Digoxin remains one of the most commonly prescribed of all cardiac medications. The main indications for digoxin usage include atrial fibrillation and heart failure; both these conditions are more prevalent in older patients. Given the aging population and the increasing incidence of heart failure we would expect prescribing of digoxin to remain as frequent or to even increase in older patients. Older patients are also more likely to develop toxicity and diagnosis of digoxin toxicity can be difficult in this group. Numerous components contribute to the development of toxicity in older patients, ranging from aging-related changes in renal function or body mass to polypharmacy and possible interactions with digoxin. It is therefore important to understand how the pharmacokinetics of digoxin may be altered in the older population. Application of basic pharmacological principles may be helpful in anticipating these problems. This review describes the pharmacokinetics of digoxin, the changes in pharmacokinetics with increasing age and how concomitant disease states or drug interactions may affect the pharmacokinetics of digoxin. Greater knowledge about the causes and prevention of digoxin toxicity should further reduce the morbidity and mortality arising from digoxin toxicity, especially in the elderly population.
For over 200 years debate has raged regarding the use of digitalis glycosides in cardiac disease. At present digoxin is the most commonly prescribed digitalis compound. This review describes the pharmacokinetics of digoxin and in particular how they are altered with increasing age. When considering the elderly population it is important to recognise the heterogeneity of response in this group, therefore there are no rules, with regards to prescribing, that can apply to the entire elderly population.

Virgin olive oil is a high quality natural product obtained only by physical means. In addition to triacylglycerols it contains nutritionally important polar and non-polar antioxidant phenols and other bioactive ingredients. The polar fraction is a complex mixture of phenolic acids, simple phenols, derivatives of the glycosides oleuropein and ligstroside, lignans, and flavonoids. These compounds contribute significantly to the stability, flavor, and biological value of virgin olive. In the various stages of production, during storage and in the culinary uses, polar phenols and other valuable bioactive ingredients may be damaged. Oxidation, photo-oxidation, enzymic hydrolysis and heating at frying temperatures have a serious adverse effect. Due to the biological importance of the oil and its unique character, analytical methods have been developed to evaluate antioxidant activity or analyse complex phenol mixtures. These are based on radical scavenging assays and chromatographic techniques. Hyphenated methods are also used including liquid chromatography-mass spectrometry and liquid chromatography-nuclear magnetic resonance spectroscopy.

A new lupane type triterpenic acid, pulsatillic acid, and two new lupane type triterpenoid glycosides, pulsatilloside A and B, along with the known 23-hydroxybetulinic acid were isolated from the roots of Pulsatilla chinensis. Their structures were characterized as 3-oxo-23-hydroxy-lup-20(29)-en-28-oic acid, 3 beta, 23-dihydroxy-lup-20(29)-en-28-oic acid 3-O-alpha-L-arabinopyranoside and 3 beta, 23-dihydroxy-lup-20(29)-en-28-oic acid 28-O-beta-D-glucopyranosyl-(1-->6)-beta-D-glucopyranoside on the basis of hydrolysis and spectral evidence including two-dimensional relay HOHAHA, one-dimensional multiple relay COSY and ROESY NMR techniques. Pulsatillic acid exhibited cytotoxic activities against P-388, Lewis lung carcinoma and human large-cell lung carcinoma.

Digoxin has been shown to have an estrogenic effect and is associated with increased risk of gynecomastia and estrogen-sensitive cancers such as breast and uterus cancer. These findings, particularly recent observations of increased breast cancer risk, raise questions about the safety of digoxin use in breast cancer patients. Therefore, we investigated whether digoxin use after breast cancer diagnosis increased the risk of breast cancer-specific mortality in breast cancer patients. A cohort of 17,842 breast cancer patients newly diagnosed from 1998 to 2009 was identified from English cancer registries (from the National Cancer Data Repository). This cohort was linked to the UK Clinical Practice Research Datalink (to provide digoxin and other prescription records) and to the Office of National Statistics mortality data (to identify breast cancer-specific deaths). Using time-dependent Cox regression models, unadjusted and adjusted hazard ratios (HR) and 95 % confidence intervals (CIs) were calculated for the association between post-diagnostic exposure to digoxin and breast cancer-specific and all-cause mortality. In 17,842 breast cancer patients, there were 2219 breast cancer-specific deaths. Digoxin users appeared to have increased breast cancer-specific mortality compared with non-users (HR 1.73; 95 % CI 1.39–2.15) but this association was entirely attenuated after adjustment for potential confounders (adjusted HR 0.91; 95 % CI 0.72–1.14). In this large population-based breast cancer cohort study, there was little evidence of an increase in breast cancer-specific mortality with digoxin use after diagnosis. These results provide some reassurance that digoxin use is safe in breast cancer patients.

Em Portugal, as indústrias corticeira e de pasta de papel constituem um importante sector económico, contudo, gerando elevadas quantidades de subprodutos. Estes subprodutos poderiam ser explorados em aplicações de alto valor acrescentado, como fonte de compostos fenólicos, por exemplo, em vez de serem apenas queimados para produção de energia. Estes compostos são conhecidos pelas suas inúmeras propriedades, entre as quais, antioxidante, anti-inflamatória e anti-trombótica. Neste estudo as frações fenólicas da maior parte dos subprodutos gerados nas indústrias corticeira e de pasta de papel foram caracterizados em detalhe, com vista à sua valorização. A fração fenólica das cascas de Eucalyptus globulus, E. grandis, E. urograndis e E. maidenii, bem como da cortiça de Quercus suber e resíduos provenientes da sua exploração, nomeadamente, o pó de cortiça e os condensados negros, foi obtida por processos convencionais de extração sólido-líquido. No caso da casca de E. globulus, foi ainda avaliado o potencial de metodologias “verdes” no processo de extração de compostos fenólicos, usando extração com CO2 supercrítico. Esta técnica foi otimizada com recurso a metodologias de superfície de resposta. Na identificação e quantificação dos compostos fenólicos foi usada cromatografia líquida de alta resolução aliada a técnicas de espectrometria de massa. O teor de fenólicos totais foi ainda determinado pelo método de Folin- Ciocalteu, essencialmente para efeitos comparativos. A caracterização da fração fenólica de cada extrato foi ainda complementada com a análise da atividade antioxidante, usando o radical 2,2-difenil-1-picrilhidrazilo (DPPH). Foram identificados trinta compostos fenólicos na casca de E. globulus, 17 deles referenciados pela primeira vez como seus constituintes, nomeadamente os ácidos quínico, di-hidroxifenilacétic, cafeico e metil-elágico, bis-hexahidroxidifenoil( HHDP)-glucose, galoil- bis-HHDP-glucose, galoil-HHDPglucose, isoramnetina—hexosídeo, quercetina-hexosídeo, ácido metil-elágicopentosídeo, miricetina-ramnosídeo, isoramnetina-ramnosídeo, mearnsetina, floridzina, mearnsetina-hexosídeo, luteolina e uma proantocianidina B. Neste trabalho, foi estudada pela primeira vez a composição fenólica das cascas de E. grandis, E. urograndis e E. maidenii. Treze, doze e vinte e quatro compostos fenólicos foram identificados nas cascas de E. grandis, E. urograndis e E. maidenii, respetivamente. Entre estes compostos encontram-se os ácidos quínico, gálico, metilgálico, protocatequínico, clorogénico e elágico, catequina, galoil-bis-HHDP-glucose, digaloilglucose, epicatequina, quercetina-glucoronídeo, di-hidroxiisopropilcromona- hexosídeo, isoramnetina-hexosídeo, ácido elágicoramnosídeo, taxifolina, quercetina-hexosídeo, di-hidroxi- (metilpropil)isopropilcromona-hexosídeo, ácido metil-elágico-pentosídeo, miricetina-ramnosídeo, isoramnetina-ramnosídeo, aromadendrina-ramnosídeo, mearnsetina, mearnsetina-hexosídeo, eriodictiol, quercetina, isoramnetina e naringenina. A análise da fração fenólica da cortiça permitiu identificar vinte e dois compostos fenólicos, dez deles referenciados pela primeira vez como seus constituintes, nomeadamente, os ácidos quínico, salicílico, p-hidroxifenillático e metilgálico, ácido carboxílico da brevifolina, eriodictiol, naringenina, um éster isoprenílico do ácido cafeico, isoramnetina-ramnosídeo e isoramnetina. No pó de cortiça industrial foram identificados dezasseis compostos fenólicos, nomeadamente os ácidos quínico, gálico, protocatequínico, cafeico, ferúlico, elágico e metilgálico, esculetina, ácido carboxílico da brevifolina, coniferaldeído, um éster isoprenílico do ácido cafeico, uma dilactona do ácido valoneico, ácido elágico-pentosídeo, ácido elágico-ramnosídeo, isoramnetinaramnosídeo e isoramnetina. Destes, apenas o ácido elágico foi previamente referenciado como componente do pó de cortiça. Do mesmo modo, treze compostos fenólicos foram identificados no condensado negro, doze deles referenciados pela primeira vez como seus constituintes. São eles os ácidos quínico, gálico, p-hidroxifenil-láctico, protocatequínico, p-coumarico, cafeico e elágico, vanilina, esculetina, coniferaldeído, um éster isoprenílico do ácido cafeico e o eriodictiol. A extração supercrítica de compostos fenólicos da casca de eucalipto permitiu não só verificar os parâmetros que afetam a qualidade e quantidade finais dos extratos, como também obter os valores ótimos para estes parâmetros. Esta extração mostrou ainda ser bastante seletiva para determinados grupos de compostos fenólicos, como as flavanonas eriodictiol e naringenina e para o flavonol O-metilado isoramnetina. Este é também o primeiro estudo envolvendo a determinação da atividade antioxidante de extratos da cortiça e dos resíduos da sua exploração, bem como da casca de E. grandis, E. urograndis e E. maidenii. A vasta gama de compostos fenólicos identificados em cada extrato analisado, assim como as prominentes atividades antioxidantes, todas na mesma gama de valores do bem conhecido antioxidante comercial, ácido ascórbico, são claramente um grande contributo para a valorização destes subprodutos industriais.

Total phenol, hydroxycinnamic acid derivatives, flavone/flavonol and flavanones/dihydroflavonol contents of hydro-alcoholic extracts, obtained by sonication, from the aerial parts of Artemisia campestris L., Anthemis arvensis L., Haloxylon scoparium Pomel, Juniperus phoenicea L., Arbutus unedo L., Cytisus monspessulanus L., Thymus algeriensis Boiss et Reut, Zizyphus lotus L (Desf.) collected in Djebel Amour (Sahara Atlas, Algeria) were quantified by spectrophotometric methods. The chemical composition of the essential oils obtained by hydrodistillation from Artemisia campestris L. and Juniperus phoenicea I aerial parts were also evaluated by gas chromatography (GC) and gas chromatography coupled to mass spectrometry (GC-MS). The antioxidant activity of the extracts and essential oils was assessed measuring the capacity for preventing lipid peroxidation using two lipidic substrates (egg yolk and liposomes), the capacity for scavenging DPPH, ABTS, superoxide anion radicals, hydroxyl radicals and peroxyl radicals. Anti-inflammatory activity was assessed by measuring the capacity for inhibiting lipoxygenase. Reducing power and chelating capacity were also assayed. The results showed different amounts of total phenols depending on the method used: A. campestris extract had the highest levels of total phenols when the measurement was made at lambda = 280 nm, whereas H. scoparium and A. unedo extracts showed the highest levels of total phenols with Folin-Ciocalteau. C. monspessulanus had the highest levels of flavones/flavonols and flavanones/dihydroflavonols. The essential oils of A. campestris and J. phoenicea were mainly constituted by alpha-pinene, beta-pinene and sabinene; and a-pinene, respectively. The methods used for assaying the capacity for preventing lipid peroxidation revealed to be inadequate for extracts due to the great interferences detected. The essential oils were more active than the generality of extracts for scavenging peroxyl radicals and for inhibiting lipoxygenase, whereas A. unedo extract was the most active for scavenging ABTS, DPPH, superoxide anion radicals and it also had the best reducing capacity. In a general way, the great majority of the antioxidant activities correlated well with the phenol content although such correlation was not so clear with the flavonoid content. Nous avons démontré l’utilité du groupement protecteur tert-butylsulfonyle (N-Bus) pour la chimie des acides aminés et des peptides. Celui-ci est préparé en deux étapes, impliquant la réaction d’une amine avec le chlorure de tert-butylsulfinyle, suivie par l’oxydation par du m-CPBA, pour obtenir les tert-butylsulfonamides correspondants avec d’excellents rendements. Le groupement N-Bus peut être clivé par traitement avec 0.1 N TfOH/DCM/anisole à 0oC en 10h pour régénérer le sel d’ammonium. Une variété d’acides aminés N-Bus protégés ainsi que d’autres aminoacides peuvent alors être utilisés pour préparer divers dipeptides et tripeptides. A l’exception du groupe N-Fmoc, les conditions de déprotection du groupe N-Bus clivent également les groupements N-Boc, N-Cbz et O-Bn. Une déprotection sélective et orthogonale des groupes N-Boc, N-Cbz, N-Fmoc et O-Bn est également possible en présence du groupe protecteur N-Bus. Le nouvel acide aminé non-naturel (3R, 2R) 3–méthyl-D-leucine (β-Me-Leu) et son régioisomère 2-méthyle ont été synthétisés par ouverture d’une N-Ts aziridine en présence d’un excès de LiMe2Cu. Chacun des régioisomères du mélange (1:1,2) a été converti en la méthylleucine correspondante, puis couplé à l’acide D-phényllactique puis au motif 2-carboxyperhydroindole 4-amidinobenzamide en présence de DEPBT. Des élaborations ultérieures ont conduit à des analogues peptidiques non-naturels d’aeruginosines telles que la chlorodysinosine A. Les deux analogues ont ensuite été évalués pour leur activité inhibitrice de la thrombine et la trypsine. La présumée aeruginosine 3-sulfate 205B et son anomère β ont été synthétisés avec succès à partir de 5 sous-unités : la 3-chloroleucine, l’acide D-phényllactique, le D-xylose, le 2-carboxy-6-hydroxyoctahydroindole et l’agmatine. La comparaison des données RMN 1H et 13C reportées avec celles obtenues avec l’aeruginosine synthétique 205B révèle une différence majeure pour la position du groupe présumé 3'-sulfate sur l’unité D-xylopyranosyle. Nous avons alors synthétisés les dérivés méthyl-α-D-xylopyranosides avec un groupement sulfate à chacune des positions hydroxyles, afin de démontrer sans ambiguïté la présence du sulfate en position C-4' par comparaison des données spectroscopiques RMN 1H et 13C. La structure de l’aeruginosine 205B a alors été révisée. Une des étapes-clés de cette synthèse consiste en la formation du glycoside avec le groupe hydroxyle en C-6 orienté en axial sur la sous-unité Choi. Le 2-thiopyridylcarbonate s’est avéré une méthode efficace pour l’activation anomérique. Le traitement par AgOTf et la tétraméthylurée en solution dans un mélange éther-DCM permet d’obtenir l’anomère α désiré, qui peut alors être aisément séparé de l’anomère β par chromatographie

L’incidence du diabète chez les premières nations du Canada est plus de trois fois celle du reste du pays, dû, en partie, aux traitements culturellement inappropriés. Notre projet vise à traiter le diabète chez ces populations à partir de leur pharmacopée de médicine traditionnelle afin d’améliorer l’acceptation des traitements. En utilisant une approche ethnobotanique, notre équipe a identifié 17 plantes médicinales utilisées pour traiter des symptômes du diabète par les Cris d'Eeyou Istchee (Baie James, Québec). Parmi eux, l'extrait éthanolique de baies de Vaccinium vitis-idaea a montré un effet stimulateur sur le transport du glucose dans les cellules musculaires squelettiques et les adipocytes en culture. Le but de cette thèse était d’élucider les mécanismes par lesquels cet extrait exerce ses effets anti-hyperglycémiants, d’identifier ses principes actifs et de confirmer in vivo, son efficacité. Les résultats démontrent que V.vitis a augmenté le transport du glucose dans les cellules musculaires en cultures, C2C12 et L6 et a stimulé la translocation des transporteurs GLUT4 dans les cellules L6. L'extrait a également inhibé la respiration dans les mitochondries isolées du foie du rat. Cet effet est semblable à celui de la metformine et en lien avec la production du stress métabolique et l'activation de l'AMPK. De plus, la voie de signalisation de l’insuline ne semble pas être impliquée dans le mécanisme d’action de V. vitis. Le fractionnement guidé par la stimulation du transport du glucose a mené à l'isolation des principes actifs; la quercétine, la quercétine-3-O-galactoside, et la quercétine-3-O-glucoside. Comparable à l'extrait brut, ses composés ont stimulé la voie AMPK. Cependant, la quércetine était la seule à inhiber la respiration mitochondriale. Pour valider l'effet de V.vitis in vivo, l'extrait (1% dans l'eau de boisson) a été administré aux souris KKAy pendant 10 jours. La glycémie et le poids corporel ont été significativement réduits par V.vitis. Ces effets ont été associés à une diminution de la prise alimentaire, ce qui suggère que V.vitis diminue l'appétit. L'étude pair-fed a confirmé que les effets de V.vitis sont, majoritairement, dû à la réduction de l’appétit. De plus, V.vitis a augmenté la teneur en GLUT4 dans le muscle squelettique, a stimulé la iv phosphorylation de l'ACC et a augmenté les niveaux de PPAR-α dans le foie des souris KKAy. Ces effets se voient être additifs à l’effet anorexigène de V. vitis. Au cours du fractionnement bioguidé de l’extrait, l’ester méthylique de l'acide caféique (CAME), un produit formé lors de la procédure du fractionnement, a démontré un effet stimulateur puissant sur le transport du glucose dans les celules C2C12 et donc un potentiel anti-diabétique. Pour identifier d'autres acides caféique active (AC) et pour élucider leurs relations structure-activité et structure-toxicité, vingt dérivés AC ont été testés. Outre CAME, quatre composés ont stimulé le transport du glucose et ont activé l'AMPK suite au stress métabolique résultant d'un découplage de la phosphorylation oxydative mitochondriale. L’activité nécessite une fonction d’AC intacte dépourvu de groupements fortement ionisés et ceci était bien corrélée avec la lipophilicite et la toxicité. Les résultats de cette thèse soutiennent le potentiel thérapeutique de V. vitis, ses composés actifs ainsi que de la famille de l’AC et pour la prévention et le traitement du diabète.

Les glycosides sont reconnus pour leur potentiel pharmaceutique tels que les antibiotiques, les agents anticancéreux et antiviraux. Ils sont impliqués dans plusieurs processus biologiques entre autres la reconnaissance cellulaire, l’inflammation, la réponse immunitaire, la croissance, le transport cellulaire, l’adhésion cellulaire et les groupes sanguin. Notre groupe excelle dans la glycosidation stéréocontrôlée avec un minimum de protection suivant le concept d’activation à distance d’aglycones hétérocycliques anomériques. La présence d’une quantité sous stoechiométrique d’acide de Lewis, les (2-pyridyl)-β-D-glycosides déprotégés sont d’excellents donneurs permettant de haute sélectivité pour l’anomère- α-D de glycosides simples et complexes. Inversement, (2-pyridyl)-α-D-glycosides donnent les β-D-glycosides avec de bonne sélectivité. Des exemples de formation stéréocontrôlée de glycosides sont présentés dans cette thèse avec des accepteurs tels que les phénols, les stéroïdes, les terpènes et les acides hydroxyaminés. Cette méthodologie de glycosidation a été appliquée sur support solide.

In the present study on natural antioxidants, the focus has been kept mainly on oil seeds, especially sesame and its by-products. Sesame, which has been under cultivation in India for centuries is called the 'Queen of oil seed crops' because of the high yield of oil obtained and the nutritional qualities of the seed, oil, and meal. Though India is the largest producer of sesame in the world, research on the various health benefits of sesame has been carried out by Japanese Sesame has an important place in the foods and tradit..ional medicine of India from time immemorial. Foreseeing the potential of sesame and its byproducts as an important antioxidant source and its availability in bulk, the present study was focussed on Sesamum species. There are not many reports on the wild species of Sesamum in India, especially of the Kerala region. Hence, in the present study we also included antioxidants of Sesamurnrnalabaricumdistributed throughout the coastal region.The important characteristics of sesame are attributed to the presence of the umquc compounds lignans. Lignans arc a group of natural products of phenyl propanoid ongm, whieh are widely distributed in nature. They display important physiological functions in plants, in human nutrition and medicine, given their extensive health promotive and curative properties. Much interest has been focussed on their effectiveness as antineoplastic agents and research in this area has revealed several modes of action by which they can regulate the growth of mammalian cells. Sesame is an important source of furofuran lignans, of which sesamin and the rare oxygenated derivative sesamoIin are the most abundant. Others include sesamol and glucosides of lignans. Sesarnin and episesamin are reported to have hypocholesterolemic effect, suppressive effect on chemically induced cancer, alleviation of allergy symptoms etc. Sesamol, sesamolin and the lignan glycosides are reported to inhibit lipid peroxidation. Present investigation on sesame and its byproducts have been carried out to explore the possibility of developing a natural antioxidant extract from available resources to be used as a substitute to synthetic ones in vegetable oils and foods. Preliminary analysis showed that sesame cake, a byproduct could still be utilized as a major source of lignans. Sesame cake, which is now used only as a cattlefeed, can be better utilized in the form of a valuable antioxidant source. The present study explains the development of a feasible process for the extraction of antioxidant compounds from sesame cake. The antioxidant extract so prepared from sesame cake has been tested for vegetable oil protection and is found to be effective at low concentration. In addition, studies also include the antioxidant, radical scavenging, anticancer, mosquitocidal and pesticidal activities of extract and individual compounds.

The objective of the study was to find out a natural way to fight white spot syndrome virus (WSSV) in cultured shrimps, as the present scenario necessitated an organic remedy for the devastating pathogen in crustaceans. Under this research programme seven mangrove plants were collected, identified and aqueous extracts screened for their protective effect on the giant tiger shrimp Penaeus monodon against WSSV. The experimental design consisted two modes of application, such as exposure of the virus to the extract and injection challenge, and oral administration of the extract coated feed followed by oral challenge. All experimental animals were monitored through a nested diagnostic PCR analysis. Of the seven mangrove extracts screened aqueous extract from Ceriops tagal imparted total protection to shrimp from WSSV when challenged by both methods. Shrimps administered with the aqueous extract from C. tagal were devoid of virions. The HPLC fingerprint of the aqueous extracts from C. tagal showed more than 25 peaks and 7 of them were larger and well separated. Preliminary phytochemical analysis revealed the presence of alkaloids, flavonoids, polyphenolics, cardiac glycosides, saponins and sterols. The study indicated suitability of the aqueous extract of C. tagal as a possible prophylaxis for WSSV infection in shrimp. This is the first report on the anti WSSV property of the mangrove plant C. tagal

Thymus is taxonomically a very complex genus with a high frequency of hybridisation and introgression among sympatric species. The variation in accumulation of leaf-surface flavonoids was investigated in 71 wild populations of Thymus front different putative hybrid swarm areas in Andalucia, Spain. Twenty-two flavones, five flavanones, two dihydroflavonols, a flavonol and two unknowns were detected by HPLC-DAD combined with LC-APCI-MS analysis. The majority of compounds were flavones with a lutelin-type substitution of the B-ring, in contrast to previous reports on Macedonian taxa, which predominantly accumulate flavones with apigenin-type substitution of the B-ring. Anatomical and morphometric studies, supported by cluster analysis, identified pure Thymus hyemalis and Thymus baeticus populations, and a large number of putative hybrids. Flavonoid variation was closely related to morphological variation in all populations and is suspected to be a result of genetic polymorphism. Principal component analysis identified the presence of species-specific and geographically linked chemotypes and putative hybrids with mixed morphological and chemical characteristics. Qualitative and quantitative flavonoid accumulation appears to be genetically regulated, while external factors play a secondary role. Flavonoid profiles can thus provide diagnostic markers for the taxonomy of Thymus and are also useful in detecting hybridising taxa. (C) 2007 Elsevier Ltd. All rights reserved.
