Abstrakt Bakgrund. Anknytning är ett viktigt band som skapas mellan mor och barn, detta är viktigt för att barnet ska utvecklas till en trygg person. För att anknytning ska uppstå krävs mycket närhet och värme. Vid brist på anknytning blir barnet passivt och apatiskt. Syfte. Syftet med denna studie var att belysa hur anknytningsprocessen mellan mor och barn kan påverkas vid olika förlossningssätt. Metod. En litteratursökning av artiklar publicerade mellan 1989-2007 gjordes, sökorden var attachment, cesarean section, delivery, mother-child relations och nurs*. 28 artiklar granskades noga och av dessa stämde 14 helt överens med inklusionskriterierna och vårt syfte. Dessa bearbetades och analyserades i litteraturstudien. Resultat. Analysen resulterade i tre kategorier. Den första var faktorer som kan påverka anknytningsprocessen, den andra var en komplicerad förlossnings inverkan på anknytningsprocessen och den tredje var omvårdnadshandlingar som kan underlätta anknytningsprocessen och sjukhusvistelsen för modern. Diskussion. Komplicerade förlossningar bidrog ofta till att kvinnan inte fick träffa sitt barn direkt efter förlossningen, något som var psykiskt påfrestande för kvinnan och negativt för anknytningsprocessen. Kvinnor som genomgick komplicerade förlossningar var i extra stort behov av stöd och information från personalen för att känna närhet till sitt barn. Konklusion. När en kvinna får känna sitt barns närhet direkt efter förlossningen så startar anknytningsprocessen fortare än om det första mötet blir uppskjutet. Om omvårdnadspersonalen använder sig av den kunskap som finns angående interaktionen mellan mor och barn så kan det underlätta för kvinnan i samspelet med sitt barn.

La sintomatologia ansiosa materna nel periodo prenatale risulta influire negativamente non sullo stato materno ma anche sul successivo sviluppo infantile, Tuttavia, sono limitati gli studi che hanno considerato lo specifico contributo dei disturbi d’ansia nel periodo prenatale. L’obiettivo generale dello studio è quello di indagare nel primo periodo post partum la relazione tra psicopatologia ansiosa materna e: temperamento e sviluppo neonatale, qualità del caregiving materno e dei pattern interattivi madre-bambino. 138 donne sono state intervistate utilizzando SCID-I (First et al., 1997) durante il terzo trimestre di gravidanza. 31 donne (22,5%) presentano disturbo d’ansia nel periodo prenatale. A 1 mese post partum il comportamento del neonato è stato valutato mediante NBAS (Brazelton, Nugent, 1995), mentre le madri hanno compilato MBAS (Brazelton, Nugent, 1995). A 3 mesi postpartum, una sequenza interattiva madre-bambino è stata videoregistrata e codificata utilizzando GRS (Murray et al., 1996). La procedura dello Stranger Episode (Murray et al., 2007) è stata utilizzata per osservare i pattern interattivi materni e infantili nell’interazione con una persona estranea. I neonati di madri con disturbo d’ansia manifestano alle NBAS minori capacità a livello di organizzazione di stati comportamentali, minori capacità attentive e di autoregolazione. Le madri ansiose si percepiscono significativamente meno sicure nell’occuparsi di loro, valutando i propri figli maggiormente instabili e irregolari. Nell’interazione face to face, esse mostrano comportamenti significativamente meno sensibilI, risultando meno coinvolte attivamente con il proprio bambino. Durante lo Stranger Episode, le madri con fobia sociale presentano maggiori livelli di ansia e incoraggiando in modo significativamente inferiore l’interazione del bambino con l’estraneo. I risultati sottolineano l’importanza di valutare in epoca prenatale la psicopatologia ansiosa materna. Le evidenze confermano la rilevanza che può assumere un modello multifattoriale di rischio in cui i disturbi d’ansia prenatali e la qualità del caregiving materno possono agire in modo sinergico nell’influire sugli esiti infantili.

Quais os efeitos do diagnóstico de prematuridade no discurso das mães e suas repercussões na relação mãe-criança? Partindo desta questão, que surgiu na experiência psicanalítica em um hospital pediátrico, o presente estudo visa analisar os efeitos do diagnóstico de prematuridade no discurso materno a partir de entrevistas psicanalíticas preliminares com mães de crianças prematuras. Trata-se de uma pesquisa clínica qualitativa que propõe uma articulação entre clínica e teoria a partir da construção de quatro casos clínicos fundamentados pelo referencial teórico da psicanálise de Freud, Lacan e autores contemporâneos. Se no imaginário social a prematuridade é associada a dificuldades em diversos contextos da vida, a análise de cada caso revela que este diagnóstico pode ou não ser encadeado pelo sujeito aos significantes que o marcam de forma prevalente. Nesta via, a prematuridade se desloca do lugar determinante de algo que sempre marca e decide, para um lugar que só pode ser escutado no um a um. Para a análise dos casos, elencou-se a divisão mãe-mulher como operador conceitual central dada sua prevalência nos discursos, em um percurso teórico que parte da história do amor materno ao exame psicanalítico da maternidade a partir da sexualidade feminina. Tais considerações partem do mito do amor materno de Badinter, em direção à equivalência do filho como substituto da falta fálica em Freud, até à ênfase ao desejo da mulher na mãe em Lacan. A discussão apresenta os diferentes lugares atribuídos à prematuridade por cada sujeito feminino e a prevalência de impasses próprios à conjunção e disjunção mãe-mulher incidindo na relação mãe-criança

Objective: To assess the effect of reduced skin exposure in preterm infants receiving overhead phototherapy treatment on total serum bilirubin (TSB). Methods: Randomized controlled trial. Preterm infants (>1500 g birthweight and less than or equal to 36 weeks gestation) were randomized to being nursed either partially clothed with only disposable nappies and in posturally supported positions (n = 30) or naked without postural support (n = 29). Primary outcome was mean TSB percentage change at 24 h of completed conventional overhead phototherapy treatment (irradiance of 6 muWcm(-2)/nm at a wavelength of 425-475 nm). The incidence of rebound jaundice, number of infants continuing to receive phototherapy treatment at 24 h periods, parental stress, mother-infant interaction and mean TSB percentage change at 24 h of completed conventional overhead phototherapy treatment were examined. Results: Mean TSB percentage change at 24 h of completed treatment for the partially clothed group was 15.4% (+/-18) and for the naked group 19% (+/-15) (mean difference 3.6% 95% CI -5.1, 12.3). No other outcomes were significantly affected by reduced skin exposure to overhead phototherapy treatment. Conclusion: Our results show no statistically significant difference in TSB level change using either nursing practice.

The leatherback turtle Dermochelys coriacea is considered to be at serious risk of global extinction, despite ongoing conservation efforts. Intensive long-term monitoring of a leatherback nesting population on Sandy Point (St. Croix, US Virgin Islands) offers a unique opportunity to quantify basic population parameters and evaluate effectiveness of nesting beach conservation practices. We report a significant increase in the number of females nesting annually from ca. 18-30 in the 1980s to 186 in 2001, with a corresponding increase in annual hatchling production from ca. 2000 to over 49,000. We then analyzed resighting data from 1991 to 2001 with an open robust-design capture-mark-recapture model to estimate annual nester survival and adult abundance for this population. The expected annual survival probability was estimated at ca. 0.893 (95% CL 0.87-0.92) and the population was estimated to be increasing ca. 13% pa since the early 1990s. Taken together with DNA fingerprinting that identify mother-daughter relations, our findings suggest that the increase in the size of the nesting population since 1991 was probably due to an aggressive program of beach protection and egg relocation initiated more than 20 years ago. Beach protection and egg relocation provide a simple and effective conservation strategy for this Northern Caribbean nesting population as long as adult survival at sea remains relatively high. Objective: To know the mothers’ perception of the main difficulties in the care of children with special needs dependent on mechanical ventilation. Methods: A qualitative research conducted from August to November 2013 at the Dr. Waldemar Alcântara Hospital in Fortaleza, CE. Respondents were seven mothers of children under mechanical ventilation. Data were collected using sociodemographic questionnaires and guiding questions, from which emerged thematic categories that underwent content analysis, with data being discussed in the light of the relevant literature. Results: The average age of mothers ranged 18-36 years. Six were married or had formed a common-law marriage. Education level ranged from illiteracy to complete secondary education. Family income was up to one minimum wage. Mothers reported anguish and fear regarding the initial impact of the diagnosis, with little understanding of the biomedical language and a lot of questions during the whole period after diagnosis and during hospitalization. Conclusion: Difficulties involve aspects related to family abandonment, the distancing from the other children at the expense of the care of the disable child, the difficulty in engaging in social relationships, and the prognosis and care of their child. The hospital environment generates psychological repercussions on the expectation and hopelessness regarding the cure of their child, as they know they might die. Healthcare professionals can favor incredible transformations, generating a new “care”, broader and humanized, facilitating the recovery/restructuring of the family within this new universe.

This research investigated the role of mother-centred issues that influence breastfeeding behaviours. The need for social marketing research for breastfeeding is indicated by the fact that despite evidence of the health benefits to both the infant and mother of longer breastfeeding duration, rates in developed countries have failed to increase in recent decades. Breastfeeding is a complex behaviour that for many women involves barriers that influence their commitment to continue breastfeeding. Structural equation modelling was used on a sample of 405 respondents to an online survey. The analysis revealed that personal social support had a significant impact on breastfeeding self-efficacy, which in turn had a significant impact on breastfeeding behaviour. The findings and implications for both social marketing theory and practice are discussed.

The past decade has witnessed the publication of a growing number of important ethnographic studies investigating the schooling experiences of Black students. Their focus has largely been upon student-teacher relations during the students' last few years of compulsory schooling. What they have highlighted is the complexity of racism and the varied nature of Black students' experiences of schooling. Drawing upon data from a year-long ethnographic study of an inner-city, multi-ethnic primary school, this paper aims to compliment these studies in two ways. Firstly the paper will broaden the focus to examine how student peer-group relations play an integral role, within the context of student-teacher relations, in shaping many Black students' schooling experiences. By focussing on African/Caribbean infant boys, it will be shown how student-teacher relations on the one hand, and peer-group relations on the other, form a continuous feed-back loop; the products of each tending to exacerbate and inflate the other. Secondly, by concentrating on infant children, the paper will assess the extent to which these resultant processes and practices are also evident for Black pupils at the beginning of their school careers - at the ages of five and six.
