OBJECTIVES: Previous literature suggests that early psychosis (EP) patients with a history of offending behavior (HOB) have specific clinical needs. The aims of this study were to assess: (1) the prevalence of HOB in a representative sample of EP; (2) the premorbid and baseline characteristics of patients with HOB, and (3) the potential differences in short-term outcome of such patients when compared to patients without HOB. METHODS: The Early Psychosis Prevention and Intervention Centre (EPPIC) admitted 786 EP patients between 1998 and 2000. Data were collected from patients' files using a standardized questionnaire. Data of 647 patients could be analyzed. RESULTS: HOB patients (29% of the sample) were more likely to be male with lower level of premorbid functioning and education, have used illicit substances and have attempted suicide. They presented with a more complex clinical picture and had poorer 18-month outcome. Most importantly, they had a significantly longer duration of untreated psychosis. CONCLUSIONS: On the basis of the high prevalence and specific features of EP patients with HOB, our study confirms a need for additional research in this domain and for the development of specific treatment strategies. Most importantly, it suggests a need for the promotion of early detection strategies among the populations of young offenders, considering that some of them may be going through the early phases of a psychotic disorder and that reduction of treatment delay and provision of well adapted interventions may have a significant impact at numerous levels in such patients.

  The Department of Health has published a White Paper on Universal Health Insurance. The White Paper sets out in detail the elements of the proposed Universal Health Insurance model for Ireland. As such, it provides detail on the overall design of the model, the proposed system for deciding on the standard package of services and the financing mechanisms for the system. This is a most fundamental reform of the health system and we recognise the importance of consulting extensively and inclusively with all interested parties.  It is important to seek your views on the policy as it is set out in the White Paper, and we view this as a valuable opportunity for citizens to contribute to the development of policy on the future of their health system.  Therefore, we would like to hear from any individual, group, organisation or other body that wishes to contribute to the consultation on the White Paper. In particular, but not limited to, we would welcome your views on the following issues: A consultation document setting out a number of key questions under each of the above headings has been developed and can be downloaded here. There is an opportunity at the end of the document for views or comments on other aspects of the White Paper to be provided. Alternatively, additional views or comments can be sent as an email or hard copy to the addresses below. It is intended to establish a separate independent Expert Commission to examine the issues around the basket of services to be provided under UHI and within the overall health system. The Minister will announce details of the Commission in the near future. Therefore, it would be useful if the submissions on the White Paper refrained from commenting in detail on the services to be provided under UHI. Views on the basket of services will be sought by the Commission when it commences its consultation process. Descriptive epidemiology research involves collecting data from large numbers of subjects. Obtaining these data requires approaches designed to achieve maximum participation or response rates among respondents possessing the desired information. We analyze participation and response rates in a population-based epidemiological study though a telephone survey and identify factors implicated in consenting to participate. Rates found exceeded those reported in the literature and they were higher for afternoon calls than for morning calls. Women and subjects older than 40 years were the most likely to answer the telephone. The study identified geographical differences, with higher RRs in districts in southern Spain that are not considered urbanized. This information may be helpful for designing more efficient community epidemiology projects.

RESUMOO Neste artigo, analisa-se como se apresenta o comprometimento organizacional de trabalhadores de um call center, localizado em Belo (A Horizonte (Minas Gerais, Brasil). Após o delineamento conceitual UJ do tema central, são expostos os resultados de um estudo de caso descritivo, realizado com abordagens quantitativa e qualitativa. Os dados de 399 questionários e 22 entrevistas são, respectivamente, tratados estatisticamente e submetidos à análise de conteúdo. A base de comprometimento que predominou entre esses infoproletários foi "obrigação pelo desempenho" e, em menor grau, "afetiva". Foi observado que quanto maior é o seu tempo de experiência nesse tipo de organização, menores são os níveis de comprometimento de modo geral, fatos esclarecidos, parcialmente, por meio das entrevistas.

Since the begínning ofAdvertising, art has been a source ofinexhaustible inspiration for the advertising creative, who have used or misused it with any limits. With more or less direct references to artists, pieces or art movements, to references to the act ofart creation itself and certain aesthetic categories. Taking into account the various semantic versions that art offers, with multiple connotative lectures, this use has not been much profitable in many occasions, not as much as it could have been. However, in other circumstances, thís use has been vampirizing, only preoccupied for the audience impact ofa well knowll reference, but despising it and reducing it to a mere reclaim. In the case ofGiocconda by Leonardo da Vinci or the Birth ofVenus by Bottícelli are significant examples ofthis use, maybe popular, ofthe art productíon. That is to say that the depository was used as a source ofreferences to enrich the lectures ofthe advertising pieces (for instance, the excellent Citroen Xsara Picasso advertisement with the fordism mass production chain) or ¡ust to allow a higher t1Otoriety (like the Chupa-Chups advertisement where the Mona Lisa heartily sucks one ofthe mythical sweets with a stick). Thanks to a selected advertising pieces, graphical and audiovisual, we will be able to go through this kind ofadvertising creation logistics, from the moment that this one decided to call out these art references that were sleeping dreamingly Oll the fair in a depository until this strictly cultural moment came.

This paper aims to estimate empirically the efficiency of a Swiss telemedicine service introduced in 2003. We used claims' data gathered by a major Swiss health insurer, over a period of 6 years and involving 160 000 insured adults. In Switzerland, health insurance is mandatory, but everyone has the option of choosing between a managed care plan and a fee-for-service plan. This paper focuses on a conventional fee-for-service plan including a mandatory access to a telemedicine service; the insured are obliged to phone this medical call centre before visiting a physician. This type of plan generates much lower average health expenditures than a conventional insurance plan. Reasons for this may include selection, incentive effects or efficiency. In our sample, about 90% of the difference in health expenditure can be explained by selection and incentive effects. The remaining 10% of savings due to the efficiency of the telemedicine service amount to about SFr 150 per year per insured, of which approximately 60% is saved by the insurer and 40% by the insured. Although the efficiency effect is greater than the cost of the plan, the big winners are the insured who not only save monetary and non-monetary costs but also benefit from reduced premiums.
