RESUMO - É recomendado o aleitamento materno exclusivo desde o nascimento até aos 6 meses. Angola apresenta uma situação bastante preocupante, pois é um dos países do mundo com maior taxa de mortalidade infantil: em 2010 era de 114,9 mortes por cada 1000 nascimentos. O desmame precoce, a contaminação dos alimentos e da água potável e a desnutrição criam condições para aparecimento de doenças que causam elevadas mortes em crianças com menos de um ano. Em muitas regiões de África, o parto ainda é um acontecimento marcado por práticas culturais ancestrais que podem prejudicar a saúde e sobrevivência dos recém-nascidos, além de pôr em perigo a vida das mães. Algumas dessas práticas incluem: (i) deitar fora o colostro; e (ii) alimentar os bebés com outros alimentos que não o leite materno. Este estudo é quantitativo com metodologia transversal e pretende identificar, descrever e analisar os fatores que contribuem para o abandono do aleitamento materno exclusivo antes dos 6 meses de idade, na província do Uíge, em Angola, bem como indicar formas de intervenção para que os índices de prevalência do aleitamento materno exclusivo possam aumentar. A população em estudo são mães com filhos até um ano de idade e utilizadoras do centro de saúde materno infantil do município sede da província de Uíge em Angola, da qual será considerada uma amostra de 418 mães. Os dados obtidos através de questionário serão registados em quadros e gráficos para posterior análise estatística através de tabelas de frequências, cálculo de percentagens e taxas de incidência.

Peu de femmes atteignent la recommandation internationale d’un allaitement exclusif d’une durée minimale de 6 mois malgré ses nombreux bienfaits pour l’enfant et pour la mère. Une raison fréquemment mentionnée pour la cessation précoce de l’allaitement ou l’introduction de préparations commerciales pour nourrissons est l’insuffisance de lait. L’origine de cette perception maternelle demeure toujours inexpliquée bien que sa prévalence dans les écrits soit bien documentée. L’insuffisance lactée relève-t-elle de pratiques d’allaitement qui contreviennent au processus physiologique de la lactation ou relève-t-elle d’un manque de confiance maternelle dans sa capacité d’allaiter? Une meilleure compréhension des déterminants de la perception d’insuffisance lactée (PIL) s’avère primordiale, un manque d’interventions infirmières permettant de prévenir, dépister et soutenir les femmes allaitant percevant une insuffisance lactée ayant été identifié. Cette étude visait à déterminer l’apport explicatif de variables biologiques et psychosociales de 252 femmes primipares allaitant sur une PIL. Une modélisation de facteurs associés à la PIL a été développée à l’aide de l’approche synthèse théorique comportant les variables suivantes: les événements entourant la naissance, les capacités infantiles et maternelles, la supplémentation, le sentiment maternel d’efficacité en allaitement, la PIL et les pratiques d’allaitement. Afin de mieux comprendre comment se développe et évolue la PIL, un devis prédictif confirmatif longitudinal a été privilégié de la naissance à la 6e semaine postnatale. Au T1, soit le premier 24 heures suivant la naissance, les participantes ont complété un questionnaire concernant leur intention d’initier et de maintenir un allaitement exclusif pour une durée de 6 mois. Au T2, soit la 3e journée postpartum, les femmes complétaient un 2e questionnaire regroupant les différentes mesures utilisées pour l’étude des variables de la modélisation PIL, incluant le prélèvement d’un échantillon de lait maternel. À la 2e semaine, soit le T3, les femmes complétaient un questionnaire similaire à celui du T2, lequel était envoyé par la poste. Finalement, au T4, soit à la 6e semaine, une entrevue téléphonique semi-dirigée concernant les pratiques d’allaitement a été réalisée. La vérification des hypothèses s’est faite principalement à l’aide de tests de corrélations de Pearson, d’analyses de régressions et d’équations structurelles. Les résultats indiquent une influence simultanée des capacités infantiles et du sentiment maternel d’efficacité en allaitement sur la PIL au T2 et au T3; le sentiment maternel d’efficacité en allaitement exerçant de surcroit un effet médiateur entre les capacités infantiles et la perception d’insuffisance lactée au T2 et au T3. Au T2, la fréquence des tétées est associée à une diminution du taux de Na+ du lait maternel, marqueur biologique de l’établissement de la lactogenèse II. Des interventions ciblant le développement d’un sentiment maternel élevé d’efficacité en allaitement devraient être privilégiées.

Le tire-lait est un appareil conçu pour aider les mères durant l’allaitement. Selon la recherche préliminaire effectuée, c’est un thème très peu exploré dans les études; il est donc difficile d’affirmer si le produit répond correctement aux besoins reliés à son usage et à son utilité ou comment il s’intègre dans l’allaitement. Les fondements de l’étude exploratoire ont été bâtis sur ces enjeux dans le but de comprendre l’expérience d’usage du tire-lait. Inspirée de l’approche phénoménologique centrée sur l’usager, l’étude a permis d’amasser des données qualitatives à travers des entrevues semi-dirigées auprès de sept participantes. L’analyse de ces données a permis d’identifier les thèmes importants qui ont mené à des constats, rédigés dans une optique de design. L’étude a trouvé que c’est souvent quand la réalité de l’allaitement au sein rattrape les mères qu’elles vont vouloir se procurer un tire-lait, en s’inspirant des marques utilisées dans les hôpitaux par manque de temps. Le tire-lait devient alors principalement utile pour se désengorger et pour la liberté qu’il procure, s’intégrant dans une routine. Le tire-lait est par contre peu ergonomique; il semble y avoir un décalage entre le concept proposé et les manipulations réelles, notamment au niveau de la mise en marche, des bouteilles et des embouts. Ces résultats de l’étude pourraient être intégrés dans le tire-lait pour mieux représenter la réalité du contexte, de la pratique et de l’usage du produit. Ils pourraient ainsi nourrir le processus de conception dans un projet futur afin d’améliorer le design du tire-lait.

Antecedentes El dolor en neonatos ha sido un problema poco explorado. Se ha propuesto el uso de las terapias no farmacológicas para su tratamiento, sin embargo existen pocas aproximaciones sistemáticas para la evaluación de su eficacia. Objetivos Determinar la eficacia de las terapias no farmacológicas en el manejo del dolor en neonatos pretérmino a través de una revisión sistemática. Metodología Se realizó una revisión sistemática de la literatura para evaluar la eficacia de las terapias no farmacológicas en el manejo del dolor en el recién nacido petérmino. La búsqueda se realizó a través de las bases de datos Embase, Cochrane, Bireme y Embase. Se identificaron estudios publicados inglés y español. Se realizó un análisis cualitativo y cuantitativo. Resultados Se incluyeron 10 ensayos clínicos. La solución de sacarosa administrada por vía oral mostró reducir la intensidad del dolor en el recién nacido. La intubación y toma de muestras facilitada por el cuidador mostró también reducir la intensidad del dolor. Conclusión Se recomienda la administración solución de sacarosa y acompañamiento del cuidador durante los procedimientos como medidas para reducir el dolor en el recién nacido pretérmino.

Várias evidências científicas têm mostrado consistentemente que o leite materno é o melhor alimento para os bebés sendo altamente benéfico para sua saúde a curto, médio e longo prazo. É recomendado às mulheres que amamentem o seu bebé exclusivamente por 6 meses e continuem a amamentar após os 6 meses, como parte de uma dieta equilibrada. Em casos em que, por qualquer motivo, a mãe não produz quantidade suficiente de leite, o bebé seja prematuro ou tenha alguma patologia, o leite materno doado pode ser a alternativa mais benéfica. Este trabalho tem por objetivo analisar a realidade dos bancos de leite doado em Portugal e no resto do mundo, bem como descrever o seu funcionamento desde a chegada do leite ao banco até à dispensa. Assim, para além do trabalho diário no banco de leite do Hospital Universitario Virgen de las Nieves em Granada, estudou-se o funcionamento de outros bancos de leite no mundo, incluindo Portugal, com o objetivo de comparar técnicas, procedimentos, e propor melhoramentos no processamento do leite materno doado. Com este estudo, foi possível concluir sobre a relevância nutricional e imunológica do Leite Humano Doado (LHD). Verificando-se que em casos de impossibilidade de a mãe amamentar, esta será a melhor opção de alimentação para bebés prematuros e/ou com muito baixo peso ao nascer. Quanto ao procedimento nos Bancos de Leite Humano (BLH), conclui-se que são necessários mais estudos e mais formação dos profissionais de saúde de forma a melhorar a qualidade do LHD bem como reduzir os riscos de infeção.

Initial bacterial colonization, including colonization with health-positive bacteria, such as bifidobacteria and lactobacilli, is necessary for the normal development of intestinal innate and adaptive immune defenses. The predominance of beneficial bacteria in the gut microflora of breast-fed infants is thought to be, at least in part, supported by the metabolism of the complex mixture of oligosaccharides present in human breast milk, and a more adult-type intestinal microbiota is found in formula-fed infants. Inadequate gut colonization, dysbiosis, may lead to an increased risk of infectious, allergic, and autoimmune disorders later in life. The addition of appropriate amounts of selected prebiotics to infant formulas can enhance the growth of bifidobacteria or lactobacilli in the colonic microbiota and, thereby, might produce beneficial effects. Among the substrates considered as prebiotics are the oligosaccharides inulin, fructo-oligosaccharides, galacto-oligosaccharides, and lactulose. There are some reports that such prebiotics have beneficial effects on various markers of health. For example, primary prevention trials in infants have provided promising data on prevention of infections and atopic dermatitis. Additional well-designed prospective clinical trials and mechanistic studies are needed to advance knowledge further in this promising field. (J Pediatr 2009;155:S61-70).

Aims: Certain milk factors may promote the growth of a gastrointestinal microflora predominated by bifidobacteria and may aid in overcoming enteric infections. This may explain why breast-fed infants experience fewer intestinal infections than their formula-fed counterparts. The effect of formula supplementation with two such factors was investigated in this study. Methods and Results: Infant faecal specimens were used to ferment formulae supplemented with glycomacropeptide (GMP) and alpha-lactalbumin (alpha-la) in a two-stage compound continuous culture model. At steady state, all fermenter vessels were inoculated with 5 ml of 0.1 M phosphate-buffered saline (pH 7.2) containing 10(8) CFU ml(-1) of either enteropathogenic Escherichia coli 2348/69 (O127:H6) or Salmonella serotype Typhimurium (DSMZ 5569). Bacteriology was determined by independent fluorescence in situ hybridization. Vessels that contained breast milk (BM), as well as alpha-la and GMP supplemented formula had stable total counts of bifidobacteria while lactobacilli increased significantly only in vessels with breast milk. Bacteroides, clostridia and E. coli decreased significantly in all three groups prior to pathogen addition. Escherichia coli counts decreased in vessels containing BM and alpha-la while Salmonella decreased significantly in all vessels containing BM, alpha-la and GMP. Acetate was the predominant acid. Significance and Impact of the Study: Supplementation of infant formulae with appropriate milk proteins may be useful in mimicking the beneficial bacteriological effects of breast milk.

The effect of formula supplementation with two such factors was investigated in this study. Infant faecal specimens were used to ferment formulas supplemented with glycomacropeptide and α-lactalbumin in a two-stage compound continuous culture model. Bacteriology was determined by fluorescence in situ hybridisation. Vessels that contained breast milk as well as α-lactalbumin and glycomacropeptide had stable counts of bifidobacteria while lactobacilli increased significantly only in vessels with breast milk. Bacteroides, clostridia and Escherichia coli decreased significantly in all runs. Acetate was the principal acid found along with high amounts of propionate and lactate. Supplementation of infant formulas with appropriate milk proteins may be useful in simulating the beneficial bacteriological effects of breast milk.

The aim of this study was to evaluate the anti-tumor activity of Amblyomin-X, a serine protease Kunitz-type inhibitor. Amblyomin-X induced tumor mass regression and decreased number of metastatic events in a B16F10 murine melanoma model. Alterations on expression of several genes related to cell cycle were observed when two tumor cell lines were treated with Amblyomin-X. PSMB2, which encodes a proteasome subunit, was differentially expressed, in agreement to inhibition of proteasomal activity in both cell lines. In conclusion, our results indicate that Amblyomin-X selectively acts on tumor cells by inducing apoptotic cell death, possibly by targeting the ubiquitin-proteasome system. (C) 2010 Elsevier Ltd. I ett internationellt perspektiv har Sverige hög amningsfrekvens, men under 2000-talet har amningsfrekvensen sjunkit och stora regionala skillnader ses. Kvinnorna rekommenderas att helamma sex månader. Mödravårdcentralen (MVC) informerar om bröstmjölkens hälsoeffekter som till exempel att bröstmjölken skyddar barn mot allergier. Forskning visar att överviktiga och feta kvinnor har kortare amningsduration, men få studier behandlar svenska förhållanden. Syfte: Syfte med fördjupningsarbetet är att studera samband mellan mammans BMI relaterat till amningsduration och mammans skattning av barnets hälsa de tre första levnadsåren. Metod: Studien är en kvantitativ retrospektiv tvärsnittstudie. Data insamlades via enkäter, 418 deltagare inkluderades. Data har analyserats med Statistical Package for the Social Sciences (SPSS). För deskriptiv och jämförande analys har parametriska och icke-parametriska analyser genomförts. Resultat: De kvinnor som inte ammade skattade sitt barns hälsa sämre vid tre års ålder i jämförelse med kvinnor som ammat någon period. Vid tre års ålder skattade kvinnor med övervikt och fetma sitt barns hälsa sämre. Slutsats: Det är viktigt att kunna identifiera kvinnor som är behov av stöd för att initiera amning och kunna bidra till anpassat stöd till dem. AbstractSweden reports high duration of breastfeeding compared to international findings. During the last century, the frequency of breastfeeding duration has decreased in Sweden, with large regional differences. Recommendation for exclusive breastfeeding is six months. Midwives provide information about healthy benefits of breast milk, such as breast milk protecting baby´s against allergies. Research has presented associations between obesity and short duration of breastfeeding. However, there is a lack of knowledge regarding women in a Swedish context. Aim: To investigate the relation between mothers BMI, duration of breastfeeding and maternal valuation of the baby´s health during the first three year of life. Method: The study is a quantitative retrospective cross-sectional study. Data was collected through questionnaires, 418 participants were included. Data was analyzed using the SPSS. For descriptive and comparative analysis parametric and nonparamateric statistics have been used.Results: Women who did not breastfed perceived their baby´s health worse at three years compared with women who did breastfeed. Women with overweight and obesity also perceived the health of their three year old worse than the other maternal group. Conclusions: It´s important to identify women who need support to initiate breastfeeding and give them customized support.

Background: Although breast milk has numerous benefits for infants' development, with greater effects in those born preterm (at < 37 gestational weeks), mothers of preterm infants have shorter breastfeeding duration than mothers of term infants. One of the explanations proposed is the difficulties in the transition from a Neonatal Intensive Care Unit (NICU) to the home environment. A person-centred proactive telephone support intervention after discharge from NICU is expected to promote mothers' sense of trust in their own capacity and thereby facilitate breastfeeding. Methods/design: A multicentre randomized controlled trial has been designed to evaluate the effectiveness and cost-effectiveness of person-centred proactive telephone support on breastfeeding outcomes for mothers of preterm infants. Participating mothers will be randomized to either an intervention group or control group. In the intervention group person-centred proactive telephone support will be provided, in which the support team phones the mother daily for up to 14 days after hospital discharge. In the control group, mothers are offered a person-centred reactive support where mothers can phone the breastfeeding support team up to day 14 after hospital discharge. The intervention group will also be offered the same reactive telephone support as the control group. A stratified block randomization will be used; group allocation will be by high or low socioeconomic status and by NICU. Recruitment will be performed continuously until 1116 mothers (I: 558 C: 558) have been included. Primary outcome: proportion of mothers exclusively breastfeeding at eight weeks after discharge. Secondary outcomes: proportion of breastfeeding (exclusive, partial, none and method of feeding), mothers satisfaction with breastfeeding, attachment, stress and quality of life in mothers/partners at eight weeks after hospital discharge and at six months postnatal age. Data will be collected by researchers blind to group allocation for the primary outcome. A qualitative evaluation of experiences of receiving/providing the intervention will also be undertaken with mothers and staff. Discussion: This paper presents the rationale, study design and protocol for a RCT providing person-centred proactive telephone support to mothers of preterm infants. Furthermore, with a health economic evaluation, the cost-effectiveness of the intervention will be assessed. Trial registration: NCT01806480

Information on the status of long-chain polyunsaturated fatty acids (LCPUFAs) in pregnancy and breast milk in very high fish-eating populations is limited. The aim of this study was to examine dietary intake and changes in fatty acid status in a population of pregnant women in the Republic of Seychelles. Serum docosahexaenoic acid (DHA) decreased significantly between 28-week gestation and delivery (n=196). DHA status did not correlate significantly with length of gestation and was not associated with self-reported fish intake, which was high at 527 g/week. In breast milk, the ratio of DHA to arachidonic acid (AA) was consistent with those observed in other high fish-eating populations. Overall the data suggest that high exposure to LCPUFAs from habitual fish consumption does not prevent the documented decrease in LCPUFA status in pregnancy that occurs as a result of foetal accretion in the third trimester of pregnancy.

Blood levels of polyunsaturated fatty acids (PUFA) are considered biomarkers of status. Alpha-linolenic acid, ALA, the plant omega-3, is the dietary precursor for the long-chain omega-3 PUFA eicosapentaenoic acid (EPA), docosapentaenoic acid (DPA), and docosahexaenoic acid (DHA). Studies in normal healthy adults consuming western diets, which are rich in linoleic acid (LA), show that supplemental ALA raises EPA and DPA status in the blood and in breast milk. However, ALA or EPA dietary supplements have little effect on blood or breast milk DHA levels, whereas consumption of preformed DHA is effective in raising blood DHA levels. Addition of ALA to the diets of formula-fed infants does raise DHA, but no level of ALA tested raises DHA to levels achievable with preformed DHA at intakes similar to typical human milk DHA supply. The DHA status of infants and adults consuming preformed DHA in their diets is, on average, greater than that of people who do not consume DHA. With no other changes in diet, improvement of blood DHA status can be achieved with dietary supplements of preformed DHA, but not with supplementation of ALA, EPA, or other precursors.

Plumbagin (PLB) has shown anti-cancer activity but the mechanism is unclear. This study has found that PLB has a potent pro-apoptotic and pro-autophagic effect on A549 and H23 cells. PLB arrests cells in G2/M phase, and increases the intracellular level of reactive oxygen species in both cell lines. PLB dose-dependently induces autophagy through inhibition of PI3K/Akt/mTOR pathway as indicated by reduced phosphorylation of Akt and mTOR. Inhibition or induction of autophagy enhances PLB-induced apoptosis. There is crosstalk between PLB-induced apoptosis and autophagy. These findings indicate that PLB initiates both apoptosis and autophagy in NSCLC cells through coordinated pathways.

BACKGROUND: Mastitis is an acute, debilitating condition that occurs in approximately 20 % of breastfeeding women who experience a red, painful breast with fever. This paper describes the factors correlated with mastitis and investigates the presence of Staphylococcus aureus in women who participated in the CASTLE (Candida and Staphylococcus Transmission: Longitudinal Evaluation) study. The CASTLE study was a prospective cohort study which recruited nulliparous women in late pregnancy in two maternity hospitals in Melbourne, Australia in 2009-2011.

METHODS: Women completed questionnaires at recruitment and six time-points in the first eight weeks postpartum. Postpartum questionnaires asked about incidences of mastitis, nipple damage, milk supply, expressing practices and breastfeeding problems. Nasal and nipple swabs were collected from mothers and babies, as well as breast milk samples. All samples were cultured for S. aureus. "Time at risk" of mastitis was defined as days between birth and first occurrence of mastitis (for women who developed mastitis) and days between birth and the last study time-point (for women who did not develop mastitis). Risk factors for incidence of mastitis occurring during the time at risk (Incident Rate Ratios [IRR]) were investigated using a discrete version of the multivariable proportional hazards regression model.

RESULTS: Twenty percent (70/346) of participants developed mastitis. Women had an increased risk of developing mastitis if they reported nipple damage (IRR 2.17, 95 % CI 1.21, 3.91), over-supply of breast milk (IRR 2.60, 95 % CI 1.58, 4.29), nipple shield use (IRR 2.93, 95 % CI 1.72, 5.01) or expressing several times a day (IRR 1.64, 95 % CI 1.01, 2.68). The presence of S. aureus on the nipple (IRR 1.72, 95 % CI 1.04, 2.85) or in milk (IRR 1.78, 95 % CI 1.08, 2.92) also increased the risk of developing mastitis.

CONCLUSIONS: Nipple damage, over-supply of breast milk, use of nipple shields and the presence of S. aureus on the nipple or in breast milk increased the mastitis risk in our prospective cohort study sample. Reducing nipple damage may help reduce maternal breast infections.
