Denervation rénale endovascu- laire par radiofréquence: perspective d'un traitement prometteur de l'hypertension artérielle [Renal sympathetic denervation: perspective of a promising treatment for hypertension].

The crucial role of the sympathetic nervous system activity in the initiation and maintenance of hypertension was already in mind in the 1920s when surgical options were proposed to severely hypertensive patients. Despite constant evolution of pharmacological treatments, one estimates that 15-30% of hypertensive patients are still not well controlled and present resistant hypertension. The development of a new endovascular catheter used for selective sympathetic renal denervation by radiofrequency offers new perspectives of treatment. Encouraged by the recent results of the first clinical trials in a targeted population, this procedure could be used in some more indications in the future. However, long term morbidity and mortality of this technique are still not known.

Place de ta vitamine D, du sevelamer et du cinacalcet dans la prise en charge des patients en insuffisance rénale [Treatment of secondary hyperparathyroidism in renal insufficiency: role of calcitriol, sevelamer and cinacalcet]

Along with the decrease in kidney function arises a secondary hyperparathyroidism, which constitutes one of the most important risk factor for mortality in patients suffering from renal insufficiency. Treating secondary hyperparathyroidism is challenging, as most of the parameters of mineral metabolism are interconnected. We review here the pathophysiology and treatment options of this entity.

Limites des procédés d'investigation de la fonction érectile dans le cadre d'expertises médicolégales [Limits of the processes of investigation of the erectile function in forensic expertises].

Résumé La responsabilité d'un expert chargé d'apprécier la capacité érectile d'un prévenu est importante, car la peine infligée par le tribunal peut dépendre de ses conclusions. Sa tâche est en plus ardue car les procédés d'investigations de la fonction érectile ont tous des limites. Malgré toutes ces limites qui sont décrites dans cet article, l'expert peut aider à la recherche de la vérité comme les auteurs le démontrent. Summary The responsability of the expert in charge to appreciate the erectile capacity of an accused is considerable, as the sentence inflicted by the Tribunal could depend directly on his conclusions. His duty is harder as all the investigation procedures of the erectile functions have limits. Despite these limits, described in this article, the expert can help to discover the truth, as demonstrated by the authors.

Déterminants précoces de la mortalité du patient âgé gravement brûlé

Introduction : Avec le vieillissement de la population, le nombre de patients brûlés âgés a augmenté. La prise en charge médicale et chirurgicale globale des brûlés s'est nettement améliorée mais demeure difficile dans cette population vulnérable. Il manque des facteurs prédictifs précoces pour prédire la mortalité de ces patients. Cette étude a pour objectif d'essayer d'en identifier certains. Méthodes : Etude descriptive rétrospective de données collectées prospectivement dans un système informatisé (MetaVision®) sur une période qui s'étend de janvier 2001 à décembre 2010: 53 variables sont colligées et soumises à des analyses univariées en fonction du devenir chez des patients âgés de plus de 50ans. Résultats : 101 patients sur 363 admissions pendant la même période ont été étudiés : ils sont âgés de 66.6 ± 11.9 ans, brûlés sur 21.5 ± 14.9 % surface corporelle (BSA), et 16 sont décédés (15.8%). Vingt variables sont statistiquement significativement associées avec un décès : BSA, % de brûlure chirurgicale, score de BAUX, BAUX modifié, abbreviated burn severity index (ABSI), SAPS II, insuffisance rénale aiguë et chronique, cardiopathie ischémique, pression intra-abdominale élevée (y.c. syndrome du compartiment abdominal), bilan liquidien cumulé à J5 et à J10, sur-réanimation liquidienne, usage de plasma frais congelé, albuminémie, CRP, bicarbonates, créatinine et nutrition entérale. Conclusion : Plusieurs variables cliniques , certains déjà identifiés, mais également plusieurs nouveaux liés à la réponse physiologique du patient. Une étude ont été identifiées comme étant associés à un pronostic défavorable des patients brûlés âgés incluant un nombre plus élevé de patients permettrait de faire des analyses multivariées et de dégager des facteurs prédictifs combinés.

The interaction beween the meaning of its information and the information reciprocally contained in its signification, within a specified context and pertinence, implies a strong level of redundancy shared by producers and users of the map, a same system of scientific and ideological values to pass on the cartographic message. We are far from the objective cartographer who would fade away so to let the facts speaks "objectively" by themselves.

Protéinurie orthostatique et phénomène de compression de la veine rénale gauche (nutcracker syndrome) [Orthostatic proteinuria and compression of the left renal vein (nutcracker syndrome)]

INTRODUCTION: The pathogenic mechanism of orthostatic proteinuria has not yet been clearly established. OBSERVATION: In a tall, thin, 21 year-old man, isolated proteinuria was discovered during an urological control conducted one year after a bilateral orchidopexy following left testicular torsion. Proteinuria was orthostatic. Doppler examination of the kidney revealed an entrapment of the left renal vein (nutcracker phenomenon-NCP). COMMENTS: An NCP was diagnosed in a young patient presenting with orthostatic proteinuria. By provoking modifications in intraglomerular haemodynamics, the NCP may, in nearly half of the cases, be at the origin of orthostatic proteinuria. Doppler examination is the diagnostic method of choice in the screening for NCP.

Dénervation rénale pour le traitement de l'hypertension artérielle résistante: proposition pour une attitude multidisciplinaire commune [Renal denervation in resistant hypertension: proposal for a common multidisciplinary attitude].

The prevalence of resistant hypertension ranges between 5-30%. Patients with resistant hypertension are at increased risk of cardiovascular events. Radiofrequency renal denervation is a recent and promising technique that can be used in the setting of resistant hypertension. However, long-term safety and efficacy data are lacking and evidence to use this procedure outside the strict setting of resistant hypertension is missing. The aim of the article is to propose a common work-up for nephrologists, hypertensiologists, cardiologists and interventional radiologists in order to avoid inappropriate selection of patients and a possible misuse of this procedure.
