant efficacy. Observational studies have indicated a significant reduction of hip fracture risk in cohorts of women who maintained HRT therapy. The Women's Health Initiative is the first prospective randomised controlled study which showed a positive effect of HRT in terms of reduction of vertebral and hip fractures risk. Unfortunately, this study has been interrupted after 5.2 years because of the unsupportable increase of risk of cardiovascular disease and breast cancer. Compliance with HRT, however, is typically poor because of the potential side effects and possible increased risk of breast or endometrial cancer. Nevertheless, there is now evidence that lower doses of estrogens in elderly women may prevent bone loss while minimizing the side effects seen with higher doses. Combination therapies using low doses estrogen should probably be reserved for patients who continue to fracture on single therapy. Selective estrogen receptor modulators (SERMs) are very interesting drugs. The goal of these agents is to maximize the beneficial effect of estrogen on bone and to minimize or antagonize the deleterious effects on the breast and endometrium. Raloxifene, approved for the prevention and the treatment of osteoporosis, has been shown to reduce the risks of vertebral fracture in large clinical trials. However, they don't reduce non vertebral fractures. Tibolone is a synthetic steroid that increased bone mineral density at lumbar spine and femoral neck. But no trial has been performed with fractures as end point.

Elaboration d'un cadre d'évaluation de l'impact du processus communautaire sur la santé des aîné(e)s à Yverdon.

Contexte et cadre: L'Institut universitaire de médecine sociale et préventive (IUMSP) de Lausanne a été mandaté par la Fondation Leenaards pour élaborer un cadre pour l'évaluation de l'impact d'un processus communautaire sur la santé des aînés dans la ville d'Yverdon, appelé "Quartiers solidaires". "Quartiers solidaires" est une méthodologie de développement communautaire, créée par l'unité de travail social communautaire de Pro Senectute Vaud. Elle a pour but d'améliorer la qualité de vie actuelle et future des personnes âgées au sein de leur quartier. Pour atteindre cet objectif, les habitants, et en particulier les aînés, sont invités à devenir auteurs et acteurs de leurs propres projets en créant une communauté locale. Le projet "Quartiers solidaires" à Yverdon s'inscrit par ailleurs dans le cadre d'un projet communal de politique d'action sociale de la ville, Qualité de vie, s'appliquant au niveau de la ville et impliquant divers acteurs et instances. L'IUMSP propose, pour l'élaboration de ce cadre, d'utiliser un outil : le "modèle de catégorisation des résultats". Cet outil, appelé aussi SMOC (Swiss Model for Outcome Classification), permet à tous les partenaires impliqués dans le projet Quartiers solidaires d'établir ensemble un état de situation à laquelle ils sont confrontés (c'est à dire une analyse des problèmes auxquels ils doivent faire face), ainsi qu'une théorie d'action (c'est à dire un schéma qui structure les activités, en cours et planifiées, selon les divers objectifs visés). Cet outil, caractérisé par sa démarche participative, a été utilisé dans le cadre d'ateliers réunissant les différentes parties prenantes du projet. [...] Le modèle est basé sur la santé dans sa définition la plus large (telle que définie par l'OMS), englobant donc la qualité de vie, et est en adéquation avec l'approche écologique, qui repose sur une vision élargie des déterminants de la santé. Covalent homodimers of murine secretory component induced by epitope substitution unravel the capacity of the polymeric Ig receptor to dimerize noncovalently in the absence of IgA ligand.

Recombinant secretory immunoglobulin A containing a bacterial epitope in domain I of the secretory component (SC) moiety can serve as a mucosal delivery vehicle triggering both mucosal and systemic responses (Corthésy, B., Kaufmann, M., Phalipon, A., Peitsch, M., Neutra, M. R., and Kraehenbuhl, J.-P. (1996) J. Biol. Chem. 271, 33670-33677). To load recombinant secretory IgA with multiple B and T epitopes and extend its biological functions, we selected, based on molecular modeling, five surface-exposed sites in domains II and III of murine SC. Loops predicted to be exposed at the surface of SC domains were replaced with the DYKDDDDK octapeptide (FLAG). Another two mutants were obtained with the FLAG inserted in between domains II and III or at the carboxyl terminus of SC. As shown by mass spectrometry, internal substitution of the FLAG into four of the mutants induced the formation of disulfide-linked homodimers. Three of the dimers and two of the monomers from SC mutants could be affinity-purified using an antibody to the FLAG, mapping them as candidates for insertion. FLAG-induced dimerization also occurred with the polymeric immunoglobulin receptor (pIgR) and might reflect the so-far nondemonstrated capacity of the receptor to oligomerize. By co-expressing in COS-7 cells and epithelial Caco-2 cells two pIgR constructs tagged at the carboxyl terminus with hexahistidine or FLAG, we provide the strongest evidence reported to date that the pIgR dimerizes noncovalently in the plasma membrane in the absence of polymeric IgA ligand. The implication of this finding is discussed in terms of IgA transport and specific antibody response at mucosal surfaces.

Factor shares, the price markup, and the elasticity of substitution between capital and labor

[spa] La participación del trabajo en la renta nacional es constante bajo los supuestos de una función de producción Cobb-Douglas y competencia perfecta. En este artículo se relajan estos supuestos y se investiga si el comportamiento no constante de la participación del trabajo en la renta nacional se explica por (i) una elasticidad de sustitución entre capital y trabajo no unitaria y (ii) competencia no perfecta en el mercado de producto. Nos centramos en España y los U.S. y estimamos una función de producción con elasticidad de sustitución constante y competencia imperfecta en el mercado de producto. El grado de competencia imperfecta se mide a través del cálculo del price markup basado en laaproximación dual. Mostramos que la elasticidad de sustitución es mayor que uno en España y menor que uno en los US. También mostramos que el price markup aleja la elasticidad de sustitución de uno, lo aumenta en España, lo reduce en los U.S. Estos resultados se utilizan para explicar la senda decreciente de la participación del trabajo en la renta nacional, común a ambas economías, y sus contrastadas sendas de capital.
