Erősődő igény van a jelenlegi sokoldalú nemzetközi felügyeleti rendszer reformjára úgy, hogy az a nemzeti gazdaságpolitikák nemzetközi hatásait is figyelembe vegye. Ehhez át kell reformálni a jelenlegi globális pénzügyi szabályozási rendszert, de ki kell alakítani a nemzeti gazdaságpolitikák egymásra hatásának koordinációját is. Ide tartozik a globális külső sokkok csillapítására szolgáló anti-ciklikus finanszírozás, a nemzetközi adózási együttműködés fokozása, a nemzetközi adósságfinanszírozás tökéletesítése, vagy a globális fizetésieszköz-tartalékok és a fizetési rendszer viszonya. Ez utóbbi területen az SDR kiterjedtebb használatának feltételeit kell kimunkálni. A jelenlegi globális intézmények – WTO, Nemzetközi Valutaalap, Világbank – alapos megújítása elkerülhetetlen. A globális gazdasági koordináció intézményi kereteinek kidolgozása viszont még várat magára. A G20-ak csoportja – bár fontos reformokat kezdeményezett –, nem tekinthető a világgazdasági egyensúlytalanságok megoldása letéteményesének. A cél csak olyan globális intézményi struktúra lehet, amely egyaránt képes a világ nagy számú gazdaságai közötti koordinációs feladatok megoldására, s a döntések végrehajtásának kikényszerítésére. / === / The present multilateral international surveillance system needs to be reformed with an eye on international repercussions of national economic policies. The present global financial architecture, the coordination of interplays of national economic policies must be modified. An anti-cyclical financing capable of absorbing global external shocks, strengthening of international tax cooperation, improving international debt financing or the relations between global financial reserves and the global payment system might be part and parcel of this process. A more extended use of SDR could be worked out. Reforms of the present global institutions – the WTO, the IMF, the World Bank – cannot be avoided any further. Institutional frameworks of global economic coordination mechanism have still not been worked out. The Group of 20 (G-20) cannot be seen as the sole player in fighting world economic disequlibria. A global institutional system is envisaged, which is able to implement economic coordination among national economic units and to enforce the implementation of decisions taken. At present there is no global institution dealing with coherence and consistency of global issues. Reforming present institutions and/or designing new ones are possible options. The basis for such an international coordination must involve general acceptance of principles, transparent implementation, and enforcement of decisions taken.

The concept of a global civil society is gaining greater acceptance among International Relations (IR) scholars, yet few studies exist that look at the role of fraternal organizations and their influence in constructing this realm. Freemasonry, one of the oldest fraternal orders, exerts a powerful influence on its membership through its symbolism, architecture and ritual, based on the tenets of mutual respect and tolerance towards all human beings. Such principles helped in creating a body of practices and institutions as early as the eighteenth century which two hundred years later were identified and conceptualized as global civil society. ^ The allegations of anti-Masons and conspiracy theorists offer a continuous account of Masonry's influence on the political scene since its modern founding in 1717 Great Britain. Conspiracy theorists portray the coming of a New World Order, orchestrated and directed by a secret hierarchy of Masons/Illuminati. Even though the lens of conspiracy theories paints a distorted view of reality, it does focus attention to Freemasonry's activities as a major player in politics over the span of three centuries. Not only do such theories challenge the novelty of practices that make up a global civil society, but also the notion that it is an inclusive and growing sector that unites people across the globe. They also provide a valuable critique by pointing out the inconsistencies and discriminatory practices of Masonry as contrasted with the lofty ideals and aims for humanity. ^ The Masonic influence in the social world is perceived as one that reflects the liberal worldview where the nation-state and power structures are in pursuit of human progress, or profit. The symbolism of Masonry, however, carries a message that can be characterized as representing republican ideals. Masonic symbolism and ritual create spaces of meaning where the contradictions between the ideals and the structures of inequality and elitism can be resolved. Freemasonry as a symbolic system proclaiming their inherent republican values does have a global reach. However, the effectiveness of these values is bounded by the constraints that are inherent in a liberal world dominated by nation-states. ^

The proliferation of weapons of mass destruction (WMD), nuclear, biological and chemical (NBC) is one of the main security challenges facing the international community today. However the new Global Security Strategy of 2016 raises the question of non-proliferation of WMD only as an incidental matter, not addressing directly the threat, a fundamental threat in the regional and global security. This is a clear step backwards for the European common security.

El objetivo del artículo es exponer una adecuada y necesaria redefinición de la política de la UE hacia el Norte de África y el Sahel, tanto en términos geográficos como en términos estratégicos de acuerdo con lo expuesto en la nueva Estrategia Global Europea. En este sentido, se revisarán los postulados y la evolución de la antigua Estrategia Europea de Seguridad (EES) y su consiguiente Política de Vecindad (ENP), en comparación con la recién lanzada Estrategia. La nueva Estrategia es concebida como una guía para el desarrollo de la política exterior y de seguridad que la UE ha de llevar a cabo, inter alia, en su periferia. Lamentablemente su concepción de esta zona regional resulta anclada en el pasado.

At the outset of the United Nations Convention on the Rights of the Child, the Committee on the Rights of the Child identified four of its provisions (non-discrimination; best interests of the child as a primary consideration; life, survival and development; and participation) as ‘general principles’. This approach has shaped implementation of, advocacy for and the scholarship on the Convention. The use of general principles has the potential to make a significant contribution in other areas of human rights law provided that the principles are selected carefully and address the distinct issues at the root of potential rights violations for particular rights-holders.

Ante la crisis mundial (por el fin de la época del Estado-nación y el inicio de la aldea global), resulta prioritario repensar las categorías con las que comprendemos el mundo. En este caso, se invita a la reflexión sobre la normatividad social, para hacer ver que no sólo es jurídica, también lo es ético-moral, entre otras. Incluso, la normatividad jurídica no se limita a un conjunto de principios y preceptos, sino que dispone también de una dimensión subjetiva, relativa a las relaciones jurídicas. Éstas dependen del grado de intervención de la voluntad que, a su vez, se ve influenciada por las normas ético-morales. En definitiva, aquí no sólo se denuncian las falacias del positivismo formalista de Estado, sino que se busca la reafección político-jurídica del sistema (frenando la entropía social en ciernes), además de ofrecerse un paradigma normativo adecuado a laglobalización: el normativismo iberoamericano basado en la ética humanista.

Remarkable gains have been made in global health in the past 25 years, but progress has not been uniform. Mortality and morbidity from common conditions needing surgery have grown in the world’s poorest regions, both in real terms and relative to other health gains. At the same time, development of safe, essential, life-saving surgical and anaesthesia care in low-income and middle-income countries (LMICs) has stagnated or regressed. In the absence of surgical care, case-fatality rates are high for common, easily treatable conditions including appendicitis, hernia, fractures, obstructed labour, congenital anomalies, and breast and cervical cancer. In 2015, many LMICs are facing a multifaceted burden of infectious disease, maternal disease, neonatal disease, non-communicable diseases, and injuries. Surgical and anaesthesia care are essential for the treatment of many of these conditions and represent an integral component of a functional, responsive, and resilient health system. In view of the large projected increase in the incidence of cancer, road traffic injuries, and cardiovascular and metabolic diseases in LMICs, the need for surgical services in these regions will continue to rise substantially from now until 2030. Reduction of death and disability hinges on access to surgical and anaesthesia care, which should be available, affordable, timely, and safe to ensure good coverage, uptake, and outcomes. Despite growing need, the development and delivery of surgical and anaesthesia care in LMICs has been nearly absent from the global health discourse. Little has been written about the human and economic effect of surgical conditions, the state of surgical care, or the potential strategies for scale-up of surgical services in LMICs. To begin to address these crucial gaps in knowledge, policy, and action, the Lancet Commission on Global Surgery was launched in January, 2014. The Commission brought together an international, multi- disciplinary team of 25 commissioners, supported by advisors and collaborators in more than 110 countries and six continents. We formed four working groups that focused on thedomains of health-care delivery and management; work-force, training, and education; economics and finance; and information management. Our Commission has five key messages, a set of indicators and recommendations to improve access to safe, affordable surgical and anaesthesia care in LMICs, and a template for a national surgical plan.

By 2005, human organ trafficking, commercialization, and transplant tourism had become a prominent and pervasiveinfluence on transplantation therapy. The most common source of organs was impoverished people in India,Pakistan, Egypt, and the Philippines, deceased organ donors in Colombia, and executed prisoners in China. Inresponse, in May 2008, The Transplantation Society and the International Society of Nephrology developed theDeclaration of Istanbul on Organ Trafficking and Transplant Tourism consisting of a preamble, a set of principles, anda series of proposals. Promulgation of the Declaration of Istanbul and the formation of the Declaration of IstanbulCustodian Group to promote and uphold its principles have demonstrated that concerted, strategic, collaborative,and persistent actions by professionals can deliver tangible changes. Over the past 5 years, the Declaration of IstanbulCustodian Group organized and encouraged cooperation among professional bodies and relevant international, regional,and national governmental organizations, which has produced significant progress in combating organ traffickingand transplant tourism around the world. At a fifth anniversary meeting in Qatar in April 2013, the DICGtook note of this progress and set forth in a Communique´ a number of specific activities and resolved to furtherengage groups from many sectors in working toward the Declaration’s objectives.

Surgery is essential for global cancer care in all resource settings. Of the 15.2 million new cases of cancer in 2015, over 80% of cases will need surgery, some several times. By 2030, we estimate that annually 45 million surgical procedures will be needed worldwide. Yet, less than 25% of patients with cancer worldwide actually get safe, affordable, or timely surgery. This Commission on global cancer surgery, building on Global Surgery 2030, has examined the state of global cancer surgery through an analysis of the burden of surgical disease and breadth of cancer surgery, economics and financing, factors for strengthening surgical systems for cancer with multiple-country studies, the research agenda, and the political factors that frame policy making in this area. We found wide equity and economic gaps in global cancer surgery. Many patients throughout the world do not have access to cancer surgery, and the failure to train more cancer surgeons and strengthen systems could result in as much as US $6.2 trillion in lost cumulative gross domestic product by 2030. Many of the key adjunct treatment modalities for cancer surgery--e.g., pathology and imaging--are also inadequate. Our analysis identified substantial issues, but also highlights solutions and innovations. Issues of access, a paucity of investment in public surgical systems, low investment in research, and training and education gaps are remarkably widespread. Solutions include better regulated public systems, international partnerships, super-centralisation of surgical services, novel surgical clinical trials, and new approaches to improve quality and scale up cancer surgical systems through education and training. Our key messages are directed at many global stakeholders, but the central message is that to deliver safe, affordable, and timely cancer surgery to all, surgery must be at the heart of global and national cancer control planning.

El objetivo de este estudio es establecer si la dexmedetomidina (DEX) es segura y efectiva para el manejo coadyuvante de síndrome de abstinencia a alcohol (SAA) a través de la búsqueda de evidencia científica. Metodología: se realiza una revisión sistemática de literatura publicada y no publicada desde enero de 1989 hasta febrero 2016 en PubMed, Embase, Scopus, Bireme, Cochrane library y en otras bases de datos y portales. Los criterios de inclusión fueron ensayos clínicos aleatorizados y no aleatorizados, estudios cuasi-experimentales, estudios de cohorte, y estudios de casos y controles; que incluyeron pacientes mayores de 18 años hospitalizados con diagnóstico de SAA y donde se usó DEX como terapia coadyuvante. Resultados: 7 estudios, 477 pacientes, se incluyeron en el análisis final. Se encontraron dos ensayos clínicos aleatorizados, tres estudios de casos y controles y dos estudios de cohorte retrospectivo. Solo uno de los estudios fue doble ciego y utilizó placebo como comparador. Análisis y conclusiones: en los estudios experimentales se determinan que el uso de DEX como terapia coadyuvante en el manejo de SAA tiene significancia clínica y estadística para disminuir dosis de BZD en las primeras 24 horas de tratamiento; pero no demostraron tener otros beneficios clínicos. En los estudios no aleatorizados existe consenso que relaciona el uso de DEX con menores dosis de BZD de forma temprana. Recomendaciones: no se recomienda el uso de DEX en SAA de forma rutinaria. Se recomienda usar DEX solo en casos en el que exista evidencia fallo terapéutico a BZD.
