A case-control study was conducted to identify risk factors for death from tetanus in the State of Pernambuco, Brazil. Information was obtained from medical records of 152 cases and 152 controls, admitted to the tetanus unit in the State University Hospital, in Recife, from 1990 to 1995. Variables were grouped in three different sets. Crude and adjusted odds ratios, p-values and 95% confidence intervals were estimated. Variables selected in the multivariate analysis in each set were controlled for the effect of those selected in the others. All factors related to the disease progression - incubation period, time elapsed between the occurrence of the first tetanus symptom and admission, and period of onset - showed a statistically significant association with death from tetanus. Similarly, signs and/or symptoms occurring on admission or in the following 24 hours (second set): reflex spasms, neck stiffness, respiratory signs/symptoms and respiratory failure requiring artificial ventilation (third set) were associated with death from tetanus even when adjusted for the effect of the others.

This work aims to evaluate the impact of drug treatment on infection by Ascaris lumbricoides (Al), Trichuris trichiura (Tt) and hookworms (Hook) in a rural community from the sugar-cane zone of Pernambuco, Brazil. Four parasitological surveys were carried out from March 2001 to March 2002. Individual diagnosis was based on eight slides (four by the Kato-Katz method and four by the Hoffman method) per survey. Infected subjects were assigned to two groups for treatment with either albendazole (n = 62) or mebendazole (n = 57). Prevalence of infection fell significantly (p < 0.05) one month after treatment: Al (from 47.7% to 6.6%); Tt (from 45.7% to 31.8%) and Hook (from 47.7% to 24.5%). One year after treatment, infections by Tt and Hook remained significantly below pre-control levels. A substantial decrease in single-infection cases and multiple infections was found. Egg-negative rate was significant for Al (94.0%), Hook (68.3%) but not for Tt (45.5%), and did not differ significantly between subjects treated with mebendazole or albendazole. Egg counts fell significantly in the individuals remaining positive for Tt. It is recommended that antihelminthic treatment should be selective and given at yearly intervals preferably with albendazole, due to its cost-effectiveness.

AIM: The aim of the study was to evaluate the effectiveness of a 10-week combined training programme (aerobic and strength exercise) compared to an aerobic training programme, and respiratory physiotherapy on COPD patients' health. METHODS: Fifty subjects with moderate to severe COPD were randomly assigned to two groups. Combined group (CG, n=25) who underwent combined training, and aerobic group (AG, n=25) who underwent aerobic training. These were compared with fifty COPD subjects who underwent respiratory physiotherapy, breathing control and bronchial clearance techniques (RP group, n = 50). We evaluated health state through two questionnaires, St. George's Respiratory Questionnaire (SGRQ) and SF-36, at the beginning and at the end of the programme. RESULTS: The CG group showed differences (p<0.0001) in modification rates in state of health compared to the AG and RP groups in the activity (64 ± 9%, 19 ± 7%, 1 ± 15%) , impact (35 ± 5%, 20 ± 18%, 1 ± 14%) and total (41 ± 9%, 26 ± 17%, 1 ± 15%) domains assessed by the SGRQ, and the physical function (109 ± 74%, 22 ± 12%, 0.1 ± 18%), physical role (52 ± 36%, 11 ± 15%, 1.3 ± 21%) and vitality (83 ± 39%, 14 ± 38%) domains assessed by SF-36. CONCLUSION: These results suggest that combined training in subjects with COPD appears to be a more effective method, with better clinical changes, and improvements in health state perception.

The objective of this report is to describe the first record of Desmodus rotundus in urban area from the city of Olinda, Pernambuco State, Northeastern Brazil, and to draw attention to the possible risk of rabies transmission in this place. After the complaint of a dog owner who observed three bats attacking his dog, images registering attacks of D. rotundus were captured with a video camera. From 09:00 p.m. on 13 February 2004 to 04:00 a.m. of the next day, a high frequency of haematophagic activity and the presence of several bites on the dog's body were observed. This finding represents a serious risk to public health. Thus, it is necessary to further study the bat fauna, with special attention to their feeding behaviour in this place, in order to better know their biology and to adopt pertinent control measures. This is, to our knowledge, the first record of D. rotundus in urban area of Olinda.

As alterações do comportamento frequentemente observadas em doentes internados nas unidades de cuidados intensivos (UCI), podem ser adequadamente designadas, na maioria das vezes, por estado confusional agudo, o qual se caracteriza por: flutuação do estado de vigília, distúrbio do ciclo vigília-sono, défice de atenção e concentração, desorganização do pensamento, manifestado entre outras formas por discurso incoerente, distúrbios da percepção sob a forma de ilusões e/ou alucinações, desorientação no tempo e no espaço, agitação ou diminuição da actividade psicomotora e perturbação da memória. O estado confusional agudo nas UCI resulta, geralmente, das seguintes situações: doenças e distúrbios melancólicos/sistémicos, tais como a sepsis, a insuficiência renal e a insuficiência hepática; exposição a agentes tóxicos exógenos, tais como medicamentos; privação de substâncias de abuso, como o álcool; e doenças primariamente intracranianas, tais como infecções do sistema nervoso central. Frequentemente, coexistem outras causas, sendo as principais: a privação de sono, os défices cognitivos prévios, o medo e a ansiedade, bem como, em certos casos, o tipo de personalidade do doente. O tratamento compreende a correcção dos distúrbios metabólicos/sistémicos; a suspensão de tóxicos e/ou o uso de antídotos; o tratamento da privação; o uso de haloperidol com ou sem benzodiazepinas; e medidas não farmacológicas que diminuam o stress ambiental e promovam o bem-estar físico e mental.
