Objetivo: Avaliar a influência da fissura na escolha de alimentos doces e alteração do peso corporal em pacientes alcoolistas. Métodos: Vinte e um pacientes alcoolistas em tratamento no Centro de Atenção Psicossocial de álcool e drogas (CAPSad), Ouro Preto/MG, foram selecionados para participar deste estudo (14 homens e 7 mulheres, com idade entre 25 e 64 anos). Foi aplicado questionário para avaliar a fissura (craving) e o consumo alimentar. A alteração do peso corporal e do índice de massa corporal (IMC) foi estimada por métodos antropométricos para avaliar o estado nutricional. As avaliações foram realizadas no momento inicial e final, contemplando até três meses de tratamento. Resultados: As mulheres apresentaram ganho de peso (1,9 ± 1,86 kg) e os homens, perda de peso corporal (-0,13 ± 2,09 kg) (p = 0,04). Não houve diferença estatística quando essa variação de peso foi comparada entre os abstinentes (AB) e não abstinentes (NA) (Homens: AB = 0,39 ± 2,19 kg; NA: -1,06 ± 1,75 kg/Mulheres: AB: 2,73 ± 1,95 kg; NA: 1,42 ± 1,85 kg). A presença de fissura inicial e final foi semelhante entre os que recaíram e os abstinentes. Os abstinentes mantiveram menor fissura e maior sensação de bem-estar com o consumo de alimentos fontes de carboidratos simples ou complexos. Conclusão: Os alcoolistas que conseguiram se abster tiveram menor grau de fissura com maior bem-estar com o consumo de alimentos fontes de carboidratos. Houve mudanças do peso corporal ao longo do tempo de acompanhamento dos alcoolistas em tratamento para a abstinência.

Na gravidez, a mulher experiencia mudanças desenvolvimentais ao nível dos seus relacionamentos significativos. Este estudo compara a qualidade do relacionamento com o companheiro e com outra figura significativa em grávidas adolescentes e adultas, e analisa preditores sócio-demográficos para a qualidade destes relacionamentos. Uma amostra de 130 grávidas (66 adolescentes e 64 adultas) foi avaliada no terceiro trimestre de gestação quanto às características sociais e demográficas e à qualidade do relacionamento com figuras significativas. Os resultados mostram que as adolescentes referem menor confiança (χ2 = 3.365, p = 0,055) e maior discórdia (χ2 = 3.842, p = 0,041) no relacionamento com o companheiro e maior sentimento de ligação (χ2 = 19.126, p = 0,000) e apatia (χ2 = 8.568, p = 0,004) no relacionamento com a outra figura significativa, comparativamente com as adultas. Verifica-se ainda que a gravidez na adolescência associa-se a relacionamentos de menor qualidade, especialmente devido a situações sócio-demográficas mais desfavoráveis e não tanto à condição de ser ou não adolescente.

OBJETIVO: Análise comparativa dos resultados hospitalares em pacientes infartados reperfundidos por meio de uma intervenção coronariana percutânea de resgate ou primária. MÉTODOS: Selecionados pacientes consecutivos, submetidos a uma intervenção percutânea no infarto do miocárdio (< 24h), entre 1997 e 2000, incluídos no registro nacional multicêntrico CENIC, comparando os resultados hospitalares, entre aqueles submetidos ao procedimento de resgate (n=840) ou primário (n=8.531). RESULTADOS: Os pacientes que realizaram intervenção de resgate eram significativamente mais jovens, com infarto anterior, associado à presença de disfunção ventricular, porém com menor porcentual de doença coronariana multiarterial, comparados aos primários. As taxas de implante de stents foram similares (56,9% vs. 54,9%; p=0,283), mas o sucesso do procedimento foi menor nos casos de resgate (88,1% vs. 91,2%; p<0,001), cursando com maior mortalidade (7,4% vs. 5,6%; p=0,034), comparado à intervenção primária; nova revascularização (< 0,5%), cirurgia de emergência (< 0,3%) e reinfarto (< 2,6%) foram similares. A análise multivariada identificou o procedimento de resgate como preditor de mortalidade ao final da fase hospitalar [RC (IC=95%) = 1,60 (1,17-2,19); p=0,003]. CONCLUSÃO: Neste registro nacional, a intervenção de resgate apresentou uma maior mortalidade hospitalar, comparada ao procedimento primário.

Plantas de milheto forrageiro, em condições controladas, foram submetidas a tratamentos em soluções nutritivas completa e com emissão de N, P, K, Ca, Mg e S, tendo como objetivos avaliar a produção de matéria, a concentração dos nutrientes na planta, bem como estabelecer o quadro sintomatológico das carências nutricionais. As produções de matéria seca alcançaram os seguintes valores (g): completo = 55,8; -N = 2,1; -P = 2,8; -K = 9,5; -Ca = 41,9; -Mg = 35,7 e -S 53,4. Os valores analíticos encontrados nas folhas de plantas sadias e deficientes foram: N% - 3,21-2,07; P% - 0,38 - 0,09; K% - 2, 19-0,54; Ca% - 0,52-0,16; Mg% -0,48-0,04; S% - 0,24-0,12. Plantas deficientes em nitrogênio, fósforo, potássio e magnésio não produziram espigas. Foi obtido o quadro sintomatológico completo, das carências dos macronutrientes.

De uma bem conduzida plantação de rúcula (Euruca sativa var. Cultivada) foram coletadas plantas aos 20, 27, 34 e 41 dias de idade. As plantas foram lavadas e divididas em folhas e raízes. Postas a secar a 80°C e analisadas para N, P, K, Ca, Mg, S, B, Cu, Fe , Mn e Zn pelos métodos convencionais de laboratório. Observou-se que o crescimento, expresso em produção de matéria seca, é crescente até os 34 dias, estabilizando-se após este período. A análise química mostrou que a concentração de K e Ca é elevada atingindo 2,19% para este último elemento aos 41 dias de idade da planta. Para os micronutrientes chama atenção a elevada concentração de zinco nas folhas atingindo 229 ppm aos 41 dias de idade. Uma área cultivada de 10 m² extrai pelas folhas , que é parte exportada as seguintes quantidades de elementos: 20 g de N, 1,86 g de P, 22,1 g de K, 7,56 g de Ca , 1,77 g de Mg, 1,99 g de S, 17,7 mg de B, 7,8 mg de Cu , 475,6 mg de Fe, 14,3 mg de Mn e 37,7 mg de Zn.

OBJECTIVES: To estimate the prevalence of youth who use cannabis but have never been tobacco smokers and to assess the characteristics that differentiate them from those using both substances or neither substance. DESIGN: School survey. SETTING: Postmandatory schools. PARTICIPANTS: A total of 5263 students (2439 females) aged 16 to 20 years divided into cannabis-only smokers (n = 455), cannabis and tobacco smokers (n = 1703), and abstainers (n = 3105). OUTCOME MEASURES: Regular tobacco and cannabis use; and personal, family, academic, and substance use characteristics. RESULTS: Compared with those using both substances, cannabis-only youth were younger (adjusted odds ratio [AOR], 0.82) and more likely to be male (AOR, 2.19), to play sports (AOR, 1.64), to live with both parents (AOR, 1.33), to be students (AOR, 2.56), and to have good grades (AOR, 1.57) and less likely to have been drunk (AOR, 0.55), to have started using cannabis before the age of 15 years (AOR, 0.71), to have used cannabis more than once or twice in the previous month (AOR, 0.64), and to perceive their pubertal timing as early (AOR, 0.59). Compared with abstainers, they were more likely to be male (AOR, 2.10), to have a good relationship with friends (AOR, 1.62), to be sensation seeking (AOR, 1.32), and to practice sports (AOR, 1.37) and less likely to have a good relationship with their parents (AOR, 0.59). They were more likely to attend high school (AOR, 1.43), to skip class (AOR, 2.28), and to have been drunk (AOR, 2.54) or to have used illicit drugs (AOR, 2.28). CONCLUSIONS: Cannabis-only adolescents show better functioning than those who also use tobacco. Compared with abstainers, they are more socially driven and do not seem to have psychosocial problems at a higher rate.

Different embolic materials for portal vein embolization (PVE) were evaluated. Twenty pigs received left and median PVE. Hydrophilic phosphorylcholine, N-butyl cyanoacrylate, hydrophilic gel, and polyvinyl alcohol (PVA) particles measuring either 50-150 microm or 700-900 microm were used in five pigs each. Portography and portal vein pressure measurement were performed before, immediately after PVE, and before being euthanized at day 7. Tissue wedges from embolized, and non-embolized liver were obtained for pathology. After complete embolization, recanalization occurred at 7 days in one gel and one 700-900 PVA embolization. Post-PVE increase in portal pressure was found in all groups (p = 0.01). The area of the hepatic lobules in non-embolized liver was larger than in the embolized liver in all groups (p = 0.001). The ratios of the areas between non-embolized/embolized livers were 1.65, 2.19, 1.57, and 1.32 for gel, NBCA, 50-150 PVA and 700-900 PVA, respectively; the ratios of fibrosis between the embolized and non-embolized livers were 1.37, 3.01, 3.49, and 2.11 for gel, NBCA, 50-150 PVA and 700-900 PVA, respectively. Hepatic lobules in non-embolized liver were significantly larger with NBCA than in other groups (p = 0.01). Fibrosis in embolized liver was significantly higher for NBCA and 50-150 PVA (p = 0.002). The most severe changes in embolized and non-embolized liver were induced by 50-150 PVA and NCBA PVE.

Purpose: To assess the clinical outcome of patients who were subjected to long-axis sacroplasty as first line treatment for sacral insufficiency fractures. Methods and materials: Nineteen patients with unilateral (n = 3) or bilateral (n = 16) sacral fractures were involved. Under local anaesthesia, each patient was subjected to CT guided sacroplasty using the long-axis approach through a single entry point. An average of 6 ml of PMMA was delivered along the path of each sacral fracture. For each individual patient, the VAS pain score before sacroplasty and at 1, 4, 24, and 48 weeks after the procedure was obtained. Furthermore, the use of analgesics (narcotic/non-narcotic) along with the evolution of post interventional patient mobility before and after sacroplasty was also recorded. Results: The mean pre-procedure VAS score was 8 ± 1.9. This has rapidly declined in the first week after the procedure (mean 4 ± 1.5) followed by gradual decrease along the rest of follow-up period at 4 weeks (mean 3 ± 1.2), 24 weeks (mean 2 ± 1.3), and 48 weeks (mean 1.3 ± 1.4), respectively. Eleven (58%) patients were under narcotic analgesia before sacroplasty, whereas, 8 (42%) patients were using non-narcotics. Corresponding values after the procedure were 2/19 (10%) (narcotic) and 10/19 53% (non-narcotic). Seven (37%) patients did not address post-procedure analgesic use. The evolution of post interventional mobility was favourable in the study group since they revealed a significant improvement in their mobility point scale. Conclusion: Long-axis percutaneous sacroplasty is a suitable minimally invasive treatment option for patients who present with sacral insufficiency fractures. Future studies with larger patient number are warranted to grasp any potential limitations of this therapeutic approach.

The Alive N Kicking (ANK) programme is a child weight management programme. Its primary aim is to support and empower parents and children from 2-19 years old to adopt healthy eating practices, increase physical activity levels, reduce sedentary behaviour and build self esteem.A secondary aim is to contribute towards children participating in the programme improving their long term health through achieving and maintaining a health body weight. The objectives of the programme are:1.To increase the level of healthy eating practices amongst participating families. 2.To increase the level of daily physical activity of participating children.3.To increase levels of self-esteem amongst children participating in the programme. 4.To contribute towards the long-term aim of achieving and maintaining a healthy body weight for children and parents participating in the programme.5.To deliver a safe, high-quality service.

Purpose: To report the magnetic resonance imaging (MRI) findings in athletic injuries of the extensor carpi ulnaris (ECU) subsheath, assessing the utility of gadolinium-enhanced (Gd) fat-saturated (FS) T1-weighted sequences with wrist pronation and supination. Methods and Materials: Sixteen patients (13 males, 3 females; mean age 30.3 years) with athletic injuries of the ECU subsheath sustained between January 2003 and June 2009 were included in this retrospective study. Initial and follow‑up 1.5-T wrist MRIs were performed with transverse T1-weighted and STIR sequences in pronation, and Gd FS T1-weighted sequences with wrist pronation and supination. Two radiologists assessed the type of injury (A to C), ECU tendon stability, associated lesions and rated pulse sequences using a three-point scale: 1 = poor, 2 = good and 3 = excellent. Results: Gd-enhanced FS T1-weighted transverse sequences in supination (2.63) and pronation (2.56) were most valuable, compared with STIR (2.19) and T1 weighted (1.94). Nine type A, one type B and six type C injuries were found. There were trends towards diminution in size, signal intensity and enhancement of associated pouches on follow‑up MRI and tendon stabilisation within the ulnar groove. Conclusion: Gd-enhanced FS T1-weighted sequences with wrist pronation and supination are most valuable in assessing and follow‑up athletic injuries of the ECU subsheath on 1.5-T MRI.

Traumatic origin of disc herniation, remains as a relevant Medico-Legal problem. The present paper makes a disc protrusion/ herniation revision, mainly about mechanic and traumatic factors involved in their origin. A multifactorial (genetic, bio-antropometric, environnement, metabolic, etc.) causation is concluded to explain etiology of disc herniation. Pure disc herniation (occurring in a healthy non degenerated disc) is considered an exceptional phenomenon. Clinical and radiological spine degeneration sings are revised, along with others that can point to a traumatic origin. Dealing with a disc herniation after an accident, Medical Expert has always to consider the possibility of a precedent disc damage status, assessing the role of a worsening of such antecedent condition in each case. A classification of degree of causation accident-herniation, and antecedent disc damage decompensation are proposed.

BACKGROUND. Exposure to xenoestrogens during pregnancy may disturb the development and function of male sexual organs. OBJECTIVE. In this study we aimed to determine whether the combined effect of environmental estrogens measured as total effective xenoestrogen burden (TEXB) is a risk factor for male urogenital malformations. METHODS. In a case-control study, nested in a mother-child cohort (n = 702) established at Granada University Hospital, we compared 50 newborns with diagnosis of cryptorchidism and/or hypospadias with 114 boys without malformations matched by gestational age, date of birth, and parity. Controls did not differ from the total cohort in confounding variables. TEXB and levels of 16 organochlorine pesticides were measured in placenta tissues. Characteristics of parents, pregnancy, and birth were gathered by questionnaire. We used conditional and unconditional regression models to estimate odds ratios (ORs) and 95% confidence intervals (CIs). RESULTS. TEXB from organohalogenated compounds was detectable in 72% and 54% of case and control placentas, respectively. Compared with controls, cases had an OR for detectable versus non-detectable TEXB of 2.82 (95% CI, 1.10-7.24). More pesticides were detected in cases than in controls (9.34 +/- 3.19 vs. 6.97 +/- 3.93). ORs for cases with detectable levels of pesticides, after adjusting for potential confounders in the conditional regression analysis, were o,p'-DDT (OR = 2.25; 95% CI, 1.03-4.89), p,p'-DDT (OR = 2.63; 95% CI, 1.21-5.72), lindane (OR = 3.38; 95% CI, 1.36-8.38), mirex (OR = 2.85; 95% CI, 1.22-6.66), and endosulfan alpha (OR = 2.19; 95% CI, 0.99-4.82). Engagement of mothers in agriculture (OR = 3.47; 95% CI, 1.33-9.03), fathers' occupational exposure to xenoestrogens (OR = 2.98; 95% CI, 1.11-8.01), and history of previous stillbirths (OR = 4.20; 95% CI, 1.11-16.66) were also associated with risk of malformations. CONCLUSIONS We found an increased risk for male urogenital malformations related to the combined effect of environmental estrogens in placenta.

BACKGROUND: This study aimed to investigate the influence of deep sternal wound infection on long-term survival following cardiac surgery. MATERIAL AND METHODS: In our institutional database we retrospectively evaluated medical records of 4732 adult patients who received open-heart surgery from January 1995 through December 2005. The predictive factors for DSWI were determined using logistic regression analysis. Then, each patient with deep sternal wound infection (DSWI) was matched with 2 controls without DSWI, according to the risk factors identified previously. After checking balance resulting from matching, short-term mortality was compared between groups using a paired test, and long-term survival was compared using Kaplan-Meier analysis and a Cox proportional hazard model. RESULTS: Overall, 4732 records were analyzed. The mean age of the investigated population was 69.3±12.8 years. DSWI occurred in 74 (1.56%) patients. Significant independent predictive factors for deep sternal infections were active smoking (OR 2.19, CI95 1.35-3.53, p=0.001), obesity (OR 1.96, CI95 1.20-3.21, p=0.007), and insulin-dependent diabetes mellitus (OR 2.09, CI95 1.05-10.06, p=0.016). Mean follow-up in the matched set was 125 months, IQR 99-162. After matching, in-hospital mortality was higher in the DSWI group (8.1% vs. 2.7% p=0.03), but DSWI was not an independent predictor of long-term survival (adjusted HR 1.5, CI95 0.7-3.2, p=0.33). CONCLUSIONS: The results presented in this report clearly show that post-sternotomy deep wound infection does not influence long-term survival in an adult general cardio-surgical patient population.

Purpose: To report the diffusion-weighted MR imaging (DWI) findings in hepatic alveolar echinococcosis (AE). To evaluate the usefulness of apparent diffusion coefficients (ADCs) for differentiating the 5 types of AE lesions (as reported by Kodama, Radiology, 2003).Methods and Materials: We retrospectively included 17 patients (10 women, mean age 64.3years) with 48 AE liver lesions (>1cm2) that had been investigated by 3-Tesla MR imaging between March 2008 and August 2011 performing our standard protocol including DWI (b-values: 0, 300 and 600s/mm2). In consensus, two radiologists assessed lesion characteristics such as diameter, cystic and/or fibrotic components including Kodama classification, signal intensity, contrast enhancement, calcifications (on CT), and measured the ADC of each lesion. AE was confirmed by serology, biopsy and/or surgery in all patients.Results: Seventeen lesions of Kodama type 1, 10 of type 2, 19 of type 3, 1 of type 4 and 1 of type 5 were found. Mean(±SD) ADC of all AE lesions was 1.75±0.45 ×10-3mm2/s. Mean(±SD) ADCs of Kodama type 1, 2, 3, 4 and 5 lesions were 1.74±0.55, 1.71±0.49, 1.82±0.36, 1.46±0 and 1.43±0 ×10-3mm2/s, respectively. No significant difference was noted between the different Kodama types (p=0.89). Presence of fibrotic (p=0.24) and/or calcified (p=0.90) components, or contrast enhancement (p=0.84) of AE lesions were not correlated with significant differences in ADCs.Conclusion: ADCs of AE lesions are relatively low compared to other cystic liver lesions, which is helpful in suggesting the diagnosis. However, ADCs were not found to be useful for differentiating Kodama types of AE lesions.

The prevalence of occult hepatitis B virus (HBV) infection was investigated in 149 hepatitis B surface antigen (HBsAg) negative injecting drug users (IDUs) in the Central-West Region of Brazil. Of these individuals, 19 were positive for HBV DNA, resulting in an occult HBV infection prevalence of 12.7% (19/149); six of these 19 individuals had anti-HBV core and/or anti-HBV surface antibodies and 13 were negative for HBV markers. All IDUs with occult hepatitis B reported sexual and/or parenteral risk behaviours. All HBV DNA-positive samples were successfully genotyped. Genotype D was the most common (17/19), followed by genotype A (2/19). These findings reveal a high prevalence of occult HBV infection and the predominance of genotype D among IDUs in Brazil's Central-West Region.
