Reinvigorating and redesigning early intervention in psychosis services for young people in Auckland

RESUMO: Auckland tem sido pioneira na implementação de modelos de Intervenção Precoce em Psicose. No entanto, esta organização do serviço não mudou nos últimos 19 anos. Segundo os dados obtidos da utilização do serviço, no período de 1996 -2012 foram atendidos 997 doentes, que tinham um número médio de 89 contactos (IQR: 36-184), com uma duração média de 62 horas de contactos (IQR: 24-136). Estes doentes passaram um número médio de 338 dias (IQR: 93-757) em contacto com o programa. 517 doentes (52%) não necessitaram de internamento no hospital, e os que foram internados, ficaram uma mediana de 124 dias no hospital (IQR: 40-380). Os doentes asiáticos tiveram um aumento de 50% de probabilidade de serem internados no hospital. Este relatório inclui 15 recomendações para orientar as reformas para o serviço e, nomeadamente, delinear a importância de uma visão organizacional e dos seus componentes-chave. As recomendações incluem o reforço da gestão e da liderança numa estrutura de equipe mais integrada, com recursos dedicados a melhorar a consciencialização da comunidade, a educação e deteção precoce, bem como a capacidade de receber referenciações diretas. Os Indicadores Chave de Desempenho devem ser estabelecidos, mas os Exames de Estado Mental em risco, devem ser removidos. Auckland deve manter a faixa etária alvo atual. A duração do serviço deve ser aumentada para um mínimo de três anos, com a opção de aumentá-la para cinco anos. A proporção de gestor de cuidados para os doentes deve ser preconizada em 1:15, enquanto o pessoal de apoio não-clínico deve ser aumentado. Os psiquiatras devem ter uma carga de trabalho de cerca de 80 doentes por equivalente de tempo completo. Um serviço local de prestação de cuidados deve ser desenvolvido com, nomeadamente, intervenções culturais para responder às necessidades da população multicultural de Auckland. A capacidade de investigação deve ser incorporada no Serviço de Intervenção Precoce em Psicoses. Qualquer alteração deverá envolver contacto com todas as partes interessadas, e a Administração Regional de Saúde deve comprometer-se em tempo, recursos humanos e políticos para apoiar e facilitar a mudança do sistema, investindo de forma significativa para melhor servir a comunidade Auckland.----------------------------------- ABSTRACT: Auckland has been pioneering in the adoption of Early Intervention in Psychosis models but the design of the service has not changed in 19 years. In service utilisation data from 997 patients seen from 1996 -2012, patients had a median number of 89 contacts (IQR: 36-184), with a median duration of 62 hours of contact (IQR: 24-136). Patients spent a median number of 338 days (IQR: 93-757) in contact with the program. 517 patients (52%) did not require admission to hospital, and those who did spent a median of 124 days in hospital (IQR: 40-380). Asian patients had a 50% increased chance of being admitted to hospital. This report includes 15 recommendations to guide reforms to the service, including outlining the importance of vision and key components. It recommends strengthened managerial leadership and a more integrated team structure with dedicated resources for improved community awareness, education and early detection as well as the capacity to take direct referrals. Key Performance Indicators (KPIs) should be established but At Risk Mental States should be excluded. Auckland should maintain the current target age range. The duration of service should be increased to a minimum of three years, with the option to extend this to five years. The ratio of care co-ordinator to patients should be capped at 1:15 whilst non-clinical supporting staff should be increased. Psychiatrists should have a caseload of about 80 per FTE. A local Service Delivery framework should be developed, as should cultural interventions to meet the needs of the multicultural population of Auckland. Research capacity should be incorporated into the fabric of Early Intervention in Psychosis Services. Any changes should involve consultation with all stakeholders, and the DHB should commit to investing time, human and political resources to support and facilitate meaningful system change to best serve the Auckland community.

Since the knowledge-based economy has become a fashion over the last few decades, the concept of the professional learning community (PLC) has started being accepted by educational institutions and governments as an effective framework to improve teachers’ collective work and collaboration. The purpose of this research was to compare and contrast the implementations of PLCs between Beijing schools and Ontario schools from principals’ personal narratives. In order to discover the lessons and widen the scope to understand the PLC, this research applied qualitative design to collect the data from two principal participants in each location by semistructured interviews. Four themes emerged: (a) structure and technology, (b) identity and climate, (c) task and support, and (d) change and challenge. This research found that the root of the characteristics of the PLCs in Beijing and Ontario was the different existing teaching and learning systems as well as the test systems. Teaching Research Groups (TRGs) is one of the systems that help Chinese to organize routine time and input resources to improve teachers’ professional development. However, Canadian schools lack a similar system that guarantees the time and resources. Moreover, standardized test plays different roles in China and Canada. In China, standardized tests, such as the college entrance examination, are regarded as the important purpose of education, whereas Ontario principals saw the Education Quality and Accountability Office (EQAO) as a tool rather than a primary purpose. These two main differences influenced principals’ beliefs, attitudes, strategies, and practices. The implications based on this discovery provide new perspectives for principals, teachers, policy makers, and scholars to widen and deepen the research and practice of the PLC.

L’école ne semble plus en mesure de répondre par elle-même aux besoins multidimensionnels des élèves en difficulté qui sont maintenant intégrés dans la classe ordinaire. (Cooper, 1999; Deslandes & Bertrand, 2001; Dryfoos, 1994; Greenberg, 2004; Ministère de l'Éducation du Loisir et du Sport, 2005; Paavola, 1995; Riddle & Tett, 2001; The Learning Partnership, 2004). Les équipes de soutien à l’enseignant semblent un moyen efficace d’appuyer l’enseignant et l’élève, mais cette approche est très peu utilisée dans les milieux éducatifs au Canada. Certains élèves présentant des problèmes complexes ne reçoivent pas l’appui nécessaire faute d’un manque de spécialiste à l’intérieur du cadre scolaire. Dans cette perspective, les services intégrés pourraient être une voie propice pour le travail de partenariat mais plusieurs obstacles semblent empêcher la collaboration et la coopération entre les intervenants scolaires et les partenaires de la communauté. La présente recherche a identifié un modèle qui permet d’appuyer l’enseignant dans sa pratique afin d’aider l’élève en difficulté et de le maintenir dans la classe ordinaire. Le modèle d’équipe de soutien à l’enseignant mis en œuvre dans les écoles d’un conseil francophone scolaire de l’Ontario est un modèle dérivé de Chalfant, Pysh et Moultrie (1979). Vers les années 1970, ces équipes ont été formées en vue d’offrir un appui à l’enseignant de classe ordinaire pour éviter les placements d'élèves en éducation spéciale l’intention est encore la même en 2010. Par ce modèle, nous favorisons l’entrée des services intégrés à l’intérieur du cadre scolaire et les partenaires travaillent en équipe afin de favoriser le succès scolaire des élèves. Un coordonnateur du milieu scolaire est le lien qui assure la coordination et les suivis des différentes recommandations proposées par l’équipe. Le travail de collaboration et de coopération est essentiel pour assurer le succès scolaire de l’élève. Les résultats de cette mise en œuvre nous ont permis de constater que le modèle est efficace dans l’atteinte de ses buts. D’une façon générale, les intervenants scolaires affirment se sentir effectivement appuyés par les équipes de soutien tant dans la formation, les échanges et les discussions que dans les ressources matérielles. Ce qui ressort le plus souvent de leurs propos est qu’ils apprécient la réelle collaboration et la concertation qui existent entre tous les intervenants qui assurent les suivis. Quatre-vingt-treize pour cent des répondants qui ont participé aux équipes de soutien affirment qu’il y a eu des changements positifs chez les élèves. Différents défis ont aussi été relevés. Mentionnons entre autre, la lourdeur du rôle du coordonnateur, la grandeur du territoire du Conseil (perte de temps sur la route), des cas de conflits existants entre les parents et l’école. Indépendamment de ces défis, l’application du modèle d’équipe de soutien à l’enseignant répond à l’objectif d’inclusion scolaire des élèves en difficulté tout en permettant aux enseignants de ne pas se sentir seuls ni d’être effectivement seuls à assumer ce mandat.

Construyendo Legado Cultural es un modelo de gestión cultural sostenible aplicado al patrimonio cultural de Suesca Cundinamarca, que involucra a la comunidad en la gestión de proyectos culturales relacionados con el patrimonio, a través de la aplicación de un instrumento de acción participativa que le permite a la comunidad, con la mediación del gestor cultural, ser autogestora junto con el sector público y privado, de sus procesos culturales fortaleciendo su identidad cultural y fomentando el desarrollo del municipio.

El Objetivo fue aplicar un método didáctico diseñado para estudiantes de Promoción y Prevención del programa de Fisioterapia de la Universidad del Rosario, en Colombia, con el fin de facilitar sus acciones de educación en salud con la comunidad. Metodología: el proyecto incluyó tres fases: la revisión teórica de las competencias del docente en la práctica y los diferentes métodos empleados para desarrollar acciones de promoción y prevención en el trabajo comunitario, el diseño del método y su aplicación con los estudiantes y, finalmente, la evaluación y el respectivo análisis de resultados. Resultados: se evidenció que frente al desarrollo de otras experiencias docentes, la aplicación de un método didáctico con los estudiantes les permitía desarrollar hábilmente sus competencias para el trabajo con grupos poblacionales. Conclusiones: la aplicación de un método didáctico en práctica favorece la adquisición de herramientas pedagógicas por los estudiantes y mejora su desempeño en el desarrollo de las actividades programadas.

The construction industry with its nature of project delivery is very fragmented in terms of the various processes that encompass design, construction, facilities and assets management. Facilities managers are in the forefront of delivering sustainable assets management and hence further the venture for mitigation and adaptation to climate change. A questionnaire survey was conducted to establish perceptions, level of commitment and knowledge chasm in practising sustainable facilities management (FM). This has significant implications for sustainable design management, especially in a fragmented industry. The majority of questionnaire respondents indicated the importance of sustainability for their organization. Many of them stated that they reported on sustainability as part of their organization annual reporting with energy efficiency, recycling and waste reduction as the main concern for them. The overwhelming barrier for implementing sound, sustainable FM is the lack of consensual understanding and focus of individuals and organizations about sustainability. There is a knowledge chasm regarding practical information on delivering sustainable FM. Sustainability information asymmetry in design, construction and FM processes render any sustainable design as a sentiment and mere design aspiration. Skills and training provision, traditionally offered separately to designers and facilities managers, needs to be re-evaluated. Sustainability education and training should be developed to provide effective structures and processes to apply sustainability throughout the construction and FM industries coherently and as common practice.

The black communities, nowadays known as quilombolas, have long been segregated in social and cultural distances in relation to other groups in the country. Gradually, the establishment of public policy has enabled the inclusion of these social communities in new instances, favoring the improvement of standards of learning. In order to know the route of the school in relation to its surroundings, this paper presents a reflection on the educational history of the Municipal School Padre Armando de Paiva, inserted in the context of afrodescendência (African descendants), in the community called Sibaúma, municipality of Tibau do Sul - RN, characterized as the object of this study. Methodologically, this study is limited in a descriptive research of the historical reconstruction of the school, a case considered worthy of analysis. From a reflection on the presence of African descent in Brazil and in Rio Grande do Norte, the study also presents information on education in Brazil. Finally, it presents the trajectories directed to the analysis of physical conditions, the dynamics of registration, and avoidance of repetition of its students and the qualifications of its teaching staff. There are several tools for measuring quality of life (QoL) and specifically, health-related quality of life (HRQoL) for persons with diabetes. A commonly-used measure, the Diabetes Quality of Life (DQOL) Survey, developed for the Diabetes Control and Complications Trial (DCCT), has been used in several experimental settings, and its reliability and validity are well-established. However, it is considered too long to be of practical use in clinical settings. Because of this, a shortened version of the tool was used recently in the Community Diabetes Education (CoDE) Project in Dallas, Texas, a clinic-based patient education program that uses a specially-trained community healthcare worker to provide patient education. However, the modified scale has never been tested for reliability and validity. Thus, one goal of this thesis was to measure these psychometric properties of the scale. After establishing the reliability and validity, the results of the scale were analyzed to determine the effects of the intervention on the subjects’ quality of life. The changes in QoL scales were compared with changes in physiologic measures which are most closely allied with diabetes, including blood glucose levels, weight/BMI, co-morbidities and health beliefs in order to determine if there is a relationship between such measures and quality of life. The results of the reliability and validity testing were not conclusive. Measures of reliability and criterion validity were established, but these contrasted with poor measures of repeatability and content validity. The effect of the intervention on quality of life, however, was more significant, particularly regarding the impact of diabetes. Those who received the counseling had significantly higher scores on the Impact scale than those who did not, and the former group had much greater improvement in scores over the twelve month period than the latter group. ^

A presente dissertação de Mestrado visa à investigação, observação e o estudo de ações de Educação para a Sustentabilidade (nos âmbitos da educação formal e não formal) que vêm sendo desenvolvidas, através de levantamento histórico e de um Estudo de Caso. O objetivo é refletir sobre aspectos das relações existentes entre ONGs socioambientalistas e escolas públicas e seu respectivo impacto nas localidades onde estão sendo implementadas, bem como, nos indivíduos nelas inseridos. Pretende-se analisar estas relações, as metodologias utilizadas, bem como, refletir sobre políticas públicas que apóiam estas ações.

Taiwan's technical vocational educational system includes three levels: (1) institutes of technology (two and four year programs), (2) junior colleges (two, three and five year programs), and (3) senior vocational schools. Two-year junior colleges enroll their students through two channels: (1) based on results of the Particular Screening Entrance Examination (PSEE), (2) based on the Joint College Entrance Examination (JCEE). The PSEE has two categories: Category I includes on-time graduates with excellent performance, which means that they are within the top ten of their classes; Category II students include award-winning students in talent contests who have achieved a minimum grade point average of 75%. The JCEE is a regular entrance examination given to any senior vocation school graduate.^ Basic courses have a great impact on the students' academic performance. The purpose of the study was to focus on the effectiveness of teaching mathematics in two-year junior colleges and to analyze and correlate the results of two-year junior college students' performance in calculus and on-time graduation. The target group consisted of 521 students enrolled at National Taipei Institute of Technology in 1993.^ Calculus is a very important course for engineering majors in two-year junior colleges and has a great impact on the students' academic performance. This retrospective study showed that there was a correlation between students' performance in calculus and on-time graduation after two years of study.^ The conclusions of the study urge the Ministry of Education to reform two-year junior college curriculum standards to emphasize basic rudimentary courses. It is recommended that engineering majors receive three hours of calculus per week as the current requirement of only two hours per week is inadequate. The future job market will require a technologically advanced labor force that can be trained in a higher education system. More channels of higher education for two-year junior college graduates should be made available for those wishing to pursue bachelor degrees. Additional work in calculus will not only enhance the opportunities for two-year junior college graduates to continue their pursuit of an advanced academic degree, but also serve them well as they seek career advancement. ^

From educational, communications, psychological, and technical points of view, the renovation of pedagogy in media education is based upon the promotion of "educational technology." The promotion of educational technology relies upon the appropriate availability and knowledge of different educational media made available by the trained media personnel.^ In the past three decades most of the junior colleges in Taiwan set up educational media centers to help students learn through the use of media which enables them to obtain optimum benefits in a short time. What are the roles the media personnel play in the media center? What responsibilities have they to bear in the center? What differences are there when a trained and untrained media personnel are presented in junior colleges media center in Taiwan? What do the trained and untrained media personnel feel toward the importance of each media service in the area of media center's administration, media production, specialized media duties, and the training of staff in media use? These are the questions addressed in this study.^ Through the study of the related literature and a survey conducted in the junior colleges in Taiwan, recommendations are offered to provide improvement of the services and training of media specialists in Taiwan that are appropriate for a changing work and environment. These recommendations are for media specialists to be formally trained to effectively serve the changing needs of school library media so as to make optimal use of media in the junior colleges. ^
