La prevalencia de coledocolitiasis es de un 10 a 20%. 10-20% tienen coledocolitiasis gigante, es decir presencia de cálculos mayores de 15 mm, aumentando la morbimortalidad por complicaciones. El objetivo principal fue determinar la frecuencia de coledocolitiasis gigante, la presencia de factores predictores del éxito o fracaso del manejo endoscópico. El éxito en el manejo endoscópico está entre 80 y 90%, un 20% requieren cirugía de exploración biliar. Se realizó la búsqueda de las variables utilizando el instrumento para la recolección de la información. Se realizó un análisis univariado y bivariado de las variables medidas y se utilizo STATA versión 10. Como principal resultado, se encontró que la frecuencia de coledocolitiasis gigante en nuestra población fue del 10%, el éxito del manejo endoscopio fue del 89.23% y el factor predictor mas fuerte para el éxito fue el diámetro del cálculo, siendo mayor para cálculos de menos de 19.09 mm. Como conclusión, en nuestro estudio, la frecuencia de coledocolitiasis gigante es cercana a la conocida en la literatura mundial. El manejo endoscópico en nuestro estudio es el pilar en estos casos, teniendo probabilidad de éxito en el manejo que es igual a la publicada en los estudios mundiales, que existe la probabilidad que el tamaño del cálculo mayor a 19 mm de diámetro indique mayor tasa de fracaso y requerimiento de técnicas endoscópicas avanzadas para su éxito. Se requieren estudios, con mayor número de pacientes para determinar la validez estadística de estos resultados.

El uso de seguro de medicamentos, dentro del proceso de atención en salud, ha tomado importancia a nivel nacional e internacional al evidenciarse el aumento de incidentes y eventos relacionados con los medicamentos, que involucran desde el procesos de adquisición, pasando por la prescripción médica, la dispensación, la administración de medicamentos, hasta llegar a la utilización en los pacientes y cuidadores. El uso adecuado de medicamentos es una de las metas de seguridad del paciente de la Organización Mundial de la Salud, en las cuales trabaja todos los días el Instituto de Ortopedia Infantil Roosevelt, y en el caso específico de medicamentos lo hace a través de su Programa de Farmacovigilancia haciendo seguimiento y clasificación de los casos reportados, donde para los años 2012 al 2013 se observan avances considerables y la consolidación de estrategias que mejoran el uso del medicamento, que se da con una inducción adecuada a cada uno de los funcionarios que intervienen en este proceso y a la educación permanente de colaboradores, pacientes y sus cuidadores. El propósito de este estudio fue analizar los datos encontrados en las bases de del programa de Farmacovigilancia en el Instituto de Ortopedia Infantil Roosevelt en los años 2012-2013, estableciendo variables, comparando los resultados, determinando el estado actual y de ser posible establecer lineamientos de mejora al mismo

Objetivo: Determinar la ocurrencia de reacciones adversas a medicamentos (RAM) como causa de ingreso a una unidad de cuidado intermedio de un hospital universitario. Materiales y Métodos: Se revisaron las historias clínicas de los pacientes admitidos a la Sala de Emergencias – Cuidado Intermedio (SALEM) entre septiembre y diciembre de 2012 que cumplieron los criterios de inclusión y se detectaron los casos sospechosos de reacción adversa a medicamento (RAM) que posteriormente fueron evaluados por cuatro investigadores respecto a la causalidad a través del Algoritmo de Naranjo, prevenibilidad usando los criterios de Shumock y Thornton y la clasificación clínica mediante el empleo del sistema DoTS. Resultados: Se encontraron 96 pacientes que presentaron 108 casos de RAM. Las RAM más frecuentes fueron las arritmias y la hemorragia de vías digestivas altas (12.04%), 20.3% de los casos correspondieron a fallos terapéuticos, y, los medicamentos mayormente asociados fueron el ácido acetil salicílico (15.74%) y el losartán (10.19%). 46 casos fueron catalogados como posibles y uno solo como definitivo. Usando la clasificación DoTS se estableció que en el 82.4% de los casos la dosis era colateral (dentro del rango de dosis terapéutica), 89.8% fueron independientes del tiempo, y entre los factores mayormente asociados a susceptibilidad a la RAM estuvieron las comorbilidades (41.7%) y la edad (49%). 44% de las RAM fueron prevenibles. Conclusión: Las RAM son una causa de ingreso no despreciable en una unidad de cuidado intermedio para las cuales existen diferentes sistemas de evaluación, y una cantidad significativa de ellas es prevenible. Se requieren más estudios a nivel nacional para evaluar la incidencia de estas y establecer estándares de clasificación y medidas para mitigar su efecto.

La osteoporosis es una de las condiciones patológicas en mayor crecimiento a medida que la población de tercera edad aumenta, esto se traduce en fracturas por fragilidad como lo son las fracturas de radio distal y las fracturas de cadera, actualmente no se cuentas con datos de la población a estudio que correlacione este tipo de fracturas. Es un estudio retrospectivo de casos y controles donde se obtuvo un grupo de pacientes con fractura de cadera que consultaron a un hospital universitario de alta complejidad en la ciudad de Bogotá, se evaluó la presencia de antecedente de fractura de radio distal y se comparó con un grupo control de trauma en cadera. Se obtuvo un total de 325 casos (72,5%) y 123 (25%) controles. El promedio de edad fue de 81 años, el 70% de los pacientes en ambos grupos correspondió a mujeres. No hubo diferencia en cuanto a la prevalencia de tabaquismo, hipertensión arterial o diabetes en los grupos. No se encontraron diferencias significativas en cuanto a niveles de glicemia, calcio, vitamina D. La presencia de antecedente de fractura de radio distal en grupo con fractura de cadera fue del 7,1% encontrando un OR de 3,91 IC 95%(1,17– 13,10). La presencia de fractura de radio distal como antecedente es un predictor para la fractura de cadera en pacientes mayores. Se necesitan más estudios que correlacionen otras variables que pueden influir en la asociación para fractura de cadera y radio, para así identificar una población específica que se beneficie de un tratamiento temprano.

Objectives: to identify factors associated with maternal intrapartum transfer from a freestanding birth centre to hospital. Design: case-control study with retrospective data collection. Participants and settings: cases included all 111 women transferred from a freestanding birth centre in Sao Paulo to the referral hospital, from March 2002 to December 2009. The controls were 456 women who gave birth in the birth centre during the same period who were not transferred, randomly selected with four controls for each case. Methods: data were obtained from maternal records. Factors associated with maternal intrapartum transfers were initially analysed using a chi(2) test of association. Variables with p < 0.20 were then included in multivariate analyses. A multiple logistic regression model was built using stepwise forward selection; variables which reached statistical significance at p < 0.05 were considered to be independently associated with maternal transfer. Findings: during the study data collection period, 111(4%) of 2,736 women admitted to the centre were transferred intrapartum. Variables identified as independently associated factors for intrapartum transfer included nulliparity (OR 5.1, 95% CI 2.7-9.8), maternal age >= 35 years (OR 5.4, 95% CI 2.1-13.4), not having a partner (OR 2.8, 95% CI 1.5-5.3), cervical dilation <= 3 cm on admission to the birth centre (OR 1.9, 95% CI 1.1-3.2) and between 5 and 12 antenatal appointments at the birth centre (OR 3.8, 95% CI 1.9-7.5). In contrast, a low correlation between fundal height and pregnancy gestation (OR 0.3, 95% CI 0.2-0.6) appeared to be protective against transfer. Conclusions and implications for practice: identifying factors associated with maternal intrapartum transfer could support decision making by women considering options for place of birth, and support the content of appropriate information about criteria for admission to a birth centre. Findings add to the evidence base to support identification of women in early labour who may experience later complications and could support timely implementation of appropriate interventions associated with reducing transfer rates. Objective: To build a life table and determine the factors related to the time of treatment of undernourished children at a nutrition rehabilitation centre (CREN), Sao Paulo, Brazil. Design: Nutritional status was assessed from weight-for-age, height-for-age and BMI-for-age Z-scores, while neuropsychomotor development was classified according to the milestones of childhood development. Life tables, Kaplan-Meier survival curves and Cox multiple regression models were employed in data analysis. Setting: CREN (Centre of Nutritional Recovery and Education), Sao Paulo, Brazil. Subjects: Undernourished children (n 228) from the southern slums of Sao Paulo who had received treatment at CREN under a day-hospital regime between the years 1994 and 2009. Results: The Kaplan-Meier curves of survival analysis showed statistically significant differences in the periods of treatment at CREN between children presenting different degrees of neuropsychomotor development (log-rank = 6.621; P = 0.037). Estimates based on the multivariate Cox model revealed that children aged >= 24 months at the time of admission exhibited a lower probability of nutritional rehabilitation (hazard ratio (HR) = 0.49; P = 0.046) at the end of the period compared with infants aged up 12 months. Children presenting slow development were better rehabilitated in comparison with those exhibiting adequate evolution (HR = 4.48; P = 0.023). No significant effects of sex, degree of undernutrition or birth weight on the probability of nutritional rehabilitation were found. Conclusions: Age and neuropsychomotor developmental status at the time of admission to CREN are critical factors in determining the duration of treatment.

QUESTIONS UNDER STUDY / PRINCIPLES: Interest groups advocate centre-specific outcome data as a useful tool for patients in choosing a hospital for their treatment and for decision-making by politicians and the insurance industry. Haematopoietic stem cell transplantation (HSCT) requires significant infrastructure and represents a cost-intensive procedure. It therefore qualifies as a prime target for such a policy. METHODS: We made use of the comprehensive database of the Swiss Blood Stem Cells Transplant Group (SBST) to evaluate potential use of mortality rates. Nine institutions reported a total of 4717 HSCT - 1427 allogeneic (30.3%), 3290 autologous (69.7%) - in 3808 patients between the years 1997 and 2008. Data were analysed for survival- and transplantation-related mortality (TRM) at day 100 and at 5 years. RESULTS: The data showed marked and significant differences between centres in unadjusted analyses. These differences were absent or marginal when the results were adjusted for disease, year of transplant and the EBMT risk score (a score incorporating patient age, disease stage, time interval between diagnosis and transplantation, and, for allogeneic transplants, donor type and donor-recipient gender combination) in a multivariable analysis. CONCLUSIONS: These data indicate comparable quality among centres in Switzerland. They show that comparison of crude centre-specific outcome data without adjustment for the patient mix may be misleading. Mandatory data collection and systematic review of all cases within a comprehensive quality management system might, in contrast, serve as a model to ascertain the quality of other cost-intensive therapies in Switzerland.

BACKGROUND: Mortality and morbidity are particularly high in the building industry. The annual rate of non-fatal occupational accidents in Switzerland is 1,133 per 100,000 inhabitants. METHODS: Retrospective analysis of the electronic database of a university emergency centre. Between 2001 and 2011, 782 occupational accidents to construction workers were recorded and analysed using specific demographic and medical keywords. RESULTS: Most patients were aged 30-39 (30.4%). 66.4% of the injured workers were foreigners. This is almost twice as high as the overall proportion of foreigners in Switzerland or in the Swiss labour market. 16% of the Swiss construction workers and 8% of the foreign construction workers suffered a severe injury with ISS >15. There was a trend for workers aged 60 and above to suffer an accident with a high ISS (p = 0.089). CONCLUSIONS: As in other European countries, most patients were in their thirties. Older construction workers suffered fewer injuries, although these tended to be more severe. The injuries were evenly distributed through the working days of the week. A special effort should be made that current health and safety measures are understood and applied by foreign and older construction workers.

BACKGROUND: Acute epidural and subdural haematomas remain among the most common causes of mortality and disability resulting from traumatic brain injury. In the last three decades improvements in rescue, neuromonitoring and intensive care have led to better outcomes. The purpose of this study was to evaluate the impact of these strategies on outcome in patients treated in a single institution in Switzerland. METHODS: A total of 76 consecutive patients who underwent emergency craniotomy for acute traumatic epidural and subdural haematoma at University Hospital Bern between January 2000 and December 2003 were included in this study. RESULTS: Thirty-seven patients presented with an epidural haematoma and 46 with a subdural haematoma. In seven patients both haematomas could be documented. The median age was 54 years (IQR 28). The median initial GCS score was 7 (IQR 6). The median time from primary injury to surgery was 3 hours (IQR 2.5 hours). The median stay in the ICU was 3 days (IQR: 3 days). The outcome was favourable (GOS 4 and 5) in 43 patients (57%). Thirteen patients (17%) remained severely or moderately disabled (GOS 3). Finally, a total of 21 patients (28%) died or remained in a persistent vegetative state (GOS 1 and 2). Mortality was 41% for acute subdural haematoma (19/46) and 3% (1/37) for patients with epidural haematoma. Only age, GCS at admission and pupil abnormalities seemed to be associated with outcome. Time to surgery was not. CONCLUSION: In patients admitted with acute traumatic epidural and subdural haematomas that are treated within a median of 3 hours after primary injury, factors such as age, initial GCS and pupil abnormalities still appear to be the most important factors correlating with outcome.

PURPOSE: The goal of this study was to analyse a possible association of admission blood glucose with hospital mortality of polytraumatised patients and to develop an outcome prediction model for this patient group. METHODS: The outcome of adult polytraumatised patients admitted to the University Hospital of Berne, Switzerland, between 2002 and 2004 with an ISS > or = 17, and more than one severely injured organ system was retrospectively analysed. RESULTS: The inclusion criteria were met by 555 patients, of which 108 (19.5%) died. Hyperglycaemia proved to be an independent predictor for hospital mortality (P < 0.0001), following multiple regression analysis. After inclusion of admission blood glucose, the calculated mortality prediction model performed better than currently described models (P < 0.0001, AUC 0.924). CONCLUSION: In this retrospective, single-centre study in polytraumatised patients, admission blood glucose proved to be an independent predictor of hospital mortality following regression analysis controlling for age, gender, injury severity and other laboratory parameters. A reliable admission blood glucose-based mortality prediction model for polytraumatised patients could be established. This observation may be helpful in improving the precision of future outcome prediction models for polytraumatised patients. These observations warrant further prospective evaluation.

INTRODUCTION Low systolic blood pressure (SBP) is an important secondary insult following traumatic brain injury (TBI), but its exact relationship with outcome is not well characterised. Although a SBP of <90mmHg represents the threshold for hypotension in consensus TBI treatment guidelines, recent studies suggest redefining hypotension at higher levels. This study therefore aimed to fully characterise the association between admission SBP and mortality to further inform resuscitation endpoints. METHODS We conducted a multicentre cohort study using data from the largest European trauma registry. Consecutive adult patients with AIS head scores >2 admitted directly to specialist neuroscience centres between 2005 and July 2012 were studied. Multilevel logistic regression models were developed to examine the association between admission SBP and 30 day inpatient mortality. Models were adjusted for confounders including age, severity of injury, and to account for differential quality of hospital care. RESULTS 5057 patients were included in complete case analyses. Admission SBP demonstrated a smooth u-shaped association with outcome in a bivariate analysis, with increasing mortality at both lower and higher values, and no evidence of any threshold effect. Adjusting for confounding slightly attenuated the association between mortality and SBP at levels <120mmHg, and abolished the relationship for higher SBP values. Case-mix adjusted odds of death were 1.5 times greater at <120mmHg, doubled at <100mmHg, tripled at <90mmHg, and six times greater at SBP<70mmHg, p<0.01. CONCLUSIONS These findings indicate that TBI studies should model SBP as a continuous variable and may suggest that current TBI treatment guidelines, using a cut-off for hypotension at SBP<90mmHg, should be reconsidered.
