O prognóstico da gestação é influenciado pelo estado nutricional materno. Sabe-se que, desde a infância, as pessoas portadoras da doença falciforme apresentam crescimento deficiente. Pouco se conhece sobre a evolução do estado nutricional materno na anemia falciforme. O objetivo foi analisar o estado nutricional de gestantes portadoras de anemia falciforme, avaliando o ganho de peso, o consumo dietético materno e os resultados adversos maternos e perinatais. Métodos: Foram avaliadas 26 gestantes portadoras de anemia falciforme SS e 23 SC, a partir do início do pré-natal especializado até o puerpério. Como grupo controle foram utilizadas 63 gestantes, sem comorbidades. O estado nutricional das gestantes foi avaliado de acordo com índice de massa corporal, pré-gestacional e ao final da gravidez, bem como analisado o ganho de peso materno. O consumo dietético foi analisado utilizando-se questionário de frequência alimentar, em cada trimestre, caracterizando-se o valor energético total e a ingestão de macronutrientes e micronutrientes. As complicações maternas e perinatais foram investigadas. O nível de significância foi 0,05 (alfa=5%). Resultados: O IMC pré-gestacional foi significativamente menor no grupo SS (mediana 20,3 kg/m2) quando comparado com os grupos SC (22,7 kg/m2, p < 0,05) e controle (23,2 kg/m2, p < 0,05). O baixo peso pré-gestacional (IMC < 18,5 kg/m2) foi significativamente mais frequente no grupo SS (15,4%) quando comparado aos grupos SC (4,4%) e controle (1,6%, p=0,009). Ao final da gestação, o grupo SS apresentou menor IMC (mediana 23,1 kg/m2) quando comparado com o grupo SC (26,1 kg/m2, p < 0,05) e controle (28,5 kg/m2, p < 0,05). O ganho ponderal na gestação foi menor no grupo SS (mediana 8,0 kg) quando comparado com o grupo SC (11,9 kg, p < 0,05) e Controle (13,7 kg, p < 0,05). Na análise do consumo dietético no 2º trimestre, constatou-se que os grupos SS e SC apresentaram menor consumo de proteínas (medianas, 73 g/d e 69 g/d) quando comparados aos controles (96 g/d, p < 0,05); e o consumo de cálcio foi menor no grupo SS comparado ao controle (mediana, 410 vs. 748 g/d, p<0,05). No 3º trimestre, houve menor consumo de proteínas pelo grupo SS quando comparado com os controles (mediana, 68 g/d vs. 93 g/d, p < 0,05); o consumo de vitamina A foi menor no grupo SS quando comparado aos controles (mediana, 447 vs. 940 mcg/d, p < 0,05); o consumo de vitamina E foi menor no grupo SS (mediana, 6 mg/d) quando comparado aos grupos SC (10 mg/d) e controle (7 mg/d, p < 0,05). As complicações maternas mais frequentes foram: crise álgica, 58% no grupo SS e 44% no SC (p=NS); infecção urinária, 31% no SS e 17% no SC (p=NS); e infecção pulmonar, 35% no SS e 9% no SC (p=NS). Houve diferença significativa no diagnóstico de sofrimento fetal: SS (36%), SC (14%) e controle (13%, p=0,032). Resultados perinatais adversos foram mais frequentes nos grupos SS e SC quando comparados aos controles. Conclusões: O estado nutricional das gestantes portadoras de doença falciforme SS caracterizou-se pela desnutrição materna, com baixo ganho ponderal na gestação. O consumo dietético de macronutrientes demonstrou ingestão inadequada de proteínas nos 2º e 3º trimestres. As complicações maternas e resultados perinatais adversos foram eventos frequentes nas gestantes portadoras de doença falciforme

Opticians and audiologists tend to see the same people. Many factors that are associated with poorer vision, whether hereditary, pre-natal, or post-natal, are also associated with poorer hearing. The most common factor is simply old age. According to the Royal National Institute for Deaf People (RNID), more than half of all people aged over 60 have some degree of hearing loss and are gradually losing their hearing as part of the ageing process – a process known as presbycusis.

The gestation process, in general, is a very important event on a woman’s life and it brings phisical, phisiological and emotional changes, which by itself is an experience full of intense feelings. By late-aged pregnancy we mean those which occurs at the age of 35 or further. The occurance of this type of pregnancy is rising in Brasil and throughout the world, factors such as, better access to birth control resources and the search for financial stability explains the pregnancy delay. Important processes like resilience and social support can help late-aged pregnant women, in a benefical way, to adapt to the gestation process. Resilience is the capacity that a certain individual or group of individuals have to go through an adverse situation, be able to overcome it and become streghtened, transforming it in motivation for its biopsichosocial development. Social support is a complex and dinamic process that involves transactions between individuals and their social networks, meeting the social needs, promoting and complementing the personal resources that they have to face new demands. This research has the intention of raising information about the issues of late-aged pregnant women in the County of Natal- RN, the main objective was to evaluate the resilience indicators and the social support on late-aged pregnant women in the Natal-RN County. A transversal cut, correlational and descriptive research that was done with 150 lateaged pregnant women. The tools that were used were: A form with sociodemographic and gestation info, the scale of resilience and social support. An eletronic spreadsheet sofware (Excel e SPSS 21.0) was used to analize data which helped on the statistics according to its variables and the objective of this work. For the nominal variables, relative frequencies were used and for continuous the Pearson correlation and determination coefficient were used, regarding that; the sample had a normal distribution. The project fulfilled the ethnic aspects prescribed by Resolution 466/12 of the National Health Council, with a favorable decision (356.436/ 2013) of the UFRN Ethics on Research Committee. Most of the pregnant women had a low money income and education level, born in the state of Rio Grande do Norte they had an average age of 37,49 (±2,577), catholic, married, house wives, they had more than one child and were on their third trimester of pregnancy; they also had a low past abortion rate, not having planned their pregnancy, with an average of 4,22 (±2,506) pre-natal appointments, residing with an average of 3,673 (±1,397) people, having used any sort of birth control device and having high indicators of resilience and social support. The correlations kept between resilience, social support and some of the social demographics and gestation variables were considered low. Such data points out the fact that most of these women were in a stable relationship; they hadn’t had a past of abortion, they were involved with some kind of religion, they were not first pregnancy mothers, had an age on which they are not considered inexperienced mothers and even had scored high on the social support scale, these may all possibly be the most contributing factors on development and resilience building on these 35 years or more mothers. We expect that the data and information from this research may add up knowledge, actions and improvements regarding late-aged pregnant women and the pregnancy phenomena in general.

Research has consistently found sex differences in mental rotation. Twin research has suggested that females with male co-twins perform better than females with female co-twins on mental rotation. Because twins share both pre-natal and post-natal environments, it is not possible to test whether this advantage is due to in-uterine transmission of testosterone from males to females or due to socialisation processes. The present study explored whether the advantage of females with brothers can be observed in non-twin siblings. Participants (N = 1799) were assessed on mental rotation. The observed group differences were overall small: males performed significantly better than females; females with sisters performed similarly to females with brothers; importantly, males with brothers performed significantly better than both female groups. The results suggest that sex differences in mental rotation are driven by the group of males with brothers.

Los socorristas de traumatizados en el área pre-hospitalaria en la ciudad de Machala, carecen de profesionales con formación académica de tercer nivel, en su mayoría son bachilleres con cursos básicos de primeros auxilios Objetivo.- Determinar el impacto de un programa educativo en el nivel de conocimiento de los protocolos de atención en trauma, usados por los socorristas que laboran en atención pre-hospitalaria de la ciudad de Machala. Métodos.-Realizamos un estudio cuasi experimental con dos grupos: Grupo estudio (n: 46) y grupo control (n: 45). Muestra obtenida de forma no aleatoria por conveniencia. Los socorristas fueron evaluados al inicio del estudio por un test previamente establecido, al grupo de estudio se aplicó un programa educativo, al grupo control no se lo intervino. Dos meses después se evaluó el impacto del programa. Resultados.- El programa educativo mejora el conocimiento global un 43% en el grupo de estudio. El impacto del programa de la atención pre-hospitalaria demostró un incremento en conocimientos: en respiratorio 56%; 67.9% en circulatorio y el 19.2% en neurológico; pero disminuye: en vía aérea 10.9% y 7.6% en la exposición de la víctima. El Riesgo relativo luego de la intervención es de 4.45 y se necesita aplicar el programa educativo a dos personas para mejorar una (NNT -2.4). Conclusión.- El programa educativo es útil para mejorar el conocimiento sobre atención pre-hospitalaria en trauma, sin lograr en los participantes alcanzar el nivel esperado por lo heterogéneo de su formación académica

A intubação endotraqueal (IET), no pré-hospitalar, é considerada o “gold standard” na manutenção da via aérea de modo a fornecer a ventilação e oxigenação à pessoa em situação crítica, no entanto, está associada a várias complicações e riscos. Com este trabalho de revisão sistemática da literatura pretendeu-se compreender a relação entre a IET em emergência pré-hospitalar e o prognóstico da pessoa em situação crítica, colocando a seguinte questão: “Qual o impacto da intubação endotraqueal, em emergência pré-hospitalar, no prognóstico clínico do doente?”. Este trabalho de investigação iniciou-se com uma pesquisa da literatura de língua inglesa e portuguesa, nas bases de dados científicas MEDLINE, CINAHL, MedicLatina e Nursing and Allied Health Collection num horizonte temporal entre 2010 e 2014. A evidência destaca que a realização da intubação endotraqueal, no pré-hospitalar, está associada a piores taxas de reanimação cardiorrespiratória, ao aumento das taxas de mortalidade e ao aumento das comorbilidades. Estas conclusões reforçam a discussão em torno da prática da IET no pré-hospitalar e sensibilizam os profissionais de saúde para a limitação desta prática a casos em que exista indicação clínica segura.

O cancer de mama e o segundo tipo mais frequente do cancer no mundo, o segundo que mais aparece na populaçao feminina, e que constitui a quarta causa de morte de mulheres com cancer no Brasil. O trabalho apresentado trata de uma intervençao que tem como objetivo melhorar e qualificar a atençao do cancer de colo de utero e controle de cancer de mama, na UBS Rodrigues , Lagoa Vermelha R/S. A intervençao foi realizada de abril a junho, no ano 2015, em 12 semanas. a equipe foi capacitada de acordo com o protocolo do Ministerio da Saude,na area adstrita a UBS residem 1287 mulheres mais atençao a entre 25 e 64 anos de idade, e 517 mulheres entre 50 e 69 anos, tinhamos como metas cadastrar 80% das usuarias para o controle de cancer de mama e utero,, participam um total de 499mulheres durante as 12 semanas de intervençao, 363 {28%] na faixa etaria de 25 a 64 anos, e 136{26%} na faixa etaria de 50 a 69 anos.Todas as mulheres cadastradas estavam com coleta satifatoria de exame citopatologico de colo de utero nos tres meses da intervençao, alcanzando assim 100% de cobertura e todas receberam orientalçoes sobre doenças sexuaimente transmissiveis.A intervençao foi importante para o equipe , ja que as açoes foram incorporadas a rotina do serviço, a mesma reviu as atribuiçoes da equipe, viabilizando atençao a um maior grupo de pessoas, melhoria em os registros mais atençao a demanda espontanea e classificaçao de risco.Pretendemos implementar outras açoes prioritarias, como atençao a hipertensos, diabeticos, pre natal e puerperio. ,

The period between offspring birth and recruitment into the breeding population is considered one of the least understood components of animal life histories. Yet, examining this period is essential for studies of parental care, dispersal, demography, and life histories. Studies of the pre-reproductive period are particularly few in tropical regions, where the organization of life histories are predicted to differ compared to northern hemisphere species. For my dissertation I used radio-telemetry, mark-resighting, and field observations to study the pre-reproductive period in a Neotropical bird, the western slaty-antshrike (Thamnophilus atrinucha), in Panama. First, I found that parental care after offspring left the nest (the post-fledging period) was greater than care during the nestling period. Prolonged care resulted in a clear trade-off for parents as they did not nest again until fledglings from the first brood were independent. Parents fed offspring for a prolonged duration during the post-fledging period and higher post-fledging survival was observed compared to many northern hemisphere species. Second, I observed that offspring that remained with parents for longer periods on the natal territory had higher survival both while on the natal territory and after dispersal compared to those dispersing earlier. Parental aggression towards offspring increased with offspring age and offspring dispersed earlier when parents renested. Contrary to other family living species, only a small proportion of antshrike offspring remained on the natal territory until the following year and all dispersed to float. Floating is when juveniles wander within other breeding pairs’ territories. These results suggest that the benefits of delayed dispersal declined with offspring age and with renesting by parents. Third, I observed that survival during the dependent period and first year was greater in slaty antshrikes compared to that of northern hemisphere species. Pre-reproductive survival relative to adult survival was equal or greater than that observed in northern hemisphere species. The date offspring left the nest, mass, and age at dispersal influenced offspring survival, whereas offspring sex and year did not. Relatively high survival during the pre-reproductive period coupled with comparatively low annual productivity clarifies how many tropical species achieve replacement. High juvenile survival appears to obtain from extended post-fledging parental care, delayed dispersal, low costs of dispersal, and a less seasonal environment. Lastly, I experimentally manipulated begging at the nest to examine changes in parental behavior. Under elevated begging, parents increased provisioning rates and reduced the time between arrival to the nest and feeding of nestlings, potentially to reduce begging sounds. Furthermore, parents switched to preferentially feed the closest offspring during the begging treatment. This suggests parents either allowed sibling competition to influence feeding decisions, or feeding the closer nestling increased the efficiency of provisioning. In summary, I found that slaty antshrikes have delayed age at reproduction, higher post-fledging and first year survival, extended post-fledging parental care, equal or greater pre-reproductive survival relative to adult survival, and delayed dispersal compared to many northern hemisphere passerines. These results suggest that this tropical species has a strategy of high investment into few offspring. Furthermore, reproductive effort is equal or greater at least in slaty antshrikes compared to northern hemisphere species, suggesting that the latitudinal gradient in clutch size is not explained by a gradient in reproductive effort.

The mandible has a mixed embryological origin, and its growth is associated with the secondary cartilage of the condyle process (CP). In this area, growth depends on an array of intrinsic and extrinsic factors that influence protein metabolism. In the present study, we used an adolescent rat model to evaluate the growth and development of the CP under conditions of pre- and postnatal protein deficiency, combined with or without the stress of severe burn injury (BI). We found that protein deficiency severely undermined the growth of the CP, by altering the thickness of its constituent layers. BI is also capable of affecting CP growth, although the effect is less severe than protein deficiency. Interestingly, the summed effect of protein deficiency and BI on the CP is less severe than protein deficiency alone. A possible explanation is that the increased carbohydrates in a hypoproteic diet stimulate the production of endogenous insulin and protein synthesis, which partially compensates for the loss of lean body mass caused by BI.

Clinical trials comparing different drug regimens and strategies for the treatment of congenital toxoplasmosis and its clinical manifestations in the liveborn child in different clinical settings should aim at formally evaluating the net benefit of existing treatments and at developing new therapeutic options. Currently, there is no ideal drug for congenital toxoplasmosis; future research should focus on the screening of new active drugs and on their pre-clinical and early clinical development, with a focus on pharmacokinetic/dynamic studies and teratogenicity. For the prenatal treatment of congenital toxoplasmosis, a trial comparing spiramycine to pyrimethamine-sulphadiazine and placebo would allow a formal estimation of the effect of both drugs in infected pregnant women. In newborn children, the net benefit of pyrimethamine-sulphadiazine should also be formally assessed. These trials will be implemented in settings where prenatal screening for Toxoplasma gondii is currently implemented. Trials should be carefully designed to allow for translation to other settings and modelling tools like cost-effectiveness analysis should be used to provide clinicians and founders with the best available evidence to establish recommendations.

Genetic background, prenatal and post-natal early-life conditions influence the development of interconnected physiological systems and thereby shape the phenotype. Certain combinations of genotypes and pre- and post-natal conditions may provide higher fitness in a specific environmental context. Here, we investigated how grey partridges Perdix perdix of two strains (wild and domesticated) cope physiologically with pre- and post-natal predictable vs. unpredictable food supply. Food unpredictability occurs frequently in wild environments and requires physiological and behavioural adjustments. Well-orchestrated and efficient physiological systems are presumably more vital in a wild environment as compared to captivity. We thus predicted that wild-strain grey partridges have a stronger immunity, glucocorticoid (GC) stress response and oxidative stress resistance (OSR) than domesticated birds, which have undergone adaptations to captivity. We also predicted that wild-strain birds react more strongly to environmental stimuli and, when faced with harsh prenatal conditions, are better able to prepare their offspring for similarly poor post-natal conditions than birds of domesticated origin. We found that wild-strain offspring were physiologically better prepared for stressful situations as compared to the domesticated strain. They had a high GC stress response and a high OSR when kept under predictable food supply. Wild-strain parents reacted to prenatal unpredictable food supply by lowering their offspring's GC stress response, which potentially lowered GC-induced oxidative pressure. No such pattern was evident in the domesticated birds. Irrespective of strain and prenatal feeding scheme, post-natal unpredictable food supply boosted immune indices, and GC stress response was negatively related to antibody response in females and to mitochondrial superoxide production. Wild-strain grey partridge showed fitness-relevant physiological advantages and appeared to prepare their offspring for the prospective environment. Negative relationships between GC stress response, immunity and oxidative indices imply a pivotal role of an organism's oxidative balance and support the importance of considering multiple physiological systems simultaneously.
