Practical techniques to manage the dangers associated with sexually transmitted diseases have varied considerably both cross culturally and historically. Adopting a Foucauldian perspective, this article examines sociohistorical aspects of the governance of venereal disease in New South Wales between 1871 and 1916. Public debates and official documents are analysed to identify strategic shifts in practices associated with venereal disease management , especially in relation to prostitution. Particular attention is paid to the development of contagious disease legislation and its role in the regulation of venereal disease . It is argued that during the period in question, two distinct governmental regimes of disease control can be identified. In the first, medical policing managed venereal disease through the mobilisation of repressive controls, requiring the isolation and detention of polluting bodies. In the second, liberal governance adopted pedagogic practices to train populations perceived as either healthy or unhealthy. It is further argued that as liberal strategies of governance came to dominate the management of venereal disease , the association of prostitution with venereal disease began to weaken. Instead, authorities became increasingly concerned with populations whose behaviour was not traditionally linked with venereal disease , such as the young and the sexually inexperienced.

INTRODUCTION Influenza vaccination in pregnancy is recommended for all women in Australia, particularly those who will be in their second or third trimester during the influenza season. However, there has been no systematic monitoring of influenza vaccine uptake among pregnant women in Australia. Evidence is emerging of benefit to the infant with respect to preventing influenza infection in the first 6 months of life. The FluMum study aims to systematically monitor influenza vaccine uptake during pregnancy in Australia and determine the effectiveness of maternal vaccination in preventing laboratory-confirmed influenza in their offspring up to 6 months of age. METHODS AND ANALYSIS A prospective cohort study of 10 106 mother-infant pairs recruited between 38 weeks gestation and 55 days postdelivery in six Australian capital cities. Detailed maternal and infant information is collected at enrolment, including influenza illness and vaccination history with a follow-up data collection time point at infant age 6 months. The primary outcome is laboratory-confirmed influenza in the infant. Case ascertainment occurs through searches of Australian notifiable diseases data sets once the infant turns 6 months of age (with parental consent). The primary analysis involves calculating vaccine effectiveness against laboratory-confirmed influenza by comparing the incidence of influenza in infants of vaccinated mothers to the incidence in infants of unvaccinated mothers. Secondary analyses include annual and pooled estimates of the proportion of mothers vaccinated during pregnancy, the effectiveness of maternal vaccination in preventing hospitalisation for acute respiratory illness and modelling to assess the determinants of vaccination. ETHICS AND DISSEMINATION The study was approved by all institutional Human Research Ethics Committees responsible for participating sites. Study findings will be published in peer review journals and presented at national and international conferences. Objectives To quantify the burden of disease attributable to smoking in South Africa for 2000. Design The absolute difference between observed lung cancer death rate and the level in non-smokers, adjusted for occupational and indoor exposure to lung carcinogens, was used to estimate the proportion of lung cancer deaths attributable to smoking and the smoking impact ratio (SIR). The SIR was substituted for smoking prevalence in the attributable fraction formula for chronic obstructive pulmonary disease (COPD) and cancers to allow for the long lag between exposure and outcome. Assuming a shorter lag between exposure and disease, the current prevalence of smoking was used to estimate the population-attributable fractions (PAF) for the other outcomes. Relative risks (RR) from the American Cancer Society cancer prevention study (CPS-II) were used to calculate PAF. Setting South Africa. Outcome measures Deaths and disability-adjusted life years (DALYs) due to lung and other cancers, COPD, cardiovascular conditions, respiratory tuberculosis, and other respiratory and medical conditions. Results Smoking caused between 41 632 and 46 656 deaths in South Africa, accounting for 8.0 - 9.0% of deaths and 3.7 - 4.3% of DALYs in 2000. Smoking ranked third (after unsafe sex/sexually transmitted disease and high blood pressure) in terms of mortality among 17 risk factors evaluated. Three times as many males as females died from smoking. Lung cancer had the largest attributable fraction due to smoking. However, cardiovascular diseases accounted for the largest proportion of deaths attributed to smoking. Conclusion Cigarette smoking accounts for a large burden of preventable disease in South Africa. While the government has taken bold legislative action to discourage tobacco use since 1994, it still remains a major public health priority.

Previous attempts to determine the degree to which exposure to environmental factors contribute to noncommunicable diseases (NCDs) have been very conservative and have significantly underestimated the actual contribution of the environment for at least two reasons. Firstly, most previous reports have excluded the contribution of lifestyle behavioral risk factors, but these usually involve significant exposure to environmental chemicals that increase risk of disease. Secondly, early life exposure to chemical contaminants is now clearly associated with an elevated risk of several diseases later in life, but these connections are often difficult to discern. This is especially true for asthma and neurodevelopmental conditions, but there is also a major contribution to the development of obesity and chronic diseases. Most cancers are caused by environmental exposures in genetically susceptible individuals. In addition, new information shows significant associations between cardiovascular diseases and diabetes and exposure to environmental chemicals present in air, food, and water. These relationships likely reflect the combination of epigenetic effects and gene induction. Environmental factors contribute significantly more to NCDs than previous reports have suggested. Prevention needs to shift focus from individual responsibility to societal responsibility and an understanding that effective prevention of NCDs ultimately relies on improved environmental management to reduce exposure to modifiable risks.

Purpose of review: To describe articles since January 2013 that include information on how costs change with infection prevention efforts. Recent findings: Three articles described only the costs imposed by nosocomial infection and so provided limited information about whether or not infection prevention efforts should be changed. One article was found that described the costs of supplying alcohol-based hand run in low-income countries. Eight articles showed the extra costs and cost savings from changing infection prevention programmes and discussed the health benefits. All concluded that the changes are economically worthwhile. There was a systematic review of the costs of methicillin-resistant Staphylococcus aureus control programmes and a methods article for how to make cost estimates for infection prevention programmes. Summary: The balance has shifted away from studies that report the high cost of nosocomial infections toward articles that address the value for money of infection prevention. This is good as simply showing a disease is high cost does not inform decisions to reduce it. More research, done well, on the costs of implementation, cost savings and change to health benefits in this area needs to be done as many gaps exist in our knowledge.

Chronic diseases are a leading cause of death and disability, largely attributable to modifiable lifestyle risk factors. Many midlife Australian are getting insufficient physical activity for health and face a range of barriers to exercise. Results of this study provide evidence that benefits and barriers are an important predictor of exercise behaviour in midlife women and, that a 12 week nurse led health promotion program can effectively promote benefits and increase physical activity. This study provides evidence about benefits and barriers to exercise that will inform health promotion practice for chronic disease risk factor reduction in midlife women.

African-born individuals in the U.S. face significant health challenges, including low utilization of preventive screening services. Using a community-based participatory research framework, we describe preliminary efforts at establishing a collaborative relationship with the East African communities of San Diego, identifying salient community health needs, and developing a framework for disseminating information and addressing identified health gaps. To this end, 40 East African-born women participated in focus groups with the purpose of eliciting community perspectives on U.S. health care services, beliefs about preventive screening, and to garner recommendations for future outreach. Qualitative analyses identified participants’ desire to engage in primary prevention techniques that incorporated best practices from their home countries and the U.S., and the need for health education programs to provide information on increasingly prevalent chronic diseases. The findings are discussed in connection with continued community-engaged efforts and the implications for health and resettlement policies to reduce inequities disfavoring resettled refugees.

Introduction Two symposia on “cardiovascular diseases and vulnerable plaques” Cardiovascular disease (CVD) is the leading cause of death worldwide. Huge effort has been made in many disciplines including medical imaging, computational modeling, bio- mechanics, bioengineering, medical devices, animal and clinical studies, population studies as well as genomic, molecular, cellular and organ-level studies seeking improved methods for early detection, diagnosis, prevention and treatment of these diseases [1-14]. However, the mechanisms governing the initiation, progression and the occurrence of final acute clinical CVD events are still poorly understood. A large number of victims of these dis- eases who are apparently healthy die suddenly without prior symptoms. Available screening and diagnostic methods are insufficient to identify the victims before the event occurs [8,9]. Most cardiovascular diseases are associated with vulnerable plaques. A grand challenge here is to develop new imaging techniques, predictive methods and patient screening tools to identify vulnerable plaques and patients who are more vulnerable to plaque rupture and associated clinical events such as stroke and heart attack, and recommend proper treatment plans to prevent those clinical events from happening. Articles in this special issue came from two symposia held recently focusing on “Cardio-vascular Diseases and Vulnerable Plaques: Data, Modeling, Predictions and Clinical Applications.” One was held at Worcester Polytechnic Institute (WPI), Worcester, MA, USA, July 13-14, 2014, right after the 7th World Congress of Biomechanics. This symposium was endorsed by the World Council of Biomechanics, and partially supported by a grant from NIH-National Institute of Biomedical Image and Bioengineering. The other was held at Southeast University (SEU), Nanjing, China, April 18-20, 2014.

Mastitis is one of the most economically significant diseases for the dairy industry for backyard farmers in developing countries and high producing herds worldwide. Two of the major factors impeding reduction in the incidence of this disease is [a] the lack of availability of an effective vaccine capable of protecting against multiple etiological agents and [b] propensity of some of the etiological agents to develop persistent antibiotic resistance in biofilms. This is further complicated by the continuing revolving shift in the predominant etiological agents of mastitis, depending upon a multitude of factors such as variability in hygienic practices on farms, easy access leading to overuse of appropriate or inappropriate antibiotics at suboptimal concentrations, particularly in developing countries, and lack of compliance with the recommended treatment schedules. Regardless, Staphylococcus aureus and Streptococcus uberis followed by Escherichia coli, Streptococcus agalactiae has become the predominant etiological agents of bovine mastitis followed Streptococcus agalactiae, Streptococcus dysagalactiae, Klebsiella pneumonia and the newly emerging Mycoplasma bovis. Current approaches being pursued to reduce the negative economic impact of this disease are through early diagnosis of infection, immediate treatment with an antibiotic found to either inhibit or kill the pathogen(s) in vitro using planktonic cultures and the use of the currently marketed vaccines regardless of their demonstrated effectiveness. Given the limitations of breeding programs, including genetic selection to improve resistance against infectious diseases including mastitis, it is imperative to have the availability of an effective broad-spectrum, preferably cross-protective, vaccine capable of protecting against bovine mastitis for reduction in the incidence of bovine mastitis, as well as interrupting the potential cross-species transmission to humans. This overview highlights the major etiological agents, factors affecting susceptibility to mastitis, and the current status of antibiotic-based therapies and prototype vaccine candidates or commercially available vaccines against bovine mastitis as potential preventative strategies. © 2013 Tiwari JG, et al.

This report provides a systematic review of the most economically damaging endemic diseases and conditions for the Australian red meat industry (cattle, sheep and goats). A number of diseases for cattle, sheep and goats have been identified and were prioritised according to their prevalence, distribution, risk factors and mitigation. The economic cost of each disease as a result of production losses, preventive costs and treatment costs is estimated at the herd and flock level, then extrapolated to a national basis using herd/flock demographics from the 2010-11 Agricultural Census by the Australian Bureau of Statistics. Information shortfalls and recommendations for further research are also specified. A total of 17 cattle, 23 sheep and nine goat diseases were prioritised based on feedback received from producer, government and industry surveys, followed by discussions between the consultants and MLA. Assumptions of disease distribution, in-herd/flock prevalence, impacts on mortality/production and costs for prevention and treatment were obtained from the literature where available. Where these data were not available, the consultants used their own expertise to estimate the relevant measures for each disease. Levels of confidence in the assumptions for each disease were estimated, and gaps in knowledge identified. The assumptions were analysed using a specialised Excel model that estimated the per animal, herd/flock and national costs of each important disease. The report was peer reviewed and workshopped by the consultants and experts selected by MLA before being finalised. Consequently, this report is an important resource that will guide and prioritise future research, development and extension activities by a variety of stakeholders in the red meat industry. This report completes Phase I and Phase II of an overall four-Phase project initiative by MLA, with identified data gaps in this report potentially being addressed within the later phases. Modelling the economic costs using a consistent approach for each disease ensures that the derived estimates are transparent and can be refined if improved data on prevalence becomes available. This means that the report will be an enduring resource for developing policies and strategies for the management of endemic diseases within the Australian red meat industry.

Trata-se de um estudo quantitativo, com abordagem descritiva e comparativa, que objetivou verificar a influência da consulta de enfermagem gerontológica no nível de adesão terapêutica dos clientes com doenças crônicas não-transmissíveis, acompanhados num ambulatório especializado. Como objetivo específico, buscou-se comparar o nível de adesão terapêutica, perfil sociodemografico e clínico entre o grupo de clientes acompanhados na consulta de enfermagem e o grupo dos não acompanhados. Este foi motivado pela práxis da autora em consultas gerontológica, onde foram percebidos obstáculos enfrentados por muitos clientes com adesão inadequada ao tratamento, acarretando dificuldades no controle de suas doenças, fato este ratificado pela literatura. Os resultados evidenciaram similaridade entre os perfis sociodemográficos, com prevalência de sexo feminino, baixa escolaridade e renda. O perfil clínico revelou, em ambos os grupos, alto índice de hipertensão, diabetes e depressão, destacando-se esta última por ser desfavorável ao comprometimento com o autocuidado. Por fim, verificou-se que os níveis de adesão terapêutica ficaram majoritariamente dentro da faixa estabelecida como adesão ampla, sem diferença expressiva entre os grupos. É relevante citar que, entre os acompanhados pela consulta de enfermagem, observou-se, um maior conhecimento sobre a doença em tratamento e suas manifestações, maior acesso aos medicamentos e auto percepção de conhecimentos sobre efeitos colaterais. Constatando-se que, este último achado, exerce grande influência na adesão ao tratamento farmacológico. Este desfecho reflete a importância de uma assistência sistematizada, embasada em uma teoria de enfermagem, que neste contexto, foi a teoria do autocuidado de Orem, utilizada previamente nas consultas de enfermagem do ambulatório investigado. Os resultados corroboram o uso desta teoria, especialmente numa perspectiva educativa da assistência ambulatorial. Diante disso, é mister buscar novas abordagens de pesquisa, a fim continuar a investigação sobre as contribuições da enfermagem para uma melhor adesão terapêutica dos clientes idosos. Finalmente, a autora espera cooperar para o aprimoramento científico nesta matéria e para uma progressiva qualificação da assistência de enfermagem na prevenção e controle de agravos à saúde.

As doenças crônicas não transmissíveis (DCNT) estão posicionadas no topo das enfermidades em termos de morbimortalidade, no Brasil e no mundo. Entre estas, as doenças cardiovasculares (DCV), e particularmente, as cerebrovasculares (DCbV), produzem um impacto significativo sobre a autonomia das pessoas, desfalcando a força de trabalho das nações e gerando um alto custo para a previdência social de todos os países. No Brasil, só muito recentemente as enfermidades circulatórias passaram a ser contempladas por políticas públicas formuladas pelo Ministério da Saúde (MS), não só pela manutenção destas doenças em altos patamares de morbimortalidade, mas também pelo crescimento exponencial de alguns dos seus fatores de risco. Partindo do pressuposto que as políticas e programas oficiais não estão sendo efetivamente implementados no âmbito da Atenção Primária à Saúde (APS), o objetivo do presente estudo foi investigar e analisar como estas iniciativas do MS vem sendo efetivamente executadas em Juiz de Fora-MG. A estratégia utilizada para essa investigação consistiu em uma pesquisa qualiquantitativa com base em observação, documentos e entrevistas semi-estruturadas com os diferentes componentes profissionais das Equipes de Saúde da Família de três unidades básicas de saúde do município citado. Foram entrevistados 40 profissionais de saúde, entre médicos, enfermeiros e agentes comunitários de saúde, buscando-se entender como os programas governamentais com interface com a prevenção das doenças cardiovasculares e, em especial, cerebrovasculares, vêm sendo implementados ao nível do Programa de Saúde da Família. Na comparação entre o que é recomendado nos programas governamentais e o que vem sendo executado nas UBS, concluiu-se que ainda há um longo caminho a ser percorrido para que estes programas sejam efetivamente implementados na porta de entrada do sistema de saúde.

Introdução: A Política Nacional de Atenção Integral à Saúde do Homem surge para compreender a singularidade masculina nos seus diversos contextos socioculturais. A política trás, como um dos objetivos, promover, na população masculina, conjuntamente com o Programa Nacional de DST/AIDS, a prevenção e o controle das doenças sexualmente transmissíveis e da infecção pelo HIV. A pesquisa em tela tem por objetivo: Identificar a representação social do ser homem para homens que se referem como heterossexuais; Descrever as práticas sociais e culturais que podem levar o homem a se expor ao HIV; e Analisar a representação social do ser homem e sua relação com a vulnerabilidade para a infecção pelo HIV. Metodologia: Trata-se de uma pesquisa qualitativa, pautada na Teoria das Representações Sociais, realizada no CTA em São Gonçalo, Rio de Janeiro, com a participação de 08 sujeitos, com os quais foram desenvolvidas entrevistas semiestruturadas. Os dados foram analisados através da análise de conteúdo. Resultado: Da análise dos dados surgiram quatro categorias, quais sejam, a representação social do ser homem: imagens, comportamentos e compromissos; O homem ideal e o romantizado: caráter, sucesso, herói e imagem; o homem real: individualismo, apego às máquinas, o jeitinho masculino e a vida sexual; e o universo reificado da aids e a construção do preconceito. A ideia do homem ideal aparece para os sujeitos como aquele que pratica o que é politicamente correto e que, por sua vez, segue as normas da sociedade vigente. Para eles a masculinidade tem como desdobramento a virilidade e o homem é, por natureza, considerado como ser insaciável sexualmente. Os entrevistados apontaram para uma dimensão avaliativa do homem real, como um sujeito individualista e com práticas hedonistas e com uma prática sexual desenfreada. Contudo, a proteção pela infecção ao HIV é representada através da fidelidade conjugal, onde o comportamento sexual considerado adequado serve como imunização contra a infecção. Os sujeitos apresentaram o homem ideal como um ser romantizado, rompendo com o estereótipo que se tem do homem na sociedade, como sujeito duro, infiel e dominador. A partir do estudo constatou-se que os entrevistados possuem uma visão reificada sobre a doença e representam a aids como a doença do outro. Conclusão/Contribuições para enfermagem: A compreensão do homem na atualidade pode nos trazer novas discussões no que diz respeito à construção social do mesmo, assim como suas implicações diante das vulnerabilidades existentes para a infecção pelo HIV/AIDS, a fim de desconstruir mitos e tabus que permeiam a questão cultural do que é ser masculino, para assim, entender esse sujeito no âmbito dos serviços de saúde e trabalhar práticas sociais e sexuais masculinas saudáveis.

O objetivo deste estudo é discutir a associação entre padrão de consumo de álcool e dimensões de risco à exposição ao HIV, desdobradas em conhecimento sobre HIV e práticas sexuais entre universitários. Trata-se de uma pesquisa transversal com abordagem quantitativa, realizada com 416 universitários de 14 cursos distintos. Os dados foram coletados através de dois instrumentos e tratados através de estatística descritiva com o software SPSS 21.0. A pesquisa foi autorizada pelo Comitê de Ética em Pesquisa da Universidade do Estado do Rio de Janeiro, sob n 003.3.2012. A população de estudo é predominantemente do sexo masculino (59,6%), com faixa etária entre 19 a 22 anos (54,3%), de cor branca (57,5%), sem companheiros (69,2%) ou filhos (92,3%) e com maior percentual de católicos (31,3%) e daqueles que não possuem religião (33,7%). Destaca-se, ainda, que são possuidores de computador ou eletrônicos portáteis (98,8%) com fácil acesso à internet (96,8%). Os principais achados apontam que a maioria faz uso de álcool (60%), com a proporção de 7 homens para cada 3 mulheres. Relacionado ao AUDIT, foi identificado predomínio das zonas I (abstinência ou baixo risco-73,8%) e II (uso nocivo-20,4%). Sobre as relações sexuais, a maioria afirmou ter experiência sexual (69,5%), com idade da primeira relação entre 16 e 18 anos (54%), no entanto, mais homens (54,3%) afirmaram manter relações sexuais após o consumo de álcool do que mulheres (45,7%). Apesar do conhecimento sobre HIV/aids e álcool ser considerado como fator protetor pela literatura vigente, constatou-se que não há associação deste conhecimento com a prática sexual mais segura. Independente do padrão de consumo de bebidas alcoólicas, os universitários apresentam o mesmo tipo de prática sexual, muitas vezes se expondo à infecção ao HIV e outras doenças sexualmente transmissíveis. Dentre os dados analisados, um dos motivos de exposição ao HIV/Aids são as relações sexuais após a ingestão de bebida alcoólicas e o não uso de preservativos nestas situações. O consumo exagerado de bebidas alcoólicas está ligado ao sexo masculino e este grupo apresenta práticas sexuais de maior risco. Sugere-se que novos estudos possam analisar a relação de causa e efeito para verificar quais fatores podem influenciar de fato a exposição ao HIV de estudantes usuários de álcool. Estas informações são relevantes para conhecermos a atual demanda desse grupo e focar nas reais necessidades que são imperiosas para a prevenção dos futuros danos nocivos à saúde individual e coletiva. Torna-se necessário compreender qual é a demanda dos jovens em se expor a diversos riscos à saúde adotando práticas não seguras e, principalmente, o que de fato modula estas condutas.

El cáncer de cuello de útero o cáncer de cérvix es el segundo tumor más frecuente en las mujeres del mundo después del de mama y el quinto de todos los canceres. En España, es el sexto más frecuente, constituyendo el 4,8% de los canceres en la mujer. Este tipo de cáncer es el resultado final de una infección no resuelta por el virus del papiloma humano (VPH). Hoy en día, el VPH representa una de las infecciones de trasmisión sexual más común. El 70-80 % de hombres y mujeres sexualmente activos están o han estado expuestos al VPH. El cambio en la conducta sexual de las mujeres ha llevado a que las infecciones de trasmisión sexual sean ahora más frecuentes que en las generaciones anteriores. Hay un inicio más precoz de las primeras relaciones sexuales y un número más elevado de compañeros sexuales. Estos cambios han hecho aumentar la prevalencia del VPH en los últimos años en las mujeres jóvenes de España. La combinación de estrategias de prevención primaria (vacunación contra el VHP) y secundaria (cribado) permitiría reducir la incidencia y la mortalidad del cáncer de cuello de útero. Existen dos vacunas frente al VPH que son Gardasil ® y Cervarix ®. Al tratarse de dos vacunas nuevas, el éxito de la introducción dependerá de la comprensión apropiada de los riesgos y beneficios de la vacuna para prevenir la infección del VPH. Esto se consigue mediante programas efectivos de educación y formación sobre el VPH.
