A resistência a fármacos antituberculose tem constituído uma grande ameaça ao controle da tuberculose em âmbito mundial. A sua detecção precoce permite ao médico instituir um esquema de tratamento mais adequado ao paciente e consequentemente quebrar a cadeia de transmissão dos bacilos. Os testes de sensibilidade a antimicrobianos atuais, embora eficientes, são caros e/ou demorados e/ou trabalhosos. Com base nesta premissa, nos propusemos a desenvolver e padronizar um método fenotípico direto para determinação da sensibilidade do Mycobacterium tuberculosis a antimicrobianos de primeira linha do tratamento da tuberculose. Para o desenvolvimento deste novo teste, utilizaram-se os princípios do método das proporções e do exame de cultura pelo método de Ogawa–Kudoh. O estudo foi dividido em duas fases. A primeira, caracterizada pelo desenvolvimento e padronização do método proposto e a segunda, pela análise da concordância entre o método desenvolvido e o método do MGIT (padrão-ouro). Na primeira fase, foram realizados diversos ensaios para definir: os volumes de absorção e de liberação de líquidos de diferentes tipos de swab, o meio de cultura, as concentrações dos antimicrobianos e o tempo de leitura/interpretação das culturas. Além disso, foi verificado se a amostra deveria ou não ser diluída. Com base nos resultados destes ensaios, padronizou-se o método com: swab comercial, em meio de cultura Ogawa- Kudoh contendo separadamente 0,2 μg/mL de isoniazida, 40,0 μg/mL de rifampicina, 10,0 μg/mL de estreptomicina e 500,0 μg/mL de ácido para-nitrobenzóico. Padronizou-se ainda a inoculação da amostra de escarro de forma direta, ou seja, sem diluir e a leitura/interpretação do resultado do teste no período entre 21 e 28 dias. A análise comparativa entre este método e o teste de sensibilidade a antimicrobianos no sistema MGIT realizada na segunda fase do projeto indicou um índice kappa igual a 1,000, ou seja, uma concordância muito boa em relação ao padrão-ouro. Diante desses resultados promissores, acreditamos que o método desenvolvido apresente um grande potencial para ser utilizado em laboratórios com pouca infra-estrutura, por ser de baixo custo, fácil execução e relativamente rápido.

Introdução: Os ensaios de liberação do interferon- γ (ELIG) surgiram como uma alternativa para o diagnóstico de infecção latente pelo Mycobacterium tuberculosis (ILTB). Neste estudo, nós comparamos o desempenho de um dos ELIG, teste Quantiferon TB Gold in tube – QFT, com a prova tuberculínica (PT) em dois pontos de corte (≥ 5 mm e ≥ 10 mm), em profissionais de saúde da atenção básica à saúde (ABS). Métodos: Estudo transversal realizado em profissionais de saúde da ABS de quatro capitais nacionais com alta incidência de TB. O resultado do teste QFT foi comparado com o resultado da PT nos pontos de corte ≥ 5mm e ≥ 10 mm. Resultados: Foram incluídos 632 profissionais de saúde. Ao considerar o ponto de corte ≥ 10 mm para a PT, a concordância entre QFT e a PT foi de 69% (k = 0,31) e para o ponto de corte ≥ 5 mm, a concordância entre os testes foi de 57% (k = 0,22). Devido a baixa concordância entre a PT e o QFT, nós avaliamos os possíveis fatores associados com a discordância entre eles. Ao comparar o grupo PT- / QFT- com o grupo PT+ / QFT-, no ponto de corte ≥ 5 mm, a idade entre 41-45 [OR = 2,70, IC 95%: 1,32-5,51] e 46-64 [OR = 2,04, IC 95%: 1,05-3,93], presença de cicatriz vacinal do BCG [OR = 2,72, IC 95%: 1,40-5,25] e trabalhar apenas na ABS [OR = 2,30, IC 95 %: 1,09-4,86] apresentaram associação estatística significativa. Para o ponto de corte ≥ 10 mm, a presença de cicatriz vacinal do BCG [OR = 2,26, IC 95%: 1,03-4,91], ter tido contato domiciliar com paciente portador de tuberculose ativa [OR = 1,72, IC 95%: 1,01-2,92] e ter feito a PT anteriormente [OR = 1,66, IC 95%: 1,05-2,62] revelaram associação estatística significativa. Curiosamente, a discordância observada no grupo PT- / QFT + não apresentou associação estatistica com nenhuma das variáveis consideradas, independentemente do ponto de corte da PT. Conclusões: Apesar de termos identificado que a vacina BCG contribuiu para a discordância entre os testes, as recomendações brasileiras para o início do tratamento da ILTB não devem ser alteradas devido as limitações do QFT.

Mycobacterium tuberculosis was isolated from a central venous catheter in a non-immunosuppressed patient with systemic tuberculosis. This case report represents a very uncommon form of isolation of Mycobacterium tuberculosis. A total improvement was obtained after treatment.

The present study was undertaken to assess the effect of the crude extract of Cleome droserifolia (CD) leaves on experimentally infected mice with Schistosoma mansoni. Two groups of mice, showing a patent infection of S. mansoni, one of them was daily treated with an alcoholic extract of CD leaves (0.31 g kg-1 body weight, i.p.) for 21 days. The schistosomicidal activity of the CD extract was evaluated, three weeks post-treatment, on some parasitological and histopathological aspects including worm load, oogram pattern, faecal eggs releasing and granuloma formation. In addition, serum thyroid hormones levels (tri-iodothyronine; T3 and tetra-iodo-thyronin; T4), serum total protein contents and hepatic reduced glutathione (GSH) were evaluated. Treatment using CD extract resulted in a weak reduction in worm burden (32.46%) and affected the viability of both mature and immature eggs as indicated by the increase in the percentage of dead eggs and the decrease in the percentage of live ones. In addition, a week post-treatment, eggs elimination was observed in the stool of the infected-treated group which was low compared to the infected group. There was a suppressive effect of the extract on granuloma formation that could be due to the antioxidant effect of the extract. These data are confirmed by increasing hepatic GSH, serum total proteins and thyroid hormone levels in the infected-treated group as compared to the infected group. Treatment significantly enhanced b globulin fractions of the protein. Based on these assumptions, CD extract has beneficial effects on thyroid hormones status and anti-schistosomiasis activity. The beneficial effects of CD extract could be related to its direct effects on the parasite, and secondary to its effect on the antioxidant capacity of the host. The present study could emphasize the precise mechanism (s) of CD extract protection.

Development and standardization of reliable methods for detection of Mycobacterium tuberculosis in clinical samples is an important goal in laboratories throughout the world. In this work, lung and spleen fragments from a patient who died with the diagnosis of miliary tuberculosis were used to evaluate the influence of the type of fixative as well as the fixation and paraffin inclusion protocols on PCR performance in paraffin embedded specimens. Tissue fragments were fixed for four h to 48 h, using either 10% non-buffered or 10% buffered formalin, and embedded in pure paraffin or paraffin mixed with bee wax. Specimens were submitted to PCR for amplification of the human beta-actin gene and separately for amplification of the insertion sequence IS6110, specific from the M. tuberculosis complex. Amplification of the beta-actin gene was positive in all samples. No amplicons were generated by PCR-IS6110 when lung tissue fragments were fixed using 10% non-buffered formalin and were embedded in paraffin containing bee wax. In conclusion, combined inhibitory factors interfere in the detection of M. tuberculosis in stored material. It is important to control these inhibitory factors in order to implement molecular diagnosis in pathology laboratories.

A ligase chain reaction DNA amplification method for direct detection of Mycobacterium tuberculosis (Abbott LCx MTB) in respiratory specimens was evaluated. Results from LCx MTB Assay were compared with those from acid fast bacilli smear, culture, and final clinical diagnosis for each patient. A total of 297 respiratory specimens (sputum and bronchial lavage) from 193 patients were tested. The sensitivity, specificity, positive predictive value and negative predictive value of LCx vs culture were 92.7%, 93%, 67.8% and 98.7%, respectively. When compared to the clinical final diagnosis, the sensitivity, specificity, PPV and NPV for LCx were 88.9%, 96.8%, 86.5% and 97.4%, respectively. The sensitivity of LCx MTB assay was 75% for smear-negative, culture positive samples. The results indicate that LCx MTB assay is a rapid, simple and valuable technique as a complementary tool for the diagnosis of tuberculosis.

This prospective study evaluated the incidence of Mycobacterium tuberculosis infection among nursing students at the Federal University of Espírito Santo, using the purified protein derivative test. In parallel, we evaluated whether knowledge on tuberculosis transmission mechanisms had any impact on nursing students' attitudes in relation to the risk of tuberculosis infection. The incidence of tuberculin conversion was 10.5% per year (p=0.035, 95% CI = 3.63-17.43), whereas the expected conversion rate in the overall population in Brazil is 0.5% per year. These results indicate nursing students as a group at risk for TB infection. Among the risk factors studied only the use of NIOSH95 masks was associated with protection against infection (RR=0.2). Furthermore there was no statistical difference among students that PPD converted and those who remained negative regarding disease knowledge and the existence of adequate facilities for patient hospitalization. Our data reinforces the need for implementing special biosafety programs targeting this professional group.

This open comparative study for evaluating hypersensitivity to tuberculin among medical students (n=441), nursing students (n=178) and economics students (n=230) in Vitória, State of Espírito Santo, had the aim of assessing whether the prevalence of positive tuberculin tests among healthcare students could be used as a sentinel event for nosocomial transmission in localities with high endemicity. There was a significant change in the frequency of purified protein derivative reactivity in the nursing students (20. 3%) and medical students (18. 4%), in comparison with the economics students (6%) (p <0. 001). As expected, the medical and nursing students had more knowledge about tuberculosis than did the economics students (p <0. 001). Our data suggest that the incidence of Mycobacterium tuberculosis infection among nursing and medical students in our region, which has high endemicity of tuberculosis, does not differ from what is found in low-incidence countries. These data reinforce the need for implementing biosafety programs for this target population.

RESUMO - A Tuberculose surge, de acordo com o último relatório da Organização Mundial da Saúde, como a segunda principal causa de morte em todo o mundo, de entre as doenças infeciosas. Em 2012, 1.3 milhões de pessoas morreram devido a esta patologia e surgiram 8.6 milhões de novos casos. De entre os grupos de risco de infeção, surgem os profissionais de saúde. A dificuldade no diagnóstico da Tuberculose, o contacto próximo com os pacientes, as medidas de controlo de infeção por vezes inadequadas são algumas das razões que explicam o risco mais elevado de contrair Tuberculose no local de trabalho. Esta Dissertação de Mestrado pretende estabelecer uma nova classificação de risco de infeção por M. tuberculosis em estabelecimentos de saúde, com vista a promover a saúde destes profissionais, inovadora nos critérios de avaliação das medidas de controlo de infeção e de análise dos casos de exposição não protegida a Tuberculose ativa. Esta metodologia de avaliação foi o resultado de uma revisão bibliográfica sobre a temática, tendo sido aplicada num hospital para verificar a sua adequabilidade e mais-valia.

INTRODUCTION: The aim of this work was to evaluate the prevalence of Mycobacterium tuberculosis (MT) strains with mutations that could result in resistance to the main drugs used in treatment in a region with one of the highest numbers of tuberculosis (TB) cases in southern Brazil. METHODS: Deoxyribonucleic acid (DNA) from 120 sputum samples from different patients suspicious of pulmonary tuberculosis who attended the Municipal Public Laboratory for Mycobacterium sp. diagnosis was directly amplified and analyzed by PCR-SSCP. The DNA was amplified in known hotspot mutation regions of the genes rpoB, ahpC, embB, katG, inhA, and pncA. RESULTS: The percentage of samples positive by culture was 9.2% (11/120); 5% (6/120) were positive by bacilloscopy and MT-PCR, and DNA fragments of the aforementioned resistance genes could be amplified from seven (7) of the eleven (11) samples with positive results, either by culture or PCR/bacilloscopy. All presented a SSCP pattern similar to a native, nonresistant genotype, with the ATCC strain 25177 as control, except for one sample (0.01%), which presented a SSCP profile demonstrating mutation at the embB gene. CONCLUSIONS: These results are consistent with the empirical observations by physicians treating TB patients in our region of a low occurrence of cases that are refractory to conventional treatment schemes, in contrast to other parts of the country. Continued surveillance, especially molecular, is essential to detect and monitor the outbreak of MT-resistant strains.
