The saphenous nerve (SaN) innervates the region from the upper medial thigh to the medial aspect of the foot and ankle. A femoral nerve block (FNB) is effective for blockade of the SaN but this causes quadriceps weekness and reduced patient mobility that is unsuitable in an ambulatory surgical setting.

Incisional hernia is one of the most common complications of laparotomy. Its repair with prosthesis has enabled a considerable improvement in the outcome, significantly reducing recurrences. This study analyses the results of open hernioplasty with mesh performed as a Day Surgery procedure in 42 patients between November 2008 and October 2010. The results were good, with low postoperative morbidity and recurrences (2.4%).

Con el objeto de probar el efecto de la analgesia preventiva, un concepto que viene siendo difundido en los últimos años para tratar el dolor agudo postoperatorio, se realizó un estudio clínico controlado aleatorizado a simple ciego de 92 pacientes ASA I-II sometidos a colecistectomía laparoscópica bajo anestesia general balanceada. Se asignaron 2 grupos de 46 pacientes cada uno, ambos recibieron una asociación de tramadol 1 mg.kg -1+Ketoprofeno 1 mg.kg -1. El gruo PRE 30 minutos antes de la incisión guirúrgica y el grupo POS al finalizar la intervención. Se midio el efecto del tratamiento según los requerimientos de analgesia suplementaria en 48 horas de postoperatorio. Los grupos fueron comparables en sus variables demográficas y en las variables hemodinámicas de la pre y posanestesia. La diferencia en los requerimientos analgésicos por número y promedio de dosis de ketoprofeno administrado en el período de control [PRE 263.04 más menos 82.62 mg vs POS 258.7 más menos 77.68 mg] no fue estadísticamente significativa [P mayor 0.05]. La incidencia de náusea y vómito postoperatorio fue mayor para el grupo POS=0.016]

Surgical site infections (SSIs) can affect body tissues, cavities, or organs manipulated in surgery and constitute 14% to 16% of all infections. This study aimed to determine the incidence of SSIs in women following their discharge from a gynecology outpatient clinic, to survey different types of SSIs among women, and to verify the association of SSIs with comorbidities and clinical conditions. Data were collected via analytical observation with a cross-sectional design, and the study was conducted in 1,026 women who underwent gynecological surgery in a teaching hospital in the municipality of Teresina, in the northeast Brazilian State of Piauí, from June 2011 to March 2013. The incidence of SSIs after discharge was 5.8% among the women in the outpatient clinic. The most prevalent surgery among the patients was hysterectomy, while the most prevalent type of SSI was superficial incisional. Comorbidities in women with SSIs included cancer, diabetes mellitus, and hypertension. Surveillance of SSIs during the post-discharge period is critical for infection prevention and control. It is worth reflecting on the planning of surgical procedures for patients who have risk factors for the development of SSIs.

OBJECTIVE: To screen for mutations in AMH and AMHR2 genes in patients with persistent Müllerian duct syndrome (PMDS). PATIENTS AND METHOD: Genomic DNA of eight patients with PMDS was obtained from peripheral blood leukocytes. Directed sequencing of the coding regions and the exon-intron boundaries of AMH and AMHR2 were performed. RESULTS: The AMH mutations p.Arg95*, p.Arg123Trp, c.556-2A>G, and p.Arg502Leu were identified in five patients; and p.Gly323Ser and p.Arg407* in AMHR2 of two individuals. In silico analyses of the novel c.556-2A>G, p.Arg502Leu and p.Arg407* mutations predicted that they were harmful and were possible causes of the disease. CONCLUSION: A likely molecular etiology was found in the eight evaluated patients with PMDS. Four mutations in AMH and two in AMHR2 were identified. Three of them are novel mutations, c.556-2A>G, and p.Arg502Leu in AMH; and p.Gly323Ser in AMHR2. Arq Bras Endocrinol Metab. 2012;56(8):473-8
