OBJECTIVE: The Veterans Health Administration has developed My HealtheVet (MHV), a Web-based portal that links veterans to their care in the veteran affairs (VA) system. The objective of this study was to measure diabetic veterans' access to and use of the Internet, and their interest in using MHV to help manage their diabetes. MATERIALS AND METHODS: Cross-sectional mailed survey of 201 patients with type 2 diabetes and hemoglobin A(1c) > 8.0% receiving primary care at any of five primary care clinic sites affiliated with a VA tertiary care facility. Main measures included Internet usage, access, and attitudes; computer skills; interest in using the Internet; awareness of and attitudes toward MHV; demographics; and socioeconomic status. RESULTS: A majority of respondents reported having access to the Internet at home. Nearly half of all respondents had searched online for information about diabetes, including some who did not have home Internet access. More than a third obtained "some" or "a lot" of their health-related information online. Forty-one percent reported being "very interested" in using MHV to help track their home blood glucose readings, a third of whom did not have home Internet access. Factors associated with being "very interested" were as follows: having access to the Internet at home (p < 0.001), "a lot/some" trust in the Internet as a source of health information (p = 0.002), lower age (p = 0.03), and some college (p = 0.04). Neither race (p = 0.44) nor income (p = 0.25) was significantly associated with interest in MHV. CONCLUSIONS: This study found that a diverse sample of older VA patients with sub-optimally controlled diabetes had a level of familiarity with and access to the Internet comparable to an age-matched national sample. In addition, there was a high degree of interest in using the Internet to help manage their diabetes.

PURPOSE: It is unclear whether sociocultural and socioeconomic factors are directly linked to type 2 diabetes risk in overweight/obese ethnic minority children and adolescents. This study examines the relationships between sociocultural orientation, household social position, and type 2 diabetes risk in overweight/obese African-American (n = 43) and Latino-American (n = 113) children and adolescents. METHODS: Sociocultural orientation was assessed using the Acculturation, Habits, and Interests Multicultural Scale for Adolescents (AHIMSA) questionnaire. Household social position was calculated using the Hollingshead Two-Factor Index of Social Position. Insulin sensitivity (SI), acute insulin response (AIRG) and disposition index (DI) were derived from a frequently sampled intravenous glucose tolerance test (FSIGT). The relationships between AHIMSA subscales (i.e., integration, assimilation, separation, and marginalization), household social position and FSIGT parameters were assessed using multiple linear regression. RESULTS: For African-Americans, integration (integrating their family's culture with those of mainstream white-American culture) was positively associated with AIRG (β = 0.27 ± 0.09, r = 0.48, P < 0.01) and DI (β = 0.28 ± 0.09, r = 0.55, P < 0.01). For Latino-Americans, household social position was inversely associated with AIRG (β = -0.010 ± 0.004, r = -0.19, P = 0.02) and DI (β = -20.44 ± 7.50, r = -0.27, P < 0.01). CONCLUSIONS: Sociocultural orientation and household social position play distinct and opposing roles in shaping type 2 diabetes risk in African-American and Latino-American children and adolescents.

BACKGROUND: Early preparation for renal replacement therapy (RRT) is recommended for patients with advanced chronic kidney disease (CKD), yet many patients initiate RRT urgently and/or are inadequately prepared. METHODS: We conducted audio-recorded, qualitative, directed telephone interviews of nephrology health care providers (n = 10, nephrologists, physician assistants, and nurses) and primary care physicians (PCPs, n = 4) to identify modifiable challenges to optimal RRT preparation to inform future interventions. We recruited providers from public safety-net hospital-based and community-based nephrology and primary care practices. We asked providers open-ended questions to assess their perceived challenges and their views on the role of PCPs and nephrologist-PCP collaboration in patients' RRT preparation. Two independent and trained abstractors coded transcribed audio-recorded interviews and identified major themes. RESULTS: Nephrology providers identified several factors contributing to patients' suboptimal RRT preparation, including health system resources (e.g., limited time for preparation, referral process delays, and poorly integrated nephrology and primary care), provider skills (e.g., their difficulty explaining CKD to patients), and patient attitudes and cultural differences (e.g., their poor understanding and acceptance of their CKD and its treatment options, their low perceived urgency for RRT preparation; their negative perceptions about RRT, lack of trust, or language differences). PCPs desired more involvement in preparation to ensure RRT transitions could be as "smooth as possible", including providing patients with emotional support, helping patients weigh RRT options, and affirming nephrologist recommendations. Both nephrology providers and PCPs desired improved collaboration, including better information exchange and delineation of roles during the RRT preparation process. CONCLUSIONS: Nephrology and primary care providers identified health system resources, provider skills, and patient attitudes and cultural differences as challenges to patients' optimal RRT preparation. Interventions to improve these factors may improve patients' preparation and initiation of optimal RRTs.

Radio advertising is suffering from a remarkable crisis of creativity as it has yet not found its role in a radio model based on voice locution and information genres. This article suggests the need for implementing a peripheral or heuristic strategy to attract and hold listeners’ attention. Within this framework, the narration and scene representation are proposed as suitable persuasion techniques. The objective is to design a useful conceptual tool for an efficient creative conception of narration at the service of certain commercial strategy. First, the concept of narrative persuasion is grounded according to the possibilities of the sound code. Second, the keys of scene representation and commercial strategy (brand, product, advantage, benefit and target) within the sound message are presented. And third, these keys are articulated in a model. This model is pre-tested by means of analyzing eight different case-radio ads.

Research into student teachers' perceptions, attitudes and prior experiences of learning suggests that these experiences can exert an influence on practice which can be relatively undisturbed by their initial teacher education. This article is based on the initial findings of an all-Ireland survey of all first-year students on B.Ed. courses in colleges in Northern Ireland and in the Republic of Ireland. The survey is the first stage in a longitudinal study which will follow the same cohort of students for the duration of their initial teacher education, seeking to map and track the development of their ideas about teaching and learning in primary history, geography and science. Based on an analysis of the quantitative data in the entry questionnaire, the initial findings suggest that subject knowledge remains a problematic issue in initial teacher education and that both location and gender interact with knowledge, attitudes and subject area to produce a complex and challenging context for teacher educators in history, geography and science education.

Background
The incidence of chronic illnesses is increasing globally. Non-adherence to medications and other medication-related problems are common among patients receiving long-term medications. Medication use review (MUR) is a service provision with an accredited pharmacist undertaking structured, adherence-centered reviews with patients receiving multiple medications. MUR services are not yet available in community pharmacies in Qatar.

Objective
The current study aims to evaluate community pharmacists' knowledge, attitudes, and perception towards establishing MUR as an extended role in patient care.

Setting
Private community pharmacies in Qatar including chains and independent pharmacies.

Methodology
A cross-sectional survey using a self-administered questionnaire was conducted among licensed community pharmacists from December 2012 to January 2013. Data analysis was conducted using descriptive and inferential statistics.

Main outcome measures
Knowledge, attitudes, and practices related to MUR concept and services.

Results A total of 123 participants responded to the survey (response rate 56 %). The mean total knowledge score was 71.4 ± 14.7 %. An overwhelming proportion of the participants (97 %) were able to identify the scope of MUR in relation to chronic illnesses and at enhancing the quality of pharmaceutical care. Furthermore, 80 % of the respondents were able to identify patients of priority for inclusion in an MUR program. However, only 43 % of the participants knew that acute medical conditions were not the principal focus of an MUR service, while at least 97 % acknowledged that the provision of MUR services is a great opportunity for an extended role of community pharmacists and that MUR makes excellent use of the pharmacist's professional skills in the community. The participants generally reported concerns about time, dedicated consultation area, and support staff as significant barriers towards MUR implementation.

Conclusion
This study suggests that community pharmacists in Qatar had sufficient knowledge about the concept of MUR and its scope, but there were still important deficiencies that warrant further education. The findings have important implications on policy and practice pertaining to the implementation of MUR as an extended role of pharmacists and as part of Qatar's National Health Strategy to move primary health care forward.

A investigação realizada encontra-se inserida na área das Ciências de Educação, teve como objetivo principal compreender a atitude diagnóstica no quadro das situações educativas e pedagógicas desenvolvidas pelas educadoras de alguns jardins-de-infância, do distrito de Évora e como finalidade construir uma teoria de médio alcance elaborada numa estrutura explicativa do conjunto de fenómenos em contexto. Em educação de infância, o ato de cuidar estabelece-se numa relação de ajuda à criança e orienta-se para objetivos como promover o bem-estar, o conforto e o desenvolvimento em todas as dimensões. Isto implica conhecer e compreender cada criança ou grupo de crianças e as situações educativas e pedagógicas, em contexto. Compreende um exercício prático e para o qual se projeta uma prática de diagnóstico conciliadora da compreensão dos fenómenos sociais, estruturados em comportamentos, ações e atitudes específicas. Verificam-se lacunas quanto à forma como este exercício se cumpre e evolui nas diferentes fases. Repetem-se fragilidades quanto à mobilização de saberes, atitudes e competências a cada momento do agir. Constatam-se conceitos psicologizados e estruturas muito ténues e, por isso, necessitam ser estudadas, documentadas e teorizadas, particularmente numa dimensão praxiológica do conhecimento. O estudo inseriu-se no quadro das metodologias qualitativas, seguiu o paradigma interpretativo e o raciocínio indutivo. O referencial metodológico reuniu os princípios e procedimentos da Grounded Theory. A colheita de dados efetuou-se em jardins-de-infância da rede pública; a amostra teórica envolveu seis educadoras de infância e as crianças com idades compreendidas entre os três e os seis anos. Adotámos o uso de multitécnicas, entre as quais, a observação, a entrevista e a narrativa escrita. Fizemos vinte observações, repartidas pelos dois períodos do dia (manhã e tarde); vinte e seis entrevistas e seis narrativas escritas. Da análise dos dados emergiu a Atitude Diagnóstica como uma predisposição que caracteriza o ato de agir e as suas características foram-se tornando evidentes com o desenvolvimento da caracterização do processo de diagnóstico. Desta emergiu o modelo teórico definido em três eixos fundamentais. O “Processo de avaliação diagnóstica e planeamento” representativo das etapas, segundo as quais o educador desenvolve um conjunto de ações propiciadoras de um conhecimento previamente organizado, visando o bem-estar, o conforto, a segurança e o desenvolvimento. Em concomitância emerge o segundo, “Processo de intervenção educativa e pedagógica”, expressivo do conjunto de ações coerentes e evolutivas, empreendidas com vista à execução dos objetivos do ensino aprendizagem. Trata-se de dois processos sistematizados e perspetivados sob as dimensões diacrónica e sincrónica, compostos por uma sequência de pensamentos, permanentemente averbados pela “Atitude Diagnóstica” que dá um caráter coerente e evolutivo às tomadas de decisão nas ações educativas. Estes dois eixos integrados e entrelaçados são auxiliados por um terceiro, o “Processo de relação” que os harmoniza e dá especificidade a cada situação experienciada. A relação consolida-se e assume-se uma ajuda na confiabilidade necessária para a promoção da confiança entre os pares e o conhecimento vai evoluindo gradualmente em função do tempo e dos compromissos. O educador age com intencionalidade e para cada ator traça objetivos a cada momento do agir. A gestão do tempo, dos sentimentos e emoções funciona como variável importante na relação orientada sob a tríade: educador, criança e família e é concomitante com o Desenvolvimento pessoal e profissional do educador; ### ABSTRACT: The Diagnostic Attitude as an Analysis Instrument in Educational Action The conducted investigation is inserted in the area of Education Sciences. Its main aim was to understand the diagnostic attitude in the frame of the educative and pedagogical activities developed by the educators of some kindergartens in the district of Évora, and its purpose was to build a medium range theory elaborated in a structure explaining the whole of the phenomena in context. In child education, the act of caring is established in a relationship of help towards the child and is oriented towards goals as specific as promoting the well-being, the comfort and the development in all dimensions. This implies knowing and understanding each child or group of children and the educational and pedagogical situations in context. It includes a practical exercise that determines a complex scenario and for which we project a conciliating diagnostic practice towards the understanding of the social phenomena, structured in specific behaviors, actions and attitudes. We recognize some gaps regarding the way how this exercise is fulfilled and evolves in the different phases. Weaknesses regarding the mobilization of knowledge, attitudes and competences in each acting moment are repeated. We notice psichologized concepts and very superficial structures that need thus to be studied, documented and theorized, particularly in a praxeological dimension of knowledge. The study is inserted in the frame of qualitative methodologies, following an interpretative paradigm and an inductive reasoning. The methodological referential has gathered the principles and procedures of the Grounded Theory. The gathering of data was done in public kindergartens; the theoretical sample involved six child educators and children aged between three and six years. Several techniques were used, such as observation, interview and written narrative. We did twenty observations, divided between the two periods of the day (morning and afternoon); twenty-six interviews and six written narratives. The analysis of the data resulted in the Diagnostic attitude, like a predisposition that characterizes the act of acting and its characteristics became evident with the development of the characterization of the diagnostic process. From this rose the theoretical model defined in three fundamental axes. The Process of diagnostic evaluation and planning that represents the stages according to which the educator develops a group of actions that allow the transmission of a previously organized knowledge that aims well-being, comfort, safety and development. Concomitantly arises the second, Process of educative and pedagogic intervention, that expresses a group of coherent and evolving actions, undertaken to reach the goals of the teaching-learning process. These are two systematized processes, perspectivated under the diachronical and synchronical dimensions, composed by a sequence of thought, permanently confirmed by the Diagnostic attitude that gives a coherent and evolving character to the decision making in educative actions. These two integrated and intertwined axis are aided by a third one, Process of relation that harmonizes them and gives specificity to each experienced situation. The relation is consolidated and one assumes a kind of help in the trustworthiness necessary for the promotion of the trust between pairs and the knowledge keeps evolving gradually according to time and commitments. The educator acts with intent and sets goals for each actor at each acting moment. Time, feeling and emotion management works as an important variable in the relation oriented according to the triad educator, child and family, being also concomitant with the Personal and professional development of the educator.

Considerando que a escola tem como missão formar cidadãos saudáveis, conscientes e responsáveis, é de importância primordial que a Educação para a Saúde (EpS) integre o Projeto Educativo (PE) e seja abordada de forma transversal, numa dimensão essencial ao percurso educativo e formativo dos jovens. Tendo em conta o contexto educativo concreto de cada escola/agrupamento e o quadro legislativo vigente, deve ser criado um Projeto de Educação para a Saúde (PEpS). Este terá como objetivo fomentar a capacitação (empowerment) de crianças e adolescentes, dotando-as de conhecimentos, atitudes e valores que os ajudem a fazer opções e a tomar decisões adequadas à saúde e ao bem-estar físico, social e mental. A partir da leitura efetuada do atual quadro de conhecimentos dos domínios abrangidos – avaliação, projetos e promoção e educação da saúde em meio escolar, constatou-se a escassez de estudos de avaliação, em Portugal, nesta área. No estudo multicasos desenvolvido, em que se explorou uma metodologia de índole qualitativa, procedeu-se à descrição e avaliação das dinâmicas de implementação de cinco PEpS. Os projetos foram avaliados a partir de cinco dimensões avaliativas (organizacional, comunitária, ecológica, psicossocial e curricular), em resultado da triangulação de dados recolhidos através de análise documental, de observações e da análise de entrevistas. A avaliação considerou o contexto e alicerçou-se sobre uma démarche de referencialização. Fez apelo a uma perspetiva de avaliação holística e interativa, como construção e coconstrução coletiva de sentido. O protocolo construído – “Referencial de Avaliação”, que se configura como um dos contributos deste estudo, compreendeu a seleção de critérios de qualidade para um PEpS, fundamentados na literatura da especialidade e de indicadores. O estudo evidenciou práticas consideradas de referência nas dinâmicas de implementação dos PEpS. Contudo, só para um dos projetos se registou os critérios de qualidade que o permitem definir como um “bom” projeto. A título de exemplo, refira-se a estrutura coesa da equipa educativa, cujos elementos têm funções claramente definidas, aliada a forte liderança. A análise dos resultados e a literatura consultada possibilitou definir recomendações, que se sistematizaram num guião de práticas de referência, nomeadamente, integração da EpS no PE, aposta numa forte liderança, trabalho de equipa e continuidade de ação. O guião apresenta-se como um contributo que pode potenciar a articulação entre teoria e prática no que respeita à implementação da EpS em escolas portuguesas.

Objectives: This paper reports on the acceptability and effectiveness of the FITS (Focussed Intervention Training and Support) into Practice Programme. This intervention was scaled up from an earlier cluster randomised-controlled trial that had proven successful in significantly decreasing antipsychotic prescribing in care homes. Method: An in depth 10-day education course in person-centred care was delivered over a three-month period, followed by six supervision sessions. Participants were care-home staff designated as Dementia Care Coaches (DCCs) responsible for implementing interventions in 1 or 2 care homes. The course and supervision was provided by educators called Dementia Practice Development Coaches (DPDCs). Effectiveness data included monitoring antipsychotic prescriptions, goal attainment, knowledge, attitudes and implementation questionnaires. Qualitative data included case studies and reflective journals to elucidate issues of implementation. Results: Of the 100 DCCs recruited, 66 DCCs completed the programme. Pre-post questionnaires demonstrated increased knowledge and confidence and improved attitudes to dementia. Twenty per cent of residents were prescribed antipsychotics at baseline which reduced to 14% (31% reduction) with additional dose reductions being reported alongside improved personalised goal attainment. Crucial for FITS into Practice to succeed was the allocation and protection of time for the DCC to attend training and supervision and to carry out implementation tasks in addition to their existing job role. Evaluation data showed that this was a substantial barrier to implementation in a small number of homes. Discussion and conclusions: The FITS into practice programme was well evaluated and resulted in reduction in inappropriate anti-psychotic prescribing. Revisions to the intervention are suggested to maximise successful implementation.

Resumo: Na promoção de comportamentos alimentares saudáveis ao longo da vida. que deve iniciar-se o mais precocemente possível, a educação alimentar é um elemento chave. O jardim-de-infância é um espaço efectivo na implementação de projectos de educação alimentar, onde os educadores de infância são actores imprescindíveis. Este estudo teve como objectivo desenvolver uma metodologia apropriada para identificar as capacidades de estudantes de educação de infância e avaliar as suas aptidões no contexto da educação alimentar infantil. A população alvo do estudo foram estudantes do 4ºano da licenciatura em educação de infância de instituições públicas e privadas da área metropolitana de Lisboa (n=287). Construiu-se um videograma, integrador dos domínios da aprendizagem (cognitivo e afectivo) e dos factores associados aos comportamentos em saúde (predisponentes, capacitadores e de esforço). A construção dos questionários estruturou-se em: conhecimentos-atitudes-comportamentos.Pretendia-se que os estudantes transmitissem a sua opinião, de acordo com os questionários.Os estudantes manifestaram conhecimentos, no âmbito da alimentação infantil e da utilidade e aplicabilidade da educação alimentar.Nenhum estudante apresentou conhecimentos negativos, numa escala de zero a 18 valores, 86,9% obtiveram uma classificação entre 11 e 17 valores e 13,1% a classificação máxima.A atitude dos estudantes revelou-se consensual com o desejável, 98,5% apresentaram atitudes positivas face aos aspectos estruturadores das actividades de educação alimentar.No domínio do "saber fazer" 51,0% dos estudantes demonstraram fragilidades no seu desempenho/comportamento. Em conclusão, a utilização do questionário demonstrou ser apropriado para a medição dos conhecimentos, das atitudes e como forma de os estudantes, após visionarem o videograma, espelharem o seu desempenho/comportamento face à representação do educador de infância. A taxa de resposta nunca foi inferior a 98,9% (n=284). No domínio dos conhecimentos, os estudantes manifestaram concepções positivas no campo da alimentação infantil e da educação alimentar. As atitudes enfatizaram-se no domínio afectivo, nos aspectos relacionais e comunicacionais. No domínio dos comportamentos, 91,8% dos estudantes apresentaram dificuldades na identificação dos aspectos menos correctos do desempenho dos actores do video, o que pode sugerir lacunas no âmbito do "saber fazer". Abstract:Nutrition education is key element to promote lifelong healthy eating behaviours and it must begin since early stages of life. Nursery schools are an effective space in the implementation of nutrition education projects, where the nursery teachers are indispensable actors. The aim of this study was to develop an appropriate methodology to identify nursery education student´s skills and evaluate their attitudes in the childhood nutrition education's context. The study's population were students of the 4th degree in private and public nursery education universities in Lisbon metropolitan area (n=287). A video integrator of learning domains (cognitive and affective) and of factors associated with health behaviours (predisposing, enabling and reinforcing) was developed. Questionnaire´s construction was structured in: knowledge-attitudes-behaviours.According to the questionnaires students were asked to transmit their opinion. Students revealed knowledge, in the scope of the childhood nutrition and in utility and applicability of nutrition education. No students presented negative results in knowledge ina zero - 18 scale; 86,9% students obtained a classification between 11 and 17 and the maximum classification was obtained by 13.1% students. Student's attitude was according to win the desirable, 98,5% student´s revealed positive attitudes in the nutrition education's structure aspects of chilhood activities.In the domain of the "know to do" 51,0% of students showed fragilities in their performance/behaviour. In conclusion, the questionnaire´s utilization confirm to be appropriated in measurement knowledge and attitudes and also, after watching the video, as a way for students reflect their performance/behaviour face to the nursery teacher´s representation. Answer´s rate was never lower than 98.9% (n=284). In the knowledge's domain students demonstrated positive conceptions in the childhood nutrition and nutrition education's fields. Attitudes were emphasized in the effective domain in the communication and relational aspects. In behaviour's domain 91.8% students presented difficulties in identifying the less correct aspects of the video actor's performance what may suggest gaps in the "know to do" scope.

ABSTRACT: Background. In India, prevalence rates of dementia and prodromal amnestic Mild Cognitive Impairment (MCI) are 3.1% and 4.3% respectively. Most Indians refer to the full spectrum of cognitive disorders simply as ‘memory loss.’ Barring prevention or cure, these conditions will rise rapidly with population aging. Evidence-based policies and practices can improve the lives of affected individuals and their caregivers, but will require timely and sustained uptake. Objectives. Framed by social cognitive theories of health behavior, this study explores the knowledge, attitudes and practices concerning cognitive impairment and related service use by older adults who screen positive for MCI, their primary caregivers, and health providers. Methods. I used the Montreal Cognitive Assessment to screen for cognitive impairment in memory camps in Mumbai. To achieve sampling diversity, I used maximum variation sampling. Ten adults aged 60+ who had no significant functional impairment but screened positive for MCI and their caregivers participated in separate focus groups. Four other such dyads and six doctors/ traditional healers completed in-depth interviews. Data were translated from Hindi or Marathi to English and analyzed in Atlas.ti using Framework Analysis. Findings. Knowledge and awareness of cognitive impairment and available resources were very low. Physicians attributed the condition to disease-induced pathology while lay persons blamed brain malfunction due to normal aging. Main attitudes were that this condition is not a disease, is not serious and/or is not treatable, and that it evokes stigma toward and among impaired persons, their families and providers. Low knowledge and poor attitudes impeded help-seeking. Conclusions. Cognitive disorders of aging will take a heavy toll on private lives and public resources in developing countries. Early detection, accurate diagnosis, systematic monitoring and quality care are needed to compress the period of morbidity and promote quality of life. Key stakeholders provide essential insights into how scientific and indigenous knowledge and sociocultural attitudes affect use and provision of resources.

The ill effects of second-hand smoke are now well documented. To protect the population from exposure to tobacco smoke, comprehensive smoking bans are necessary as expressed in the WHO Framework Convention on Tobacco Control and its guidelines. Switzerland has only a partial smoking ban full of exceptions which has been in effect since 2010, which reproduces the so-called Spanish model. In September 2012, the Swiss citizens refused a proposal for a more comprehensive ban. This case study examines the reasons behind this rejection and draws some lessons that can be learnt from it.
