In contrast to the impressive advances in somatic research of erectile dysfunction (ED), scientific literature shows contradictory reports on the results of psychotherapy for the treatment of ED. Authors conducted a meta-analysis to evaluate the effectiveness of psychological interventions for the treatment of ED compared to oral drugs, local injection, vacuum devices, or other psychological intervention. Distinct sources of randomized controlled trials (RCTs) were searched: electronic databases (between 1966 and 2007), cross checking of references, and contact with scientific societies. For dichotomous outcomes the pooled relative risks were calculated and for continuous outcomes mean differences between interventions. Statistical heterogeneity was addressed. Eleven RCTs involving 398 men met the inclusion criteria. There is evidence that group therapy improves ED. Focused sex group therapy showed greater efficacy than control group. Men randomized to receive psychotherapy plus sildenafil showed significant improvement of ED and were less likely than those receiving only sildenafil to drop out. Regarding to the effectiveness of psychological interventions for the treatment of ED compared to local injection and vacuum devices no difference was found. Melnik T, Soares BGO, and Nasello AG. Introduction. Physical exercise including pelvic floor muscle (PFM) training seems to improve the sexual function of women with urinary incontinence. This effect in postmenopausal women who are continent has not yet been determined. Aim. The aim of this study was to assess the effect of a 3-month physical exercise protocol (PEP) on the sexual function and mood of postmenopausal women. Methods. Thirty-two sedentary, continent, sexually active women who had undergone menopause no more than 5 years earlier and who had follicle stimulating hormone levels of at least 40 mIU/mL were enrolled into this longitudinal study. All women had the ability to contract their PFMs, as assessed by vaginal bimanual palpation. Muscle strength was graded according to the Oxford Modified Grading Scale (OMGS). A PEP was performed under the guidance of a physiotherapist (M. M. F.) twice weekly for 3 months and at home three times per week. All women completed the Sexual Quotient-Female Version (SQ-F) and the Hospital Anxiety and Depression Scale (HADS) before and after the PEP. Main Outcome Measures. SQ-F to assess sexual function, HASDS to assess mood, and OMGS to grade pelvic floor muscle strength. Results. Thirty-two women (24 married women, eight women in consensual unions) completed the PEP. Following the PEP, there was a significant increase in OMGS score (2.59 +/- 1.24 vs. 3.40 +/- 1.32, P < 0.0001) and a significant decrease in the number of women suffering from anxiety (P < 0.01), but there was no effect on sexual function. Conclusion. Implementation of our PEP seemed to reduce anxiety and improve pelvic floor muscular strength in sedentary and continent postmenopausal women. However, our PEP did not improve sexual function. Uncontrolled variables, such as participation in a long-term relationship and menopause status, may have affected our results. Erectile dysfunction (ED) and urinary incontinence are common complications following radical prostatectomy (RP). Although pelvic-floor biofeedback training (PFBT) may improve urinary continence following RP, its effects on the recovery of potency are unknown. Fifty-two patients selected for RP were prospectively randomized for a treatment group (n=26) receiving PFBT once a week for 3 months and home exercises or a control group (n=26), in which patients received verbal instructions to contract the pelvic floor. Erectile function (EF) was evaluated with the International Index of Erectile Function-5 (IIEF-5) before surgery and 1, 3, 6 and 12 months postoperatively. Patients were considered potent when they had a total IIEF-5 score >20. Continence status was assessed and defined as the use of no pads. Groups were comparable in terms of age, body mass index, diabetes, pathological tumor stage and neurovascular bundle preservation. A significant reduction in IIEF-5 scores was observed after surgery in both groups. In the treatment group, 8 (47.1%) patients recovered potency 12 months postoperatively, as opposed to 2 (12.5%) in the control group (P=0.032). The absolute risk reduction was 34.6% (95% confidence interval (CI): 3.8-64%) and the number needed to treat was 3 (95% CI: 1.5-17.2). A strong association between recovery of potency and urinary continence was observed, with continent patients having a 5.4 higher chance of being potent (P=0.04). Early PFBT appears to have a significant impact on the recovery of EF after RP. Urinary continence status was a good indicator of EF recovery, with continent patients having a higher chance of being potent.

Introduction and Objective: Because of the improvements on detection of early stage prostate cancer over the last decade, focal therapy for localized prostate cancer (PC) has been proposed for patients with low-risk disease. Such treatment would allow the control of cancer, thereby diminishing side effects, such as urinary incontinence and sexual dysfunction, which have an enormous impact on quality of life. The critical issue is whether it is possible to preoperatively predict clinically significant unifocal or unilateral prostate cancer with sufficient accuracy. Our aim is to determine whether there is any preoperative feature that can help select the ideal patient for focal therapy. Material and methods: A total of 599 patients who underwent transrectal ultrasound, (TRUS)-guided prostate biopsy followed by radical prostatectomy to treat PC were examined in our laboratory between 2001 and 2009. We established very restricted criteria to select patients with very-low-risk disease for whom focal therapy would be suitable (only I biopsy core positive, tumor no larger than 80% of a single core, no perineural invasion, PSA serum level < 10 ng/ml, Gleason score < 7 and clinical stage T1c, T2a-b). We defined 2 groups of patients who would be either adequately treated or not treated by focal therapy. The primary endpoint was the evaluation of preoperative features in order to identify which parameters should be considered when choosing good candidates for focal therapy. Results: Fifty-six out of 599 patients met our criteria. The mean age was 59 years, and the mean number of biopsy cores was 14.4. Forty-seven (83.9%) were staged T1c, and 9 (16.1%) were staged T2a-b. Forty-four (78.6%) patients could be considered to have been adequately treated by focal therapy, and 12 (21.4%) could not. There was no statistical difference between the 2 groups considering age, clinical stage, PSA levels, Gleason score, and tumor volume in the biopsy. All 12 patients who could be considered inadequately treated had a bilateral, significant secondary tumor, 58.3% had Gleason >= 7, and 25% were staged pT3. Conclusion: Although focal therapy might be a good option for patients with localized prostate cancer, we are so far unable to select which of them would benefit from it based on preoperative data, even using very restricted criteria, and a considerable proportion of men would still be left undertreated. CONTEXT Most patients with neuro-urological disorders require life-long medical care. The European Association of Urology (EAU) regularly updates guidelines for the diagnosis and treatment of these patients. OBJECTIVE To provide a summary of the 2015 updated EAU Guidelines on Neuro-Urology. EVIDENCE ACQUISITION Structured literature searches in several databases were carried out to update the 2014 guidelines. Levels of evidence and grades of recommendation were assigned where possible. EVIDENCE SYNTHESIS Neurological disorders often cause urinary tract, sexual, and bowel dysfunction. Most neuro-urological patients need life-long care for optimal life expectancy and quality of life. Timely diagnosis and treatment are essential to prevent upper and lower urinary tract deterioration. Clinical assessment should be comprehensive and usually includes a urodynamic investigation. The neuro-urological management must be tailored to the needs of the individual patient and may require a multidisciplinary approach. Sexuality and fertility issues should not be ignored. Numerous conservative and noninvasive possibilities of management are available and should be considered before a surgical approach is chosen. Neuro-urological patients require life-long follow-up and particular attention has to be paid to this aspect of management. CONCLUSIONS The current EAU Guidelines on Neuro-Urology provide an up-to-date overview of the available evidence for adequate diagnosis, treatment, and follow-up of neuro-urological patients. PATIENT SUMMARY Patients with a neurological disorder often suffer from urinary tract, sexual, and bowel dysfunction and life-long care is usually necessary. The update of the EAU Guidelines on Neuro-Urology, summarized in this paper, enables caregivers to provide optimal support to neuro-urological patients. Conservative, noninvasive, or minimally invasive approaches are often possible.

Introducción: Las esferas vaginales también son conocidas como bolas chinas, bolas vaginales, bolas del amor, bolas de geisha, Rin no tama, bolas Ben Wa. Ayudan a fortalecer el suelo pélvico. Estas producen una microvibración al moverse la bola del interior, favoreciendo la musculatura del suelo pélvico y aumentando el riego sanguíneo a nivel de la pelvis. Hay esferas de diferente peso, par su indicación el suelo pélvico debe estar tónico. Objetivo: Revisar la literatura sobre la efectividad de las esferas vaginales e incrementar los conocimientos de los profesionales sobre este tipo de tratamiento propioceptivo, que resulta eficaz para muchas mujeres por su fácil utilización. Metodología: Búsqueda bibliográfica en las bases de datos: Cochrane Plus, Medline, Gerion, Scielo, Cuiden, Biblioteca de Salud Reproductiva de la OMS y google académico. Resultados: Las esferas vaginales son eficaces en el tratamiento de la incontinencia urinaria y fecal y para el entrenamiento del suelo pélvico. La incontinencia urinaria es un problema frecuente y molesto, que puede llegar a interferir en el trabajo, en la vida social y sexual. Hay necesidad de más ensayos para evaluar los métodos y protocolos de entrenamiento adecuados para la prevención de la incontinencia, del prolapso y de la disfunción sexual. Conclusión: Se ha evidenciado que las esferas vaginales son eficaces en el tratamiento de mujeres con incontinencia urinaria de esfuerzo e igualmente efectivas, en el entrenamiento muscular del suelo pélvico.

Introduction Female sexual functioning is affected by a range of factors including motivation, psychological well-being, and relationship issues. In understanding female sexual dysfunction (FSD), there has been a tendency to privilege diagnostic and medical over relationship issues. Aim To investigate the association between women’s experience of intimacy in close relationships - operationalized in terms of attachment and degree of differentiation of self - and FSD. Methods Two hundred and thirty sexually active Australian women responded to an invitation to complete a set of validated scales to assess potential correlates of sexual functioning. Main Outcome Measures The Female Sexuality Function Index, the Experiences in Close Relationships Scale, the Differentiation of Self Inventory, as well as a set of study-specific questions were subject to hierarchical multiple regression analyses Results Relational variables of attachment avoidance and to a lesser degree, attachment anxiety were associated with FSD. Participants with lower levels of differentiation of self were more likely to report sexual difficulties. The inability to maintain a sense of self in the presence of intimate others was the strongest predictors of sexual problems. A history of sexual abuse in adulthood and higher levels of psychological distress were also associated with sexual difficulties. Conclusions The findings provide support for a relational understanding of female sexual functioning. Attachment avoidance, attachment anxiety, and degree of differentiation of self are shown to be associated with sexual difficulties. The findings support the need to focus on relational and psychological factors in women’s experience of sex.

Após o sucesso de vendas do Viagra, medicamento indicado para o tratamento da disfunção erétil, lançado em 1998, houve uma rápida proliferação de artigos, livros e encontros sobre as disfunções sexuais femininas. Desde 2000, um intenso debate sobre o envolvimento da indústria farmacêutica na produção biomédica sobre as disfunções sexuais femininas e a concomitante busca por um medicamento similar ao Viagra destinado às mulheres tem envolvido profissionais de diferentes disciplinas. Esta dissertação teve como objetivo investigar os discursos científicos sobre as disfunções sexuais femininas, através do exame dos artigos publicados no periódico Archives of Sexual Behavior, desde sua fundação, em 1971, até 2007. O periódico foi escolhido por sua legitimidade neste campo de saberes, por abranger um amplo período (36 anos) e seu caráter multidisciplinar. Pretendeu-se investigar quando, como e por quais grupos profissionais as disfunções sexuais femininas foram descritas e abordadas no periódico. No caso das chamadas disfunções sexuais, as descrições científicas, que vêm aumentando significativamente nos últimos anos, dão origem a prescrições de terapias, medicamentos, intervenções cirúrgicas, programas de educação sexual e políticas públicas. Ou seja, subjacente a esse discurso, que afirma ser empírico e imparcial, estão processos que se encontram muito além dos limites de um laboratório ou das atividades de um pesquisador. Buscou-se, assim, pensar a produção científica como produto de articulações e negociações que se desenrolam em esferas diversas, envolvendo processos culturais, sociais, econômicos e também cognitivos ou científicos, em contraposição às concepções que caracterizam a ciência como um projeto que apenas revela verdades. Para tanto, foi apresentado o contexto do surgimento de uma ciência da sexualidade, no decorrer do século XIX e, em seguida, o contexto no qual emergiram os discursos sobre as disfunções sexuais femininas, o que propiciou sua emergência naquele dado momento, o modo como foram definidas e por quem, como se articularam a processos sociais, econômicos e culturais e que transformações sofreram ao longo dos anos.

A Neuropatia autonômica cardiovascular (NAC), apesar de ter sido apontada como fator de risco independente para doença cardiovascular (DCV) em pacientes com diabetes tipo 1 (DM1), permanece subdiagnosticada. Os objetivos do trababalho foram determinar a prevalência de NAC e seus indicadores clínicos e laboratoriais em pacientes com DM1 e a associação com outras complicações crônicas do diabetes, além de avaliar a concordância entre os critérios diagnósticos da NAC determinados pelos parâmetros da análise espectral e pelos testes reflexos cardiovasculares. Pacientes com DM1, duração da doença ≥ 5 anos e com idade ≥ 13 anos foram submetidos a um questionário clínico-epidemiológico, a coleta de sangue e de urina para determinação da concentração urinária de albumina, ao mapeamento de retina, e exame clínico para pesquisa de neuropatia diabética sensitivo motora além da realização de testes reflexos cardiovasculares. Cento e cinquenta e um pacientes com DM1, 53.6 % do sexo feminino, 45.7% brancos, com média de idade de 33.4 13 anos, idade ao diagnóstico de 17.2 9.8 anos, duração de DM1 de 16.3 9.5 anos, índice de massa corporal (IMC) de 23.4 (13.7-37.9) Kg/m2 e níveis de hemoglobina glicada de 9.1 2% foram avaliados. Após realização dos testes para rastreamento das complicações microvasculares, encontramos neuropatia diabética sensitivo motora, retinopatia diabética, nefropatia diabética e NAC em 44 (29.1%), 54 (38%), 35 (24.1%) e 46 (30.5%) dos pacientes avaliados, respectivamente. A presença de NAC foi associada com idade (p=0.01), duração do DM (p=0.036), HAS (p=0.001), frequência cardíaca em repouso (p=0.000), HbA1c (p=0.048), uréia (p=0.000), creatinina (p=0.008), taxa de filtração glomerular (p=0.000), concentração urinária de albumina (p=0.000), níveis séricos de LDL-colesterol (p=0.048), T4 livre (p=0.023) e hemoglobina (p=0.01) e a presença de retinopatia (p=0.000), nefropatia (p=0.000) e neuropatia diabética sensitivo motora (p=0.000), além dos seguintes sintomas; lipotimia (p=0.000), náuseas pós alimentares (p=0.042), saciedade precoce (p=0.031), disfunção sexual (p=0.049) e sudorese gustatória (p=0.018). No modelo de regressão logística binária, avaliando o diagnóstico de NAC como variável dependente, foi observado que apenas a FC em repouso, presença de neuropatia diabética sensitivo motora e retinopatia diabética foram consideradas variáveis independentes significativamente. A NAC é uma complicação crônica comum do DM1, atingindo cerca de 30% dos pacientes estudados e encontra-se associada à presença de outras complicações da doença. Indicadores da presença de NAC nos pacientes avaliados incluíram a idade, duração do diabetes, presença de HAS, frequência cardíaca de repouso e presença de sintomas sugestivos de neuropatia autonômica. O presente estudo ratifica a importância do rastreamento sistemático e precoce desta complicação.

OBJECTIVE: We tested the hypothesis that intraventricular hemorrhage (IVH) is associated with incontinence and gait disturbance among survivors of intracerebral hemorrhage (ICH) at 3-month follow-ups. METHODS: The Genetic and Environmental Risk Factors for Hemorrhagic Stroke study was used as the discovery set. The Ethnic/Racial Variations of Intracerebral Hemorrhage study served as a replication set. Both studies performed prospective hot-pursuit recruitment of ICH cases with 3-month follow-up. Multivariable logistic regression analyses were computed to identify risk factors for incontinence and gait dysmobility at 3 months after ICH. RESULTS: The study population consisted of 307 ICH cases in the discovery set and 1,374 cases in the replication set. In the discovery set, we found that increasing IVH volume was associated with incontinence (odds ratio [OR] 1.50; 95% confidence interval [CI] 1.10-2.06) and dysmobility (OR 1.58; 95% CI 1.17-2.15) after controlling for ICH location, initial ICH volume, age, baseline modified Rankin Scale score, sex, and admission Glasgow Coma Scale score. In the replication set, increasing IVH volume was also associated with both incontinence (OR 1.42; 95% CI 1.27-1.60) and dysmobility (OR 1.40; 95% CI 1.24-1.57) after controlling for the same variables. CONCLUSION: ICH subjects with IVH extension are at an increased risk for developing incontinence and dysmobility after controlling for factors associated with severity and disability. This finding suggests a potential target to prevent or treat long-term disability after ICH with IVH.

Treatment of urinary incontinence with the artificial urinary sphincter has been available in centres such as London and Liverpool for a number of years. This service is now available in the department of urology of the Belfast City Hospital. Twelve patients have had successful implantation of an artificial urinary sphincter for urinary incontinence, and ten are now fully continent. One patient with Wegener's granulomatosis developed active disease in his urethra which has precluded activation of the device. One patient has had the device removed because of erosion into the urethra.

OBJECTIVE: To document prostate cancer patient reported 'ever experienced' and 'current' prevalence of disease specific physical symptoms stratified by primary treatment received.
PATIENTS: 3,348 prostate cancer survivors 2-15 years post diagnosis.
METHODS: Cross-sectional, postal survey of 6,559 survivors diagnosed 2-15 years ago with primary, invasive PCa (ICD10-C61) identified via national, population based cancer registries in Northern Ireland and Republic of Ireland. Questions included symptoms at diagnosis, primary treatments and physical symptoms (impotence/urinary incontinence/bowel problems/breast changes/loss of libido/hot flashes/fatigue) experienced 'ever' and at questionnaire completion ("current"). Symptom proportions were weighted by age, country and time since diagnosis. Bonferroni corrections were applied for multiple comparisons.
RESULTS: Adjusted response rate 54%; 75% reported at least one 'current' physical symptom ('ever':90%), with 29% reporting at least three. Prevalence varied by treatment; overall 57% reported current impotence; this was highest following radical prostatectomy (RP)76% followed by external beam radiotherapy with concurrent hormone therapy (HT); 64%. Urinary incontinence (overall 'current' 16%) was highest following RP ('current'28%, 'ever'70%). While 42% of brachytherapy patients reported no 'current' symptoms; 43% reported 'current' impotence and 8% 'current' incontinence. 'Current' hot flashes (41%), breast changes (18%) and fatigue (28%) were reported more often by patients on HT.
CONCLUSION: Symptoms following prostate cancer are common, often multiple, persist long-term and vary by treatment. They represent a significant health burden. An estimated 1.6% of men over 45 is a prostate cancer survivor currently experiencing an adverse physical symptom. Recognition and treatment of physical symptoms should be prioritised in patient follow-up. This information should facilitate men and clinicians when deciding about treatment as differences in survival between radical treatments is minimal.

A dor é uma experiência perceptualmente complexa, influenciada por um conjunto variado de fatores biológicos e também psicossociais. A sua vivência varia de pessoa para pessoa, havendo diferentes níveis de impacto no funcionamento emocional, interpessoal, motivacional e físico. A dor sexual, mais conhecida por dispareunia e vaginismo, é uma problemática de natureza habitualmente crónica que afeta muitas mulheres. Apesar de ser um importante alvo de estudo nas últimas décadas, e apesar do impacto que tem nas vidas de muitas mulheres, é ainda uma temática pouco abordada junto dos profissionais de saúde, sendo igualmente difícil a determinação da sua causa e respetivo tratamento. A sua concetualização tem sido um dos principais alvos de discussão entre investigadores e clínicos, havendo quem defenda que a mesma deve ser considerada, ou como uma perturbação de dor, ou como uma disfunção sexual. Contudo, mesmo com um crescimento significativo da literatura, não existem ainda dados que clarifiquem o papel que determinadas variáveis psicossociais exercem no desenvolvimento e manutenção da dor sexual e que forma estas aproximam, ou distanciam, este quadro clínico da dor crónica e de outras disfunções sexuais. Neste contexto, o objetivo do presente estudo consistiu em avaliar a influência do Mindfulness, do afeto-traço, dos pensamentos automáticos, das crenças sexuais, da perceção, vigilância e catastrofização face à dor, da perceção da resposta do outro significativo à dor, da autoestima, da autoestima sexual, do ajustamento diádico e do funcionamento sexual em mulheres com dor sexual, comparando-as com três grupos específicos: mulheres com dor crónica, mulheres com outras dificuldades sexuais e mulheres da população geral, sem nenhuma destas dificuldades. Por outro lado, foi avaliada a capacidade preditiva de cada uma destas variáveis psicossociais na intensidade da dor em mulheres que sofrem de dor sexual e dor crónica. Um total de 1233 mulheres colaboraram no presente estudo: 371 mulheres com dor sexual, 245 mulheres com dor crónica, 94 mulheres com disfunção sexual e 523 mulheres da população geral. As participantes responderam a um conjunto de questionários que foram disponibilizados através de um link online e que avaliaram cada uma das dimensões em estudo. Os resultados mostraram que as mulheres com dor sexual e disfunção sexual apresentaram uma menor capacidade para ser mindful, mais pensamentos automáticos negativos de fracasso/desistência, uma maior escassez de pensamentos eróticos, uma menor autoestima e autoestima sexual e uma menor qualidade do ajustamento diádico e funcionamento sexual, quando comparadas com as mulheres com dor crónica e da população geral. Por outro lado, as mulheres com dor sexual e dor crónica apresentaram maiores níveis de perceção, vigilância e catastrofização face à dor, quando comparadas com as mulheres com disfunção sexual e da população geral. Ao nível da perceção da reposta do outro significativo, as mulheres com dor sexual apresentaram significativamente uma menor perceção de respostas solícitas que as mulheres com dor crónica e da população geral. Não foram encontradas diferenças entre os grupos ao nível do afeto-traço e crenças sexuais disfuncionais. No que diz respeito à intensidade da dor nas mulheres com dor sexual, emergiram como preditores significativos os pensamentos de fracasso, as crenças sexuais de desejo sexual como pecado, a magnificação e o desânimo face à dor, a atenção à dor, a perceção de resposta de punição do outro significativo, o ajustamento diádico, a autoestima e a autoestima sexual. Em relação ao grupo com dor crónica, surgiram como preditores significativos o afeto negativo, o desânimo face à dor, a atenção à dor e a perceção de resposta de punição do outro significativo. Uma análise conjunta de todos estes preditores para cada um dos grupos, demonstrou que a perceção da resposta de punição da parte de outro significativo se constituiu como o melhor preditor da intensidade da dor nas mulheres com dor sexual, enquanto que o desânimo face à dor se mostrou como o mais significativo nas mulheres com dor crónica. De uma forma geral, os resultados demonstraram a importância das diferentes variáveis psicossociais na vivência da dor sexual e na respetiva intensidade da dor. Revelaram ainda que a dor sexual apresenta aspetos em comum, quer com a dor crónica, principalmente ao nível da relação com a dor, quer com outras disfunções sexuais, nomeadamente em termos cognitivos e relacionais. O presente estudo vem assim reforçar a ideia de que este é um quadro clínico multidimensional e complexo, trazendo consigo importantes implicações ao nível da sua concetualização, avaliação e tratamento.
