Orbital myositis is a subgroup of the nonspecific inflammatory syndrome or orbital pseudotumor and is characterized by a primary inflammation of extraocular muscles. The authors describe a 70-year-old patient with acute proptosis, ocular pain and right ophthalmoplegia, whose orbital computed tomographic scan showed enlargement of the homolateral extraocular muscles. Clinical presentation and complementary tests were compatible with the diagnosis of orbital myositis however, because of the particular aspects, which included retinal central vein occlusion, optic nerve lesion, distension of the superior ophthalmic vein and the homolateral cavernous sinus, the differential diagnosis with cavernous sinus pathology and thyroid ophthalmopathy was considered. The importance of a rapid diagnosis and treatment is stressed.

The ability to control human immunodeficiency virus type 1 (HIV-1) infection and progression of the disease is regulated by host and viral factors. This cross-sectional study describes the socio-demographic and epidemiological characteristics associated with HIV-1 infection in 1,061 subjects attended in Londrina and region, south of Brazil: 136 healthy individuals (Group 1), 147 HIV-1-exposed but uninfected individuals (Group 2), 161 HIV-1-infected asymptomatic patients (Group 3), and 617 patients with AIDS (Group 4). Data were obtained by a standardized questionnaire and serological tests. The age of the individuals ranged from 15.1 to 79.5 years, 54.0% and 56.1% of the Groups 3 and 4 patients, respectively, were men. The major features of groups 2, 3, and 4 were a predominance of education level up to secondary school (55.8%, 60.2% and 62.4%, respectively), sexual route of exposure (88.4%, 87.0% and 82.0%, respectively), heterosexual behavior (91.8%, 75.2% and 83.7%, respectively), and previous sexually transmitted diseases (20.4%, 32.5%, and 38.1%, respectively). The patients with AIDS showed the highest rates of seropositivity for syphilis (25.6%), of anti-HCV (22.3%), and anti-HTLV I/II obtained by two serological screening tests (6.2% and 6.8%, respectively). The results documenting the predominant characteristics for HIV-1 infection among residents of Londrina and region, could be useful for the improvement of current HIV-1 prevention, monitoring and therapeutic programs targeted at this population.

Apesar das infecções urinárias serem relativamente frequentes durante a gravidez, a ocorrência de um abcesso renal é muito raro. A patogénese do abcesso renal tem mudado ao longo dos anos. Actualmente, mais de 75% dos casos têm origem numa infecção do aparelho urinário. Os autores apresentam o caso clínico de uma grávida de 35 anos, internada às 38 semanas e cinco dias de gestação por febre, náuseas, vómitos e dor lombar à direita. Os dados clínicos e os exames complementares realizados levaram ao diagnóstico de abcesso renal. É feita uma revisão da literatura acerca do tema.

Purpose: The purpose of this article is to discuss the impact of Supplier orientation and the resulting Supply Chain Management (SCM) approach, on the organizational performance of ISO 9001 Quality Management Systems certified organizations. Methodology/Approach: Following a literature review, a full structural conceptual model was proposed. An online survey was administered to managers of Portuguese organizations with certified ISO 9001 Quality Management Systems. Descriptive Statistics and Structural Model Equations were used to validate the proposed conceptual model. Findings: There are positive relationships between Organization Strategy and Supplier Orientation, between Supplier Orientation and Stakeholders Satisfaction, and between Stakeholders Satisfaction and Organizational Performance Orientation, supporting ISO 9001:2015. These findings provide insights that supplier orientation, mediated by stakeholder satisfaction, is an essential tool for organizational competitive sustainable advantage. Research Limitation/implication: The analysis was based on managers of ISO 9001 certified organizations perceptions, so additional studies with actual data and longitudinal studies should be useful for further validation. Originality/Value of paper: The importance of the overall organizational ecosystem is highlighted with potential impact on the more than 1 Million ISO 9001 organizations certified worldwide and in their suppliers.

SUMMARY Background. Disseminated histoplasmosis is common in AIDS patients with advanced immunosuppression in Ceará, Northeastern Brazil. The goal of this study was to determine the prevalence of Histoplasma infection in patients with HIV/AIDS living in Fortaleza, the capital of Ceará. Methods. Intradermal tests with histoplasmin (mycelial phase) were performed in 161 HIV patients with CD4 ≥ 350 cells/mm 3 . Evidence of recent illness was evaluated with immunodiffusion (ID) tests in 76 of these individuals. Results. A total of 11.8% of patients reacted to histoplasmin and 2.63% had ID test positive to Histoplasma. The presence of mango trees (Mangifera indica) in the patient neighborhood (OR = 2.870; 95% CI = 1.081-7.617; p = 0.040) and past activity involving soil (OR = 2.834; 95% CI = 1.045-7.687; p = 0.045) or visits to a farm (OR = 3.869; 95% CI = 1.189-12.591; p = 0.033) were significantly associated with Histoplasma infection. Conclusions. Patients with HIV living in Fortaleza have an expressive prevalence of infection with Histoplasma.

Na abordagem da cicatrização de uma ferida cirúrgica deve equacionar-se a cicatrização por segunda intenção como uma alternativa válida, revelando excelentes resultados e claras vantagens após selecção adequada dos doentes. Os autores descrevem o caso de um doente do sexo masculino com 66 anos de idade, submetido a excisão de um carcinoma espinocelular da concha do pavilhão auricular direito e subsequente cicatrização por segunda intenção. Discutem-se algumas particularidades desta técnica revendo a literatura relevante.

Foi feita uma revisão dos levantamentos malacológicos realizados pela Fundação Nacional de Saúde em 52 municípios de 8 microrregiões do estado de Santa Catarina, Brasil, de 1981 a 1994. Cinqüenta deles foram positivos para Biomphalaria tenagophila, o único vetor do Schistosoma mansoni identificado nos 94.535 exemplares examinados. Das 1.358 localidades trabalhadas, 617 (45,4%) foram positivas, variando de 4,3% a 89,4%, por município. Os caramujos foram coletados em 0,2% a 26,3% das coleções hídricas pesquisadas, por município, sendo que 7,1% (2.013/28.120) serviam de criadourospara oplanorbídeo. Nos municípios de Araquari, Massaranduba, Joinville, Jaraguã do Sul e São Francisco do Sul, na microrregião de Joinville, foram encontrados caramujos infectados com S. mansoni. Os dois últimos são os focos de esquistossomose mansoni mais meridionais do Brasil. Outras pesquisas identificaram a B. peregrina, B. oligoza, B. schrammi, B. straminea e B. occidentalis.

O presente trabalho teve como principais objectivos o estudo do mecanismo da libertação do ferro em proteínas da família da ferritina (DNA-binding proteins from starved cells, Dps), bem como a identificação, produção e caracterização de potenciais parceiros redox destas proteínas através da utilização de técnicas bioquímicas e espectroscópicas apropriadas. Foram identificadas no genoma de Marinobacter hydrocarbonoclasticus duas flavoproteínas (2375 e 3073) reconhecidas como possíveis parceiros redox da Dps de Pseudomonas nautica 617. Todas as proteínas foram expressas heterologamente em células de E. coli BL21 (DE3) e delineados protocolos de purificação em dois passos, por cromatografias de permuta iónica e de exclusão molecular, que permitiram obter rendimentos expressivos (Dps — 31,9 mg/L de cultura, Flavoproteína 2375 — 73 mg/L de cultura e Flavoproteína 3073 — 79,4 mg/L de cultura). Aquando da purificação da 2375 verificou-se que a estabilidade da holoproteína depende da força iónica, característica que limita a sua utilização como parceiro redox da Dps. O estudo da reacção de transferência electrónica foi iniciado com testes preliminares através da espectroscopia de UV/Visível, permitindo avaliar da ocorrência da reacção de redução do core férrico da Dps e concomitante libertação do produto final ferroso, por análise de uma mistura contendo NADH, flavoproteína 3073 oxidada e Dps core Fe. Este estudo não permite, contudo, estabelecer uma relação de causa efeito concreta e fiável que nos permita identificar o NADH como doador inicial de eletrões e ferro ferroso como produto final desta reacção. O mecanismo de libertação foi estudado em maior detalhe através de espectroscopia de Mössbauer permitindo estabelecer a natureza das diferentes espécies intervenientes na reacção e assim verificar, pela primeira vez, que na presença dos três componentes, acima mencionados, existe redução e libertação de ferro previamente incorporado na Dps, na forma de iões ferrosos, bem como determinar parâmetros cinéticos apropriados.

Polymer binder modification with inorganic nanomaterials (NM) could be a potential and efficient solution to control matrix flammability of polymer concrete (PC) materials without sacrificing other important properties. Occupational exposures can occur all along the life cycle of a NM and “nanoproducts” from research through scale-up, product development, manufacturing, and end of life. The main objective of the present study is to analyse and compare different qualitative risk assessment methods during the production of polymer mortars (PM) with NM. The laboratory scale production process was divided in 3 main phases (pre-production, production and post-production), which allow testing the assessment methods in different situations. The risk assessment involved in the manufacturing process of PM was made by using the qualitative analyses based on: French Agency for Food, Environmental and Occupational Health & Safety method (ANSES); Control Banding Nanotool (CB Nanotool); Ecole Polytechnique Fédérale de Lausanne method (EPFL); Guidance working safely with nanomaterials and nanoproducts (GWSNN); Istituto Superiore per la Prevenzione e la Sicurezza del Lavoro, Italy method (ISPESL); Precautionary Matrix for Synthetic Nanomaterials (PMSN); and Stoffenmanager Nano. It was verified that the different methods applied also produce different final results. In phases 1 and 3 the risk assessment tends to be classified as medium-high risk, while for phase 2 the more common result is medium level. It is necessary to improve the use of qualitative methods by defining narrow criteria for the methods selection for each assessed situation, bearing in mind that the uncertainties are also a relevant factor when dealing with the risk related to nanotechnologies field.

In this work we develop a new mathematical model for the Pennes’ bioheat equation assuming a fractional time derivative of single order. A numerical method for the solu- tion of such equations is proposed, and, the suitability of the new model for modelling real physical problems is studied and discussed

In this work we perform a comparison of two different numerical schemes for the solution of the time-fractional diffusion equation with variable diffusion coefficient and a nonlinear source term. The two methods are the implicit numerical scheme presented in [M.L. Morgado, M. Rebelo, Numerical approximation of distributed order reaction- diffusion equations, Journal of Computational and Applied Mathematics 275 (2015) 216-227] that is adapted to our type of equation, and a colocation method where Chebyshev polynomials are used to reduce the fractional differential equation to a system of ordinary differential equations
