Closed-form solutions are presented for approximate equations governing the pulsatile flow of blood through models of mild axisymmetric arterial stenosis, taking into account the effect of arterial distensibility. Results indicate the existence of back-flow regions and the phenomenon of flow-reversal in the cross-sections. The effects of pulsatility of flow and elasticity of vessel wall for arterial blood flow through stenosed vessels are determined.

The propagation characteristics of a visco-elastic fluid in a distensible tube tube are studied. The linear visco-elastic nature of the fluid is described by a complex coefficient of viscosity η*. The equation of motion of the vessel wall takes into account the pulsatile nature of the wall. Results are presented for wave propagation velocity, the resistance and the reactance of the fluid and the wall impedance. It is seen that the visco-elastic influence is significant for high values of the frequency of oscillation in various arterial vessels.

Migraine is a complex neurological disorder with a well-documented genetic basis. Migraine is a product of allelic variation in genes of neurological, vascular and hormonal origin interacting with environmental triggers. Presentation can include attacks of head pain with symptoms of nausea, emesis, photophobia, phonophobia, and occasionally, visual sensory disturbances, known as aura. Migraine pain is difficult to ignore, associated with a deep sense of malaise and manifests as a throbbing, pulsatile headache, localized to one side of the head that intensifies with physical activity and that can last from 4-72 hours. Migraine is diagnosed according to criteria developed by the International Headache Society (IHS) and is subdivided into two main types based on the occurrence of aura symptoms that may be present in the early stages of the headache: migraine with aura (MA) and migraine without aura (MO). The majority (about 70%) of migraineurs are diagnosed with the MO subtype whilst the remaining 30% experience MA accompanied by neurological symptoms that manifest as fully reversible, visual, sensory and/or dysphasic speech disturbances in conjunction with their headache. Glutamate is the primary excitatory neurotransmitter in the central nervous system (CNS) and over-excitation of glutamate receptors is regarded as a contributing factor, through various mechanisms, to the pathology of migraine. In this chapter we present an overview of the pathophysiology and co-morbidity of migraine with other psychiatric disorders and discuss the role of the glutamatergic system in migraine, its molecular components as potential drug targets, in addition to the current treatments and progress of modulators of glutamatergic signaling.

The adequacy of anesthesia has been studied since the introduction of balanced general anesthesia. Commercial monitors based on electroencephalographic (EEG) signal analysis have been available for monitoring the hypnotic component of anesthesia from the beginning of the 1990s. Monitors measuring the depth of anesthesia assess the cortical function of the brain, and have gained acceptance during surgical anesthesia with most of the anesthetic agents used. However, due to frequent artifacts, they are considered unsuitable for monitoring consciousness in intensive care patients. The assessment of analgesia is one of the cornerstones of general anesthesia. Prolonged surgical stress may lead to increased morbidity and delayed postoperative recovery. However, no validated monitoring method is currently available for evaluating analgesia during general anesthesia. Awareness during anesthesia is caused by an inadequate level of hypnosis. This rare but severe complication of general anesthesia may lead to marked emotional stress and possibly posttraumatic stress disorder. In the present series of studies, the incidence of awareness and recall during outpatient anesthesia was evaluated and compared with that of in inpatient anesthesia. A total of 1500 outpatients and 2343 inpatients underwent a structured interview. Clear intraoperative recollections were rare the incidence being 0.07% in outpatients and 0.13% in inpatients. No significant differences emerged between outpatients and inpatients. However, significantly smaller doses of sevoflurane were administered to outpatients with awareness than those without recollections (p<0.05). EEG artifacts in 16 brain-dead organ donors were evaluated during organ harvest surgery in a prospective, open, nonselective study. The source of the frontotemporal biosignals in brain-dead subjects was studied, and the resistance of bispectral index (BIS) and Entropy to the signal artifacts was compared. The hypothesis was that in brain-dead subjects, most of the biosignals recorded from the forehead would consist of artifacts. The original EEG was recorded and State Entropy (SE), Response Entropy (RE), and BIS were calculated and monitored during solid organ harvest. SE differed from zero (inactive EEG) in 28%, RE in 29%, and BIS in 68% of the total recording time (p<0.0001 for all). The median values during the operation were SE 0.0, RE 0.0, and BIS 3.0. In four of the 16 organ donors, EEG was not inactive, and unphysiologically distributed, nonreactive rhythmic theta activity was present in the original EEG signal. After the results from subjects with persistent residual EEG activity were excluded, SE, RE, and BIS differed from zero in 17%, 18%, and 62% of the recorded time, respectively (p<0.0001 for all). Due to various artifacts, the highest readings in all indices were recorded without neuromuscular blockade. The main sources of artifacts were electrocauterization, electromyography (EMG), 50-Hz artifact, handling of the donor, ballistocardiography, and electrocardiography. In a prospective, randomized study of 26 patients, the ability of Surgical Stress Index (SSI) to differentiate patients with two clinically different analgesic levels during shoulder surgery was evaluated. SSI values were lower in patients with an interscalene brachial plexus block than in patients without an additional plexus block. In all patients, anesthesia was maintained with desflurane, the concentration of which was targeted to maintain SE at 50. Increased blood pressure or heart rate (HR), movement, and coughing were considered signs of intraoperative nociception and treated with alfentanil. Photoplethysmographic waveforms were collected from the contralateral arm to the operated side, and SSI was calculated offline. Two minutes after skin incision, SSI was not increased in the brachial plexus block group and was lower (38 ± 13) than in the control group (58 ± 13, p<0.005). Among the controls, one minute prior to alfentanil administration, SSI value was higher than during periods of adequate antinociception, 59 ± 11 vs. 39 ± 12 (p<0.01). The total cumulative need for alfentanil was higher in controls (2.7 ± 1.2 mg) than in the brachial plexus block group (1.6 ± 0.5 mg, p=0.008). Tetanic stimulation to the ulnar region of the hand increased SSI significantly only among patients with a brachial plexus block not covering the site of stimulation. Prognostic value of EEG-derived indices was evaluated and compared with Transcranial Doppler Ultrasonography (TCD), serum neuron-specific enolase (NSE) and S-100B after cardiac arrest. Thirty patients resuscitated from out-of-hospital arrest and treated with induced mild hypothermia for 24 h were included. Original EEG signal was recorded, and burst suppression ratio (BSR), RE, SE, and wavelet subband entropy (WSE) were calculated. Neurological outcome during the six-month period after arrest was assessed with the Glasgow-Pittsburgh Cerebral Performance Categories (CPC). Twenty patients had a CPC of 1-2, one patient had a CPC of 3, and nine patients died (CPC 5). BSR, RE, and SE differed between good (CPC 1-2) and poor (CPC 3-5) outcome groups (p=0.011, p=0.011, p=0.008, respectively) during the first 24 h after arrest. WSE was borderline higher in the good outcome group between 24 and 48 h after arrest (p=0.050). All patients with status epilepticus died, and their WSE values were lower (p=0.022). S-100B was lower in the good outcome group upon arrival at the intensive care unit (p=0.010). After hypothermia treatment, NSE and S-100B values were lower (p=0.002 for both) in the good outcome group. The pulsatile index was also lower in the good outcome group (p=0.004). In conclusion, the incidence of awareness in outpatient anesthesia did not differ from that in inpatient anesthesia. Outpatients are not at increased risk for intraoperative awareness relative to inpatients undergoing general anesthesia. SE, RE, and BIS showed non-zero values that normally indicate cortical neuronal function, but were in these subjects mostly due to artifacts after clinical brain death diagnosis. Entropy was more resistant to artifacts than BIS. During general anesthesia and surgery, SSI values were lower in patients with interscalene brachial plexus block covering the sites of nociceptive stimuli. In detecting nociceptive stimuli, SSI performed better than HR, blood pressure, or RE. BSR, RE, and SE differed between the good and poor neurological outcome groups during the first 24 h after cardiac arrest, and they may be an aid in differentiating patients with good neurological outcomes from those with poor outcomes after out-of-hospital cardiac arrest.

Data on molar excess enthalpy on mixing at 298.15 K and 308.15 K, vapor-liquid equilibrium, latent heats of vaporization at 91.444 kPa and vapor pressures for the system toluene – 1, 1, 1-trichloroethane are presented. A simple adiabatic calorimeter designed for molar excess enthalpy measurements is described, tested and used. On présente, dans le cas du système toluène – 1, 1, 1-trichloréthane, des résultats relatifs aux grandeurs suivantes: a) enthalpie molaire d'excès à 298.15 K et 308.15 K; b) équilibre liquid-vapeur; c) chaleurs latentes de vaporisation à une pression absolue de 91.444 kP; d) pressions de vapeur. On décrit un calorimètre adiabatique simple, conçu pour mesurer l'enthalpie molaire d'excès, dont on a fait l'essai.

We study theoretically the hydrodynamics of a fluid drop containing oriented filaments endowed with active contractile or extensile stresses and placed on a solid surface. The active stresses alter qualitatively the wetting properties of the drop, leading to new spreading laws and novel static drop shapes. Candidate systems for testing our predictions include cytoskeletal extracts with motors and ATP, suspensions of bacteria or pulsatile cells, or fluids laden with artificial self-propelled colloids.

A droplet introduced in an external convective flow field exhibits significant multimodal shape oscillations depending upon the intensity of the aerodynamic forcing. In this paper, a theoretical model describing the temporal evolution of normal modes of the droplet shape is developed. The fluid is assumed to be weakly viscous and Newtonian. The convective flow velocity, which is assumed to be incompressible and inviscid, is incorporated in the model through the normal stress condition at the droplet surface and the equation of motion governing the dynamics of each mode is derived. The coupling between the external flow and the droplet is approximated to be a one-way process, i.e., the external flow perturbations effect the droplet shape oscillations and the droplet oscillation itself does not influence the external flow characteristics. The shape oscillations of the droplet with different fluid properties under different unsteady flow fields were simulated. For a pulsatile external flow, the frequency spectra of the normal modes of the droplet revealed a dominant response at the resonant frequency, in addition to the driving frequency and the corresponding harmonics. At driving frequencies sufficiently different from the resonant frequency of the prolate-oblate oscillation mode of the droplet, the oscillations are stable. But at resonance the oscillation amplitude grows in time leading to breakup depending upon the fluid viscosity. A line vortex advecting past the droplet, simulated as an isotropic jump in the far field velocity, leads to the resonant excitation of the droplet shape modes if and only if the time taken by the vortex to cross the droplet is less than the resonant period of the P-2 mode of the droplet. A train of two vortices interacting with the droplet is also analysed. It shows clearly that the time instant of introduction of the second vortex with respect to the droplet shape oscillation cycle is crucial in determining the amplitude of oscillation. [ES] En el presente estudio se evalúa la relación entre el estado nutricional, las actitudes y comportamientos dietéticos, y la percepción de la imagen corporal en universitarios. Se registraron más casos de sobrepeso en hombres que en mujeres. Sin embargo, la tendencia a “hacer dietas” fue mayor en las mujeres, lo que podría deberse a la presión social por mantener una imagen estilizada.

[ES] Actualmente, cada vez es mayor la preocupación entre los jóvenes por el aspecto físico. Por eso nos propusimos evaluar la percepción de la imagen corporal en universitarios. Las mujeres eligieron figuras significativamente más delgadas que los hombres, y presentaron mayor insatisfacción con su imagen. Estos resultados pueden estar influenciados por la presión social sobre la imagen de la mujer.

[ES] La presión que la sociedad ejerce sobre sus miembros hace que , exista un tendencia generalizada a la excesiva preocupación por el peso y la apariencia física. Se estudiaron las actitudes frente al peso de 526 estudiantes, 158 hombres y 368 mujeres. Muestran un deseo de ser más delgados de lo que son aunque consideran su peso menor al saludable.

O objetivo do estudo foi mensurar as alterações na quantidade do volume de fluido gengival (FG) dos dentes submetidos à movimentação ortodôntica, em adultos com periodontite crônica na fase de manutenção periodontal. O FG foi coletado com uma tira de papel absorvente padrão das áreas de pressão dos dentes de 10 pacientes (sete homens e três mulheres) e medido em microlitro no Periotron em seis dias distintos (dia -7, dia 0, dia 1, dia 7, dia 14 e dia 21). Os pacientes foram submetidos a um rigoroso programa de controle de placa. O teste de Wilcoxon foi aplicado para comparar os dados obtidos. Os resultados mostraram que o controle de placa efetivo baixou a quantidade de FG do dia -7 para o dia 0, e em seguida houve uma elevação do FG até o dia 7 por conta do início da movimentação ortodôntica, e uma nova queda do dia 7 até o 21. O maior volume de FG foi encontrado no dia 7, uma semana após o início da movimentação. O menor volume no dia 0. Concluindo, a movimentação ortodôntica aumentou o volume de FG do dia 0 até o dia 7.

Na última década, surgiram evidências de que a Síndrome Metabólica (SM), relatada de forma crescente entre adolescentes, tem início na vida intrauterina e seus sinais e sintomas já estão presentes na adolescência, porém, ainda faltam critérios diagnósticos específicos para essa faixa etária. O ciclo menstrual representa o resultado do funcionamento normal não apenas do eixo Hipotálamo-Hipófise-Ovário (HHO), do útero e do aparelho genital, mas também, do equilíbrio metabólico do organismo. Alterações no ciclo menstrual podem representar sinais de desequilíbrio e anormalidade. A SM está também relacionada à Síndrome dos Ovários Policísticos (SOP), disfunção ovariana caracterizada por oligoanovulação, hiperandrogenismo e/ou ovários policísticos. A resistência à insulina (RI) tem um papel central na fisiopatologia e na inter-relação dos componentes tanto da SM como também da SOP. A RI é compensada pelo aumento da produção de insulina pelas células beta pancreáticas, e essa hiperinsulinemia compensatória tem conseqüências no endotélio, nos fatores inflamatórios, no metabolismo glicídico e lipídico, além de afetar o ciclo menstrual pelo estímulo da androgênese ovariana, suprimindo a SHBG e possivelmente alterando o padrão da secreção pulsátil do GnRH. Estas alterações menstruais podem apresentar-se de forma precoce, antes das alterações metabólicas da RI, portanto, a avaliação atenta do padrão menstrual de adolescentes pode representar um valioso sinal que alerta para o risco metabólico e cardiovascular. Avaliamos o comportamento de parâmetros da Síndrome Metabólica e sua relação com o ciclo menstrual em adolescentes através de um estudo observacional transversal com 59 adolescentes do sexo feminino entre 12 e 19 anos e presença de pelo menos um dos seguintes fatores de risco para SM: Sobrepeso - Obesidade - Acantose Nigricans. Todas as adolescentes foram submetidas a uma avaliação clínica com levantamento de dados antropométricos, e laboratoriais composta de: Glicose de Jejum, Colesterol Total, HDL-Colesterol, Triglicerídeos, Teste Oral de Tolerância a Glicose (Glicose 120), Insulina pré (insulina jejum), pós TOTG (insulina 120), Folículo-Estimulante (FSH), Hormônio Luteinizante (LH), Testosterona Total (TT), Androstenediona, Foram criados 2 grupos:G-1- adolescentes com ciclos irregulares, e G-2- adolescentes com ciclos regulares. Das 59 adolescentes avaliadas, 36 formaram o G-1, e 23 o G-2. A média da idade ginecológica foi de 4,5 anos e da menarca 11,3 anos. Na análise estatística das diferenças nas variáveis clínicas e laboratoriais entre os grupos, observou-se que o G-1 apresentou: Cintura (p=0,026), Insulina de jejum (p=0,001), Glicose 120 (p=0,002), insulina 120 (p=0,0001), HOMA-IR (p=0,0008), Triglicerídeos (p=0,013), SM (p<0,0001) e SOP (p<0,0001) significativamente maiores e QUICK (p=0,008), G/I (p=0,002), HDL (p=0,001) significativamente menores que o G-2. (88,8% das adolescentes com ciclos irregulares no ultimo ano apresentavam irregularidade desde a menarca. Estes resultados demonstram uma associação significativa entre a irregularidade menstrual, RI, SM e SOP na população estudada. Todas as adolescentes com diagnóstico de SM apresentavam irregularidade desde a menarca e destas, 93,5% tiveram o diagnóstico de SOP. O nosso estudo chama a atenção para o comportamento do ciclo menstrual na adolescência em relação aos riscos cardiovasculares e metabólicos, sinalizando assim que outros estudos precisam ser desenvolvidos nesta população.

A presente pesquisa investiga a produção de subjetividade em seminários católicos. Por meio de diversos teóricos, girando em torno do eixo conceitual da Análise Institucional (em sua vertente socioanalítica e, principalmente, esquizoanalítica), pesquisou-se um importante seminário católico diocesano na região Sudeste, um dos primeiros fundados no Brasil. Um seminário é a instituição de internato em que muitos homens vivem, estudam e trabalham, durante oito anos em média, para se tornarem padres. Nossa pesquisa de campo utilizou-se da observação participante, oriunda da antropologia e sociologia, em diversas visitas ao estabelecimento, durante as quais fomos autorizados a participar de todas as atividades dos 110 seminaristas internos. Em seguida, alguns seminaristas, de todas as etapas da formação, e todos os 5 padres formadores foram entrevistados. Recorremos a entrevistas abertas de história de vida, segundo os procedimentos da História Oral italiana. Guiados pela distinção da filosofia de Spinoza de três gêneros de conhecimento, buscamos dramatizar a formação clerical em três níveis de compreensão: os Signos ou afectos, as Noções ou conceitos, as Essências ou perceptos. Por meio dessa trajetória, constatamos o predomínio da dimensão do instituído e de uma modelização homogeneizadora de tipo romana, resultando em que os clérigos ali formados se fechem em uma identidade sacerdotal claramente identificável e encapsulada na obediência aos centros de poder eclesiais: a Cúria romana, a Mitra diocesana e a Paróquia. Busca-se, na formação, reproduzir a subjetividade serializada segundo um Modelo sacerdotal institucionalizado, no qual as dimensões litúrgica e disciplinar são ressaltadas, em detrimento das dimensões místico-políticas sendo a perseguição à Teologia da Libertação um importante analisador dessa característica. A constante vigilância da pureza doutrinal, litúrgica, organizacional e teológica indicou-nos a pressão em reprimir a dimensão místico-profética, que range, querendo se expressar. Em vista disso, bem como de inúmeros outros analisadores, conceitos e personagens produzidos ao longo da pesquisa, pudemos constatar que o desejo clerical, modulado na formação seminarística, oscila entre dois pólos: um pólo sacerdotal-romano-paranóico e um pólo profético-libertador-esquizo. No primeiro, há redução à identidade hegemônica nascida nos centros de poder eclesiais, fechando-se à diferença, na busca de um projeto de imortalidade frente às intempéries da vida e transformações da História, produzindo práticas hierárquicas a partir de um pensamento de caráter transcendente, representativo. No segundo pólo, há busca de singularização, nascida do seguimento a Jesus, o conseqüente compromisso com os menores e excluídos dentro e fora da Igreja e o processo inerente a esta produção de sentido, criando-se, em conseqüência, uma radical imanentização da vida cristã e de seus pólos e transcendências: material/espiritual, fé/vida, Igreja/Mundo, mística/política. O seminário pesquisado, indicador das transformações micropolíticas da Igreja contemporânea, produz hegemonicamente um desejo sacerdotal-romano-paranóico, forjando funcionários do poder da Igreja, burocratas do aparelho de Estado romano, aplicadores de suas rubricas litúrgicas e normas doutrinais e morais, e não profetas do Reino de Deus, máquinas de guerra libertadoras quanto a tudo o que oprime a potência da vida e suas inauditas expressões singulares.

Computational analyses of dendritic computations often assume stationary inputs to neurons, ignoring the pulsatile nature of spike-based communication between neurons and the moment-to-moment fluctuations caused by such spiking inputs. Conversely, circuit computations with spiking neurons are usually formalized without regard to the rich nonlinear nature of dendritic processing. Here we address the computational challenge faced by neurons that compute and represent analogue quantities but communicate with digital spikes, and show that reliable computation of even purely linear functions of inputs can require the interplay of strongly nonlinear subunits within the postsynaptic dendritic tree.Our theory predicts a matching of dendritic nonlinearities and synaptic weight distributions to the joint statistics of presynaptic inputs. This approach suggests normative roles for some puzzling forms of nonlinear dendritic dynamics and plasticity.
