O presente texto trata de um estudo de caso que foi norteado pelo seguinte problema: “Qual a influencia dos fatores depressivos no trabalho do enfermeiro psiquiátrico?”. Nesse sentido, verificar os fatores responsáveis pela dualidade sofrimento e prazer no trabalho do enfermeiro psiquiátrico, levando em consideração os fatores depressivos existentes nesta profissão, será a questão mostradora desta pesquisa, que tem como base à teoria de Cristoph Dejours. A coleta de dados foi realizada por meio de um roteiro composto de questões fechadas com o intuito de caracterizar os entrevistados. Na seqüência, as entrevistas sofreram um processo de semi-estruturação, primeiramente, gravadas e posteriormente, transcritas. Essa coleta utilizou-se de sete enfermeiros da categoria psiquiátrica da unidade psiquiátrica do hospital geral (UPHG), ligada a uma Instituição Federal de Ensino Superior localizada na cidade de Santa Maria no interior do Estado do Rio grande do Sul. Para análise das respostas referentes às questões das entrevistas, foi utilizada a metodologia da Análise de Conteúdo. Os resultados estão apresentados em quatro categorias, que foram: “Trabalho em linhas gerais”, relata o trabalho num sentido mais amplo; “A valorização do bem-estar do enfermeiro”, que ressalta a importância de se ter um ambiente agradável de trabalho; “A busca do prazer no trabalho”, que mostra caminhos para a busca do prazer no trabalho e por fim “Ações preventivas para combater a depressão no ambiente de trabalho”, que compreende formas de evitar aspectos depressivos na realidade do enfermeiro psiquiátrico.

In Brazil, the mental health network proposed by the Psychiatric Reform inserts the intermediate and replacement services in the pursuit of alignment or resocialization of patients with mental and behavioral disorder in the community. Was adopted, among other services, the Center for Psychosocial Care, Home Therapy, Sheltered Home, Day Hospital and psychiatric beds in general hospital. In this context, the State of Rio Grande do Norte implanted the Day Hospital Dr. Elger Nunes (HDEN) in Natal / RN in 1996, linked to State Department of Public Health. At HDEN happened a multi and interdisciplinary therapeutic work, besides being the scene of disciplinary practices, and extension projects for graduate courses in Higher Education Institutions in the city. However, with the process of decentralization of local services, the hospital was terminated by an administrative state act in 2006, leaving damage to the activities provided to users, disciplinary practices and extension activities. From this breakdown, the objective was to narrate the trajectory of HDEN through a multidisciplinary team of professionals and teachers who used it as a field of disciplinary practices. It is characterized as a documental and qualitative, backed in the technique of thematic oral history, following the phases: authorization of the interviewee, interview recording, transcription, textualization and transcreation of the material obtained. We used documents, ordinances, general reports of activities, among others, plus interviews to fifteen employees who used this service, being thirteen part of the multidisciplinary team of professionals and two graduation professors of health care area, nursing and medicine. The stories collected were organized according to the technique chosen, respecting its steps. In preparing the body subjected to ALCESTE computer program, priority was given to the vital tone for the formation of categories and classes elected by the program, structured in three thematic areas. In the first axis, called Trajectory of HDEN, were recalled the beginning of its activities, the steps of that time, their activities, and its actors - users, families, professionals, and teaching practices. The second axis has dealt with the process of extinction of HDEN, rescuing the feelings of employees, the main reasons given at the time and immediate postextinction scenario. And the third axis revealed in an articulated form the situation of mental health in Natal / RN, listing to the challenges and prospects for the psychosocial care, starting from the trajectory of HDEN with emphasis on activities. Moreover, the trajectory of HDEN provides recognition of the historical basis outlined in the constitution of the network of substitute services present in the current scenario of psychosocial care in the city of Natal and in RN.

In this article I introduce the therapeutic potential that the psychodrama gives to the treatment of the patients with severe mental disorder. I propose the specific characteristics that, from the clinic practice in a Psychiatric Day Hospital of Madrid, have specially revealed fertile to the persons that participate in and also to the community framing and its assembly in the framework of a transdisciplinary work.

Introdução: A recusa escolar, definida como ausência escolar por causas emocionais, é o motivo de encaminhamento mais frequente para o Hospital de Dia de Adolescentes da Clí- nica da Juventude. Objectivos: Este estudo pretendeu caracteri- zar os jovens admitidos por recusa escolar e avaliar a sua situação 27 a 60 meses após a sua admissão no Hospital de Dia da Clínica da Juventude. Métodos: Consulta dos processos clínicos de todos os doentes admitidos entre 01 de Janei- ro de 2010 e 31 de Julho de 2012, de modo a avaliar as variáveis: motivo de admissão, sexo, idade, retenções escolares prévias, diag- nóstico, presença de psicopatologia parental, duração do seguimento e número de sessões efectivadas. Foi, ainda, realizado um ques- tionário por via telefónica a todos os doentes que tinham sido admitidos por recusa esco- lar, 27 a 60 meses após a admissão, de modo a apurar a situação escolar/laboral actual, manutenção de seguimento psiquiátrico e sintomatologia emocional e comportamen- tal actual.

O presente relatório descreve as atividades de estágio desenvolvidas no âmbito do Ramo de Aprofundamento de Competências Profissionais (RACP) do mestrado em Reabilitação Psicomotora da Faculdade de Motricidade Humana. O estágio profissionalizante realizou-se no serviço de psiquiatria do Hospital Prof. Doutor Fernando Fonseca, e contou com sessões de observação participada e intervenção psicomotora na equipa de pedopsiquiatria e de psiquiatria de adultos. O relatório inclui um enquadramento teórico com uma revisão legal da saúde mental e o enquadramento profissional do psicomotricista, da prática psicomotora em saúde mental e uma revisão de literatura das psicopatologias mais prevalentes, destacando os principais objetivos da psicomotricidade em cada uma delas. Inclui também o enquadramento da prática profissional, no qual se descreve a organização das atividades de estágio, a caracterização dos utentes acompanhados e do processo terapêutico com dois estudos-caso. Este estágio permitiu a participação da aluna numa equipa multidisciplinar, e contribuiu positivamente para a sua formação profissional e pessoal, no âmbito dos objetivos definidos pelo RACP no contexto da prática em saúde mental. Considerando as evoluções observadas nos diferentes utentes acompanhados conclui-se que a intervenção psicomotora realizada foi pertinente quer no contexto pedopsiquiátrico, quer no contexto psiquiátrico.

Hospital acquired infections (HAI) are costly but many are avoidable. Evaluating prevention programmes requires data on their costs and benefits. Estimating the actual costs of HAI (a measure of the cost savings due to prevention) is difficult as HAI changes cost by extending patient length of stay, yet, length of stay is a major risk factor for HAI. This endogeneity bias can confound attempts to measure accurately the cost of HAI. We propose a two-stage instrumental variables estimation strategy that explicitly controls for the endogeneity between risk of HAI and length of stay. We find that a 10% reduction in ex ante risk of HAI results in an expected savings of £693 ($US 984).

Objective: To compare the effectiveness of the STRATIFY falls tool with nurses’ clinical judgments in predicting patient falls. Study Design and Setting: A prospective cohort study was conducted among the inpatients of an acute tertiary hospital. Participants were patients over 65 years of age admitted to any hospital unit. Sensitivity, specificity, and positive predictive value (PPV) and negative predictive values (NPV) of the instrument and nurses’ clinical judgments in predicting falls were calculated. Results: Seven hundred and eighty-eight patients were screened and followed up during the study period. The fall prevalence was 9.2%. Of the 335 patients classified as being ‘‘at risk’’ for falling using the STRATIFY tool, 59 (17.6%) did sustain a fall (sensitivity50.82, specificity50.61, PPV50.18, NPV50.97). Nurses judged that 501 patients were at risk of falling and, of these, 60 (12.0%) fell (sensitivity50.84, specificity50.38, PPV50.12, NPV50.96). The STRATIFY tool correctly identified significantly more patients as either fallers or nonfallers than the nurses (P50.027). Conclusion: Considering the poor specificity and high rates of false-positive results for both the STRATIFY tool and nurses’ clinical judgments, we conclude that neither of these approaches are useful for screening of falls in acute hospital settings.

Objective-To establish the demographic, health status and insurance determinants of pre-hospital ambulance non-usage for patients with emergency medical needs. Methods-Triage category, date of birth, sex, marital status, country of origin, method and time of arrival, ambulance insurance status, diagnosis, and disposal were collected for all patients who presented over a four month period (n=10 229) to the emergency department of a major provincial hospital. Data for patients with urgent (n=678) or critical care needs (n=332) who did not use pre-hospital care were analysed using Poisson regression. Results-Only a small percentage (6.6%) of the total sample were triaged as having urgent medical needs or critical care needs (3.2%). Predictors of usage for those with urgent care needs included age greater than 65 years (prevalence ratio (PR)=0.54; 95% confidence interval (CI)= 0.35 to 0.83), being admitted to intensive care or transferred to another hospital (PR=0.62; 95% CI=0.44 to 0.89) or ward (PR=0.72; 95% CI=0.56 to 0.93) and ambulance insurance status (PR=0.67; 95% CI=052 to 0.86). Sex, marital status, time of day and country of origin were not predictive of usage and non-usage. Predictors of usage for those with critical care needs included age 65 years or greater (PR=0.45; 95% CI=0.25 to 0.81) and a diagnosis of trauma (PR=0.49; 95% CI=0.26 to 0.92). A non-English speaking background was predictive of non-usage (PR=1.98; 95% CI=1.06 to 3.70). Sex, marital status, time of day, triage and ambulance insurance status were not predictive of non-usage. Conclusions-Socioeconomic and medical factors variously influence ambulance usage depending on the severity or urgency of the medical condition. Ambulance insurance status was less of an influence as severity of condition increased suggesting that, at a critical level of urgency, patients without insurance are willing to pay for a pre-hospital ambulance service.
