O desenvolvimento dos modelos de produção acarretou diversas transformações na concepção da relação homem/trabalho. Com isso, o trabalho tornou-se um dos aspectos centrais na vida do homem moderno. Na relação com o trabalho, emergem diversos processos de subjetivação baseados nas práticas presentes nos contextos em que ele se realiza, bem como nos processos de saúde e doença. No Brasil, esse processo vem sendo delineado por questões políticas e sociais que levaram à emergência da chamada Política Nacional de Segurança e Saúde do Trabalhador, em 2004. Entretanto, esse tema e seus desdobramentos ainda são fortemente debatidos, uma vez que tal política não se encontra em intenso vigor, demonstrando um percurso em constante construção e ainda permeável à diferentes influências. Este trabalho busca problematizar as práticas que produzem processos de subjetivação do “sujeitotrabalhador” pautados em dispositivos biopolíticos, a partir da análise da gestão do cuidado em saúde do trabalhador no Brasil. Partindo dessa perspectiva, buscou-se analisar a construção das Políticas de Saúde do Trabalhador no país, focando a formulação da PNSST e sua perspectiva atual. Para isso, foram analisadas as estratégias de cuidado presentes nesta Política, bem como a forma como essas estratégias estão articuladas com a perspectiva da integralidade, uma vez que a integralidade é um novo olhar sobre a gestão do cuidado em saúde, criando novas possibilidades de um trabalho em saúde. Centra-se no fluxo do usuário, com mudanças na produção do cuidado em todos os níveis da rede pública de saúde. Primeiro, foram abordados os processos de construção e desenvolvimento da chamada “Saúde do Trabalhador” e, posteriormente, foi analisada a Política Nacional de Segurança e Saúde do Trabalhador – PNSST (2004) enquanto dispositivo de regulação das práticas de saúde, a partir do método genealógico de Michel Foucault, com foco na análise documental. O processo de construção da PNSST iniciou a partir da 1ª Conferência Nacional em Saúde do Trabalhador, o qual se delineou nas demais conferências realizadas de 2001 e 2005. Neste processo, podemos observar o conflito entre o trabalho como risco (trabalho-risco) e o trabalho como produção de subjetividade (trabalho-subjetividade), que levam a construção das noções entre saúdecontrole versus saúde-integralidade. A análise documental da PNSST de 2004 denota que ainda há uma prevalência do olhar da Saúde Ocupacional, pelo viés do trabalho-risco/saúdecontrole, uma vez que as estratégias de cuidado apresentam discursos de risco/agravo no trabalho, na patologização do sujeito e na monetarização da saúde. Além disso, de 2005 até meados de 2011, não houve a concretização e implantação da Política, sendo criados diferentes sentidos e, inclusive, convergindo as ações de Saúde do Trabalhador para o campo da Vigilância em Saúde, onde se encontra tal área no Ministério da Saúde hoje. Com isso, observamos que ao pensarmos na proposta de articular o campo da integralidade com a Saúde do Trabalhador, encontramos, na verdade, a construção de discursos pautados em estratégias biopolíticas de transformar a atividade laboral em risco que deve ser vigiado e medicalizado. Além disso, não há interface para absorção das demandas referentes à saúde do trabalhador no SUS. A criação de linhas de cuidado em Saúde do Trabalhador em Unidades Básicas de Saúde ou mesmo a criação de Unidade de Referência Especializada em Saúde do Trabalhador nos permite inserir este tema cada vez mais no campo da saúde pública no Brasil e diminuir a dispersão dos casos de sofrimento dos trabalhadores que ficam no nível do não dito.

A hanseníase, doença milenar, ainda constitui um grande desafio para a saúde pública, inclusive no que se refere a diagnóstico precoce e vigilância em saúde. Com objetivo de estimar a soroprevalência de anticorpos antiPGL-1 através do teste rápido ML-Flow em casos de hanseníase e seus contatos intradomiciliares de municípios endêmicos do Pará, realizou-se um estudo transversal incluindo 73 casos novos de hanseníase e 135 contatos intradomiciliares, selecionados no período de abril de 2011 a janeiro de 2012, nas unidades de referência para tratamento de hanseníase nos municípios de Belém, Marituba, Igarapé-Açú e Santarém. Os resultados demonstraram uma prevalência de 14,8/10.000hab de casos de hanseníase entre os contatos examinados. A soropositividade do ML Flow nos casos índices foi de 53,42% em pacientes multibacilares e 13,33% nos contatos intradomiciliares. Houve associação direta de positividade do ML Flow nos contatos intradomiciliares com o índice baciloscópico do caso índice. Não houve associação direta do tempo de convivência e a consanguineidade do caso índice em relação ao teste ML Flow. Houve associação entre a positividade do teste ML Flow e a realização da vacina BCG entre os contatos. Estes resultados indicam que a introdução do teste ML Flow poderia servir como instrumento auxiliar no monitoramento dos casos e seus contatos, tornando-se de grande relevância na vigilância epidemiológica da hanseníase.

A Leishmaniose é uma doença causada pelo protozoário Leishmania sp, podendo acometer homem e animais dependendo da espécie do parasita, São transmitidos pelos flebotomíneos fêmeas, insetos do gênero Lutzomyia, que ao exercer o hematofagismo inoculam as formas promastigota infectantes, mas recentemente, tem sido levantado hipóteses sobre a transmissão por carrapatos. Segundo a vigilância epidemiológica de Imperatriz-MA a cidade é endêmica tanto para Leishmaniose Tegumentar (LT) quanto para a Leishmaniose Visceral (LV). Este trabalho teve como objetivo principal investigar a presença de DNA de Leishmania sp em carrapatos coletados de cães atendidos em petshop e Centro de Controle de Zoonoses do município de Imperatriz utilizando a técnica de PCR. O DNA foi extraído a partir de 640 carrapatos fêmeas e testadas utilizando o primer que amplifica o gene de mini-exon de Leishmania sp. Os carrapatos foram coletados de 41 cães de diferentes bairros da cidade de Imperatriz. A maioria dos carrapatos foram identificados como Rhipicephalus sanguineus. Os seguintes sinais clínicos sugestivos de leishmaniose foram observados nos cães: onicogrifose em 53,65% (22/41); úlceras em 63,41% (26/41), a perda de cabelo e inapetência em 39,02% (16/41). Cento e setenta carrapatos (26,56%) coletados de 16 cães apresentaram DNA de Leishmania do subgênero Viannia, responsável pela forma cutânea da doença. Não foi detectado nenhum DNA de Leishmania infantum chagasi. Carrapatos infectados foram coletados de ambos os cães sintomáticos e assintomáticos. Embora ainda não tenha sido demonstrado que os carrapatos possam transmitir Leishmania aos cães sob condições naturais, o resultado deste estudo tem vários aspectos importantes, pois é um método não-invasivo de detecção, capaz de diferenciar os grupos de parasitas em circulação, em especial se os animais não têm lesões, pode ser um indicador biológico em locais onde não é feito uma investigação sorológica e nem entomológica, podendo dar suporte aos programas de vigilância de saúde local.

This article offers theoretical and practical elements for an analysis of the Worker's Health Care Program developed by the Brazilian Unified Health System (SUS). From a determined locus, the region of Franca, State of São Paulo, it aims at identifying the challenges for implementation of the National Occupational Health Policy, due to the particularities of the work/health relationship within the sugarcane agro-industry. It is based on group interviews involving health workers, specifically sanitary inspectors and the worker's health interlocutors. The results showed that health care professionals must be better prepared, from the technical point of view, to be able to perform actions related to workers' health. These results also indicated the need for a greater integration between health services, particularly when damage notification and workers' health surveillance are concerned.

This study aimed at apprehending and analyzing the perspective of Primary Health Care managers concerning nurses’ work in Children’s Health Surveillance in a city in São Paulo state. The study population consisted of eight professionals from different professional categories with direct activity in the city’s management of the population’s Health Surveillance. It is a descriptive, qualitative study. Data were collected by means of recorded semi-structured interviews. The framework used for data analysis was the thematic Content Analysis Method. The results were systematized into three themes: 1- Managers’ conceptualizations concerning Children’s Health Surveillance and its application in practice; 2- Managers’ perspectives concerning nurses’ work in Children’s Health Surveillance; 3- Qualification of Children’s Health Surveillance under the view of the municipal management. The conceptualizations concerning Children’s Health Surveillance that were apprehended showed to be convergent as they indicated this model’s appropriateness to identify and prioritize children’s care in vulnerability conditions in the territory where they live. However, some managers did not include, in their statements, health promotion aspects as one of the cornerstones of their managerial action. Nurses were considered to be fundamental in the Children’s Health Surveillance process due to their competencies and responsibilities undertaken in this health provision level. The main difficulties for adequate implementation of Children’s Health Surveillance in Primary Health Care and the proposal to overcome them were pointed out. It was concluded that, under the managers’ perspectives, nurses can greatly contribute to Children’s Health Surveillance in Primary Health Care as members of the health care team; however, to that end, they need professional qualification, structural conditions and institutional support with that regard

The purpose of this work is to provide quality control requirements and security in dental x-rays in order to obtain good quality image which allows the correct diagnosis, which reduces the dose to the patient, mainly due to the repetition of tests, and decreasing cost. The requirements apply to related activities to quality control and procedures using ionizing radiation for diagnostic imaging in dentistry by evaluating a minimum set of parameters to be tested or verified. Quality control follows the Ordinance No. 453 of the Ministry of Health of 06.01.1998, SS Resolution No. 625 of 12.14.1994 and Resolution No. 64 of the Health Surveillance Center – Department of Health of Sao Paulo and National Health Surveillance Agency – Ministry of Health of Brazil. This study was conducted in the city of Marilia, Sao Paulo, along with the Company P&R Consulting and Medical Physics, in a dental clinic of the University UNIMAR in the x-ray equipment used on that site. The physical parameters of the device were tested with the aid of ionization chambers to measure rates of radiation, electrometer to measure rates of time, kV and doses, radiographic films and positioning devices. Finally, this work demonstrates the need and importance of quality control, which one ensures the proper use of x-ray machines, maintaining efficiency and at the same time it reduces the risks to the patient, to the dentist and to the general public

Along with the advance of technology, in terms of the expansion of medical exams that uses the ionizing radiation for diagnosis, there is also the concern about quality control for maintaining quality in radiographic imaging and for delivering low dose to the patient. Based on the Federal Order 453 of the Secretariat of Health Surveillance, which takes account of the practical and justification of individual medical exposures, the optimization of radiological protection, limitation of individual dose, and the prevention of accidents, were done through this paper radiodiagnostic tests on medical equipment in order to accept it or not, according to SVS-453. Along with the help and support of P&R Consulting and Medical Physics Marilia, SP, were made Quality Control and Radiometric Control in equipment from various cities across the state of São Paulo. The equipment discussed in this work is classified as conventional X-ray. According to the Federal Order SVS-453, the quality control in the program of quality assurance should include the following minimum set of constancy tests, with following minimum frequency: biennial tests for representative values of dose given to the patients of radiography and CT performed in the service; annual tests for accuracy of the indicator tube voltage (kVp), accuracy of exposure time, half-value layer, aligning the central axis of the beam of x-ray tube, performance (mGy / mA.min.m²), linearity of the rate of kerma on air with the mAs, reproducibility of the kerma on air rates, reproducibility of the automatic exposure, focal spot size, integrity of accessories and clothing for individual protection; semiannually for collimation system accuracy; weekly for temperature processing system and sensitometry processing system. After the discovery of ionizing radiation, its applications in various fields of science began to take significant proportions. In the case of medicine, there are the application areas in radiotherapy, diagnostic radiology and nuclear medicine. It was then necessary to create the field of radiological protection to establish the conditions necessary for the safe use of such ionizing radiation. Apply knowledge obtained during the graduation stage and in the practice of radiological protection in the areas of nuclear medicine and diagnostic radiology. In the area of nuclear medicine, tests were made in the Geiger-Muller counters (GM) and the dose calibrator (curiometer), the monitoring tests of radiation, waste management, clean of the Therapeutic room and testing the quality control of gamma-chambers. In the area of radiology, were performed tests of quality control equipment for conventional X-ray equipment and x-ray fluoroscopy, all following the rules of the National Health Surveillance Agency (ANVISA), and reporting of tests. The routine developed in the fields of nuclear medicine in hospitals has proved very useful, since the quality control of GM counters contribute to the values of possible contamination are more reliable. The control of dose calibrator enables the patient not to receive different doses of the recommended amounts, which prevents the repetition of tests and unnecessary exposure to radiation. The management of waste following the rules and laws established and required for its management. Tests for quality control of gamma chambers help to evaluate its medical performance through image. In part of diagnostic radiology, tests for quality control are performed in order to verify that the equipment is acceptable for usage or if repairs are needed. This qualitative study aimed at apprehending conceptualizations and at describing experiences of nurses working in the Family Health Strategy in a medium-sized city in São Paulo state in relation to Children’s Health Surveillance, with a special focus on nursing consultation. Data were collected from 12 nurses by means of recorded semi-structured interviews that were analyzed according to the Thematic Content Analysis Method. Results were systematized into four themes: Conceptualizations about Children’s Health Surveillance; Children’s Health Surveillance in the practice of the Family Health Strategy; Nursing consultation and Children’s Health Surveillance; Proposals to qualify Children’s Health Surveillance in the realm of the Family Health Strategy. It was possible to apprehend amplified and actual conceptualizations of Children’s Health Surveillance as well as to describe experiences surrounded by difficulties to incorporate the premises of this form of Children’s Health Care in the Family Health Strategy. Nursing consultation was noteworthy as a privileged moment, but not unique, to develop such practice. Finally, based on nurses’ proposals, the importance of triggering permanent education processes targeted at these themes in family-health services was considered

Frying is a widely used method of preparing food, because it is a very fast and convenient process, and because it provides specific characteristics of color, flavor, odor, and texture, besides having great consumer acceptance. Therefore, the interest in the physiological effects that oils heated at high temperatures can cause to the human body arises. The aim of this work was to analyze the levels of frying used oil alteration in different kinds of foods served at the university restaurant at “Instituto de Biociências, Letras e Ciências Exatas” (IBILCE), in São José do Rio Preto-SP. In addition, it aims to make the restaurant aware of the good ways to fry and the commitment of providing good quality food to students and professionals of the Institute. With this purpose, determination of total polar compounds, conjugated dienoic acids, peroxide value, and fatty acid profile were analyzed. Two rapid tests were also used: 3M Fat Monitor and Oil Test kit. The results were compared with limits recommended for the disposal of oils and fats used in frying by the Brazilian Health Surveillance Agency (ANVISA), by other countries, and by other researches. Thus, it was concluded that the university restaurant provides the consumers with fried food of good quality, since none of the analyses showed results above recommended, although there is a need of improvement in the quality control of the oil used, in order to avoid unnecessary costs to the university restaurant.

The Brazilian federal government Agency for Health Surveillance detected pesticide residues in fresh food available for consumers all over the country. The current study investigated the effects of a mixture of some of those pesticides (dichlorvos, dicofol, dieldrin, endosulfan, and permethrin) on the reproductive system of Sprague-Dawley (SD), Wistar (WT), and Lewis (LEW) rats. Female rats from each strain were randomized into three experimental groups and were fed a control diet or diets added with pesticides mixture at their respective no-observed-effect level (NOEL)/no-observed-adverse-effect level (NOAEL) (low dose) (mg/kg/d): dichlorvos (0.23), dicofol (0.5), dieldrin (0.025), endosulfan (0.7), permethrin (5), or lowest-observed-effect level (LOEL)/lowest-effect level (LEL)/ lowest-observed-adverse-effect level (LOAEL) (toxically effective dose) (mg/kg/d): dichlorvos (2.3), dicofol (2.1), dieldrin (0.05), endosulfan (3.8), and permethrin (25) as reported in the literature. Euthanasia was performed between wk 10 and 12, during the estrous stage. Decreased body weights gain (SD and WT) and increased liver weights (SD, WT, and LEW) were observed in each strain fed the pesticides mixture at the higher levels. At that dose level, rat strains also varied in their responses regarding the estrous cycle, hormonal levels, and number of developing ovarian follicles. The studied mixture of pesticides was found to interfere with the female reproductive system when individual pesticides were mixed above a certain level, indicating a threshold exists for each of the strains studied.
