Foram comparados o estado nutricional e parâmetros do metabolismo do ferro de adultos HIV-positivos, com ou sem resposta de fase aguda (RFA). Adultos HIV-positivos (n = 29) submeteram-se a antropometria, recordatório alimentar e determinação sérica de albumina, proteína C reativa (PCR), ferritina e capacidade total de ligação do ferro (CTLF), além de creatinina urinária. Infecção mais PCR > 7mg/dl foram critérios de positividade da RFA. Índice de massa corporal (IMC < 18,5kg/m2) e índice creatinina-altura (ICA < 70%) definiram subnutrição. Subnutrição (77,8 vs 40%) e tuberculose pulmonar (44,4 vs 9,5%) foram mais freqüentes nos pacientes RFA-positivos, que também apresentaram menores níveis de albumina (3,7 ± 0,9 vs 4,3 ± 0,9g/dl), CTLF (165,8 ± 110,7 vs 265,9 ± 74,6mg/dl) e hemoglobina (10,5± 1,8 vs 12,6 ± 2,3g/dl). A ingestão de ferro foi adequada e similar entre RFA-positivos e RFA-negativos, o mesmo ocorrendo, respectivamente, quanto à ferritina sérica (mediana; variação, 568; 45,3-1814 vs 246; 18,4-1577ng/ml). Pacientes HIV-positivos com resposta de fase aguda são nutricionalmente mais comprometidos e têm anemia que parece não depender da ingestão recente de ferro.

O objetivo deste trabalho foi comparar a ingestão alimentar habitual e a freqüência de retardo do crescimento de crianças com reação intradérmica positiva para leishmaniose (Montenegro-positivas), com um grupo Montenegro-negativo. A ingestão alimentar habitual foi avaliada pelo recordatório de 24 horas e o retardo do crescimento definido segundo critérios da OMS. Crianças Montenegro-positivo (n = 9) e Montenegro-negativo (n = 17) ingeriam, respectivamente, quantidades similares de energia (1456,8 ± 314,8 vs 1316,2 ± 223,8kcal/dia) e proteínas (50,4 ± 16,7 vs 49,9 ± 13,9g/dia). Déficit de altura foi mais comum em crianças Montenegro-positivas (44,4 vs 5,9). Estes dados sugerem que a infecção prévia pela Leishmania sp afeta desfavoravelmente o estado nutricional de crianças vivendo em área endêmica.

Endemic pemphigus foliaceus, and long-term corticotherapy may affect serum lipid levels. The aim of this study was to compare serum lipids of pemphigus foliaceus patients on glucocorticoid therapy to a healthy control group. Fifteen patients receiving prednisone (0.33 ± 0.22mg/kg) for at least 12 months and 15 controls were submitted to 48-h food intake records, anthropometry, and biochemical measurements. Data were compared by chi2, Mann-Whitney and Student "t" tests. The groups were matched for gender, age, weight, body mass index, arm circumference and triceps skin fold. No differences were observed in relation to energy, fat, protein and carbohydrate daily intakes, total cholesterol, HDL, LDL, uric acid, and serum creatinine levels. Pemphigus foliaceus patients had higher triglyceride [159 (64-371) vs. 100 (45-133) mg/dl], VLDL [32 (13-74) vs. 20 (9-114) mg/dl] and ESR [44 (9-87) vs. 7 (1-30) mm/h] levels than controls, probably due to metabolic effects of inflammatory disease and corticotherapy.

We report such a case of malignant syphilis in a 42-year-old HIV-infected man, co-infected with hepatitis B virus, who presented neurolues and the classical skin lesions of lues maligna. The serum VDRL titer, which was 1:64 at presentation, increased to 1:2,048 three months after successful therapy with penicillin, decreasing 15 months later to 1:8.

To study characteristics of neurological disorders in HIV/AIDS patients and their relationship to highly active antiretroviral treatment, a cross-sectional study was conducted in an infectious disease public hospital in Belo Horizonte, Brazil, between February 1999 and March 2000. Of the 417 patients enrolled, neurological disease was observed in 194 (46.5%) and a new AIDS-defining neurological event developed in 23.7% of individuals. Toxoplasmosis (42.3%), cryptococcosis meningitis (12.9%) and tuberculosis (10.8%) were the most common causes of neurological complications. The majority (79.3%) of patients were on highly active antiretroviral treatment and these individuals using HAART showed higher CD4 cell counts (p = 0.014) and presented stable neurological disease (p= 0.0001), although no difference was found with respect to the profile of neurological complications. The neurological diseases continue to be a frequent complication of HIV/AIDS and infections are still its main causes in Brazil, even in the highly active antiretroviral treatment era.

In this work, a volumetric unit previously assembled by the research group was upgraded. This unit revamping was necessary due to the malfunction of the solenoid valves employed in the original experimental setup, which were not sealing the gas properly leading to erroneous adsorption equilibrium measurements. Therefore, the solenoid valves were substituted by manual ball valves. After the volumetric unit improvement its operation was validated. For this purpose, the adsorption equilibrium of carbon dioxide (CO2) at 323K and 0 - 20 bar was measured on two different activated carbon samples, in the of extrudates (ANG6) and of a honeycomb monolith (ACHM). The adsorption equilibrium results were compared with data previously measured by the research group, using a high-pressure microbalance from Rubotherm GmbH (Germany) – gravimetric. The results obtained using both apparatuses are coincident thus validating the good operation of the volumetric unit upgraded in this work. Furthermore, the adsorption equilibrium of CO2 at 303K and 0 - 10 bar on Metal-Organic Frameworks (MOFs) Cu-BTC and Fe-BTC was also studied. The CO2 adsorption equilibrium results for both MOFs were compared with the literature results showing good agreement, which confirms the good quality of the experimental results obtained in the new volumetric unit. Cu-BTC sample showed significantly higher CO2 adsorption capacity when compared with the Fe-BTC sample. The revamping of the volumetric unit included a new valve configuration in order to allow testing an alternative method for the measurement of adsorption equilibrium. This new method was employed to measure the adsorption equilibrium of CO2 on ANG6 and ACHM at 303, 323 and 353K within 0-10 bar. The good quality of the obtained experimental data was testified by comparison with data previously obtained by the research group in a gravimetric apparatus.

INTRODUCTION: Report the incidence of nosocomial infections, causative microorganisms, risk factors associated with and antimicrobial susceptibility pattern in the NICU of the Uberlândia University Hospital. METHODS: Data were collected through the National Healthcare Safety Network surveillance from January 2006 to December 2009. The patients were followed five times/week from their birth to their discharge or death. RESULTS: The study included 1,443 patients, 209 of these developed NIs, totaling 293 NI episodes, principally bloodstream infections (203; 69.3%) and conjunctivitis (52; 17.7%). Device-associated infection rates were as follows: 17.3 primary bloodstream infections per 1,000 central line-days and 3.2 pneumonias per 1000 ventilator-days. The mortality rate in neonates with NI was 11.9%. Mechanical ventilation, total parenteral nutrition, orogastric tube, previous antibiotic therapy, use of CVC and birth weight of 751-1,000g appeared to be associated with a significantly higher risk of NI (p < 0.05). In multiple logistic regression analysis for NI, mechanical ventilation and the use of CVC were independent risk factors (p < 0.05). Coagulase- negative Staphylococcus (CoNS) (36.5%) and Staphylococcus aureus (23.6%) were the most common etiologic agents isolated from cultures. The incidences of oxacillin-resistant CoNS and S. aureus were 81.8% and 25.3%, respectively. CONCLUSIONS: Frequent surveillance was very important to evaluate the association of these well-known risk factors with NIs and causative organisms, assisting in drawing the attention of health care professionals to this potent cause of morbidity.

INTRODUÇÃO: O objetivo desta pesquisa foi verificar a ocorrência dos principais sorovares de Leptospira spp. em cães domésticos e humanos, notificados no ano de 2008, bem como os principais fatores de riscos em uma abordagem geográfica relacionados à doença no município de Uberlândia, Estado de Minas Gerais, Brasil. MÉTODOS: Foram examinadas 268 amostras de soro sanguíneo de cães de diferentes bairros pertencentes aos distritos sanitários norte, sul, leste, oeste e central deste município, colhidas durante a campanha de vacinação antirrábica animal, em agosto de 2008. Foi realizada uma abordagem geográfica do município e avaliada a localização de áreas periféricas, aterro sanitário, coleta de lixo, notificação de roedores, casos de leptospirose humana e áreas de alagamento decorrente de enchentes, durante o ano de 2008. A leptospirose foi diagnosticada pela técnica de soroaglutinação microscópica (SAM), padrão-ouro para diagnóstico da leptospirose animal e humana. RESULTADOS: Os cães reagiram principalmente aos sorovares Autumnalis (34,2%) e Tarassovi (23,7%), sendo este, também detectado em humanos em 2008. A ocorrência destes sorovares pode estar relacionada com uma fonte de infecção comum as duas espécies, ou a hipótese de que o cão possa ser a fonte de infecção para o ser humano. O distrito sanitário leste apresentou um maior número de cães reagentes. CONCLUSÕES: A leptospirose ocorreu nos cães e humanos no município de Uberlândia no ano de 2008. Esta doença muitas vezes negligenciada deve ser prevenida por representar risco à saúde pública e se parecer com outras doenças também endêmicas como a dengue.

INTRODUCTION: HIV-infected children and adolescents treated with highly active antiretroviral therapy (HAART) regimens that include a protease inhibitor (PI) can show significant improvements in clinical outcomes, nutritional status and quality of life. The study aimed to report nutritional and metabolic alterations for pediatric patients continuously exposed to HAART and for healthy controls for up to 1 year. METHODS: Clinical, anthropometric, lipid profile and food intake data were collected prospectively over approximately 12-months for each patient. RESULTS: Fifty-one individuals were studied, of these, 16 were healthy. After 12 months follow-up, HIV-positive individuals remained below the healthy control group parameters. No change was observed concerning food intake. Triglyceride serum levels were higher in patients using protease inhibitor at the onset of the study [PI groups: 114 (43 - 336), and 136 (63 - 271) versus control group: 54.5 (20 - 162); p = 0.003], but after twelve months follow-up, only the group using protease inhibitor for up to two months presented higher values [140 (73 - 273) versus 67.5 (33 - 117); p = 0.004]. HDL-cholesterol was lower in HIV-positive individuals [HIV-positive groups: 36 (27 - 58) and 36 (23 - 43); control 49.5 (34 - 69); p = 0.004]. CONCLUSIONS: HIV-infected children and adolescents treated with highly active antiretroviral therapy showed compromised nutritional parameters compared to a paired healthy control group. Individuals using protease inhibitor presented worse triglyceride serum levels compared to their healthy counterparts.

INTRODUÇÃO: O presente estudo teve como objetivo avaliar a suscetibilidade de larvas de Culex quinquefasciatus a dois piretróides (Cipermetrina e Deltametrina), dois derivados da Avermectina (ivermectina e abamectina) e a um organofosforado (Temefós). MÉTODOS: Larvas de 3º e 4º instares de C. quinquefasciatus foram expostas a diferentes concentrações destes (onze repetições) seguindo o protocolo da Organização Mundial de Saúde. Uma hora após a exposição, as larvas foram lavadas em água desclorada, transferidas para recipientes plásticos contendo água sem cloro, alimentadas e observadas por períodos de 24h, até se transformarem em adultos. Para a determinação das concentrações letais, os valores foram submetidos à análise de regressão usando o modelo probit pelo programa Minitab 15. RESULTADOS: Diferenças entre as estimativas da CL50 e CL90 justificaram que a população de mosquitos testada apresenta heterogeneidade em resposta aos inseticidas, sendo a maior concentração utilizada para a CL50, a partir da análise de probit para o Temefós. Todos os inseticidas avaliados causaram mortalidade mais acentuada nas primeiras 24h exceto quando expostas à ivermectina. CONCLUSÕES: As larvas são suscetíveis a todos os inseticidas testados e há uma necessidade de um monitoramento dos inseticidas utilizados.

INTRODUÇÃO: A competência vetorial de triatomíneos é determinada a partir de estudos sobre biologia e comportamento alimentar em condições de campo e/ou laboratório. Fatores como número de picadas, quantidade de sangue ingerido e tempo de defecação têm implicações na transmissão de tripanosomatídeos. Parâmetros biológicos e comportamentais de Rhodnius neglectus e R. robustus foram comparados experimentalmente para estimar diferenças no potencial de transmissão de tripanosomatídeos. MÉTODOS: Os triatomíneos foram observados diariamente para determinar o período de desenvolvimento ninfal, mortalidade, detecção da fonte alimentar, número de picadas, tempo do repasto sanguíneo, quantidade de sangue ingerido, tempo entre o final do repasto e a primeira defecação e frequência de defecação. RESULTADOS: Apesar do período ninfal de R. neglectus (156,4 ± 25,05d) ter sido menor que o de R. robustus (204,7 ± 13,22d), a mortalidade foi similar entre as espécies (63,8 e 65%, respectivamente). R. robustus e R. neglectus detectaram rapidamente a fonte alimentar, especialmente no primeiro estádio (2,5 e 1,6min, respectivamente). Apesar do tempo de repasto sanguíneo ter sido similar entre as espécies, R. robustus ingeriu em média uma maior quantidade de sangue em todos os estádios, com maiores valores para as ninfas V. As ninfas de R. neglectus picaram mais vezes as fontes alimentares, defecaram mais rápido e mais frequentemente que as de R. robustus. CONCLUSÕES: Sob as condições de laboratório usadas, R. neglectus possui um maior potencial para transmissão de Trypanosoma cruzi e T. rangeli que R. robustus, atributo que deve ser avaliado em infecções experimentais.

INTRODUCTION: Catheter-associated bloodstream infection (CA-BSI) is the most common nosocomial infection in neonatal intensive care units. There is evidence that care bundles to reduce CA-BSI are effective in the adult literature. The aim of this study was to reduce CA-BSI in a Brazilian neonatal intensive care unit by means of a care bundle including few strategies or procedures of prevention and control of these infections. METHODS: An intervention designed to reduce CA-BSI with five evidence-based procedures was conducted. RESULTS: A total of sixty-seven (26.7%) CA-BSIs were observed. There were 46 (32%) episodes of culture-proven sepsis in group preintervention (24.1 per 1,000 catheter days [CVC days]). Neonates in the group after implementation of the intervention had 21 (19.6%) episodes of CA-BSI (14.9 per 1,000 CVC days). The incidence of CA-BSI decreased significantly after the intervention from the group preintervention and postintervention (32% to 19.6%, 24.1 per 1,000 CVC days to 14.9 per 1,000 CVC days, p=0.04). In the multiple logistic regression analysis, the use of more than 3 antibiotics and length of stay >8 days were independent risk factors for BSI. CONCLUSIONS: A stepwise introduction of evidence-based intervention and intensive and continuous education of all healthcare workers are effective in reducing CA-BSI.

Introduction We hypothesized that nutritional deficiency would be common in a cohort of postpartum, human immunodeficiency virus (HIV)-infected women and their infants. Methods Weight and height, as well as blood concentrations of retinol, α-tocopherol, ferritin, hemoglobin, and zinc, were measured in mothers after delivery and in their infants at birth and at 6-12 weeks and six months of age. Retinol and α-tocopherol levels were quantified by high performance liquid chromatography, and zinc levels were measured by atomic absorption spectrophotometry. The maternal body mass index during pregnancy was adjusted for gestational age (adjBMI). Results Among the 97 women 19.6% were underweight. Laboratory abnormalities were most frequently observed for the hemoglobin (46.4%), zinc (41.1%), retinol (12.5%) and ferritin (6.5%) levels. Five percent of the women had mean corpuscular hemoglobin concentrations < 31g/dL. The most common deficiency in the infants was α-tocopherol (81%) at birth; however, only 18.5% of infants had deficient levels at six months of age. Large percentages of infants had zinc (36.8%) and retinol (29.5%) deficiencies at birth; however, these percentages decreased to 17.5% and 18.5%, respectively, by six months of age. No associations between infant micronutrient deficiencies and either the maternal adjBMI category or maternal micronutrient deficiencies were found. Conclusions Micronutrient deficiencies were common in HIV-infected women and their infants. Micronutrient deficiencies were less prevalent in the infants at six months of age. Neither underweight women nor their infants at birth were at increased risk for micronutrient deficiencies.

INTRODUCTION: Schistosomiasis, a parasitic disease, is an important public health issue in Brazil, particularly Northern Brazil. Since the first recorded occurrence of Biomphalaria glabrata in the States of Santa Catarina and Rio Grande do Sul, it has spread to South Brazil. METHODS: Using the Information System for Notifiable Diseases (SINAN), we assessed the disease spread in Southern Brazil. RESULTS: In Rio Grande do Sul, nine localities had confirmed cases (n = 95, 2001-2013). CONCLUSIONS: We confirmed disease expansion to Southern Brazil. We demonstrated the effectiveness of SINAN to monitor notifiable diseases. Our results are useful to develop preventive actions for schistosomiasis control.
